Prevalence, species, and antimicrobial resistance of Acinetobacter in surgical practice and laboratory dog husbandry room environments

  • Acinetobacter is a bacteria found in the environment and clinical specimens, causing nosocomial infection and antimicrobial resistance (AMR) threats. This study examined the prevalence, species, and AMR characteristics of Acinetobacter isolated from surgical practice and the laboratory dog husbandry room environments (n = 235) at Rajamangala University of Technology Tawan-ok veterinary hospital during 2018-2019. The prevalence of Acinetobacter in the laboratory dog husbandry room and veterinary belongings were 2.55% and 0.43%, respectively. Species determination was Acinetobacter hemolyticus (2.13%) and Acinetobacter baumannii (0.43%) from environments in the laboratory dog husbandry room, and Acinetobacter junii (0.43%) from the shoes used in the surgical practice room. AMR was observed in both study environments and the specimens sent to the Veterinary Diagnostic Center. These isolates had a high resistant percentage to amoxicillin-clavulanic acid (84.62%), sulfamethoxazole-trimethoprim (61.54%), and cephalexin (53.85%) but were susceptible to imipenem. Compared to the isolates recovered from the clinical specimens, most isolates derived from environments exhibited multidrug resistance and shared correlated resistance patterns. These results highlight the need for sanitization in the dog husbandry room. Furthermore, the AMR results can be used as a preliminary baseline for studying AMR Acinetobacter contamination in animals and their environments.

수술실 간호사의 직업적 손상 경험 현황과 안전 분위기의 상관관계 (The Correlation between Occupational Injuries and Safety Climate among Operating Room Nurses)

  • Purpose: This study aimed to examine the occupational injuries experienced by operating room nurses and investigate the relationship between these injuries and safety climate. Methods: The study involved 198 operating room nurses working in general and tertiary hospitals with more than 300 beds. Results: Among the four sub-factors of occupational injuries experienced by operating room nurses, risks due to ergonomic hazards were the most prevalent and those due to physical hazards were the least. The scores related to occupational injury showed a significantly negative correlation with the safety climate. Conclusions: The findings indicate that higher operating room nurses' awareness of the safety climate is associated with lower rates of occupational injury. Therefore, organizational efforts and interventions are imperative to improve the safety climate within hospitals. Additionally, in order to investigate the safety of operating room nurses more in-depth, it is necessary to expand not only the sample size but also the geographic scope.

응급실과 119 안전센터의 접근성을 고려한 응급의료 취약지 분석 (Vulnerable Analysis of Emergency Medical Facilities based on Accessibility to Emergency Room and 119 Emergency Center)

  • The purpose of this study was to identify vulnerable area of emergency medical care. In the existing method, the emergency medical vulnerable area is set as an area that can not reach the emergency room within 30 minutes. In this study, we set up an area that can not reach within 30 minutes including the accessibility of 119 emergency center. To accomplish this, we obtained information on emergency room and 119 emergency center through Open API and constructed road network using digital map to perform accessibility analysis. As a result, 509 emergency room are located nationwide, 78.0% of them are concentrated in the region, 1,820 emergency center are located, and 61.0% of them are located in rural areas. The average access time from the center of the village to the emergency room was analyzed as 15.3 minutes, and the average access time considering the 119 emergency center was 21.8 minutes, 6.5 minutes more. As a result of considering the accessibility of 119 emergency center, vulnerable areas increased by 2.5 times, vulnerable population increased by 2.0 times, and calculating emergency medical care vulnerable areas, which account for more than 30% of the urban unit population, it was analyzed that it increased from 17 to 34 cities As a further study, it will be necessary to continuously monitor and research the real-time traffic information, medical personnel, medical field, and ambulance information to reflect the reality and to diagnose emergency medical care in the future.

Storage of laboratory animal blood samples causes hemorheological alterations : Inter-species differences and the effects of duration and temperature

  • Hemorheological results may be influenced by the time between blood sampling and measurement, and storage conditions (e.g., temperature, time) during sample delivery between laboratories may further affect the resulting data. This study examined possible hemorheological alterations subsequent to storage of rat and dog blood at room temperature ($22^{\circ}C$) or with cooling ($4{\sim}10^{\circ}C$) for 2, 4, 6, 24, 48 and 72 hours. Measured hemorheological parameters included hematological indices, RBC aggregation and RBC deformability. Our results indicate that marked changes of RBC deformability and of RBC aggregation in whole blood can occur during storage, especially for samples stored at room temperature. The patterns of deformability and aggregation changes at room temperature are complex and species specific, whereas those for storage at the lower temperature range are much less complicated. For room temperature storage, it thus seems logical to suggest measuring rat and dog cell deformability within 6 hours; aggregation should be measured immediately for rat blood or within 6 hours for dog blood. Storage at lower temperatures allows measuring EI up to 72 hours after sampling, while aggregation must be measured immediately, or if willing to accept a constant decrease, over 24~72 hours.

화재모델 CFAST를 이용한 원전 화재구역의 CCDP평가(II) (CCDP Evaluation of the Eire Areas in NPP Applying CEAST Model (II))

  • 본 논문에서는 화재 모델 CFAST를 사용하여 원자력발전소에 있는 8개 펌프실의 화재 안전 수준을 평가해 보고, 그 결과를 조건부 노심손상확률에 반영하여 분석해 보았다. 분석 대상 구역은 원자력발전소 일차보조건물에 위치한 고압안전주입 펌프실 A/B,저압안전주입 펌프실 A/B, 격납건물 살수 펌프실 A/B 및 모터구동 보조급수 펌프실 A/B 총 8곳이었다. 각 화재구역의 상부층 가스 온도를 분석하였으며, 분석 결과를 바탕으로 상부층에 위치한 케이블의 손상 유무를 파악하였다. 분석 결과에 의하면 총 8개 화재구역의 상부층 케이블은 건전성을 유지하는 것으로 확인되었다. 케이블의 건전성 평가 결과를 바탕으로 각 화재구역의 조건부 노심손상확률을 평가한 결과, 기존의 보수적인 가정 하에 수행된 화재위험도분석 결과보다 약 2배정도 감소하는 보다 현실적이고 불확실성이 감소한 결과를 도출할 수 있었다.

