A varicose vein of the lower limbs is dilated, tortuous and elongated vein. Western medical treatment involves conservative management, sclerotherapy, and surgery. Currently, internal medicinal treatment is inadequate. Recently a patient was diagnosed with varicose veins of the lower limbs based on clinical symptoms. In this case, Guibi-tang was administered and clinical symptoms improved. This report suggests that oriental medicinal treatment is efficacious in treating varicose veins of the lower limbs. We think that further oriental medical studies on various surgical diseases are needed.
Background: Cryosurgery was recently introduced as a treatment for varicose veins in the lower extremities. Cryosurgery with freezing probes can be used to remove the great saphenous vein (GSV) via an inguinal incision alone. The aim of this study was to assess early outcomes and the feasibility of cryosurgery for varicose veins. Materials and Methods: Forty patients were enrolled in the present study from March 2009 to July 2010. All patients underwent careful physical examinations, and their GSV reflux was demonstrated by duplex ultrasonography. Clinical severity was measured according to the clinical-etiology-anatomy-pathophysiology (CEAP) classification. The impaired GSVs were removed with rigid cryoprobes after freezing. Patients had follow-up appointments at 1 week, 1 month, 3 months, and 6 months after surgery. Sclerotherapy was performed during follow-up on an outpatient basis as needed. Results: A total of 19 men and 21 women were enrolled. The mean follow-up duration was 3.4 months (range, 1 to 12 months). The clinical severity ranged from CEAP 2 to CEAP 6a. Thirty-eight patients underwent concomitant phlebectomy during surgery. No recurrences were reported during follow-up. There were three cases of minor complications. Two patients had paresthesia, and one had thrombophlebitis. Conclusion: Using cryosurgery techniques to treat varicose veins, the complication rates were minimal and outcomes were comparable to those of previous reports. This procedure is a safe and feasible treatment modality for varicose veins.
Dieulafoy's disease, a vascular anomaly mainly in the upper stomach, is a rare but potentially life-threatening cause of upper gastrointestinal bleeding. Pathogenesis is still controversial, but the most accepted theory is that a persistent caliber vessel in the submucosa is exposed by a small mucosal erosion leading to massive bleeding. The bleeding site is usually within 6 cm of the esophagogastric junction in the cardia or fundus of the stomach. The treatment of choice is therapeutic endoscopy or surgery. The age of patients reported is mainly between 50 and 70 years, and patients of pediatric age are extremely rare. We are reporting a 5-year-old male patient who had Dieulafoy's disease which was diagnosed by emergency upper gastrointestinal endoscopy. Endoscopic finding was a nodular lesion with an adherent clot on the lessor curvature of the stomach 2 cm below the esophagogastric junction. Epinephrine and $Beriplast^{(R)}$ was injected in the lesion. On the second day after endoscopic sclerotherapy, the patient had recurred massive hematemesis and accompanying shock. So we performed gastrotomy and ligation. After the operation, he showed an improved general condition and was discharged at the 12th hospital day.
증식치료는 정상세포나 조직을 성장시키기 위해 성장인자 또는 성장인자 생성 자극제를 주사하는 것이다. 비록 수십 년 동안 논쟁이 되어 오고 있지만 갈수록 더 많은 의사들이 증식치료를 시술하고 있다. 증식치료라는 단어는 1950년 대에 Hacket에 의해 기술되었는데 인대나 근건에 있는 정상조직의 증식을 의미 하였다. 인대 염좌 등의 불완전한 치료는 만성 통증뿐만 아니라 관절불안 등을 초래하고 골관절염의 진행을 촉진할 수 있다. 증식치료는 통상적인 치료에 불응하는 이러한 근골격계 질환에 사용된다. 사용되는 증식제나 수기는 의사마다 수련과정 또는 선호도에 따라 차이가 있을 수 있지만 최근 가장 많이 사용되는 증식제는 10~25%의 포도당이다. 고해상도 초음파기기의 발전에 따라 인대나 근건 등을 실시간으로 볼 수 있게 됨에 따라 이를 증식치료에 접목하여 수기의 정밀도를 높이고 주입되는 약의 확산을 확인 할 수 있게 되었고 그 치료 결과를 기록할 수 있게 되었다.

Background Venous malformations (VMs) are a common type of vascular malformation. However, their causes and management remain unclear, and few studies specific to VMs of the head and neck have been reported. This study describes our experiences with VMs of the head and neck. Methods This retrospective study included 82 patients who underwent treatment for head and neck VMs, among 222 who visited our vascular anomalies center. Medical records between 2003 and 2016 were reviewed to identify common features in the diagnosis and treatment. The diagnosis of suspected head and neck VMs was based on the results of imaging studies or biopsies, and the VMs were analyzed based on magnetic resonance imaging, computed tomography, and Doppler sonography findings. Results VMs were slightly more common in female patients (59.8%), and 45.1% of patients developed initial symptoms at the age of 10 or younger. Lesions were slightly more common on the right side (47.3%). The main sites involved were the cheek (27.7%) and lip area (25.5%). The muscle layer was commonly involved, in 98.7% of cases. Small lesions less than 5 cm in diameter accounted for 60.8% of cases, and well-defined types were slightly more prevalent at 55.4%. Improvement was observed in 77.1% of treated patients. Conclusions Early and accurate diagnosis and appropriate treatment according to individual symptoms are important for successful treatment of VMs. If treatment is delayed, the lesions can worsen, or recurrence becomes more likely. Therefore, VMs require a multidisciplinary approach for early and accurate diagnosis.
