  • Optimal screening procedures with dichotomous performance variable T and continuous screening variable X are presented for assuring with a specified degree of confidence that at least ${\ell}$ out of m items found acceptable in screening inspection are conforming. It is assumed that T is a Bernoulli random variable and that the conditional distribution of X given T=t is normal. When m is also to be determined, optimal m and cut-off value of X minimizing the total expected cost are obtained. Cases of known and unknown parameters are considered and for unknown parameter cases, Bayesian approaches are used to find the optimal screening procedures.

  • In this paper, economic screening procedures with dichotomous performance variable T and continuous screening variable X are considered when the rejected items are reprocessed. Two models are considered; normal and logistic models. It is assumed that X given T is normally distributed in the normal model, and P(T=1|X=x) is given by a logistic function in the logistic model. Profit models are constructed which involve four price/cost components; selling price, cost from an accepted nonconforming item, and reprocessing and inspection costs. Methods of finding the optimal screening procedures are presented and numerical examples are given.

  • 본 논문에서는 부유식 방파제와 잠제의 단점을 보완하고 장점을 활용하기 위하여 부유식방파제와 잠제를 혼합한 혼합형 방파제에 의한 파랑 차단 성능을 2차원 유한요소법에 의해 검토하고 있다. 혼합형 방파제의 경우 부유식 방파제 또는 잠제를 단독으로 사용하였을 때 보다 우수한 파랑 차단 성능을 보였고, 단파 및 비교적 장파에서도 우수한 파랑 차단 성능을 보여 파랑의 일부 주기에 국한되지 않고 넓은 주기 영역에서의 파랑 차단 성능이 우수함이 나타났다.

  • This study was designed to identify the performance of occupational health services of 37 industries located in Kyung in area. The data was collected by a structured questionnaire developed by the Academic society of Community Nursing. This analysis had two factors, one was related to six of the industries, and the other was the actual assignment procedures of the health team members. This study was undertaken from December 4, 1992 to January 21, 1993. The results of the study were as follows: 1. The study group was primarily manufacturing industries which employed 300-1,000 employees. 2. The actual assignment state of occupation health team members with the exception of nurses was not kept to the regulations of the industrial safety health law. 3. The following was the analysis of the performance of occupational health services with two factors: 1). The larger the size of the six industries, the more the performance of health education, health assessment, and health screening. 2) The actual. practice of occupational health team members working environmental measurement, was more frequently performed by a nurse other than health team members together. 4. 1) The subscription rate of the laborers for primary health screening was 94%, and of these 10% had the need of secondary health screening. As a results of the secondary health screening the degrees were 'A' 45%, 'C' 92%, 'R' 21%. Of these degrees 'C', 'R' 4% were follow up cases. 2) 43% of laborers needed special health screening and .of these the subscription rate was 99%. The main item of the special health screening was physical factors. After the special health screening 46% required .follow up, 30% required medical treatment, only reporting 18%, change work 8%, suspension from work place 2%.

Organization and Evaluation of Performance Indicators of a Breast Cancer Screening Program in Meknes-Tafilalt Region, Morocco

  • Objective: The benefits of screening and early detection of breast cancer, including reduced morbidity and mortality, have been well-reported in the literature. In 2011, a breast cancer screening program was launched in Meknes-Tafilalt region of Morocco. The aim of this study was to evaluate the early performance indicators of this program. Materials and Methods: This retrospective evaluative study was conducted between April 2012 and December 2014, in Meknes-Tafilalt region of Morocco. Several performance indicators of the breast cancer screening program were calculated: the compliance rate, the positivity rate, the referral rate, the cancer detection rate and the organizational indicators. Results: During 2012-2014, a total of 184,951 women participated in the breast cancer screening program. The compliance rate was 26%, the positive rate was 3.3%, the referral rate was 36.7%, and the cancer detection rate was 1.2 per 1,000 women. The median time between the date of clinical breast examination and the date of biopsy (or cyto-puncture) was 36 days. The median time between the date of positive mammography and the date of biopsy (or cyto-puncture) was 6 days. The median time between the date of clinical breast examination and the date of the first received treatment was 61 days. Conclusions: The program needs better monitoring, as well as implementation of quality assurance tools to improve performance in our country.

