Pulmonary Aspergillosis - Pulmonary Aspergillosis - (폐 Aspergillosis -6예 보고-)

  • Pulmonary aspergillosis is being recognized with increasing frequency in recent years and the-rising incidence of this infection parallels certain medical advances in antibiotics, chemotherapeutic and immunosuppressive therapy. The cavities of lungs resulting from tuberculosis, histoplasmosis or neoplasm are apt, to be infected by one of the species of the genus Aspergillus and eventually mycetomas are formed within the cavities. Authors have experienced 6 cases of pulmonary aspergillosis forming mycetoma in Dept. of Thoracic Surgery, Catholic Medical Center from Aug. 1976 to Feb. 1979. Hemoptysis or blood tinged sputum, the predominant symptom, occurred in all cases. All patients underwent pulmonary resection, 1 pneumonectomy, 3 lobectomies, 1 lobectomy with segmental resection and 1 segmental resection and survived well without death or complication. Primary aspergillosis was in 2 cases and underlying diseases were present in 4 cases: 3 pulmonary tuberculosis, 1 bronchiectasis. The common diagnostic study of intracavitary mycetoma was the posterioanterior chest roentgenogram; in cavities suspected of being diseased or in doubtful cases, tomography was most available to find fungus ball with air-meniscus shadow.

Surgical Observation of Bronchiectasis (기관지확장증의 외과적 치료)

  • Sixty-one bronchiectatic patients were reviewed who were treated by surgical intervention in the Department of Thoracic and Cardiovascular Surgery, Chonnam University Hospital during the recent 14 years from January of 1965 to August of 1978. 1. Various pulmonary resection was performed; lobectomy was 55.5%, lobectomy and segmental resection 20.6%, bilobectomy 13.1%, lobectomy, segmental resection and thoracoplasty 4.9% and pneumonectomy was 3. 2%. 2. The most common associated disease was pulmonary Tbc, its frequency was 47.5% and next was chronic bronchitis [31%]. 3. The postoperative complication was observed in 19.6% [12 cases] and the most common was wound infection. 4. The possibility of recurrence was relatively high in the cases of residual lesion and it needs more intensive medical care. 5. The surgical result was satisfactory in 86.9% (53 cases), slight improvement in 4.9% (3 cases) and the mortality rate was 1.6% (1 case).

Completing the Seohae Grand Bridge Construction Supervision (서해대교 감리를 마치며)

  • Seohae Grand Bridge is a part of the new West Coast Highway(353km) under construction, which connects Inchon and Mokpo. It is the longest bridge in Korea (7.31 km), and has 97 spans of 60m each precast segmental approach bridges, 2 main spans of 165m each free cantilever segmental bridge(500m), and 1 stay cable bridge of 990m In total length. During the seven year long construction period, many new construction technologies and methods were utilized for the first time in Korea, and gave invaluable opportunities to experience and master these in completing the project on time with safety and precision. I am proud of being a member of this project, and wish to express deep appreciations to those who participated in the project.

  • In recent years, precast prestressed concrete segmental box girder bridges have been increasingly constructed Expansion disphragm segment of this type bridge transfers forces from the superstructure onto bearing or column, and plays an important roll of anchorage zone for longitudinal prestressed forces. Non-linear stresses occur inside of diaphragms by these extensive concentrated forces. In this study, the strut-and-tie models are proposed to design an expansion segment rationally. A formula to determine the effective transverse prestressed forces is proposed on the basis of these models. The present study is expected to provide an effective tool to design expansion segment of prestressed concrete bridges rationally.

Flexural Tests on Post-Tensioned Segmental Composite Beam (세그멘탈 합성보의 휨 실험)

  • The interior portion of Gerber's beam are constructed with post-tensioned segmental composite beams in this study. A precast concrete member which is larger than the limits of domestic transportation regulation in weight, length, and volume is divided into three parts, transported separately, and erected with a composite member by post-tensioning in site. Static flexural loading tests are performed on Gerber's type frames which are consisted with 2.5m overhangs and 5m interior beams composited from three pieces. The connection of overhang to interior composite beam and beam to beam, and flexural performance of interior portion of Gerber's beam are examined thoroughly. All of the tests are ended with a compression failure of the interior composite beams over the design strength of homogeneous beams. The brittle connection failures or tensile failures with the failure of lower strand was not observed in any test frames.