Acetic acid에 의한 저온저장고 내의 균발생 억제 효과 (Inhibition of Incidence of Fungi in Cold Storage Room by Acetic Acid)

  • Acetic acid는 상온($25^{\circ}C$)에서 저온저장고에 발생하는 과실의 부패병균인 Botrytis cinerea 및 Penicillium expansum의 포자발아 및 발아관 생장 억제에 효과가 나타났다. $4.8mg{\cdot}L^{-1}$의 acetic acid 함유배지에서는 처리 12시간 후에도 B. cinerea 및 P. expansum의 포자가 전혀 발아하지 않았으며, 처리 7일 후에도 거의 생장하지 못할 정도로 완전한 균사생장 억제효과를 나타내었다. 저온($2^{\circ}C$)에서도 B. cinerea 및 P. expansum에 대해 acetic acid의 농도가 높을수록 균사생장 억제효과가 증가하였으며 $4.8mg{\cdot}L^{-1}$를 함유한 배지(PDA)에서는 처리한 후 20일이 경과하여도 균총이 전혀 형성되지 않았다. 저온저장고를 가동하기 전에 기존의 $SO_2(5g{\cdot}m^{-3})$ 가스와 acetic acid($4.8g{\cdot}m^{-3}$)를 처리한 저장고에서 출현하는 균의 발생을 감소시켰다. 그러나 실제 적용시 인체에 대한 자극성이 있다는 점에 대한 보완책이 필요할 것으로 생각되었다.

산업체 급식소에서 제공되는 음식의 조리후 보관방법에 따른 품질평가 (A Study on the Quality Control for the Holding Method of Food Served by an Industry Foodservice Establishment)

  • The purpose of this study was that to estimate the microbial and sensory quality by the method and time of holding in the step of holding of Cow's Knee Broth, Stir-Fry Pork and Cooked Cuttlefish that were provide at the industry foodservice establishment, then to suggest safer feeding by the reasonable holding method. The results are as follows: Cow's Knee Broth: holding at room temperature was shown that the number of mi- crobiology by passage of the holding time was higer than the case of holding at steam table, but it was lower after reheating it. As a result of sensory test, it was exellent to holding at steam table and to reheat after holding room temperature within four hours and six hours each. Stir-Fry Pork: as a result of microbial analysis, in the case of holding at the steam table microbes increased safer two hours, but after four and six hours they little reduced. In the case of holding at room temperature the number of microbes increased according to the passage of time. As a result of sensory analysis, its taste of room temperature holding was fine until four and six hours each. Cooked Cuttlefish: as a microbial analysis, it was shown that the amount of total plate counts of room temperature holding ten times as high as the latter after six hours. As the result of sensory analysis, the cold holding was better until six hours and the room temperature holding was fine just until two hours. As a result of food poisoning bacteria, it was negative in every test in sample against Salmonella, Staphylococcus aureus, Vibrio parahaemolyticus.

Noise and Room Acoustic Conditions in a Tertiary Referral Hospital, Seoul National University Hospital

  • Background and Objectives: Noise levels and room acoustic parameters at a tertiary referral hospital, Seoul National University Hospital (SNUH) in Korea, are investigated. Materials and Methods: Through a questionnaire, acoustically problematic rooms are identified. Noise levels in emergency rooms (ERs) and intensive care units (ICUs) are measured over about three days. Acoustically critical and problematic rooms in the otolaryngology department are measured including examination rooms, operating rooms, nurse stations, receptions, and patient rooms. Results: The A-weighted equivalent noise level, LAeq, ranges from 54 to 56 dBA, which is at least 10 dB lower than the noise levels of 65 to 73 dBA measured in American ERs. In an ICU, the noise level for the first night was 66 dBA, which came down to 56 dBA for the next day. The noise levels during three different ear surgeries vary from 57 to 62 dBA, depending on the use of surgical drills and suctions. The noise levels in a patient room is found to be 47 dBA, while the nurse stations and the receptions have high noise levels up to 64 dBA. The reverberation times in an operation room, examination room, and single patient room are found to be below 0.6 s. Conclusions: At SNUH, the nurse stations and receptions were found to be quite noisy. The ERs were quieter than in the previous studies. The measured reverberation times seemed low enough but some other nurse stations and examination rooms were not satisfactory according to the questionnaire.

  • 바륨, 스트론튬, 이트륨 및 티탄의 질산염과 옥살산을 이용한 습식화학합성법으로 금속-옥살레이트의 침전물을 얻고, 이를 하소하여$(Ba_{0.8-x}Sr_{0.2})Y_xTiO_3(BSYT)$ 분말을 제조하였다. Y의 혼입량, 소결 온도 및 냉각 속도를 실험 인자로 설정하여 BSYT의 상온비저항을 위한 모델식을 반응표면분석법으로 구하였다. 그 결과 Y의 혼입량 변화가 상온비저항에 가장 크게 영향을 미치며, Y의 함량이 약 0.24 mol% 일 때 상온비저항은 최소값을 나타내었다. 실제 실험으로부터 구한 상온비저항 값과 모델식으로부터 계산한 상온비저항 값을 비교한 결과 잘 일치함을 확인하였다.
