  • Although adolescents usually show peculiar aspects in oral health status, there have been not so many studies about it, while there have been innumerable studies for children. This study aimed at evaluating the prevalence of dental caries of adolescents in Yangsan in Republic of Korea, detecting its contributing factors, and distinguishing the high-risk group by epidemiological mass survey. Besides, the significance including early caries lesion in diagnosis criteria was assessed with minimal invasive concept. This survey was performed on 1,371 adolescents aged 14 to 16 in Yangsan as subject examinees by a single trained examiner. The dental caries status was checked by oral examination at schools and questionnaire survey was accompanied for some relevant risk factors. The data were analyzed to yield the results as follows: Regarding the caries experience, the DMFT score by WHO criteria with ECL including or not was 4.79 and 3.97 respectively. In both the criteria, female showed higher values and that of high-risk group was almost twice compared with whole examinees by Sic Index. About pit & fissure sealed rate, it revealed the tendency that the more sealed teeth, the lower DMFT values. Among the studied risk factors, only the habit of brushing before going to bed showed positive relationship (p < 0.05).

  • Many studies on the prognosis of avulsed teeth were published but follow-up studies on Korean children and adolescents are insufficient. The aim of this study was to identify clinical factors determining the healing and prognosis of permanent incisors that were replanted after avulsion injury. This study included 184 permanent teeth in 142 patients aged 6-14 years. The clinical and radiographic data of the patients were analyzed from electronic medical records. Immature teeth had a higher probability of pulp revascularization than mature teeth. When extra-alveolar time was longer than 60 minutes, the incidence of inflammatory root resorption (IRR) was higher in immature teeth than mature teeth. However, the incidence of replacement root resorption (RR) was higher in mature teeth than immature teeth under the same condition. The incidence of IRR was increased when pulp was extirpated more than 20 days after replantation. IRR had a more significant influence on the decrease of survival expectation than RR. There was no significant correlation between the root development stage and survival expectation. However, when RR occurred, immature teeth survived shorter than mature teeth. Long term follow-up results from this study are expected to be used as fundamental data for the treatment guidelines and evaluation of the prognosis of replanted teeth.

  • Background: Orbital fractures are the most common pediatric facial fractures. Treatment is conservative due to the anatomical differences that make children more resilient to severe displacement or orbital volume change than adults. Although rarely, extensive fractures may result in enophthalmos, causing cosmetic problems. We aimed to establish criteria for extensive fractures that may result in enophthalmos. Methods: We retrospectively reviewed the charts of patients aged 0-15 years diagnosed with orbital fractures in our hospital from January 2010 to February 2019. Computed tomography images were used to classify the fractures into linear, trapdoor, and open-door types, and to estimate the defect size. Data on enophthalmos severity (Hertel exophthalmometry results) and fracture pattern and size at the time of injury were obtained from patients who did not undergo surgery during the follow-up and were used to identify the surgical indications for pediatric orbital fractures. Results: A total of 305 pediatric patients with pure orbital fractures were included-257 males (84.3%), 48 females (15.7%); mean age, 12.01±2.99 years. The defect size (p=0.002) and fracture type (p=0.017) were identified as the variables affecting the enophthalmometric difference between the eyes of non-operated patients. In the linear regression analysis, the variable affecting the fracture size was open-door type fracture (p<0.001). Pearson's correlation analysis demonstrated a positive correlation between the enophthalmometric difference and the bony defect size (p=0.003). Using receiver operating characteristic curve analysis, a cutoff value of 1.81 ㎠ was obtained (sensitivity, 0.543; specificity, 0.724; p=0.002). Conclusion: The incidence of enophthalmos in pediatric pure orbital fractures was found to increase with fracture size, with an even higher incidence when open-door type fracture was a cofactor. In clinical settings, pediatric orbital fractures larger than 1.81 ㎠ may be considered as extensive fractures that can result in enophthalmos and consequent cosmetic problems.

  • The purpose of this study was to compare the color of maxillary primary central incisors, lateral incisors, canines with the color of various composite resins using a spectrophotometer VITA Easyshade®V. One researcher measured the color of sound maxillary primary anterior teeth of 100 children aged 1 to 6 and specimens which were made by composite resins with a total of 10 types and 31 shades, and CIE L*, a*, b* values were obtained. There was no clinically recognizable color difference between the maxillary primary central and lateral incisors. However, the maxillary primary canines showed difference in color above the clinical acceptability threshold with the primary central and lateral incisors. These findings showed no significant color differences between men and women, and no significant color changes with age. A1 shade is the most common in all maxillary primary anterior teeth. Composite resins, which are thought to be similar to the color of maxillary primary anterior teeth were selected from 9 types of maxillary primary central incisors, 6 types of primary lateral incisors, 6 types of primary canines.

