Accuracy of Freehand versus Navigated Thoracolumbar Pedicle Screw Placement in Patients with Metastatic Tumors of the Spine

  • Objective : To compare the accuracy and breach rates of freehand (FH) versus navigated (NV) pedicle screws in the thoracic and lumbar spine in patients with metastatic spinal tumors. Methods : A retrospective review of adult patients who underwent pedicle screw fixation in the thoracic or lumbar spine for metastatic spinal tumors between 2012 and 2018 was conducted. Breaches were assessed based on the Gertzbein and Robbins classification and only screws placed >4 mm outside of the pedicle wall (lateral or medial) were considered breached. Results : A total of 62 patients received 547 pedicle screws (average 8 per patient) - 34 patients received 298 pedicle screws in the FH group and 28 patients received 249 screws in the NV group. There were 40/547 breaches, corresponding to a breach and accuracy rate of 7.3% and 92.7%, respectively. The breach rate was 9.7% in the FH group and 4.4% in the NV group (chi-squared test, p=0.017); this corresponded to an accuracy rate of 90.3% and 95.6%, respectively. Only one patient from the overall cohort (in the FH group) required revision surgery due to a medial breach abutting the spinal cord (1.6% of all patients; 2.9% of FH patients); no patient suffered organ, vessel, or neurological injury from screw breaches. Conclusion : Navigated pedicle screw placement in patients with metastatic spinal tumors has a significantly higher radiographic accuracy compared to the FH technique. However, the revision surgery was low and no patient suffered from clinically-relevant breach. Navigation also offers the advantage of real-time localization of spinal tumors and aids in targeting and resection of these lesions.

덮개 나사 조기 노출이 임플란트의 생존율에 미치는 영향 (Effect of the early exposure of cover screws on the survival rate of implants)

  • The early exposure of cover screws is a common complication of 2-stage implant technique. The exposure of cover screws between stage I and IT surgery may cause inflammation in the soft tissues surrounding the implants, and lead to peri-implantitis or marginal bone loss. The purpose of this study was to evaluate the effect of the early exposure of cover screws on implants placed using 2-stage technique. Two hundred and nineteen implants in 77 patients were examined for cumulative survival rate, radiographic marginal bone level change, cause and frequency of the early exposure. The results were as follows: 1. Twenty-five implants showed early exposure of cover screws with a frequency of 11.4%. 2. Cumulative survival rate of the implants with early cover screw exposure was 88.0%, and that of the implants without cover screw exposure was 96,9%. 3. At the time of stage IT surgery and 1 year after loading, the marginal bone loss was greater around the implants with early exposure of cover screws than around the implants without cover screw exposure(p <0.05), 4, There was no statistically significant difference in the frequency of the early exposure according to the implant diameter, gender, and smoking(p >0,05).

Determining the Wood (Parashorea spp.) Decaying and Metal Corroding Abilities of Eight Fungi

  • Fungi are wood-decaying organisms, and this is an important trait that should be considered in wood utilization. When fungi attack wood, it decreases the quality of the wood. The use of metal screws has become an important part of woodworking. The ability of fungi to decay wood and damage metal screws that are embedded into wood is varied. In this study, eight fungal species were evaluated with respect to their ability to decay Parashorea smythiesii and P. tomentella wood. In addition, the effect of fungi on corroding metal screws was determined using the Kolle flask method. The evaluation showed that the fungal species Schizophyllum commune, Pycnoporus sanguineus, and Polyporus arcularius were highly capable of decaying Parashorea spp. woods. The greatest wood weight loss occurred with the heartwood of P. tomentella exposed to S. commune. Based on the classification of wood resistance against fungal attack, the two Parashorea spp. were classified as moderately resistant woods (class III). Schizophyllum commune was classified as highly capable of decaying wood that was embedded with metal screws and was highly capable of corroding metal screws placed in fungi-culture media. The greatest weight of rust powder formed because of screw corrosion was obtained from screw-embedded wood exposed to S. commune. Additionally, the most severe corrosion of metal screws that were embedded into woods was caused by the activities of P. arcularius. Moreover, the average weight loss of screw-embedded wood was greater than that of unscrewed wood.

