  • Treatment of giant cell tumor of distal radius can be treated in several ways according to the aggressiveness of the tumor. But the management of giant cell tumor involving juxta-articular portion has always been a difficult problem. In some giant cell tumors with bony destruction, a wide segmental resection may be needed for preventing to recur. But a main problem is preserving of bony continuity in bony defect as well as preservation of joint function. We have attempted to overcome these problems by using a microvascular technique to transfer the fibula with peroneal vascular pedicle or anterior tibial vessel as living bone graft. From April 1984 to July 2005, we performed the reconstruction of wide bone defect after segmental resection of giant cell tumor in 14 cases, using Vascularized Fibular Graft, which occur at the distal radius. VFG with peroneal vascular pedicle was in 8 cases and anterior tibial vessel was 6 cases. Recipient artery was radial artery in all cases. Method of connection was end to end anastomosis in 11 cases, and end to side in 3 cases. An average follow-up was 6 years 6 months, average bone defect after wide segmental resection of lesion was 6.8 cm. All cases revealed good bony union in average 6.5 months, and we got the wide range of motion of wrist joint without recurrence and serious complications. Grafted bone was all alive. In functional analysis, there was good in 7 cases, fair in 4 cases and bad in 1 case. Pain was decreased in all cases but there was nearly normal joint in only 4 cases. Vascularized fibular graft around wrist joint provided good functional restoration without local recurrence.

  • In recent tunneling projects, encounters with multi-layered strata have become more frequent as the desired scale of tunneling increases. Despite substantial practical experience, the design of large-diameter shield-driven tunnels often simplifies the surrounding ground as uniform, overlooking the complexities introduced by non-uniform geotechnical factors. This study comparatively analyzed the influence of design factors, particularly segment stiffness and joint parameters, on segmental lining behavior in layered ground conditions using numerical methods. A comprehensive parametric study revealed the significant impact of deformative interaction between the lining and the soft top soil layer on overall tunnel behavior. Permitting lining deformation in the soft soil layer effectively mitigated the induced internal forces but resulted in considerable tunnel lining convergence, adopting a peanut-shaped appearance. From a practical design perspective, application of a soft segment with lower stiffness near the stiff soil layer is an economically advantageous approach, alleviating internal forces within an acceptable convergence level. Notably, around the interfaces between soil layers with different stiffnesses, the induced internal forces in the lining were minimized based on joint rotational stiffness and location. This indicates the possibility of achieving an optimal design for segmental lining joints under layered ground conditions. Additionally, a preliminary design method was proposed, which sequentially optimizes parameters for joints located near soil layer interfaces. Subsequently, a specialized design based on the proposed method for complex multi-layered strata was compared with a conventional design. The results confirmed that the internal force was effectively relieved at an allowable lining deflection level.

  • Although posterior segmental fixation technique is becoming increasingly popular, surgical treatment of craniovertebral junctional disorders is still challenging because of its complex anatomy and surrounding critical neurovascular structures. Basilar invagination is major pathology of craniovertebral junction that has been a subject of clinical interest because of its various clinical presentations and difficulty of treatment. Most authors recommend a posterior occipitocervical fixation following transoral decompression or posterior decompression and occipitocervical fixation. However, both surgical modalities inadvertently sacrifice C0-1 and C1-2 joint motion. We report two cases of basilar invagination reduced by the vertical distraction between C1-2 facet joint. We reduced the C1-2 joint in an anatomical position and fused the joint with iliac bone graft and C1-2 segmental fixation using the polyaxial screws and rods C-1 lateral mass and the C-2 pedicle.

  • This paper presents a study on the behaviour of the joints of segmental concrete bridges with external prestressing, focusing on the structural response of dry non-epoxied joints with shear keys. A Finite Element joint model to study such structures is validated modelling eight concrete panel tests. The most important feature of this model is that it has been validated with experimental tests on concrete panels which were specifically designed to fail in shear. Interface elements are used to reproduce the non linear behaviour of the joint and parameters deduced from the tests are used to define the constitutive law of these elements. This joint model is of great importance because it will permit the development of a structural model that faithfully reproduces the behaviour of these structures under combined flexure and shear and the study of its global behaviour after the opening of the joints. Interesting conclusions about the behaviour of the dry joints, about the contribution of the different mechanisms transferring shear (friction and cohesion) and about the shear stress distribution in the joint have been reached.

  • The development of transportation in large cities requires the construction of twin tunnels located at shallow depth. As far as twin tunnels excavated in parallel are concerned, most of the cases reported in literature focused on considering the effect of the ground condition, tunnel size, depth, surface loads, the relative position between two tunnels, and construction process on the structural lining forces. However, the effect of the segment joints was not taken into account. Numerical investigation performed in this study using the $FLAC^{3D}$ finite difference element program made it possible to include considerable influences of the segment joints and tunnel distance on the structural lining forces induced in twin tunnels. The structural lining forces induced in the first tunnel through various phases are considerably affected by the second tunnel construction process. Their values induced in a segmental lining are always lower than those obtained in a continuous lining. However, the influence of joint distribution in the second tunnel on the structural forces induced in the first tunnel is insignificant. The critical influence distance between two tunnels is about two tunnel diameters.

In certain some giant cell tumors with bony destruction, a wide segmental resection may be needed for preventing to recur. But a main problem is preserving of bony continuity in bony defect as well as preservation of joint function. The traditional bone grafts have high incidence in recurrence rate, delayed union, bony resorption, stress fracture despite long immobilization and stiffness of adjuscent joint. We have attemped to overcome these problems by using a microvascular technique to transfer the fibula with peroneal vascular pedicle as a living bone graft. An average follow-up was 2 years 8 months, average bone defect after wide segmental resection of lesion was 11.4cm. These all cases revealed good bony union in average 6.5months, and we got the wide range of motion of adjacent joint without recurrence and serious complications.

  • The purpose of this study was to assess the inter-segmental trunk motion during which multi-segmental movements of the spinal column was designed to interpret the effect of segmentation on the total measured spine motion. Also it analyzed the relative motion at three types of the spine models in drop landing. A secondary goal was to determine the intrinsic algorithmic errors of spine motion and the usefulness of such an approach as a tool to assess spinal motions. College students in the soccer team were selected the ten males with no history of spine symptoms or injuries. Each subject was given a fifteen minute adaptation period of drop landing on the 30cm height box. Inter-segmental spine motion were collected Vicon Motion Capture System (250 Hz) and synchronized with GRF data (1000 Hz). The result shows that Model III has a more increased range of motion (ROM) than Model I and Model II. And the Lagrange energy has significant difference of at E3 and E4 (p<.05). This study can be concluded that there are differences in the three models of algorithm during the phase of load absorption. Especially, Model III shows proper spine motion for the inter-segmental joint motion with the interaction effects using the seven segments. Model III shows more proper observed values about dynamic equilibrium than Model I & Model II. The findings have shown that the dynamic stability strategy of Model III toward multi-directional spinal motion supports for better function of the inter-segmental motor-control than the Model I and Model II.
