가상현실 기반 운동이 측만 각도 및 근활성도에 미치는 영향 (Effect of Virtual Reality-Based Exercise on Scoliometer and Muscle Activity)

  • The aim of this study is to determine the effect of virtual reality(VR) exercise using XBOX $Kinect^{TM}$ on the scoliometer angle and muscle activities of the trunk. The subjects of the study were 13 young adults who devided into a virtual reality-based exercise group(VREG, n = 7) and a traditional stabilization exercise group(TSEG, n = 6). The VREG received virtual reality game for 5 days a weeks, 30 minutes a day, for a 2 weeks and the TSEG received general trunk stabilization exercise for the same period and frequency. The subjects were measured and compared for muscle activities of trunk stabilization before and after the program. VREG tend to decrease more than TSEG in scoliometer angle. In addition, VREG showed a significant decrease in muscle activities of multifidus. This study presents the impact of virtual reality-based exercise program on trunk balance and stabilization. The following studies need customized programs for subjects that trunk balance and stabilization is required.

Reliability and Validity of Angle of Trunk Rotation Measurement Using Smartphone and 3D Printing Technology in Scoliosis

  • Purpose: The purpose of this study was to compare and analyze the method of measuring the angle of the trunk rotation using a smartphone with 3D smartphone holder compared to a scoliometer, which is a measuring tool used as a method for diagnosing scoliosis in scoliosis patients. Methods: Angle of trunk rotation was measured in 21 subjects diagnosed with scoliosis. scoliometer measurement method, a smartphone measurement method with a 3D smartphone holder, a smartphone blind measurement method with a 3D smartphone holder, a smartphone measurement method without a smartphone holder, a smartphone blind measurement method without a smartphone holder, and a total of five measurement methods were repeated three times for comparison and analysis. Results: The smartphone measurement method with a 3D smartphone holder has excellent intra-rater reliability of angle of trunk rotation (Rater A; ICC3, 2≥0.993, Rater B; ICC3, 2≥0.992). The smartphone blind measurement method with a 3D smartphone holder has excellent inter-rater reliability of angle of trunk rotation (ICC2, 2≥0.968). The scoliometer measurement method had the highest validity (r=0.976) with the smartphone measurement method with a 3D smartphone holder, and the blind measurement method without a smartphone holder had the lowest validity (r=0.886). Conclusion: These findings, the angle of trunk rotation measured by the smartphone measurement method with a 3D smartphone holder in scoliosis patients showed high reliability and validity compared to the scoliometer measurement method.

Changes in Gait Parameter with Adolescent Idiopatic Scoliosis

  • Purpose: The purpose of this study will demonstrate that relationship between scoliosis and gait factor and foot weight bearing in ambulation. Methods: Subjects were 40 elementary students. A normal control group consisted of a total of 20 children without any known musculoskeletal disorders and an AIS group of 20 children with mild AIS (defined by a Cobb angle between 10 and $25^{\circ}$) were recruited. Measurements were scoliometer screening test, Cobb angle, gait parameter (rate of swing/stance phase, gait velosity), foot weight bearing (entire, fore, hind). Results: Scoliometer screening test (P = 0.000) and X-ray Cobb angle (P = 0.000) significant difference of group which was significantly higher in the AIS group. Gait parameter not showed significant difference. Forefoot weight bearing was significantly higher in the AIS group than more normal group. Conclusion: It seems that the results of weight bearing analysis in ambulation may be used in modifying rehabilitation programs for individual needs of patients with idiopathic scoliosis.

A Study on Asymmetric Lifting Capacity Due to Spine Deformity

  • Scoliosis can be biomechanically described as a three dimensional deformity of the spine, with deviations from the physiologic curves in the sagittal and frontal planes, usually combined with intervertebral rotation. Various factors are suspected such as genetic defects, uneven growth of the vertebrae, hormonal effects, abnormal muscular activity, postural problems, or a mix of some of these elements, but its initial cause is known in only 15-20% cases. The screening test for diagnosing scoliosis is called the Adams Forward Bend Test. During the experiment, the subjects were asked to bend over, with arms dangling, until a curve could be observed. The Scoliometer was placed on the back of the subjects and used to measure the difference between the left and right apex of the curve in the thoracic, thoracolumbar and lumbar area. Then, the subjects were asked to perform Maximum Voluntary Contractions (MVCs) using the digital back muscle dynamometer in three different postures: (1) 0o (sagittally symmetric); (2) 30o from the mid-sagittal plane (clockwise); and (3) 30o from the mid-sagittal plane (counterclockwise). In addition to the experimental data, subject-dependent variables including Body Mass Index (BMI), percentage of body fat and muscle mass of left/right arms and legs were employed to reveal the cause of difference among three MVC conditions. All those variables were tested using statistical methods.