Objective: This study aimed to evaluate the efficacy and safety of ultrasound (US)-guided minimally invasive treatment in patients with parathyroid lesions. Materials and Methods: This study included 27 patients who had undergone US-guided radiofrequency ablation (RFA) or ethanol ablation (EA) for parathyroid lesions between January 2010 and 2018. RFA was performed in 19 patients with primary hyperparathyroidism (PHPT, n = 11) or secondary hyperparathyroidism (SHPT, n = 8), and EA was performed in eight patients with symptomatic nonfunctioning parathyroid cysts (SNPCs). Nodule size, volume, serum parathyroid hormone (PTH) and calcium levels were recorded before and after treatment. Complications were evaluated during and after treatment. Results: In patients with PHPT, significant reductions in size and volume were noted after RFA at 6- and 12-month follow-up (all, p < 0.05). Seven nodules nearly completely disappeared (residual volume < 0.1 mL); serum PTH and calcium levels were reduced to normal ranges (7/11, 63.6%). Four patients experienced partial reductions of serum PTH and calcium levels (4/11, 36.4%). In patients with SHPT, three experienced therapeutic response of serum PTH (3/8, 37.5%), while five showed persistent hyperparathyroidism (5/8, 62.5%) within 6 months after RFA. In patients with SNPCs, EA resulted in significant reductions in cyst size and volume (all, p < 0.05) at the last follow-up. A total of four complications (two transient hypocalcemia [RFA], one permanent [RFA], and one transient [EA] hoarseness) were observed. Conclusion: Minimally invasive treatments, such as RFA and EA, may serve as therapeutic alternatives for patients with PHPT or SNPCs; they may have limited usefulness in patients with SHPT.
목적: 동정맥 혈관기형은 혈역학적 변화를 일으켜 생명에 위험을 줄 수 있으므로, 사지의 선천성 혈관기형 환자에서 동정맥 혈관기형과 비동정맥 혈관기형을 정확히 감별진단 하는 것이 중요하다. 이 연구에서는 혈관조영술과 비교하여 경동맥 폐관류 신티그라피(transarterial lung perfusion scintigraphy, TLPS)가 선천성 혈관기형 중 상하지에 위치하는 동정맥 혈관기형의 진단과 치료 효과 평가에 유용한 지를 알아보았다. 대상 및 방법: 색전술/경화요법 전에 처음 TLPS를 시행 받은 57명(나이: $21{\pm}13$세, 남:여 = 26:31)이 연구대상이 되었다. 병변의 위치는 상지가 9명이었고 하지가 48명이었다. TLPS에서 폐의 시간 방사능 곡선을 이용하여 혈액 단락량을 구하였다. 모든 환자들에서 혈관조영술을 시행하여 혈관기형의 해부학적 및 혈역학적 평가를 하였다. 혈관조영술로 동전맥 혈관기형이 진단된 환자들은 치료 후 TLPS를 추적 시행하였다. 결과: 혈관조영술상 16명은 동정맥 혈관기형(상지 8명, 하지 8명), 나머지 41명은 비동정맥 혈관기형(상지 1명, 하지 40명)으로 진단되었다. 동정맥 혈관기형 환자들의 치료 전 평균 혈액 단락량은 비동정맥혈관기형 환자들에 비하여 유의하게 높았다($66.4{\pm}25.8%\;vs.\;2.8{\pm}4.3%$, p<0.0001). TLPS의 치료 전 상하지 동정맥 혈관기형을 진단하는 예민도와 특이도, 정확도는 각각 93.8% (15/16), 100% (41/41), 98.2% (56/57)이었다(혈액 단락량 기준 역치 = 20%). 동정맥 혈관기형 환자 16명은 추적 TLPS를 시행하였고 이 중 13명은 치료 평가에서 혈관조영술과 일치하는 결과를 보여주었다(81.3%). 색전술/경화요법 시행 후 TLPS 혈액 단락량은 유의하게 감소하였다($69.5{\pm}24.0%\;vs.\;41.0{\pm}34.7%$, p = 0.01). 결론: TLPS는 상하지동정맥 혈관기형에서 혈액 단락량의 반정량적인 정보를 제공해주며 혈관 조영술과 높은 검사결과 일치도를 보여주었다. 그러므로, TLPS는 상하지 동정맥 혈관기형의 진단과 치료 평가에 유용하다.
