Retiform hemangioendothelioma in the infratemporal fossa and buccal area: a case report and literature review

  • We report a case of retiform hemangioendothelioma (RH) located in the infratemporal fossa and buccal area in a 13-year-old Korean boy. The tumor originated from the sphenoid bone of the infratemporal fossa area and spread into the cavernous sinus, orbital apex, and retro-nasal area with bone destruction of the pterygoid process. Tumor resection was conducted via Le Fort I osteotomy and partial maxillectomy to approach the infratemporal fossa and retro-nasal area. The diagnosis of RH was confirmed after surgery. In the presented patient, surgical excision was incomplete, and close follow-up was performed. There was no evidence of expansion or metastasis of the residual tumor in the 8 years after surgery. In cases of residual RH with low likelihood of expansion and metastasis, even though RH is an intermediate malignancy, close follow-up can be the appropriate treatment choice over additional aggressive therapy. To date, 29 papers and 48 RH cases have been reported, including this case. This case is the second reported RH case presenting as primary bone tumor and the first case originating in the oromaxillofacial area.

Moderate hypofractionated image-guided thoracic radiotherapy for locally advanced node-positive non-small cell lung cancer patients with very limited lung function: a case report

  • Patients with locally advanced lung cancer and very limited pulmonary function (forced expiratory volume in 1 second $[FEV1]{\leq}1L$) have dismal prognosis and undergo palliative treatment or best supportive care. We describe two cases of locally advanced node-positive non-small cell lung cancer (NSCLC) patients with very limited lung function treated with induction chemotherapy and moderate hypofractionated image-guided radiotherapy (Hypo-IGRT). Hypo-IGRT was delivered to a total dose of 45 Gy to the primary tumor and involved lymph nodes. Planning was based on positron emission tomography-computed tomography (PET/CT) and four-dimensional computed tomography (4D-CT). Internal target volume (ITV) was defined as the overlap of gross tumor volume delineated on 10 phases of 4D-CT. ITV to planning target volume margin was 5 mm in all directions. Both patients showed good clinical and radiological response. No relevant toxicity was documented. Hypo-IGRT is feasible treatment option in locally advanced node-positive NSCLC patients with very limited lung function ($FEV1{\leq}1L$).

  • An odontoma is a slow growing and nonaggressive odontogenic tumor composed of enamel, dentin, cementum, and pulp tissue. The etiology of odontomas is unknown, although local trauma, infection, and genetic factors have been suggested. Odontomas are classified as compound odontoma or complex. A 20-year retrospective study was performed on 36 odontomas from the files of the Department of Oral Pathology at Chosun University School of Dentistry. Fifty-six percent of the patients were compound odontoma and 44% were complex odontoma. 56 percent of the patients were female and 44% were male. The odontoma is most often diagnosed in the second decade of life, during routine radiographic examination. The usual presenting symptoms are an impacted or and unerupted tooth, a retained primary tooth. Other less frequent signs and symptoms are pain, swelling, suppuration, foul odor, tooth mobility. In our patients were treated by enucleation of the tumor, and related teeth were treated by surgical extraction or orthodontically assisted eruption.

  • 유두상 섬유탄력종은 양성 심장 종양 중 두번째로 많이 발생하며, 주로 대동맥 판막이나 승모판막에 발생한다. 그러나, 폐동맥 판막에서 발생한 경우는 아주 드물게 보고되고 있다. 이에 저자들은 심부전증 환자에서 판막 치환술로 치료된 폐동맥 판막의 유두상 섬유탄력종을 드문 증례로 보고하는 바이다.

  • 선양낭성암종은 귀밑샘 및 턱밑샘에서 발생하는 비교적 드문 종양으로 치료는 수술적 제거와 재발 방지를 위하여 방사선요법을 시행하지만, 국소적 재발은 흔하고, 원격 전이는 드물지 않게 발생한다. 저자들은 일차성 턱밑샘 종양을 제거한 9년 후에 폐로 원격 전이가 된 59세 남자환자를 좌측 폐쐐기절제술 후 추적 관찰 84개월 동안 재발 없음을 보고하는 바이다.