8주간의 슈로스운동(3차원적 융합운동)이 특발성 척추 측만증 환자의 폐기능과 Cobb's angle, Scoliometer angle, 척추세움근의 근활성도에 미치는 영향 (Effect of 8 Weeks of Schroth Exercise (Three-dimensional Convergence Exercise) on Pulmonary Function, Cobb's Angle, and Erector Spinae Muscle Activity in Idiopathic Scoliosis)

  • 본 연구는 schroth 운동(3차원적 융합운동)이 폐기능과 Cobb's angle, scoliometer angle과 척추세움근의 근활성도에 미치는 영향을 알아보기 위해 실시되었다. Cobb's angle $10^{\circ}$ 이상의 특발성 척추측만증을 가지고 있는 학생 40명을 대상으로 각각 20명의 슈로스운동군과 전통적인 치료군으로 나누어 총 8주간 주 3회, 회당 60분의 운동을 실시하였다. 8주간의 Schroth 운동을 적용한 그룹에서는 Cobb's angle과 갈비뼈 돌출부, 폐활량, 체간의 확장, 오른쪽 흉추부위의 활성도에 유의한 차이를 보였다. 8주간의 전통적인 측만증 운동을 실시한 그룹에서는 체간의 확장성에서 유의한 차이를 보였다. 본 연구의 결과를 통해 3차원적 접근법인 슈로스운동이 특발성 척추측만증 환자의 흉추측만각도의 감소와 폐기능의 향상을 목적으로 한 중재기법으로 기존의 운동방법보다 효과적이라고 할 수 있다. 따라서 차후의 연구에서는 척추측만증이 장기간의 관리와 치료가 필요한 질환임을 이해하고 이에 따라 체계적인 자세관리와 3차원적 융합운동 프로그램의 적용이 필요하다고 사료된다.

대학생의 척추측만각 및 체질량지수의 관계 (Relationship between Spine Angle and Body Mass Index in Undergraduate Students)

  • 본 연구는 초기 성인에 해당하는 대학생의 척추측만 실태를 조사하고 선행연구에서 관련된 변수로 보고된 체질량 지수와의 관계를 알아보고자 시행되었다. 연구설계는 서술적 조사연구이며, 연구대상은 서울시, 대전시, 대구시, 경기도 소재의 대학교에 재학 중인 학생 158명이었다. 연구도구는 일반적 특성, 자세특성, 체질량지수 문항이 포함된 구조화된 설문지를 사용하였고, 척추측만각은 scoliometer를 사용하여 체간회전각을 직접 측정하였다. 체간회전각 $6^{\circ}$ 이상이면 척추측만이 있는 것으로 분류하였다. 자료수집기간은 2014년 11월 7일부터 3주간 시행하였고 자료분석은 척추측만각 및 체질량지수는 빈도와 백분율, 두 변수의 관계를 확인하기 위해 ${\chi}^2$-test, Logistic 회귀분석을 이용하였다. 대상자의 체간회전각 $5^{\circ}$ 이하는 114명 (72.2%), $6^{\circ}$ 이상은 44명(27.8%)이었다. 교란변수를 제외하였을 때 체간회전각이 $6^{\circ}$ 이상이 될 확률은 체질량지수 정상군을 기준으로 과체중 군이 2.63배 높았으며 통계적으로 유의하였다. 그리고 체질량지수 정상군에 비해 저체중군은 0.24배 낮았지만 통계적으로 유의하지 않았다. 우리나라에서 척추측만증의 조기검진 및 관리는 주로 아동 청소년을 대상으로 이루어지고 있다. 본 연구결과에서 대학생의 척추측만증 유병률이 높은 수준이었으므로 이들을 위한 적극적인 조기검진 및 관리가 필요할 것이다.

Short Segment Anterior Correction of Adolescent Idiopathic Scoliosis

  • Objective : The purpose of this study was to introduce our surgical experiences of scoliosis and to evaluate the effectiveness of anterior correction and fusion in adolescent idiopathic scoliosis (AIS). Methods : Between August 2004 and August 2007, four patients with AIS were treated with anterior segmental fusion and fixation at our hospital. Mean follow-up period was 9 (6-12) months. The average age was 14.0 (13-15) years. According to Lenke classification, three patients showed Lenke 1 curve and one patient with Lenke 5 curve. Single rod instrumentation was performed in one patient, dual rod instrumentation in one patient and combined rod instrumentation in two patients. Coronal Cobb measurements were performed on all curves in thoracic, thoracolumbar and, lumbar spine and the angle of hump was measured by a scoliometer pre- and postoperatively. Results : The average operative time was 394 minutes (255-525) with an average intraoperative blood loss of 1,225 ml (1,000-1,700). The mean period of hospital stay was 19.3 days and there was no complication related to the surgery. The mean Cobb angle was reduced from $43.3^{\circ}$ to $14.8^{\circ}$ (65.8% correction) postoperatively and the rib hump corrected less than $5^{\circ}$. All patients and their parents were satisfied with the deformity correction. Conclusion : Anterior spinal correction and fusion of AIS with Lenke 1 and 5 curve showed excellent deformity correction without any complications. In particular, we recommend anterior dual rod instrumentation because of mechanical stability, better control of kyphosis, and a higher fusion rate.