The Effect of Phosphodiesterase-4-Specific Inhibitor in the Rat Model of Spinal Nerve Ligation

  • Objective : Peripheral neuropathy is characterized by hyperalgesia, spontaneous burning pain, and allodynia. The purpose of this study was to investigate the effect of rolipram, a phosphodiesterase-4-specific inhibitor, in a segmental spinal nerve ligation model in rats. Methods : Both the L5 and L6 spinal nerves of the left side of the rats were ligated. Phosphodiesterase-4 inhibitor (rolipram) and saline (vehicle) were administered intraperitoneally. We measured mechanical allodynia using von Frey filaments and a nerve conduction study. Results : The mechanical allodynia, which began to manifest on the first day, peaked within 2 days. Multiple intraperitoneal injections of rolipram ameliorated the mechanical allodynia. Furthermore, an intraperitoneal administration of rolipram improved the development of pain behavior and nerve conduction velocity. Conclusion : This study suggests that the phosphodiesterase-4 inhibitor, rolipram, alleviates mechanical allodynia induced by segmental spinal nerve ligation in rats. This finding may have clinical implications.

Investigation of the seismic performance of precast segmental tall bridge columns

  • Precast segmental bridge columns (PSBC) are alternatives for monolithic cast-in-situ concrete columns in bridge substructures, with fast construction speed and structural durability. The analytical tool for common use is demonstrated applicable for seismic performance prediction of PSBCs through experiment conducted earlier. Then the analytical program was used for parameter optimization of PSBC configurations under reversal cyclic loading. Shear strength by pushover analysis was compared with theoretical prediction. Moreover, seismic response of PSBC with energy dissipation (ED) bars was compared with its no ED bar counterpart under three history ground acceleration records. The investigation shows that appropriate ED bar and post-tensioned tendon arrangement is important for higher lateral bearing capacity and good ductility performance of PSBCs.

Bamboo-like Te Nanotubes with Tailored Dimensions Synthesized from Segmental NiFe Nanowires as Sacrificial Templates

  • Bamboo-like Te nanotubes were synthesized via the galvanic displacement reaction of NiFe nanowires with Ni-rich and Fe-rich segments. The thick and thin components of the synthesized Te nanotubes were converted from the Ni-rich and Fe-rich segments in the NiFe nanowires respectively. The dimensions of the Te nanotubes were controlled by employing sacrificial NiFe nanowires with tailored dimensions as the template for the galvanic displacement reaction. The segment lengths of the Te nanotubes were found to be dependent on those of the sacrificial NiFe nanowires. The galvanic displacement reaction was characterized by analyzing the open circuit potential and the corrosion resistance.

  • Among the occlusal discrepancies, maxillary transverse deficiency is quite common in several reasons. The reasons are comprised of maxillary hypoplasia, thumb sucking habits, non-syndromic palatal synostosis and syndromal patients including cleft patients. Orthodontic treatment is used routinely to correct a deficiency in young patients while it has limitations for a skeletally mature patient. Surgical treatments help provide effective maxillary expansion to correct a deficiency in adults. Surgical methods can be categorized to segmental Le Fort I osteotomy and surgically assisted rapid maxillary expansion(SARME). Both methods seem successful but each method would have its own indication. We give a review on transverse maxillary deficiency and two surgical methods.

  • This paper presents a case history of a geosynthetics-reinforced segmental retaining wall, which collapsed during a sever rainfall immediately after the completion of the wall construction. In an attempt to identify possible causes for the collapse, a comprehensive investigation was carried out including physical and strength tests on the backfill, stability analyses on the as-built design based on the current design approaches, and slope stability analyses with pore pressure consideration. The investigation revealed that the inappropriate as-built design and the bad-quality backfill were mainly responsible for the collapse. This paper describes the site condition including wall design, details of the results of investigation and finally, lessons learned. Practical significance of the findings from this study is also discussed.
