Novel anatomical proposal for botulinum neurotoxin injection targeting depressor anguli oris for treating drooping mouth corner

  • The depressor anguli oris (DAO) muscle is a thin, superficial muscle located below the corner of the mouth. It is the target for botulinum neurotoxin (BoNT) injection therapy, aimed at treating drooping mouth corners. Hyperactivity of the DAO muscle can lead to a sad, tired, or angry appearance in some patients. However, it is difficult to inject BoNT into the DAO muscle because its medial border overlaps with the depressor labii inferioris and its lateral border is adjacent to the risorius, zygomaticus major, and platysma muscles. Moreover, a lack of knowledge of the anatomy of the DAO muscle and the properties of BoNT can lead to side effects, such as asymmetrical smiles. Anatomical-based injection sites were provided for the DAO muscle, and the proper injection technique was reviewed. We proposed optimal injection sites based on the external anatomical landmarks of the face. The aim of these guidelines is to standardize the procedure and maximize the effects of BoNT injections while minimizing adverse events, all by reducing the dose unit and injection points.

Effect of home-based exercise program on balance, activities of daily living, depression and quality of life in elderly with chronic diseases (방문재활운동 프로그램이 만성질환 노인의 균형, 일상생활활동과 우울 및 삶의 질에 미치는 영향)

  • Background: The purpose of this study was to identify the effect of home-based exercise program in elderly with chronic diseases on balance, activities of daily living, depression and quality of life. Design: One group pretest-posttest design Methods: The subjects consisted of 21 elderly people over the age of 65 years with chronic diseases lived in seo-gu, Gwangju. Home-based exercise program was performed twice a week, for an hour, during 6 weeks under supervision of an experienced instructor. The home-based exercise program was structured to last a total of 1 hour, consisting of 10 minutes of warm-up, 30 minutes of main exercises, and 20 minutes of concluding exercises. For the warm-up, we performed range of motion, stretching, and flexibility exercises. The main exercises comprised of strength, weight-bearing and balance exercises, and endurance exercises. The concluding exercises were tailored to the individual's home environment and needs, including task-oriented daily living training such as getting up from bed, moving to the restroom, and walking indoors. The subjects were assessed before and after the exercise program with berg balance scale(BBS), Korean-version modified barthel index(K-MBI), euro quality of life questionnaire 5-dimensional classification(EQ-5D), and geriatric depression scale short form-Korea(GDSSF-K). The collected data was analyzed using SPSS Win 18.0 program. Results: The results showed statistically significant difference in all domains of the BBS, K-MBI, EQ-5D, GDSSF-K after the exercise program. Conclusion: These results suggest that Home-based exercise program has resulted in a significant improvement on ADL, depression and quality of life in elderly with chronic diseases.

A Correlation of Interleukin-6 of Pancreatic Cancer Patients and Cancer Progression: Case Series (췌장암 환자의 IL-6 수치와 암 진행의 상관 관계에 대한 3례 증례보고)

  • Objectives: To report a correlation of IL-6 of pancreatic cancer and cancer progression in three pancreatic cancer patients. Method: Three pancreatic cancer patients were monitored for changes in IL-6 levels, tumor markers (CEA, CA19-9), and clinical outcomes over their treatment period. Results: Patient 1's IL-6 levels rose with liver metastasis and tumor progression, coinciding with increases in tumor markers. Patient 2's IL-6 levels remained elevated during chemotherapy, correlating with tumor growth. Patient 3's IL-6 levels spiked prior to cancer progression. Conclusion: Elevated IL-6 levels were observed in advancing pancreatic cancer patients, suggesting IL-6 as a potential biomarker for monitoring cancer progression in pancreatic cancer. Regular IL-6 monitoring could improve prognostic evaluations and treatment strategies.

Epidemiology of early esophageal adenocarcinoma

  • Background/Aims: Endoscopic resection has become the preferred treatment approach for select early esophageal adenocarcinoma (EAC); however, the epidemiology of early stage disease has not been well defined. Methods: Surveillance Epidemiology and End Results (SEER) data were analyzed to determine age-adjusted incidence rates among major epithelial carcinomas, including EAC, from 1973 to 2017. The percent change in incidence over time was compared according to tumor subtype. Early T-stage, node-negative EAC without metastasis was examined from 2004 to 2017 when precise T-stage data were available. Results: The percent change in annual incidence from 1973 to 2017 was 767% for EAC. Joinpoint analysis showed that the average annual percent change in EAC from 1973 to 2017 was 5.11% (95% confidence interval, 4.66%-5.56%). The annual percent change appeared to plateau between 2004 and 2017; however, early EAC decreased from 2010 to 2017, with an annual percent change of -5.78%. Conclusions: There has been a 7-fold increase in the incidence of EAC, which was significantly greater than that of the other major epithelial malignancies examined. More recently, the incidence of early EAC has been decreasing. Approximately one in five patients has node negative, potentially resectable early stage disease.