척추 수술에 사용되는 나사못의 반복 삽입과 인출이 인장항력에 미치는 영향 : 생체 역학적 연구 (The Effect of Repetitive Insertion and Pullout of Spinal Screws on Pullout Resistance : A Biomechanical Study)

  • 목적 : 최근 척추 수술에 나사못을 사용하는 빈도와 범위가 넓어 지고 있는데 때로 수술 중 한번 삽입하였던 나사못을 다시 사용하는 경우가 있다. 인체 골과 타이타늄의 탄성계수가 크게 차이 나지만 반복 삽입 과정에서 나사못의 이가 손상될 가능성이 있다. 저자들은 나사못의 반복 삽입이 나사못의 인출 저항에 미치는 영향을 조사하였다. 방법 : 각각 6개의 세가지 다른 종류 cortical lateral mass screw, cancellous lateral mass screw and cervical vertebral body screw의 나사못을 시험하였다. 나사못을 인체의 골과 비슷한 밀도의 인공합성골에 삽입하였으며 삽입 중 삽입력을 측정하였고 그 후 Instron(Model TT-D, Canton, MA)을 이용하여 2.4mm의 속도로 인출하여 인장항력을 digital oscilloscope에 기록하였다. 위의 과정을 3회 반복하여 기록한 뒤 나사못을 광학 현미경으로 확대하여 관찰하였다. 결과 : cortical lateral mass screws의 평균 인장항력(1회인장 시험 $185.66N{\pm}42.60$, 2회 인장시험 $167.10N{\pm}27.01$, 3회인장 시험 $162.52N{\pm}23.83$ : p=0.03)과 cervical vertebral body screws ($386.0N{\pm}24.1$, $360.2N{\pm}17.5$ and $330.9N{\pm}16.7$ : p=0.0024)은 반복하여 삽입, 인장 검사 할 때 마다 감소하였으나 cancellous lateral mass screws의 평균 인장항력($194.00N{\pm}36.47$, $219.24N{\pm}26.58$ and $199.49N{\pm}36.63$ : p=0.24)은 감소하지 않았다. 전자현미경 소견에서 나사이의 끝이 무디어지고 표면이 문드러진 것을 관찰할 수 있었다. 결론 : 일부 나사못을 반복하여 삽입한 후 나사못의 인장항력이 감소되었으므로 수술중 여러번 삽입하였던 나사못은 최종 구조물에 사용되지 않아야 한다.

Stability of the prosthetic screws of three types of craniofacial prostheses retention systems

  • Objectives: This study aimed to evaluate the stability of prosthetic screws from three types of craniofacial prostheses retention systems (bar-clip, ball/O-ring, and magnet) when submitted to mechanical cycling. Materials and Methods: Twelve models of acrylic resin were used with implants placed 20 mm from each other and separated into three groups: (1) bar-clip (Sistema INP, São Paulo, Brazil), (2) ball/O-ring (Sistema INP), and (3) magnet (Metalmag, São Paulo, Brazil), with four samples in each group. Each sample underwent a mechanical cycling removal and insertion test (f=0.5 Hz) to determine the torque and the detorque values of the retention screws. A servo-hydraulic MTS machine (810-Flextest 40; MTS Systems, Eden Prairie, MN, USA) was used to perform the cycling with 2.5 mm and a displacement of 10 mm/s. The screws of the retention systems received an initial torque of 30 Ncm and the torque values required for loosening the screw values were obtained in three cycles (1,080, 2,160, and 3,240). The screws were retorqued to 30 Ncm before each new cycle. Results: The sample was composed of 24 screws grouped as follows: bar-clip (n=8), ball/O-ring (n=8), and magnet (n=8). There were significant differences between the groups, with greater detorque values observed in the ball/O-ring group when compared to the bar-clip and magnet groups for the first cycle. However, the detorque value was greater in the bar-clip group for the second cycle. Conclusion: The results of this study indicate that all prosthetic screws will loosen slightly after an initial tightening torque, also the bar-clip retention system demonstrated greater loosening of the screws when compared with ball/O-ring and magnet retention systems.