  • Purpose: Eating disorders often result in somatic complications, including cardiac abnormalities. Cardiac abnormalities may involve any part of the heart, including the cardiac conduction system, and can lead to sudden cardiac death. The current study aimed to evaluate the incidence of cardiac complications in pediatric patients with eating disorders and their associated factors. Methods: We retrospectively analyzed patients aged 10-18 years who were diagnosed with DSM-V (Diagnostic and Statistical Manual of Mental Disorder-V) eating disorders and underwent electrocardiography (ECG) and/or echocardiography between January 2015 and May 2020. Results: In total, 127 patients were included, of whom 113 (89.0%) were female. The median body mass index (BMI) was 15.05±3.69 kg/m2. Overall, 74 patients (58.3%) had ECG abnormalities, with sinus bradycardia being the most common abnormality (91.9%). Patients with ECG abnormalities had significantly lower BMI (14.35±2.78 kg/m2 vs. 16.06± 4.55 kg/m2, p<0.001) than patients without ECG abnormalities, as well as lower phosphorus and higher cholesterol levels. Among the 46 patients who underwent echocardiographic evaluation, 23 (50.0%) had echocardiographic abnormalities, with pericardial effusion being the most common (60.9%). The median left ventricular mass (LVM) and ejection fraction were 67.97±21.25 g and 66.91±28.76%, respectively. LVM and BMI showed a positive correlation (r=0.604, p<0.001). After weight gain, the amount of pericardial effusion was reduced in 3 patients, and 30 patients presented with normal ECG findings. Conclusion: Cardiac abnormalities are relatively frequent in patients with eating disorders. Physicians should focus on this somatic complication and careful monitoring is required.

  • As physical fitness measured was muscle strength (hand grips, leg extention, back lift, and arm pull and thrust), skinfold thickness (5 different sites), circulatory function (resting heart rate and blood pressure), speed (kinesiological analysis during 100m sprint, record, maximal and final speed), motor function (50 meter dash, ball throwing, standing broad jump, and pull-ups), maximal aerobic power (maximum oxygen intake by field running method), muscle power (leg and arm by inertia ergometer), and general endurance (maximum endurance running time on the treadmill at the speed of 5 MPH and grade of 15.5%) of 1131 Korean children (boys 572, girls 559) aged of 6 to 17 years, who were randomly sampled from 24 primary, middle and high schools at the two districts of Seoul and KyungKi. The results are summarized as follows: 1) The status (height and weight) of the children was almost same as that of the previously reported Korean and Japanese children of same ages. 2) Muscle strength was a gained linearly with geting age in the boys and girls but there was a little improvement in girls aged of 13 years or more. 3) The mean skin fold thickness was increased linearly with geting ages in both sexes, but the girls from 12 to 17 years of age were increased rapidly, and maximum value was 17mm, while boys was 7.0 mm. 4) In the circulatory function, the resting heart rate was decreased, but the blood pressure was increased with ages in both sexes within the normal limits. 5) The maximum and final speed during 100 meter sprint increases with age in boys but girls who are 12 years old or older, were not improved any mere. The patterns of running were same in both sexes, and maximum speed reached at about 30 meters from starting line. 6) The motor function was increased with age in both sexes, but there was no improvement in 12 years of age or older girls. More over records of all functions except standing broad jump was less than those of Japanese in the same age, respectively. 7) The maximum oxygen intake (MOI) was increased considerably with ages and maximum values were 2.93 L/min (boys) and 2.09 L/min (girls) at the age of 17years. This result was almost same as that of the Japanese and Easter Island population, but the value was lower than that of Europe. The average of the maximum oxygen intake per kg body weight per minute from 9 to 17 years of age were around 53 ml in the boys and 42 ml in the girls. 8) Muscle power was increased linearly with ages in boys while there was relatively a little increment in girls. The maximum values of leg muscle in boys and girls at the 17 years of age were 0.168 and 0.088 horse power, respectively. 9) The maximum endurance running time was increased considerably from the age of 9 in boys, while there was no improvement in girls. The maximum values were 6.0 min and 1.8 min, respectively.

  • Purpose : We aimed to investigate the clinical manifestations of and antibiotic resistance in culture-proven childhood salmonellosis. Methods : Clinical manifestations and laboratory data of and antibiotic use in subjects with culture-proven childhood salmonellosis, who were treated at the Chonbuk National University Hospital between September 1998 and August 2008, were analyzed retrospectively. Patients with underlying diseases or concomitant illnesses were excluded. Results : We assessed a total of 72 patients. There were 68 stool culture-positive cases, 7 blood culture-positive cases, and 3 both stool culture- and blood culture-positive cases. Salmonella group D was the most frequent pathogen in stool (63.9%) and blood (71.4%) cultures. Salmonella typhi was isolated in 1 case. Of the 72 patients, 45 (62.5%) were male children, of which 29 (40.3%) were aged <3 years. The patients most commonly presented with diarrhea (90.2%) and fever (83.3%). Leukocytosis (leukocyte count, >$15,000/{\mu}L$) and leukopenia (leukocyte count, <$4,000/{\mu}L$) were detected in 8.3% and 5.6% of the patients, respectively. Elevated serum C-reactive protein concentration (>5 mg/dL) and erythrocyte sedimentation rate (>20 mm/h) were observed in 88.9% and 58.3% of the patients, respectively. Fifty-two (85.2%) of 61 patients who had undergone antibiotic treatment received a third-generation cephalosporin as definitive antibiotic therapy. Multidrug resistance rate was 40.0 % in the first 5 years of the study and 71.4% in the last 5 years. No fatalities occurred in this series. Conclusion : Children with culture-proven salmonellosis showed relatively benign clinical outcomes. Appropriate antibiotic treatment of <2 weeks is probably adequate for those without a suppurative focus of infection. The incidence of antibiotic resistant isolates was recently seen to increase.