대동맥 판막에 위치한 다발성 심장 유두상 섬유탄력종 (Multiple Cardiac Papillary Fibroelastoma of the Aortic Valve)

  • 심장 유두상 섬유탄력종은 원발성 종양 중 두 번째로 많이 호발하는 양성종양으로 주로 심장판막에 발생한다. 그러나 대부분 한곳에서 발생하며 다발성으로 발생하는 경우는 전체 심장 유두상 섬유탄력종의 5%로 아주 드문 경우이다. 55세 여자가 내원 10일전부터 발생한 순간적인 어지러움 및 실신으로 내원 시 검사한 심초음파상 대동맥 판막에 부착된 다발성 종양이 발견되어 수술 시행하였으며, 대동맥 판막의 손상 없이 대동맥 판막에 부착된 종양만 제거하였고, 환자 퇴원 후 외래 추적 관찰 중이다.

Late Occurrence of Multiple Bone Metastasis in Patient with Well Controlled Advanced Pancreatic Cancer

  • A 67-year-old male was admitted due to abdominal pain. Abdominal CT scan performed in a local clinic showed about 2 cm sized pancreatic tail mass with extensive liver and multiple regional lymph node metastasis. Histology of liver biopsy revealed poorly differentiated adenocarcinoma. He underwent chemotherapy with gemcitabine and erlotinib for 5 cycles followed by 8 cycles of second line chemotherapy with 5-fluorouracil and cisplatin. At 12 months after diagnosis, follow-up abdominal CT scan revealed marked reduction of tumor mass in the liver and pancreas with small residual tumor. After one month of last chemotherapy, he complained radiating pain along left leg. Blood chemistry revealed isolated elevation of alkaline phosphatase (ALP) and multiple bone metastasis were demonstrated in bone scan. Palliative radiation therapy to pelvic bone was performed for the relief of bone pain. The prognosis of advanced pancreatic cancer is extremely poor. We report late occurrence of multiple bone metastasis in a patient with well controlled advanced pancreatic cancer with chemotherapy.

Neoadjuvant Chemotherapy and Subsequent Adjuvant Chemotherapy with Hydroxyurea after Craniotomy in a Cat with a Meningioma

  • An 11-year-old neutered male domestic short-haired cat presented with neurological symptoms that developed over a three-month period. These included mental dullness, vocalization, ataxia, and visual impairment. The patient was diagnosed with a primary intracranial tumor at a local animal hospital. After the first diagnosis, the cat was administered hydroxyurea, prednisolone, omeprazole, and gabapentin for 3 months. After the initiation of medical treatment, the patient's clinical symptoms did not improve and the size of the tumor was static on the second magnetic resonance imaging (MRI). The dosage of hydroxyurea and prednisolone was increased for two weeks. The patient's clinical signs improved, and subsequently, a craniotomy was performed. The clinical signs completely resolved six days after surgery. Adjuvant chemotherapy with hydroxyurea was continuously administered after the craniotomy. The patient demonstrated a good clinical status during the nine-month follow-up period. Neoadjuvant chemotherapy has not yet been reported for meningiomas in cats. Further clinical trials with longer follow-up periods and larger patient cohorts will be required to confirm the effectiveness of neoadjuvant chemotherapy with hydroxyurea in feline meningioma.

  • Li-Fraumeni 증후군(이하 LFS)은 TP53 종양 억제 유전자의 생식세포 돌연변이로 인한 상염색체 우성 종양 유발 유전 질환이다. LFS 환자에서는 치료 후 치료와 관련된 암이 발생하기 때문에 LFS 환자에서 발생한 유방암의 치료에 있어 방사선 요법을 피해야 한다. 저자들은 LFS 환자에서 발생한 유방 육종의 방사선 및 화학 요법 치료 후 발생한 유방암 증례를 보고한다.

  • Cholangiocarcinoma is a heterogeneous group of aggressive tumors that correspond to the second most common primary liver tumor. They can be classified according to their anatomical position concerning the biliary tree, and each subtype demonstrates different behavior and treatment. A 38-year-old male patient presenting solely right lumbar pain was diagnosed with a 7 cm hepatic tumor involving segments I, Iva, and VIII associated with involvement of the hepatic veins. He underwent a bloc resection of hepatic segments I, II, III, IV, partial V, partial VII, and VIII; right, middle, and left hepatic veins; and inferior vena cava segment, with perfusion of the remaining liver in situ with a preservation solution. As the patient had a large accessory inferior right hepatic vein draining the remaining liver, no reimplantation of hepatic veins was necessary. He remained clinically stable in outpatient follow-up, with excellent performance status-current survival of 2 years 6 months after surgical treatment.
