Impacts of Household Income and Economic Recession on Participation in Colorectal Cancer Screening in Korea

  • To assess the impact of household income and economic recession on participation in CRC screening, we estimated annual participating proportions from 2007 to 2009 for different CRC screening modalities according to household income levels. A total of 8,042 subjects were derived from the fourth Korean National Health and Nutrition Examination Survey (KNHANES IV). Multivariate logistic regression analysis was used to estimate odds ratios and 95% confidence intervals for CRC screening with household income quartiles by gender in each year. People were less likely to attend a high-cost CRC screening such as a sigmoidoscopy or colonoscopy independent of the income quartile during the economic recession. Income disparities for participating in opportunistic cancer screening appear to have existed among both males and females during the three years (2007-2009), but were most distinctive in 2009. An increase in mortality of CRC can therefore be expected due to late detection in periods of economic crisis. Accordingly, the government should expand the coverage of CRC screening to prevent excess deaths by reducing related direct and indirect costs during the economic recession.

System for Nutrition Counseling and Screening

  • The studies on nutrition counseling and screening have been based on stand-alone program. This study introduces the nutrition counseling and screening management system. This system offers convenient user interface and the synthetic results of counseling and screening with various utilities, The system for nutrition counseling and screening consists of the general information of clients, the anthropometry data, the calculation of obesity and body mass index, the state of eating habits, the computation of calorie expenditure, the clinical symptoms, the convenient method for analysis of calorie and nutrients, nutrition prescription and alcoholism screening tests. Having interoperability, these functions preserve the information of clients and manage the historical data. The system inserts, stores and generates the synthetic information of clients, so it is able to provide suitable and efficient counseling to clients. The proposed system gathers various information of clients. With accumulated client data, it does the nutrition education, screening and counseling simultaneously. Managing clients' information connected to database, it can provide systematic and formal information. It is possible for the system to retrieve information and counsel in real time. It is expected that the nutrition counseling management system can improve the national health with animated nutrition counseling. (J Community Nutrition 7(4) : $220\∼229$, 2005)

장애인의 건강검진 수검에 영향을 미치는 요인 분석 (Determinants analysis of affecting to Health screening among Persons with Disabilities)

  • Purpose: The purpose of this study was to examine the factors affecting of health screening among persons with disabilities. Method: The study examines the factors affecting of health screening using over 20 years of age who participated in the 2017 National survey of the disabled data. Subjects were 6,332 person with disabilities over 20 years of age. Data were analyzed using descriptive statistics, X2-test, multiple logistic regression with the SPSS win 21.0 program. Results: Health screening among persons with disabilities is differentiated with education level, marital status, Subjective House economic status, Health insurance, Economic activity, Type of disability, Grade of disability, Stress recognition, Depressive symptom, Suicidal thinking, Suicidal attempt, and grade of disability. The significant predictors of the health screening were age, Subjective House economic status, Economic activity, and Grade of disability. Conclusion: We should consider age, disability level, and economic level when developing a screening program for people with disabilities. Especially, the development and promotion of the health screening program for persons with disabilities and related education are required.

A rapid screening method for selection and modification of ground motions for time history analysis

  • A three-step screening process is presented in this article for selection of consistent earthquake records in which number of suitable ground motions is quickly screened and reduced to a handful number. Records that remain at the end of this screening process considerably reduce the dispersion of structural responses. Then, an effective method is presented for spectral matching and modification of the selected records. Dispersion of structural responses is explored using different statistical measures for each scaling procedure. It is shown that the Uniform Design Method, presented in this study for scaling of earthquake records, results in most cases in the least dispersion measure.