Standardization of Substituent Effects based on Benzobisoxazole Molecule (Benzobisoxazole 분자를 활용한 치환기 효과의 표준화)

  • Recently, it was reported that the benzobisoxazole (BBO) molecule can be used for independent modulation of the HOMO and LUMO energy levels. In this study, we utilized this interesting property of BBO to quantitatively investigate the substituent effects according the substituents. We designed and proposed four BBO model systems (1BBO, 2BBO, 3BBO and 4BBO) by extending the π-conjugation lengths based on a BBO molecule. Two directions of substitution (x-axis and y-axis) and 15 various substituents were considered. From strong correlation between the LUMO energy levels of x-axis substitution and HOMO energy levels of y-axis substitution calculated from DFT method, it is found that the standardization of substitution effects can be established from BBO model systems. In addition, we demonstrated that the HOMO values of y-axis substituted 3BBO show the best performance to define the order of substituent effects. Our proposed way can be used to expect the substituent effect of any arbitrary substituents and develop the organic sensors and organic light emitting diodes.

A Big Data Analysis to Prevent Elderly Solitary Deaths by High-risk Area Clusterization (노인 고독사 방지를 위한 빅데이터 기반 고독사 고위험 지역 탐지 연구)

  • This study proposes a big data-based analytical method to detect high-risk areas for solitary deaths among the elderly in Seoul. The study categorizes and analyzes the risk factors of solitary deaths into demographic, health, economic, and socio-environmental factors. Using data collected from the Seoul Open Data Plaza and Public Data Portal, variables were generated and scatter plots were created using K-means clustering, followed by visual implementation through map creation. The analysis identified Jungnang-gu, Gangbuk-gu, Nowon-gu, Eunpyeong-gu, Gangseo-gu, and Gwanak-gu as the highest-risk areas. This study addresses the limitations of previous survey-based research through big data analysis. The findings are expected to enhance the efficiency of solitary death prevention programs and serve as a basis for informed decision-making in budget allocation across districts.

  • The construction sector, in relation to human's residential issues, serves as the cornerstone of societal systems, economic security, the national economy, individuals and households, economic activity, national safety, management, business, and government. Also, the essential sectors, such as electric power generation, transmission, and distribution, and natural gas distribution, water, sewage, and stormwater systems, and so on, form the heart of our social system. These essential sectors of our society provide core goods and services for continuous public health services, construction services, government operations, multiple businesses, and national and economic safety. Therefore, those systems are the cornerstone of our society, and strengthening their security and resilience is of utmost significance. However, the existing framework for assessing and evaluating the problems with regard to those systems lacks efficient methods and mechanisms. The main objective of this research is to define correlations between different infrastructures within an economic system to strengthen the resilience and security of the construction sector. This study will be conducted to identify physical relationships between different industries within an economic system and to define deterministic relationships through the values of interconnectedness and interdependency. In addition, this research attempts to complete a probabilistic estimation of economic impacts using historical economic data and to develop an assessment model that can be used in the future to measure economic impact in terms of the construction sector. In case of loss in the construction sector due to exogenous factors, identifying which critical infrastructures or sectors will be the most affected will help minimize the risks and property damages. Furthermore, improving the resiliency of the construction sector will help speed recovery from or resistance to unpredictable external elements.

Development of Nutrition Education Materials for Prevention and Management of Diabetes Mellitus for Older Adults

  • Nutrition is important in the management of diabetes mellitus, however, there are few little education materials specifically designed for older adults. The objective of this study was to develop nutrition education materials for prevention and management of diabetes moll for older adults. Materials developed were a booklet and four leaflets. The contents of materials were based on lesson plans. After several revisions of the draft of materials, illustrations and icons appropriate to the contents were designed using illustrator 9.0 and Photoshop 6.0. The booklet was composed of five chapters and 40 pages. The first chapter began with an introduction about diabetes and diabetes management by diet, exercise and medication. The second chapter dealt with ideal body weight, calculation of adequate caloric intake and food exchange list. The third chapter provided information for meal planning and sample menus. The fourth chapter focused on practical tips on nutritional care of diabetes, by providing tips on reducing sugars, fat and salt, and suggestions on eating for special occasions. The fifth chapter dealt with information in case of low blood sugars, exercise and foot care. The topics of the four leaflets were “Diabetes, what is it and care”, “Food exchange list and meal planning”, “Healthy eating for diabetes”, “Special care for diabetes low blood sugars, exercise and foot care” Each leaflet was composed of six sections and was printed in large paper (B4 size) for older adults. The draft of educational materials were re-viewed by four nutrition professionals and finally pilot-tested with ten adults aged 50 and older. The characteristics of the developed materials are as follows, i) messages are delivered using simple, specific information, ⅱ) messages focused on practical applicable tips, ⅲ) various pictures, illustrations and artwork were created and inserted to enhance understanding and interest, ⅳ) sections including risk factor assessment, calculation of ideal body weight and meal planning were designed to induce the user's participation, ⅴ) sample menus and food pictures were inserted in the booklet, vi) characteristics of older adults and transformed characteristics are diversely used to help the user feel familiarity. These materials are self-explanatory and can be used by older adults. These materials also can be used widely in nutrition education at public health centers or senior centers.