코팅된 지대주 나사의 반복 착탈 후 풀림력과 표면변화에 대한 연구 (Detorque force and surface change of coated abutment screw after repeated closing and opening)

  • 연구목적: 최근 지대주 나사에 WC/C 또는 TiN 코팅이 연구되고 있으며, 이들을 금속표면에 코팅 시 마찰계수의 감소와 부식에 대한 저항, 물리적 취약함의 해소 등의 효과를 얻을 수 있으며. 코팅된 지대주 나사에서 마모와 적합도, 풀림력이 향상되었다는 결과들이 보고되고 있다. 본 연구에서는 지대주 나사에 적용된 코팅이 나사 풀림에 미치는 영향을 알아보아 임상적으로 문제가 되고 있는 지대주 나사의 풀림현상을 해소하는데 그 목적이 있다. 연구재료 및 방법: 코팅되지 않은 타이타늄 지대주 나사와 (그룹 A) WC/C (그룹 B)와 TiN (그룹 C)으로 코팅된 지대주 나사의 10회 반복 착탈시 풀림력 및 표면의 변화를 비교해본 결과 다음과 같은 결론을 얻었다. 결과: 1. 반복 착탈 전, 그룹 A는 일정한 방향성을 가진 다소 거친 표면이 관찰되었으 나 그룹 B와 그룹 C는 코팅된 입자들이 관찰되었으며 코팅층이 전체적으로 균일하고 매끈한 모양을 나타내었다. 2. 반복 착탈 전, 그룹 B에 비해 그룹 C의 표면에서 코팅 입자의 크기가 더 크고 두껍게 코팅 된 모양을 나타내었다. 3. 반복 착탈 후, 그룹 A와 그룹 B에서는 지대주 나사 표면의 마모와 변형이 관찰 되었으며 그룹 B에서는 코팅 입자의 탈락 현상을 볼 수 있었다. 4. 반복 착탈 전과 후의 지대주 나사 무게의 변화를 측정한 결과 그룹 A가 가장 큰 감소를 보였으며, 그룹 C가 그룹 B 보다 무게의 변화가 적었으나 두 그룹간 통계적으로 유의한 차이는 없었다. 5. 그룹 B와 그룹 C는 그룹 A에 비해 높은 평균 풀림력을 나타내었고 통계학적으로 유의할 만한 차이가 있었다. 6. 그룹 A에서 그룹B 와 그룹 C보다 평균 풀림력의 감소 경향이 더욱 현저 하게 나타났다. 결론: 결론적으로, WC/C 또는 TiN 코팅된 지대주 나사는 반복적인 사용 후에도 코팅되지 않은 타이타늄 지대주 나사에 비해 뚜렷한 표면 변화를 보이지 않았으며 마모에 대한 저항성이 우수하였고 높은 풀림력을 나타내었다. 따라서 지대주 나사에 WC/C 또는 TiN 코팅을 적용 시 나사 풀림의 문제점을 개선할 수 있을 것으로 사료된다.

구강 악안면 영역의 생체 흡수성 고정판 사용에 관한 임상 연구 (A CLINICAL STUDY OF BIODEGRADABLE PLATES AND SCREWS IN ORAL AND MAXILLOFACIAL SURGERY)

  • Metallic bone plates and screws have been commonly used in oral and maxillofacial surgery for internal fixation. However, there are several disadvantages such as atrophy of cortical bone inherent to excessive rigid fixation systems, growth disturbance in growing individual, allergy reaction, interference with radiographic imaging, palpability, thermal sensitibity and the need for subsequent removal. To overcome these disadvantages and avoid additional surgery of removal of plates and screws, there have been many studies of biodegradable plates and screws. But, It also has complication such as foreign body reactions. We have undertaken a clinical and retrospective study on 140 patients in Dept. of Oral and Maxillofacial Surgery, Inha University Hospital from February 2006 to March 2009. The purpose of this study is to report the clinical cases and review of the literatures with biodegradable plates and screws. And we concluded following results. 1. 6 cases(3.4%) of the 177 operation sites(140 patients) experienced complications. 1 case(0.6%) was a failure of initial fixation, 1 case(0.6%) was a postoperative infection, 4 cases(2.3%) were inflammations or foreign body reaction. 2. Postoperative infections, inflammations and foreign body reactions were completely recovered with incision and drainage, supporative care with antibiotic coverage and removal of biodegradable plates. 3. Biodegradable plates and screws provide acceptable rigidity and stability clinically. But, long-term observation is required for the tissue reactions around the biodegradable plates and screws because of long resorption periods of the biodegradable materials.