목적 : 말기암 환자에서 발생하는 악성 흉막삼출액의 치료로 흉관삽입후 경화제를 주입하여 흉악유착을 유발하는 대증요법이 많이 이용된다. 그러나 이들 경화제의 종류 및 투여방법에 따라서 그 효과 및 부작용은 달라진다. 이에 대표적으로 이용되는 경화제인 doxycycline 과 bleomycin의 단기간의 효과 및 부작용을 비교 분석하기 위하여 본 연구를 실시하였다. 방법 : 1993년 l월부터 1995년 9월까지 세포조직학적으로 악성 흉막전이가 확인되었으며 증상완화를 위해서 반복천자가 필요하였던 34명(폐암 28 명, 기타암 6명)을 대상으로 무작위로 doxycycline (16례)과 bleomycin(18례)를 흉막강내에 주입하여 입원기간 중의 단기간의 치료반응과 처치시에 생기는 부작용을 비교하였다. 그리고 치료 반응군과 실패군에서의 중앙생 존기간을 조사하였다. 성적 : Doxycycline 군의 성공률은 87.5 %로 bleomycin 군의 50.0%보다 유의하게 높았다(p=0.02). 부작용으로 통증은 두 군사이에 유의한 차이는 없었으나 발열, 오심은 bleomycin군에서 유의하게 많이 생겼다(각각 p=0.01. 0.04). 그러나 이들 대부분의 부작용은 대증요법으로 호전되었다. 치료약제의 종류에 상관없이 치료반응군의 중앙생존기간은 11개월로 치료 실패군의 5개월보다 유의하게 연장되었다(p<0.01). 결론 : 이상의 성적으로 경화제를 이용한 흉막유착술은 악성 흉막삼출액을 가진 환자의 대증요법으로 효과가 좋았다. Doxycline과 bleomycin을 비교해 보면 doxycycline이 부작용도 적었고 단기간의 치료성적도 우수하였다.

Background: Conventional stripping is considered to be the standard procedure for great saphenous vein (GSV) varicosities, but many other alternative treatments such as cryostripping, endovenous laser therapy (EVLT), radiofrequency ablation, and ultrasound-guided foam sclerotherapy have been developed. Among them, both cryostripping and laser therapy have been reported to be less traumatic, with lower rates of complications and recurrences when compared to conventional stripping. To compare the efficacy of these treatments, we have analyzed and compared the mid-term clinical outcomes of cryostripping and EVLT. Methods: Patients diagnosed with varicose veins of the GSV and treated with cryostripping or laser therapy between September 2008 and April 2013 were enrolled in this study. Duplex ultrasonography was used for the diagnosis and evaluation of varicosity and reflux, and the clinical-etiology-anatomy-pathophysiology classification was used to measure the clinical severity. The symptoms, Venous Clinical Severity Score (VCSS), recurrence rates, and complication rates of the cryostripping and laser therapy groups were analyzed and compared. Results: A total of 68 patients were enrolled in this study. 32 patients were treated with cryostripping, and 36 patients were treated with laser therapy. The median follow-up period was 29.6 months. Recurrence was noted in three patients from the cryostripping group and in two patients from the EVLT group. There was no difference in the VCSS score, operative time, duration of hospital stay, and complication rate between the cryostripping group and the EVLT group. Conclusion: The mid-term clinical outcomes of cryostripping were not inferior to those of EVLT. Further, considering its cost-effectiveness, cryostripping seems to be a safe and feasible method for the treatment of varicose veins.
목 적 : 폐암은 악성 흉막 삼출액의 가장 흔한 원인으로 흉관 삽입후 흉막유착술을 시행하는 것이 치료에 널리 이용 되어왔다. 흉막경화제는 여러 종류가 알려져 있으나 그 효과나 부작용은 약제마다 차이가 있다. 이에 저자들은 대표적 흉막경화제인 doxycycline과 새로운 약제인 OK-432의 효과, 부작용 및 무병기간의 차이를 알아보기 위해 본 연구를 시행하였다. 방 법 : 1994년 1월부터 2001년 8월까지 흉막천자 세포진검사 혹은 흉막조직검사상 악성 흉막염이 확인된 폐암환자 중 OK-432나 doxycycline으로 흉막유착술을 시행한 79명의 환자에게서 흉막 유착술 후 30일째의 성공률을 조사하였으며, 약제의 부작용과 무병 생존기간을 후향적으로 조사하였다. 결 과 : OK-432와 doxycycline을 이용한 흉막유착술 후 30일째의 성공률은 각각 83%와 87%으로 유의한 차이가 없었으며 (p=0.677), 부작용으로 발열은 OK-432군 (59%, p=0.001)에서 유의하게 많았고, 통증은 doxycycline군 (73%, p=0.008)에서 유의하게 많았다 무병기간은 두 군 사이에 유의한 차이가 없었다(OK-432 : 13.6개월, doxycycline : 11.6개월)(p=0.532). 결 론 : 폐암으로 인한 악성 흉막액을 치료하는 OK-432는 doxycycline과 비슷한 흉막유착술치료 효과가 있을 것으로 생각된다.
