Objectives: To prevent smoking among adolescents, we should facilitate school-based smoking prevention programs and provide supports for teachers to participate actively in these programs. This study investigated Korean middle teachers' intentions, perceptions, and attitudes toward participating in smoking prevention programs. Methods: The study design was cross-sectional. Our conceptual framework utilized the Theory of Planned Behavior by Fishbein and Ajzen. We conducted open-ended elicitation interviews with teachers. We developed the survey questionnaire contents with data from these interviews, and distributed the questionnaires in 2002 to 194 school teachers from four schools in Seoul and Kyounggi-Do. Teachers' beliefs(behavioral, normative & control) and intentions about participating in smoking prevention programs were measured by 7-point scales. Results: The majority of teachers surveyed reported participating in adolescents' smoking prevention programs, while only less than 10% teachers reported having educational training for such programs. Teachers' attitudes toward participating in smoking prevention programs were positive, but they did not feel strong subjective norms about participating in the programs. They reported several barriers as well as facilitating conditions in participating in those programs. In correlation analysis, teachers' subjective norms and attitudes toward participating in smoking prevention were significantly correlated with their intentions to participating in those programs. Teachers with positive perceptions about smoking prevention programs were more likely to have strong intentions to participation in them, while teachers who received educational training and instructions on how to teach in smoking prevention programs were more likely to have positive perceptions than those who did not. Conclusion: We concluded that smoking prevention programs for adolescents can be made more effective by increasing societal expectations that teachers participate in these programs, and by providing additional resources dedicated to facilitating teachers' active participation in them.
Objectives: The purpose of this study was to investigate the characteristics of smoking prevention programs and their effectiveness for Korean school-aged children and adolescents by using systematic review and meta-analysis. Methods: Electronic searches were performed in RISS, NAL, DBPia, KISS using keywords according to inclusion criteria. 21 studies published from 2003 to the first half of 2017 that dealt with effects of smoking prevention programs for school-aged children and adolescents were selected for systematic review. Results: All 21 studies were quasi-experimental research designs. More than half of the programs(66.7%) were conducted for male and female. Most of the programs were conducted more than once a week(71.4%). 14 studies(66.7%) did not report using a theoretical model. Five dependent variables(knowledge of smoking, attitude toward smoking, non-smoking intention, self-assertiveness, and self-efficacy) were selected to measure the effectiveness of the smoking prevention programs. Knowledge of smoking was the most effective at hedge's g=0.673. Self-efficacy and self-assertiveness variables were statistically significant at hedge's g=0.461 and hedge's g=0.279, respectively. Effect sizes of attitude toward smoking and non-smoking intention were not statistically significant compared to the control group. As a result of the moderator effect analysis on the knowledge of smoking variable, the statistically significant variables were 'gender of participants'(p<0.01) and 'duration of the program'(p<0.001). Conclusions: The results of this study using systematic review and meta-analysis will be evidence-based data for researchers conducting smoking prevention programs in school-aged children and adolescents.
This study aimed at examining the risk factors of smoking behavior among middle school students and preparing school-based smoking prevention program. This study surveyed at February 1995 from 892 students at 3 middle schools in Seoul. The major findings of this study are follows; The proportion of current smoker among students in this study was 3.8%, and the proportion of intentions to smoking was 8.4%. At demographic variables male, pocket money of month was significantly positive association with smoking behavior, but economic status and education status of father were significantly negative association with smoking behavior. Results indicated that social influence variables(peer influence), alcohol, and positive attitudes of smoking were significantly positive association with smoking behavior. So implications for smoking prevention programs may be more effective at risk populations than using general adolescent population. Also ‘School-based smoking prevention programs’ may be learning social pressure resistance skills and giving knowledge and information about negative attitudes about smoking.
