Effects of Self-stretching and Joint Mobilization to Upper Thoracic Vertebrae in Craniovertebral Angle and Autonomic System Function in Chronic Cervical Pain (만성 목통증에 대한 자가신장과 위등뼈 관절가동술이 머리척추각 및 자율신경계 기능에 미치는 효과)

  • PURPOSE: This study aimed to examine the effects of self-stretching (SS) and joint mobilization (JM) on pain, craniovertebral angle (CVA), autonomic system function in chronic cervical pain patient with forward head posture (FHP). METHODS: A total of 30 male college students were selected as study subjects, and were divided into Group I (general physical therapy; GPT, n=10), Group II (GPT+SS, n=10), Group III (GPT+JM, n=10). All groups were evaluated three times a week for 4 weeks. Pain was measured by visual analogue scale (VAS), CVA was measured using digital goniometer and autonomic system function (heart rate; HR, skin conductivity; SC, LF norm, HF norm, LF/HF ratio) was measured by Biofeedback ProComp Infiniti. After 4 weeks, paired t-test was used to compare the changes within the group and one way ANOVA was used to compare those between the groups. RESULTS: In Group I, VAS was significantly decreased. In Group II and III was a significantly change in all items. In comparison between Group I and II was a difference in all items except HR. In comparison between Group I and III was a difference in all items. In comparison between Group II and III was a difference in VAS, LF norm and LF/HF ratio. CONCLUSION: This study showed that SS and JM can effectively reduce pain and normalize the autonomic system function.

Clinical Results of Mitral Valve Repair (승모판막질환의 판막재건술36례 성적)

  • From January 1962 to March 1987, 97 patients underwent operation for mitral valve disease. Of these patients, 61 [62.9%] required mitral valve replacement. Thirty-six patients [37.1%] had mitral valve repair. The mean age was 26.9*11.6 years [range 5 months to 48 years]. There were 32 [88.9%] cases of rheumatic valve disease, 4 [11.1%] cases of congenital mitral valve disease. Valve dysfunction was classified into three types: type I [normal leaflet motion], 6 patients; type II [prolapsed leaflet], 1 patient; type III [restricted leaflet motion], 29 patients. Twenty-nine patients [80.6%] had pure or predominant stenosis and 7[19.4%] had pure regurgitation. No patient was in NYHA functional class I. Three patients [16.7%] were in functional class II, 15[83.3%] were in functional class II. The techniques used included closed mitral commissurotomy [16 patients], open mitral commissurotomy [13 patients], localized Wooler type annuloplasty [4 patients], suture repair of leaflet defect [3 patients], chordal shortening [1 patient], Carpentier ring annuloplasty [1 patient], and fenestration of fused chordae [1 patient]. There were two perioperative deaths [5.6%], related to left ventricular failure and reoperation. The survivors were followed up for 94 patient-years [mean 4.68*5.54 years]. One late death [1.1*1.1% per patient-year] occurred and was valve related. Reoperation was required in 3 patients, of whom 2 were deaths. There was 1 case [1.1*1.1% per patient-year] of thromboembolism. No patient received anticoagulant after operation. At 10 year, 92*7.4% of the patients were still alive. The actuarial survival rate of patients free of valve-related complication was 79*6.4% at 6 years, 27*12.1% at 11 years. After surgery, 18 patients [88.9%] were in NYHA functional class I or II.

Expression and pH-dependence of the Photosystem II Subunit S from Arabidopsis thaliana

  • Photosynthesis uses light energy to drive the oxidation of water at an oxygen-evolving catalytic site within photosystem II (PSII). Chlorophyll binding by the photosystem II subunit S protein, PsbS, was found to be necessary for energy-dependent quenching (qE), the major energy-dependent component of non-photochemical quenching (NPQ) in Arabidopsis thaliana. It is proposed that PsbS acts as a trigger of the conformational change that leads to the establishment of nonphotochemical quenching. However, the exact structure and function of PsbS in PSII are still unknown. Here, we clone and express the recombinant PsbS gene from Arabidopsis thaliana in E. coli and purify the resulting homogeneous protein. We used various biochemical and biophysical techniques to elucidate PsbS structure and function, including circular dichroism (CD), fluorescence, and DSC. The protein shows optimal stability at $4^{\circ}C$ and pH 7.5. The CD spectra of PsbS show that the conformational changes of the protein were strongly dependent on pH conditions. The CD curve for PsbS at pH 10.5 curve had the deepest negative peak and the peak of PsbS at pH 4.5 was the least negative. The fluorescence emission spectrum of the purified PsbS protein was also measured, and the ${\lambda}_{max}$ was found to be at 328 nm. PsbS revealed some structural changes under varying temperature and oxygen gas condition.