DLC 표면 처리에 따른 임플랜트 지대주 나사의 풀림 현상에 관한 연구 (THE STUDY ON THE REMOVAL TORQUE OF THE DIAMOND LIKE CARBON COATED TITANIUM ABUTMENT SCREWS)

  • Statement of problem : Implant screw loosening remains a problem in implant prosthodontics. Some abutment screws with treated surfaces were introduced to prevent screw loosening and to increase preload. DLC(Diamond Like Carbon) film has similar properties on hardness, wear resistance, chemical stability, biocompatibility as real diamond materials. Purpose : The purpose of this study was to investigate the effect of lubricant layer on abutment screw and to discriminate more effective method between soft lubricant and hard lubricant to prevent screw loosening. Material and method : In this study, $1{\mu}m$ thickness DLC was used as protective, lubricating layer of titanium screws and 3 times removal torque was measured on the abutment screws to investigate the difference in 10 coated and 10 non-coated abutment screws. Results : The results indicated that the implants with DLC coating group were not more resistant to the applied force in screw loosening. At 32Ncm, the 3 times removal torque in DLC group were $27.75{\pm}2.89,\;25.85{\pm}2.35$ and $26.2{\pm}2.57$. The removal torque in no-coated abutment screws were $27.85{\pm}4.23,\;27.35{\pm}2.81$ and $27.9{\pm}2.31$, respectively. Conclusion : The lubricant layer used in this study was Diamond Like Carbon(DLC) and it have a properties of hard and stable layer. The DLC coating layer was hard enough to prevent distortion of screws in the repeated unscrewing procedure in clinical situation. The reduced friction coefficient in hard DLC layer was not effective to prevent screw loosening.

Biomechanical Comparison of Inter-fragmentary Compression Pressures : Lag Screw versus Herbert Screw for Anterior Odontoid Screw Fixation

  • Objective : The purpose of the present study was to compare inter-fragmentary compression pressures after fixation of a simulated type II odontoid fracture with the headless compression Herbert screw and a half threaded cannulated lag screw. Methods : We compared inter-fragmentary compression pressures between 40- and 45-mm long 4.5-mm Herbert screws (n=8 and n=9, respectively) and 40- and 45-mm long 4.0-mm cannulated lag screws (n=7 and n=10, respectively) after insertion into rigid polyurethane foam test blocks (Sawbones, Vashon, WA, USA). A washer load cell was placed between the two segments of test blocks to measure the compression force. Because the total length of each foam block was 42 mm, the 40-mm screws were embedded in the cancellous foam, while the 45-mm screws penetrated the denser cortical foam at the bottom. This enabled us to compare inter-fragmentary compression pressures as they are affected by the penetration of the apical dens tip by the screws. Results : The mean compression pressures of the 40- and 45-mm long cannulated lag screws were $50.48{\pm}1.20N$ and $53.88{\pm}1.02N$, respectively, which was not statistically significant (p=0.0551). The mean compression pressures of the 40-mm long Herbert screw was $52.82{\pm}2.17N$, and was not statistically significant compared with the 40-mm long cannulated lag screw (p=0.3679). However, 45-mm Herbert screw had significantly higher mean compression pressure ($60.68{\pm}2.03N$) than both the 45-mm cannulated lag screw and the 40-mm Herbert screw (p=0.0049 and p=0.0246, respectively). Conclusion : Our results showed that inter-fragmentary compression pressures of the Herbert screw were significantly increased when the screw tip penetrated the opposite dens cortical foam. This can support the generally recommended surgical technique that, in order to facilitate maximal reduction of the fracture gap using anterior odontoid screws, it is essential to penetrate the apical dens tip with the screw.