병원간호사의 정기적인 유방암 검진행위 영향 요인 (Factors Affecting Periodic Screening Behaviors for Breast Cancer among Hospital Nurses)

  • Purpose: The purpose of this study was to analyze periodic screening behaviors for breast cancer and factors affecting the screening behaviors among hospital nurses. Methods: A total of 461 nurses were recruited from 15 hospitals located in two southern areas of Korea. The Champion's Health Beliefs Model Scale-Korean version and a structured study questionnaire were used for data collection. Data were collected during July and August 2008. Results: Nurses who performed periodic mammography accounted for 15.6% of the total, while 22.8% performed periodic breast self-examination (BSE). Among married nurses, the rates of periodic mammography and BSE were significantly different by age, menopausal status, delivery experience, family history of breast cancer, and experiences of cervical cancer screening. Significant factors affecting periodic mammography were family history of breast cancer, experiences of cervical cancer screening, age above 40, and performance of periodic BSE. For health beliefs, levels of severity, confidentiality and barrier were significant factors for periodic BSE. Conclusion: Nurses, who are role models for health promoting behaviors, did not adequately performing periodic BSE and mammography. A health promotion program for breast cancer designed for hospital nurses is needed to improve performance rates for periodic screening behaviors for breast cancer.

  • For situations where there are several markets with different profit/cost structures, an economic two-sided screening procedure using a correlated variable is developed. It is assumed that the performance variable and the screening variable are jointly normally distributed. A profit model is constructed which involves selling price, cost incurred by imperfect quality, and screening inspection cost. Methods of finding the optimal screening procedure are presented and numerical examples are given.

  • Optimum screening procedures using prior information are presented. An optimal cutoff value on the screening variable X minimizing the expected total cost is obtained for the normal model; it is assumed that a continuous screening variable X given a dichotomous performance variable T is normally distributed and that costs are incurred by screening inspection and misclassification errors. Methods for finding optimal cutoff values based on the prior distributions for unknown parameters are presented.

Adaptive Recommendation System for Health Screening based on Machine Learning

  • As the demand for health screening increases, there is a need for efficient design of screening items. We build machine learning models for health screening and recommend screening items to provide personalized health care service. When offline, a synthetic data set is generated based on guidelines and clinical results from institutions, and a machine learning model for each screening item is generated. When online, the recommendation server provides a recommendation list of screening items in real time using the customer's health condition and machine learning models. As a result of the performance analysis, the accuracy of the learning model was close to 100%, and server response time was less than 1 second to serve 1,000 users simultaneously. This paper provides an adaptive and automatic recommendation in response to changes in the new screening environment.

Breast Cancer Screening in Morocco: Performance Indicators During Two Years of an Organized Programme

  • Background: Breast cancer is commonly diagnosed at late stages in countries with limited resources. In Morocco, breast cancer is ranked the first female cancer (36.1%) and screening methods could reduce the proportion presenting with a late diagnosis. Morocco is currently adopting a breast cancer screening program based on clinical examination at primary health facilities, diagnosis at secondary level and treatment at tertiary level. So far, there is no systematic information on the performance of the screening program for breast cancer in Morocco. The aim of this study was to analyze early performance indicators. Materials and Methods: A retrospective evaluative study conducted in Temara city. The target population was the entire female population aged between 45-70 years. The study was based on process and performance indicators collected at the individual level from the various health structures in Tamara between 2009 and 2011. Results: A total of 2,350 women participated in the screening program; the participation rate was 35.7%. Of these, 76.8% (1,806) were married and 5.2% (106) of this group had a family history of breast cancer. Of the women who attended screening, 9.3% (190) were found to have an abnormal physical examination findings. A total of 260 (12.7%) were referred for a specialist consultation. The positive predictive value of clinical breast examination versus mammography was 23.0%. Forty four (35.5%) of the lesions found on the mammograms were classified as BI-RADs 3; 4 or 5 category. Cancer was found in 4 (1.95%) of the total number of screened women and benign cases represented 0.58%. Conclusions: These first results of the programme are very encouraging, but there is a need to closely monitor performance and to improve programme procedures with the aim of increasing both the participation rate and the proportion of women eligible to attend screening.
