자율형 고등학교의 자기주도적 학습전형 운영 실태와 개선방안 (Improvement of Screening System of Self-initiated Learning in Self-governing High Schools)

  • The purpose of this study is to explore improving ways of screening of self-directed learning in self-governing high schools for expanding self-initiated learning screening system. To achieve the above aim, it was used survey method to 99 admission teachers of 12 self-governing high schools and analyzed responses with SPSS program. The research findings are as follows. To expand self-initiated learning screening system in self-governing high schools, it needs to obtain admission members of entrance committee, pooling of external screening members, quick decision of school admission members, increasing payment, training and understanding of the system and consideration of admission documents and study plan. Some improving ways of the expanding of self-initiated learning screening may be listed as follows: 1) development of screening model for self-governing schools 2) policy consideration for self-governing public high school 3) expanding autonomy of school 4) supporting budget 5) sharing of operation case and reinforcement of public relations.

Adaptive Recommendation System for Health Screening based on Machine Learning

  • As the demand for health screening increases, there is a need for efficient design of screening items. We build machine learning models for health screening and recommend screening items to provide personalized health care service. When offline, a synthetic data set is generated based on guidelines and clinical results from institutions, and a machine learning model for each screening item is generated. When online, the recommendation server provides a recommendation list of screening items in real time using the customer's health condition and machine learning models. As a result of the performance analysis, the accuracy of the learning model was close to 100%, and server response time was less than 1 second to serve 1,000 users simultaneously. This paper provides an adaptive and automatic recommendation in response to changes in the new screening environment.

한일간 암검진 수검율에 영향을 미치는 정책적 요인에 대한 융합연구 (Convergence Study on Factors that Influence Cancer Screening Rate in Korea and Japan)

  • 본 연구에서는 한국과 일본의 암검진 사업의 수검율에 영향을 주는 정책적 요인에 대한 융합연구를 통해 분석하였다. 그 결과는 첫째로 국민건강보험제도의 보험자가 단일보험자일 경우 통합적인 암검진 사업이 가능하기 때문에 검진율에 영향을 미칠 것으로 분석되었다. 둘째로 암과 같은 중증질병에 걸렸을 때 예상되는 의료비 부담이 높으면 경제적 인센티브가 작동하게 되어 검진율에 영향을 미칠 것으로 분석되었다. 일본의 경우에는 건강보험에서 보장하지 않는 비급여항목이 거의 없고 본인부담상한제 등이 잘 정비되어 있어 중증질환이라도 의료비부담이 낮기 때문에 암검진의 수검율이 개선되지 않는 측면이 있다. 일본은 건강보험제도의 보장성 강화가 암검진에 대한 경제적 인센티브를 약화시키는 정책적 요인으로 작용한 것으로 나타났다.

An Off-site Screening Process for the Public in Radiation Emergencies and Disasters

  • Background: A contamination screening process for the local population in radiation emergencies is discussed. Materials and Methods: We present an overview of the relevant Korean governmental regulations that underpin the development of an effective response system. Moreover, case studies of foreign countries responding to mass casualties are presented, and indicate that responses should be able to handle a large demand for contamination screening of the local public as well as screening of the immediate victims of the incident. Results and Discussion: We propose operating procedures for an off-site contamination screening post operated by the local government for members of the public who have not been directly harmed in the accident. In order to devise screening categories, sorting strategies assessing contamination and exposure are discussed, as well as a psychological response system. Conclusion: This study will lead to the effective operation of contamination screening clinics if an accident occurs. Furthermore, the role of contamination screening clinics in the overall context of the radiation emergency treatment system should be clearly established.

Information Engineering and Workflow Design in a Clinical Decision Support System for Colorectal Cancer Screening in Iran

  • Background: Colorectal cancer is a major cause of morbidity and mortality throughout the world. Colorectal cancer screening is an optimal way for reducing of morbidity and mortality and a clinical decision support system (CDSS) plays an important role in predicting success of screening processes. DSS is a computer-based information system that improves the delivery of preventive care services. The aim of this article was to detail engineering of information requirements and work flow design of CDSS for a colorectal cancer screening program. Materials and Methods: In the first stage a screening minimum data set was determined. Developed and developing countries were analyzed for identifying this data set. Then information deficiencies and gaps were determined by check list. The second stage was a qualitative survey with a semi-structured interview as the study tool. A total of 15 users and stakeholders' perspectives about workflow of CDSS were studied. Finally workflow of DSS of control program was designed by standard clinical practice guidelines and perspectives. Results: Screening minimum data set of national colorectal cancer screening program was defined in five sections, including colonoscopy data set, surgery, pathology, genetics and pedigree data set. Deficiencies and information gaps were analyzed. Then we designed a work process standard of screening. Finally workflow of DSS and entry stage were determined. Conclusions: A CDSS facilitates complex decision making for screening and has key roles in designing optimal interactions between colonoscopy, pathology and laboratory departments. Also workflow analysis is useful to identify data reconciliation strategies to address documentation gaps. Following recommendations of CDSS should improve quality of colorectal cancer screening.