Effects of Swimming Exercise and Joint Mobilization on HSP 70 Levels in Osteoarthritic Rats

  • Purpose: This study was performed to investigate the effect of joint mobilization on pain relief and cartilage repair in an induced osteoarthritis rat model by analyzing the expression of heat shock protein 70 in articular cartilage. Methods: MIA was injected into SD rats to induce osteoarthritis. These rats were divided into 4 groups: control group (n=30), no further treatment after the MIA injection ; experimental group I(n=30), performed swimming exercise after the MIA injection experimental group II (n=30), underwent joint mobilization after the MIA injection and experimental group III (n=30), performed swimming exercise and underwent joint mobilization after the MIA injection. For the histologic and pathophysiologic evaluation, safranin-O staining and for the immunohistochemical evaluation, the expression of HSP 70 in articular cartilage was analyzed 1, 7, 14, and 21 days after the MIA injection. Results: The inflammatory response and loss of tissue declined in experimental groups I and II over time, whereas the greatest decreases were noted in experimental group III. In the articular cartilage, low expression of HSP 70 was observed in every group on day 1, whereas HSP 70 expression was elevated on days 7 and 14 in experimental groups II and III. After 21 days, experimental group II displayed the strongest positive reaction, whereas HSP 70 was higher in experimental group III at this time point compared to that after 14 days. Conclusion: Our results showed that swimming exercise and joint mobilization had positive effects on pain relief and histologic and functional recovery in an induced osteoarthritis rat model.

Improvement Effect of the Water Extract from the Root of Cirsium japonicum var. ussuriense on Type II Collagen-induced Rheumatoid Arthritis Animal Models (엉겅퀴 뿌리 물 추출물의 류마티스 관절염 동물 모델에 대한 개선 효과)

  • Background: The roots of Cirsium japonicum var. ussuriense (RCJ) have been used as traditional medicine in Korea for hematuria and hematemesis. These extracts exert anti-oxidative and anti-inflammatory effects by scavenging for free radical and regulating the inflammatory response. However, the effect of RCJ on rheumatoid arthritis (RA) has not been elucidated. Thus, we evaluated the water extract of RCJ (WRCJ) using type II collagen-induced RA models. Methods and Results: RA was induced by immunization with type II collagen. All experimental materials were orally administered daily for three weeks. The positive control group was administered with 0.2 mg/kg methotrexate (n = 7), while the experimental group was administered with WRCJ (100 or 500 mg/kg, n = 7). Serum levels of TNF-alpha, Interleukin 6 (IL-6), and type II collagen IgG (CII) were measured using ELISA. Administration of 500 mg/kg WRCJ decreased the levels of TNF-alpha, IL-6, and CII. Moreover, WRCJ treatment diminished swelling of hind legs and infiltration of inflammatory cells in RA models' synovial membrane. Conclusions: These results indicate that WRCJ could improve RA, reduce inflammatory indicators and synovial inflammation. However, further experiments are required to determine how WRCJ can influence the signal transduction pathway in RA.

Efficacy and Safety of α-Lipoic Acid and Low Dose Pregabalin Combination in Painful Diabetic Neuropathy (당뇨병성 신경병증성 통증의 조절에 대한 α-Lipoic Acid와 저용량 Pregabalin 병용의 효능 및 안정성)

  • Purpose: The efficacy and safety of low-dose pregabalin and alpha lipoic acid in diabetic neuropathy were evaluated and analyzed. Materials and Methods: This study designed a retrospective study that included patients with diabetic neuropathic pain. From 2009 to 2022, 100 patients who suffered from diabetic neuropathic pain were included in this study. The patients were divided into group I (pregabalin 150 mg/day with alpha lipoic acid 600 mg/day) and group II (pregabalin 300 mg/day with alpha lipoic acid 600 mg/day). The visual analogue scale (VAS), medication side effects, and neurometer results were compared. Results: The mean follow-up period of the above patients was 120.23 weeks in group I and 149.05 weeks in group II. The average VAS score in group I decreased by 3.23 points, and the average VAS score in group II decreased by 2.86 points. Approximately 24.3% of group I had side effects, such as dizziness, sleepiness, and gastrointestinal trouble, while 76.7% of patients in group II had side effects. Sixtyseven patients had a neurometer examination before and after the medication, and there is no statistical difference between the two groups. Conclusion: The combination of low-dose pregabalin (pregabalin 150 mg/day) and alpha lipoic acid in diabetic neuropathy had a similar clinical effect and less frequent medication side effects than regular dose pregabalin (pregabalin 300 mg/day) and alpha lipoic acid. Therefore, low-dose pregabalin (pregabalin 150 mg/day) and alpha lipoic acid should be considered in treating diabetic neuropathy.