  • Background: Most developmental screening tools in Korea are adopted from foreign tests. To ensure efficient screening of infants and children in Korea, a nationwide screening tool with high reliability and validity is needed. Purpose: This study aimed to independently develop, standardize, and validate the Korean Developmental Screening Test for Infants and Children (K-DST) for screening infants and children for neurodevelopmental disorders in Korea. Methods: The standardization and validation conducted in 2012-2014 of 3,284 subjects (4-71 months of age) resulted in the first edition of the K-DST. The restandardization and revalidation performed in 2015-2016 of 3.06 million attendees of the National Health Screening Program for Infants and Children resulted in the revised K-DST. We analyzed inter-item consistency and test-retest reliability for the reliability analysis. Regarding the validation of K-DST, we examined the construct validity, sensitivity and specificity, receiver operating characteristic curve analysis, and a criterion-related validity analysis. Results: We ultimately selected 8 questions in 6 developmental domains. For most age groups and each domain, internal consistency was 0.73-0.93 and test-retest reliability was 0.77-0.88. The revised K-DST had high discriminatory ability with a sensitivity of 0.833 and specificity of 0.979. The test supported construct validity by distinguishing between normal and neurodevelopmentally delayed groups. The language and cognition domain of the revised K-DST was highly correlated with the K-Bayley Scales of Infant Development-II's Mental Age Quotient (r=0.766, 0.739), while the gross and fine motor domains were highly correlated with Motor Age Quotient (r=0.695, 0.668), respectively. The Verbal Intelligence Quotient of Korean Wechsler Preschool and Primary Scales of Intelligence was highly correlated with the K-DST cognition and language domains (r=0.701, 0.770), as was the performance intelligence quotient with the fine motor domain (r=0.700). Conclusion: The K-DST is reliable and valid, suggesting its good potential as an effective screening tool for infants and children with neurodevelopmental disorders in Korea.

4세 말소리발달 선별검사 개발과 한국어말소리분석도구(Korean Speech Sound Analysis Tool, KSAT)의 활용 (Developing the speech screening test for 4-year-old children and application of Korean speech sound analysis tool (KSAT))

  • 본 연구는 4세 아동에 대한 말소리발달 평가를 위해 세 문장 따라말하기 선별검사를 개발하고 또래와 비교할 수 있는 규준을 제공하기 위한 것이다. 이를 위해 4세 전반과 후반 각각 24명씩 총 48명의 아동에게 선별검사를 실시하였다. 선별검사 결과는 기존의 말소리장애 평가 검사 결과와 .7의 상관을 보였다. 선별검사를 통해 구한 음운발달 지표와 오류패턴에서 4세 전반과 후반으로 나눈 두 집단에 차이가 있는지 비교하였다. 후반 아동의 발달지표가 높은 것으로 나왔지만 통계적으로 유의한 차이는 없었다. 모든 분석은 한국어말소리분석도구(Korean Speech Sound Analysis Tool, KSAT)를 사용하였으며, 자동분석 결과와 임상가의 수동분석 내용을 비교하였다. 자동분석과 수동분석의 오류패턴분석 일치도는 93.63%였다. 본 연구의 의의는 유도 문장수준에서 세 문장 따라말하기 선별검사의 4세 아동의 말소리 규준을 제시했다는 것과 KSAT의 임상과 연구 현장에서 적용 가능성을 검토하였다는 것이다.

  • 한국산 녹차로부터 HPLC On-line $ABTS^{+}$ screening기법을 사용하여 catechin compounds의 특성중의 하나인 항산화활성을 빠르게 분석하였으며 녹차로부터 catechin compound의 추출을 다양한 온도와 시간의 추출방법을 적용하였다. 전 처리한 추출액에 포함된 catechin compounds을 분석하고 최적의 추출조건을 실험적으로 모색하였다. 실험결과에 의하면 추출온도 $60^{\circ}C$, 추출시간 3분으로 추출한 시료가 항산화활성이 가장 우수하였다.

Cost-effectiveness Outcomes of the National Gastric Cancer Screening Program in South Korea