The age of the very first smoking is a significant indicator of life-long health status. Smoking prevention intervention was actively conducted based in middle and high schools with the support of the Korean Act for Health Promotion since 1998. These governmental supports became large and various smoking prevention programs were developed and conducted in diverse standards and perspectives. This study made a comprehensive descriptive literature review on smoking prevention educational programs for youth in Korea to identify the intervention quality and effectiveness of them for future smoking prevention program development. A total of 28 peer-reviewed journal articles published between May, 1995 and May 2005 (for the last 10 years) were finally included in this review process. The study participants should be adolescences in middle or high schools in Korea and the intervention types of the review studies should be smoking prevention or cessation educational programs. The outcomes of the reviewed studies were reanalyzed by the participants' characteristics, theory basement, evaluation design, intervention period, evaluation outcomes, and program effectiveness Most smoking intervention programs were not specified by sex and smoking status. Largely most intervention programs focused on male students although female students' smoking percent is increasing including both smokers and nonsmokers. Based on the school grades, the large percents of smoking intervention were conducted to the first grade of middle school and the first grade of high school. Almost 70% of the reviewed studies did not apply any health behavior change theories and the Transtheoretical model and social cognitive theory were utilized in 7 studies among the last reviewed 30%. The theory-based intervention studies had greater effectiveness than the non-theory based studies. More than 90% had quasi-experimental evaluation design and the effectiveness of the non-experimental designed study seemed over estimated than the quasi-experimental or experimental designed studies. More than 60% of the reviewed studies made their education for less than j days, over a short period and the evaluation factors were knowledge and attitude in general which can be obtained in short intervention period. Therefore, smoking intervention programs for youth in Korea need to be modified in terms of research design such as the intervention period, intervention-evaluation design, theory-based approach, and population-focused intervention specification.
Purpose: The purpose of this study is to understand a variety of intervention studies to prevent smoking by adolescents in U.S. and find out implications for Korea. Methods: This study reviewed articles found in the internet and analysed the data of US DHHS and CDC. Results: The School Health Policies and Programs Study (SHPPS) is a national survey periodically conducted to assess school health policies and programs of U.S. The main components of SHPPS are health education and physical education, health services, mental health and social services, school policies, and school environments. The CDC guidelines for school health programs to prevent tobacco use and addiction are composed of policy, instruction, curriculum, training, family involvement, tobacco-use cessation efforts, and evaluation. School-based interventions to prevent smoking can be classified into the categories of information-giving curricula, social competence curricula, social influence approaches, combined methods draw on social competence and social influence approaches and multi-modal programmes and Youth Empowerment study. The key programs for adolescent smoking prevention are ALERT Project, HSPP, TNT Project, MPP, NC YES. Conclusions: As smoking is often the first step of unhealthy behaviour such as alcohol drinking, illegal drugs, and violence, smoking prevention programmes for adolescents in U.S. have been comprehensive school-based health programs. In smoking prevention programs for adolescents, CDC plays a critical role by supporting survey, research, policy, and funds. The effectiveness of the programs was high when it was based on school and involved parents, community, and mass media. As the effect of each programme is not expected to last for a long time, consistent repetition of these interventions is essential. Current smoking prevention programs for adolescents are exploring the empowerment approach focused on the active involvement of participants rather than traditional approaches using order and discipline.

This study had two objectives. The first was to assess the frequency and content of school-based anti-smoking education received by Romanian adolescents aged 14-15. Secondly, the study aimed to evaluate to what extent the implementation of a specific 5 lessons smoking prevention program influences the quality of anti-smoking school education among Romanian adolescents. The investigation was performed in twenty schools from Cluj-Napoca, Romania, which were randomly assigned to the control and experimental conditions, resulting in 55 participating classes from the seventh grade (28 in the control group and 27 in the experimental group). The experimental group participated in a school-based smoking prevention program consisting of 5 lessons. The control group beneficiated only in the standard anti-smoking education offered by their schools. Six months after the program implementation, students from both experimental and control groups filled in a questionnaire, assessing several issues regarding their exposure to anti-smoking school education in the last year. The results showed a low exposure to anti-smoking school education among the Romanian adolescents. The implementation of the specific school-based smoking prevention program increased the exposure of Romanian adolescents to a higher number of lessons of smoking prevention and influenced positively the quality of these lessons. The study identified several gaps with respect to anti-smoking education in Romanian schools. It underlines the benefits of the implementation of a school based smoking prevention program with a clear structure, which contains appropriate educational messages and it is easy to implement.