A Study on the Occurrences and Policy Development for Accident Prevention (한국의 사고발생 실태와 사고예방을 위한 정책 연구)

  • There is no national system to manage, evaluate, and analyse the information about accidents, even though it is necessary for accident prevention policies and health promotion of the general public. The existing studies are, also limited as they focus only on particular group(for example : inpatients, emergency room patients) or on particular geographic areas. Thus the results of the studies cannot be ap-plied to the general public. In order to overcome these limitations, this research focuses on data collection and analysis from accident information for the general population. By providing the analysis on types and causes af accidents, this research aims to produce the basic data necessary for accident prevention policy development. The specific aims of this research are to : 1. Analyse the actual occurrences and characteristics of accidents. 2. Suggest for the accident prevention policies and safety education. Accident report form three major newspaper printed in Korea between January 1, 1989 and December 31, 1993 were collected, and the cause, place, time, and personal injury related to the accidents were classified and then analyzed by de-scriptive statistics. The results of this research conclude : 1. The number of accidents reported by the three newspapers were 2155. 2. The highest proportion of accidents were as follows ; occurred during the June-August(31.2%), Sunday(24.8%), and 5 p.m. (7.6%) of the day. 3. The highest proportion of the accident occurred in Seoul(33. l%), Kyunggi province(14.5%), Kangwon province (7.8%), and Kyungnam prov-ince(6.7%) were next highest. 4. The main causes include car accidents(32.4%), drowning(10.9%), falls(8.8%), explosion(7.1%), and poisoning(5.8%). 5. Slightly more than half of injuries(50.5%) and about two fifths of deaths(40.3%) were caused by car accidents. Therefore, the most serious type of accidents were car accidents. 6. The number of males in accidents were almost three times higher than that of females(males 72.3%, females : 27.7%). 7. The age group from 10 to 19 years old represents the highest proportion(21%) of accidents. 8. The number of the accidents in Korea, based on the information collected from the newspapers, is estimated to be 14, 367 per year. The number of in-juries is estimated to be 88, 480 persons, and the number of deaths 29, 007 respectively. It is said that ninety percent of accidents can be prevented. Several accidents prevention policies are suggested here. 1. Safety education should be done more actively throughtout life, with special emphasis on safety education for children. 2. Safety measures for children(halmets for cycling, children car seats, seats belts, and so on) should be emphasized. 3. An injury surveillance system should be initiated. The initiation of injury reporting system in each factory and school could contribute considerably to the reduction of accidents.

Relationship of Self-Perceived Symptoms of Periodontal Disease to Quality of Life in Adults (일부 성인에서 치주질환 자각증상과 삶의 질의 관련성)

  • The aim of this study was to investigate the correlations between the self-reported symptoms of periodontal diseases and the quality of life. This study conducted its questionnaire survey over the subjects of 450 adults who had ever visited dental clinics among the adults of age 35-65 living in Seoul and Gyeonggi-do region from January 7 to March 14, 2010. The data were analyzed using a chi-square test, t-test, binominal logistic regression analysis in the SPSS version 12.0 program. Among the self-reported symptoms of periodontal diseases, 65% of respondents perceived that 'I bleed from the gums in brushing my teeth', which was the most, whereas 18.8% of respondents perceived to 'tooth mobility', which was the least. Self-reported symptoms of periodontal diseases was associated with socio-demographic characteristics such as sex, age, level of education and periodic oral examination, dental scaling(p<0.05). People who perceived the symptoms of periodontal diseases experienced functional limitation, physical pain, psychological discomfort, disability in diverse domains, and social handicap more than the group who did not perceive them(p<0.001). OHIP-14 scores were significantly correlated with sex, bleeding gums, swollen gums and bad breath(p<0.05). This study showed the self-reported periodontal problem had a negative impact on the public's leading their happy life. Therefore, in order to enhance the quality of life in adults, it is necessary to prevent periodontal diseases and reduce the subjective symptoms.