  • Background: Although screening is necessary where gastric cancer is particularly common in Asia, the performance outcomes of mass screening programs have remained unclear. This study was conducted to evaluate cost-effectiveness outcomes of the national cancer screening program (NCSP) for gastric cancer in South Korea. Materials and Methods: People aged 40 years or over during 2002-2003 (baseline) were the target population. Screening recipients and patients diagnosed with gastric cancers were identified using the NCSP and Korea Central Cancer Registry databases. Clinical outcomes were measured in terms of mortality and life-years saved (LYS) of gastric cancer patients during 7 years based on merged data from the Korean National Health Insurance Corporation and National Statistical Office. We considered direct, indirect, and productivity-loss costs associated with screening attendance. Incremental cost-effectiveness ratio (ICER) estimates were produced according to screening method, sex, and age group compared to non-screening. Results: The age-adjusted ICER for survival was 260,201,000-371,011,000 Korean Won (KW; 1USD=1,088 KW) for the upper-gastrointestinal (UGI) tract over non-screening. Endoscopy ICERs were lower (119,099,000-178,700,000 KW/survival) than UGI. To increase 1 life-year, additional costs of approximately 14,466,000-15,014,000 KW and 8,817,000-9,755,000 KW were required for UGI and endoscopy, respectively. Endoscopy was the most cost-effective strategy for males and females. With regard to sensitivity analyses varying based on the upper age limit, endoscopy NCSP was dominant for both males and females. For males, an upper limit of age 75 or 80 years could be considered. ICER estimates for LYS indicate that the gastric cancer screening program in Korea is cost-effective. Conclusion: Endoscopy should be recommended as a first-line method in Korea because it is beneficial among the Korean population.

성인 여성의 자궁경부암 선별검사 수검에 관한 예측인자 (The Predictive Factors to Participation in Cervical Cancer Screening Program)

  • Objectives : To examine the screening rate of cervical cancer in women and to find out the predictive factors for participation in cervical cancer screening programs within their life-time and within the last two years. Methods : The data was based on self-reported questionnaires from 1,613 women whose ages ranged from 26 to 60 years; this survey was peformed between December 1999 and January 2000. This study analyzed the predictive factors for participation in cervical cancer screening programs within their life-time and within the last two years. A logistic regression analysis was performed in order to derive the significant variables from the predisposing factors(demographic factor, health promotion behavior, reproductive factor), intervention factors(information channel, relation with medical stan, and proximal factors(attitude, social influence, self-efficacy). All analyses were peformed by the PC-SAS 6.12. Results : Our analyses showed that the screening rate for the women who received a cervical cancer screening(Pap smear) more than once within their life-time was 56.1% while those who had received one within the last two years was 34.5%. The significant factors for participation in cervical cancer screening program within their life-time were their income, married age, health promotion score, relation with medical staffs, social influence, and self-efficacy. On the other hand, age, number of pregnancies, menarche age, relation with medical staffs, social influences, and self-efficacy were significant factors for those being screened within the last two years. The predictive power of the logit model within their life-time was 68.8% and that within the last two years was 66.6%. Conclusion : The predictive factors for participation in cervical cancer screening program within their life-time are different from those for within the last two years. and that women's relations with medical staffs and social influences were the critical factors impacting on cervical cancer screening rates.

Information Engineering and Workflow Design in a Clinical Decision Support System for Colorectal Cancer Screening in Iran

  • Background: Colorectal cancer is a major cause of morbidity and mortality throughout the world. Colorectal cancer screening is an optimal way for reducing of morbidity and mortality and a clinical decision support system (CDSS) plays an important role in predicting success of screening processes. DSS is a computer-based information system that improves the delivery of preventive care services. The aim of this article was to detail engineering of information requirements and work flow design of CDSS for a colorectal cancer screening program. Materials and Methods: In the first stage a screening minimum data set was determined. Developed and developing countries were analyzed for identifying this data set. Then information deficiencies and gaps were determined by check list. The second stage was a qualitative survey with a semi-structured interview as the study tool. A total of 15 users and stakeholders' perspectives about workflow of CDSS were studied. Finally workflow of DSS of control program was designed by standard clinical practice guidelines and perspectives. Results: Screening minimum data set of national colorectal cancer screening program was defined in five sections, including colonoscopy data set, surgery, pathology, genetics and pedigree data set. Deficiencies and information gaps were analyzed. Then we designed a work process standard of screening. Finally workflow of DSS and entry stage were determined. Conclusions: A CDSS facilitates complex decision making for screening and has key roles in designing optimal interactions between colonoscopy, pathology and laboratory departments. Also workflow analysis is useful to identify data reconciliation strategies to address documentation gaps. Following recommendations of CDSS should improve quality of colorectal cancer screening.