Purpose: The purpose of this study was to describe the current status of managing educational programs for tobacco use prevention at middle and high schools located in Busan. Methods: Using a survey method with a 29-item self-administration questionnaire, the data were collected from January 10 through February 18, 2005, from school nurses as well as teachers major in healthcare of 227 participant schools. Results: A strategic plan for school-based educational programs of tobacco use prevention was mainly developed by a school nurse at middle school and a teacher in charge of school discipline at high school. The educational programs for tobacco use prevention were implemented for approximately 40% of schools in Busan with the budget ranged from 110,000 won to 200,000 won a year. Eighty five percepts of the schools had anti-tobacco regulations and guidelines in place. Twenty two percents of the middle and high schools collected the data regarding adolescents' smoking status periodically. Over 70% of schools provided tobacco use prevention education programs as a part of regular semester curriculum. Conclusions: For making the management of educational programs for tobacco use prevention effective and consistent with government's anti-smoking policy, schools need (a) development of tobacco management guideline and regulations with participation of teachers, students, and parents, (b) clear job and role descriptions for personnel to plan and implement anti-tobacco programs, (c) compilation of the budget enough to implement the programs, (d) periodically assessment of adolescents' smoking status, and evaluation of the programs to monitor whether those are effective for adolescents' health promotion; and (e) to make an effort to less range of quality in educational programs for tobacco use.
The National Health Promotion Law passed in 1995 was a milestone for initiating a national health promotion program in Korea, and local governments and health related organizations are developing and providing health promotion programs for the public. The purpose of this study was to explore the effectiveness of a smoking prevention video for adolescents. There was no educational video on smoking prevention for school students in Korea. Based on funding from the National Health Promotion Fund, an educational video on smoking prevention has been developed and tested for the effectiveness. The primary subject of the video was middle school students. The result of the study has shown that there was a statistically significant difference between pre-and post test among middle school students and elementary school students. Students' knowledge on the harm of smoking has been increased after the video education. In addition, more than half of the students responded that the educational video was interesting and helpful. To reduce initiation of smoking among adolescents, both policies and school health education programs should be reinforced. Policies on tobacco should focus on demand of smokers and potential smokers. So government officials should keep monitor obedience of the law on prohibition of smoking for those under age 19. School teachers should focus on social influence of smoking for adolescents.
Purpose: This study was to evaluate effect of an educational program based upon experiment for smoking prevention in middle school students. Method: A quasi-experimental design was used. The participants were 107 middle school students (53 in the experimental group, 54 in the control group). Students in the experimental group participated in three 45-minute educational programs over a period of 2 weeks. The dependent variables were measured before and 2 & 4 weeks after the treatment. The data were analyzed using $x^2$ test, Fisher's exact test, t-test, and repeated measures ANOVA. SPSS was used to aid analysis. Results: 1) The experimental group showed higher levels of knowledge of smoking prevention compared to the control group. However this program did not affect their attitude towards smoking. 2) There was a higher possibility of smoking in the future among the students who had smoked than those who had not. 3) The majority (84.9%) of the experimental group said that the program was effective. Conclusion: Knowledge of smoking for the experimental group was increased, so it is assumed that the program itself had a positive effect on the students. To change attitudes regarding smoking, smoking prevention programs need to be conducted repeatedly, not just temporarily.

Adolescent psychosocial smoking prevention programs have been successful, but limited in the magnitude of program effects on the reduction of smoking behaviors. This is primarily due to the mixed results of independent studies with program variations. This systematic, quantitative research synthesis is designed to identify program key factors that are likely to increase program effects. The present study examined school-based psychosocial smoking prevention programs (1978-1997)among students in grades 6 to 12 in the United States. Theoretical formulations in this analysis of mediating effects posit that program impact on knowledge and/ or skills is associated with less smoking behaviors among program participants. Knowledge factors investigated in this study included health effects, social consequences, social influences, and social norms. Skill factors included affective skill, self-efficacy, assertiveness, self-control, problem solving, and refusal skill. The findings from this study highlight the importance of social consequences knowledge factor and assertive communication skill factor in decreasing adolescent smoking behaviors.