국가별 건강보험체계에 따른 국가건강검진제도 비교 연구 (Comparative Study on the National Health Screening Programs according to the Health Insurance System)

  • Background: Korea's health screening program has been faced the need for changes as the population and diseases structure are changing. In addition to Korea, many countries operate state-led health check-up programs to improve the health level of the people, and the operating methods of the program appear in various forms according to each country's health insurance system. This study examines other state-led health screening programs and proposes a direction for the development of Korea's health screening program. Methods: The study was conducted using the literature review method, and the "country" was set as a unit for the case analysis. The operating method of the health screening programs and the financial resources were compared according to the health insurance system. Five countries were selected as Korea, the United States, the United Kingdom, Japan, and Taiwan. Results: The analyzed countries mainly operate the health screening program as a management method for chronic diseases, but there were differences in the operating method, financing, and targeted subjects and examination items. In most countries, a risk assessment was performed prior to the examination (screening), and the subjects who needed the examination were first selected, and a follow-up management service was provided in accordance with the risk each individual exposed. Conclusion: Rather than applying the same screening method to populations with different risk levels, a health screening program will be constructed in consideration of the individual's health level and exposure risk, and the healthcare delivery system will be reorganized so that screening and treatment services can be linked.

트리즈로 분석한 코로나19 대응 선별진료소의 진화 (The Evolution of Screening Center for COVID-19 Analyzed by TRIZ)

  • Korea's Corona 19(COVID-19) quarantine, referred to as 'K-Quarantine', is a globally recognized quarantine system that has achieved both conflicting goals: health and economy. The quarantine system represented by 3T(Test-Trace-Treat) is not a method of blocking an area, but a method of screening and treating infected and non-infected persons. The screening center, one of the key elements of this screening treatment system, has evolved to suit the timing and situation of COVID-19, and has succeeded in initial response by conducting large-scale tests quickly and safely. By analyzing the evolution of screening centers that produced such significant results from a problem-solving point of view, it proved its meaning as a practical success case of creative problem-solving. In addition, the usefulness of TRIZ (Russian abbreviation of Theory of Solving Inventive Problem), a creative problem-solving theory, was confirmed through an analysis of actual verified cases of COVID-19 response. TRIZ is a problem-solving theory created by analyzing the regularity of invention patents, and is widely used not only in the technical field but also in the non-technical fields such as design, management, and education. The results of this study are expected to provide useful meaning and practical examples to researchers interested in system analysis and TRIZ application from a problem-solving perspective.

내과계 중환자 섬망발생 선별모형 개발 (Development of a Delirium Occurrence Screening Model for Patients in Medical Intensive Care Units)

  • Purpose: The purpose of this study was to investigate risk factors related to delirium and to develop screening model on delirium occurrence in MICU (Medical Intensive Care Unit) patients. Methods: For developing a preliminary tool for delirium, the data of 166 patients were collected and analyzed. In order to estimate the accuracy and discriminating power for the developed screening model, 98 patients were enrolled. The data used in this study were collected by EMR (Electronic Medical Record) review from January to September in 2012. The collected data were analyzed using SPSS/PC Win 18.0 program. Results: Screening model on delirium in MICU patients was developed using the results of logistic regression. The total score of screening model was 24 point and measuring point was 10 point. When the measuring point is over 10 point, it means that the risk of delirium occurrence is high. The discriminating power and the validity of screening model showed AUC .908 (p <.001) and .935 (p <.001) respectively. This result showed that the screening model on delirium which developed in this study was an appropriate model for screening the delirium risk group in MICU. The sensitivity of the screening model was 83%, specificity 89% and accuracy 84%. Conclusion: The developed screening model on delirium occurrence in MICU should be combined with EMR for screening and preventing delirium in a high risk group.

  • Enoyl-Acyl Carrier Protein Reductase (Fabl) of bacteria is hem as an important target for new antibacterial drugs and plays a determinant role in completing cycles of elongation in type-H fatty acid synthase system. In this study, a fabI gene from Staphylococcus aureus 6538p cloned in pET-l4b vector and FabI protein was over-produced in Escherichaia coli BL2l (DE3). $NH_2$-terminal His-tagged FabI protein was purified by nickel-nitrilotriacetic acid (Ni-NTA) metalaffinity chromatography Purified 6xHis-tagged FabI showed a catalytic activity on tram - 2 - octenoyl - N -acethlcysteamine by utilizing NADPH as a cofactor. For the discovery of new FabI inhibitors from chemical libraries, a target-oriented screening system using a 96-well plate was developed. About 10,000 chemical libraries from Korea Chemical Bank wore tested in this screening system, and 26 chemicals (0.25%) among them showed an inhibitory activity against FabI enzyme. This result showed that a new screening system can be used for the discovery of new FabI inhibitors.

Yeungnam University type drive-through (YU-Thru) coronavirus disease 2019 (COVID-19) screening system: a rapid and safe screening system

  • Active and prompt scale-up screening tests are essential to efficiently control the coronavirus disease 2019 (COVID-19) outbreak. The goal of this work was to identify shortcomings in the conventional screening system (CSS) implemented in the beginning of the outbreak. To overcome these shortcomings, we then introduced a novel, independently developed system called the Yeungnam University type drive-through (YU-Thru), and distributed it nationwide in Korea. This system is similar to the drive-throughs utilized by fast food restaurants. YU-Thru system has shortened the time taken to test a single person to 2-4 minutes, by completely eliminating the time required to clean and ventilate the specimen collection room. This time requirement was a major drawback of the CSS. YU-Thru system also reduced the risk of subjects and medical staff infecting one another by using a separate and closed examination system. On average, 50 to 60 tests were conducted per day when using the CSS, while now up to 350 tests per day are conducted with the YU-Thru system. We believe that the YU-Thru system has made an important contribution to the rapid detection of COVID-19 in Daegu, South Korea. Here, we will describe the YU-Thru system in detail so that other countries experiencing COVID-19 outbreaks can take advantage of this system.