청소년의 척추측만증이 스트레스 및 학습태도에 미치는 영향 (The Influence of Adolescent's Scoliosis on Stress and Study-attitude)

  • Purpose: The purpose of this study was to investigate the influence of adolescent's scoliosis on the stress level and study-attitude, to inform the public that there exist an important correlation between adolescent's scoliosis and stress, as well as, study-attitude, using a questionnaire. Methods: For this study, questionnaires were distributed to elementary school in G city from May 10 through May 31, 2011. This study measured a primary scoliosis-test by a scoliometer with 300 elementary school students and a secondary test by Cobb's angle using an X-ray. The subjects were divided into 2 groups (normal group (n=20), scoliosis group (n=20)). The collected data were analyzed by a t-test and Pearson analysis. Results: As a result of this study, the Cobb's angle showed significant difference between the scoliosis group and the normal group (p<0.001). Also, it tended to have a greater stress level in the scoliosis group than the relationship between scoliosis and the stress or study-attitude compared to the normal group (p<0.05). The correlation of stress according to Cobb's angle showed a positive relationship (r=0.36), and the correlation of study-attitude to Cobb's angle showed a negative relationship (r=-0.04). Conclusion: In conclusion, this study showed that scoliosis affects stress and study-attitude. We hope that this result would help diagnose adolescent's scoliosis in anearly stage and prevent any phychologic and social problems relating to it.

Immediate Effects of a Postural Correction Garment Designed for Postural Kyphosis on Adolescents With Thoracic Hyperkyphosis: A Pilot Study

  • The purpose of this study was to investigate the changes in the flexicurve kyphosis index (KI), the flexicurve lordosis index (LI) and the distance from the inferior angle of the scapula to the nearest vertebral spinous process (DS), as a dependent variable of scapular protraction, after applying of adolescents with thoracic hyperkyphosis using three different garments. A repeated measures design was used. Ten adolescents ($15.8{\pm}1.0$ years) with thoracic hyperkyphosis ($40.1{\pm}3.7$ Cobb angle) were recruited from a university hospital. A flexicurve ruler was used to measure KI and LI and a scoliometer was used to measure DS under three different conditions: wearing-a hospital garment (HG), wearing-a sham garment (SG), and wearing-an experimental garment (EG). KI under EG condition was significantly decreased compared with that wearing the HG. However, there was no significant difference between wearing the SG and HG. LI when wearing the EG was significantly increased compared with that when wearing HG. However, there was no significant difference under SG and HG conditions. DS when wearing the EG was significantly decreased compared with wearing HG. However, there was no significant difference between the SG and HG. The results of this study show that the EG was effective in decreasing KI, but not effective in decreasing LI. Hence, the effect of the EG for correcting sagittal spine angle in adolescents with thoracic hyperkyphosis is still debatable. However, since we showed that DS decreases in the EG, this method could be applied in correcting the scapular protraction.

척추측만증 평가 척도에 관한 문헌 고찰: 비방사선 방법을 중심으로 (Review Study on the Measurement Tools of Scoliosis: Mainly on Non-radiological Methods)

  • Objectives: The purpose of this study is to investigate the characteristics, validity, and reliability of non-radiological assessment tools of scoliosis that have been studied so far. Methods: Electronic databases including Pubmed, Cochrane Library, CNKI, Science On, RISS, OASIS were searched by keywords including 'scoliosis assessment', 'scoliosis screening', 'physical examination', 'functional measurement', 'photography', and 'smartphone'. Results: 32 articles using radiation-free assessments were identified from 1,011 records. The mostly used non-radiological methods were Surface topography, Scoliometer, Ultrasound, Digital Infrared Thermal Imaging, and Photography. The other methods were Gait analysis, 3D depth sensor imaging, and Low intensity electromagnetic scan. Conclusions: It was found that non-radiological assessment tools might reduce the number of radiographs taken in scoliosis patients. To increase the reliability and validity, further research on the measurement tools of scoliosis will be needed.