Gold-Silver Mineralization of the Au-Ag Deposits at Yeongdong District, Chung-cheongbuk-Do (충청북도(忠淸北道) 영동지역(永同地域) 금은광상(金銀鑛床)의 금은광화작용(金銀鑛化作用)에 관한 연구(硏究))

  • Most of the gold (-silver) vein deposits at Yeongdong District are mainly distributed in the precambrian metamorphic rocks. Based on the Ag/Au total production and ore grade ratios, the chemical composition of electrum and the associated sulfides, the gold(-silver) deposits at Yeongdong District may be classified into 4 classes: pyrrhotite - type gold deposits( I), pyrite - type gold deposits (IT A; massive vein), pyrite - type gold deposits (II B; nonmassive vein) and argentite - type gold - silver deposits(III). The chemical study on electrum(including native gold) revealed that Au content (2.8 to 92.4 atomic%) of electrums varies very widely for different classes of deposits. The Au content of electrum associated with pyrrhotite (Class I), ranging from 47.1 to 92.4 atomic% Au, is clearly higher than that associated with pyrite (Classes IIA, IIB and III). In contrast, classes I, II, and III deposits do not show clear differences in Au content of electrum. In general, pyrrhotite - type gold deposits(I) are characterized by features such as simply massive vein morphology, low values in the Ag/Au total production and ore grade ratios, the absence or rarity of silver - bearing minerals except electrum, and distinctively simple mineralogy. Although the geological and mineralogical features and vein morphology of pyrite - type gold deposits(IIA)are very similar to those of pyrrhotite - type gold deposits (I), Class II A deposits reveal significant differences in the associated iron sulfide (i. e. pyrite) with electrum and Au content of electrum. The Ag/Au total production and ore grade ratios from Class II A deposits are relatively slightly higher than those from Class I deposits. Pyrite - type gold deposits(II B) and argentite - type gold - silver deposits (III) have many common features; complex vein morphology, medium to high values in the Ag/Au total production and ore grade ratios and the associated iron sulfide (i. e. pyrite). In contrast to Class II B deposits, Class III deposits have significantly high Ag/Au total production and ore grade ratios. It indicates distinct difference in the abundance of silver minerals (i. e. native silver and argentite). The fluid inclusion analyses and mineralogical data of electrum tarnish method indicate that the gold mineralization of Classes I and II A deposits was deposited at temperatures between $230^{\circ}$ and $370^{\circ}C$, whereas the gold (-silver) mineralization of Classes ITB and ill formed from the temperature range of $150^{\circ}-290^{\circ}C$. Therefore, Classes I and IT A deposits have been formed at higher temperature condition and/or deeper positions than Classes IIB and III.

An Analysis of Nerve Block for Cancer Pain Patients (암성통증환자(癌性痛症患者)의 신경차단요법(神經遮斷療法)의 분석(分析))

  • We have treated patients who have visited pain clinic and those admitted to the other departments of Pusan National University Hospital from Mar. 1987 to Feb. 1988 with complaints of severe cancer pain on both upper and lower abdomen and extremities by continuous administration of a very small amount of morphine in the lumbar subarachnoid(group I) and epidural(group II) space in 10 cases respectively. The results of analgesia obtained are as follows: 1. The average duration in onset of analgesia was 8 mins. in group I and 23 mins. in group II. 2. The average duration In maintaining analgesia was 12.4 hrs. in group I and 18.4 hrs. in group II. 3. The efficiency of analgesia in group I was excellent in 5, good in 4, and null in 1, and group II was excellent in 5, good in 2, and moderate in 3. 4. The degree of tolerance in group I was rather mild comparable ti that of pain score 4 till the 22nd. day of morphine administration. 5. The complications are: 2 of respiratory distress in group I, 2 of voiding difficulties in both group I and II, 1 of itching sensation in both group I and II, 1 of euphoria with hallucination In group I, and 1 of C.S.F leakage in group II. As results, it is thought that epidural administration is safer than subarachnoid administration in achieving analgesia with morphine among patients with malignant pain if the problem of tolerance is solved.

Enzyme Replacement Therapy in Patients Who Have Mucopolysaccharidosis and Are younger than 5 years old (5세 미만 뮤코다당체침착증 환자에서의 효소 대체 요법)

  • Enzyme replacement of therapy (ERT) is one of the most promising therapeutic strategies for the treatment of lysosomal storage disorders. ERT is available in three types of Mucopolysaccharidosis (MPS): for MPS I (Aludrazyme$^{(R)}$), MPS II (Elaprase$^{(R)}$) and MPS VI (Naglazyme$^{(R)}$) patients who are over 5 years old. But recently, early diagnosis can be done by expert clinicians and even in prenatal case. We describe the case of ERT under 5 years old MPS patients. Up to June, 2010 in Samsung Medical Center, there are 6patients who were diagnosed as MPS and started ERT under 5 years old. 3 patients were MPS I, 3 patients were MPS II. 2 patient who was diagnosed as MPS I was female and others were male. Their age at diagnosis were 4 to 37month-old (4, 13, 16, 25, 27, 37 month-old) and they are now 9 to 60 month-old (9, 39, 32, 81, 60 month-old). The youngest patient was started ERT at 4 month-old and others were started at their 13 to 49 month-old (13, 29, 27, 28, 49 month-old). First manifested symptoms of patients were macrocephaly, kyphosis and coarse face appearance. Especially, in 2 of them, one was MPS I and the other was MPS II had elder brother with same disease. And the youngest one was diagnosed by the iduronate-2-sulfatase (IDS) gene analysis from chorionic villi sampling. His mother knew that she was a heterozygous carrier of IDS gene mutation because her younger brother died from MPS II. All of them confirmed as MPS by the enzyme assay in leukocytes and fibroblast skin culture. We started ERT with ${\alpha}$-L-iduronidase(Aldurazyme$^{(R)}$) to MPS I and did recombinant human iduronate-2-sulfatase (Elaprase$^{(R)}$) to MPS II patients as recommended dose as over 5 years old. But for MPS II patient who was 4 month old, we started ERT by recombinant human IDS (Elaprase$^{(R)}$) with reduced dose 0.1 mg/kg and increased dose every 2 weeks by 0.1mg/kg up to 0.5mg/kg IV infusion. During ERT, all patients had no adverse effects and the excretion of GAGs were decreased. We have evaluated other clinical symptoms such as liver/ spleen volume, heart function and neurologic evaluation. We describe a successful ERT to MPS I and MPS II patient under 5 years old without any adverse event. It indicates that ERT in young children are well tolerated and that it has several effects which may confer clinical benefits with long-term therapy.

Cd 수용액 처리 조건에 따른 $Cu(In,Ga)Se_2$ 태양전지의 성능변화

  • 최근 $Cu(In,Ga)Se_2$(CIGS)와 같은 박막 태양전지에 대한 연구가 많은 관심을 끌고 있다. CIGS 태양전지의 광투과층으로 사용되고 있는 II-VI족 화합물 반도체인 CdS는 상온에서의 에너지 밴드 갭(band gap)이 2.42eV 정도로서, 가시광영역의 많은 빛을 투과시키고, 적절한 제작 조건하에서 비교적 낮은 비저항을 나타내기 때문에 널리 사용되고 있다. 하지만 CIGS 태양전지 연구는 주로 CIGS 흡수층 제조공정에 편중되어 있으며, CdS 버퍼층 공정조건에 대한 체계적인 연구가 부족하다고 판단된다. 습식공정인 Chemical Bath Deposition (CBD)에 의해 주로 제조되는 CdS는 단순한 제조공정에도 불구하고 CIGS 태양전지의 성능에 지대한 영향을 미치는 것으로 알려져 있다. 특히, CdS합성반응이 개시되기 전까지의 용액잔류시간 (dip time)은 CIGS내로의 Cd이온 농도를 결정하는 중요한 공정변수로 판단된다. CIGS 표면에 Cd이 도핑될 경우, CIGS는 n형 전도성을 갖는 얇은 층을 갖게 되어 전체적으로 n-CIGS/p-CIGS의 동종 접합을 형성하는 장점을 부여할 것으로 기대된다. 따라서 본 논문에서는 dip time을 주요변수로 하여 CIGS 태양전지의 성능에 미치는 영향을 주로 고찰하였다. Cd의 확산 정도는 secondary ion mass spectroscopy (SIMS)를 이용하여 정량화하였으며, 제조된 CIGS 태양전지의 전류-전압 특성과 상관성을 제시하고자 한다.