These kinds of diseases can be prevented simply by changing one's lifestyle to be more healthy. As a result of realization our interest in general health has become stronger. The most basic environment for human-being in society is the home. Humans secure their livelihood, physically. mentally, and socially at home. Therefore health care at home is very important. In modern society the responsibility for this task is traditionally given to housewives. The purpose of this study was to measure the degree of the health knowledge, health concern, health behavior and family health care of the married women and to analyze its related factors. The subjects for this study. 1,100 married women who studied at social education institutes and who had children attending an elementary school or a kindergarten, were surveyed with questionnaires. The preliminary survey was carried out from Aug. 7, to Aug. 19, 1995. With complement of questions, the main survey was carried out from Sep. 11, to Sep. 30, 1995. The data was analysed by using the SAS program. The results were as follows. 1. General Characteristics (1) In the individual characteristics of the respondents, the married women aged 30-39 were 54.8%, the average age was 39.8 years old. 33.8% of respondents had 6-10 years of marriage period, and the average marriage period was 14.9 years. Most of them(96.5%) lived with their husband. Those who graduated from college and graduate school were 53.4%. And 68.3% of respondents had no job. (2) In the family characteristics, 69.3% of the married women had 3 or 4 family members and the average family size was 4.1 person. 60.0% of the respondents had 2 children. Most of the respondents(90.9%) had no married children. 84.8% of the respondents lived with their parents. Those who reported that the total family income was more than 2,500,000 won a month were 32.3%. When making the decisions, 68.5% of the married women discussed the family matter with their husband. (3) In the individual characteristics of the respondents, 51.5% answered they were in good health. 61.7% of the married women answered they obtained the health knowledge through mass media. 24.3% of the women answered they had patients in their family in these days. 67.5% of the respondents answered they could generally control their health by themselves. 2. The Health Knowledge, Concern and Behavior. (1) For the health knowledge, the average score was 11.8. The lowest percent of correct answer(27.8%) was in the item about the skin tests for tuberculosis. And the highest percent(97.%) was in the item about taking a rest. (2) For the health concern, the married women had the highest concern about washing hands. But they were indifferent to smoking. (3) For the health behavior, the highest score was in "changing socks and underwear everyday", and the lowest one was in "taking a regular dental examination". 3. The Family Health Care (1) For the family health care, the item of "using a drug with the order of doctor or pharmacist" had the highest grade(4.78), and "consulting with the family physician about the health problem" had the lowest grade(2.03). (2) Older women and the women with a longer period of marriage had the highest level of the family health care(p<0.001). The married women who had 3 children had the highest level of the family health care(p<0.001). Those who had 5 or 6 family member and higher income had the highest level had the high level of the family health care(p<0.01). Women in good health and those who had the health knowledge from health experts had a high level of the family health care. (3) For the correlation of the family health care and other variables, the health behavior showed the highest correlation with family heath care practice(r=0.74) and the second was health concern(r=0.43). The variables which could explain the family health care were health behavior, the health concern and married women's health status(r²=55.87). The most closely associated with family health care was health behavior(r²=54.93)

  • Korean society is experiencing rapid changes which are certain to shape the lives of the older people and their families. So, the purpose of this study was to improve the quality of the elderly welfare service in Korea. Even though the interest about the elderly welfare in Korea is rapidly on the increase, the reality of Korean Society still leaves much to be desired, quantitatively as well as qualitatively. Pay attent to this point, now going to grasp the situation of not the handicapped elder people but the general elder people actual life condition and demand for welfare. So this study focused on the demand of not out-of home service but in-home care service. It was based on the data from by Korea National Statistical Office, having observed 6,139 elderly people. The results were as follows. They hope to get the elderly welfare services about health examination service, nursing survice, supporting service for household. That was, the most of them wanted in-home care service than out-of home care service. To ensure effective care to the elderly, it needed development and settlement of welfare service in face of their daily living. And, even though they recognized that they should cover the elderly life expenses themselves, the rate was hit that indicated their adult children as a parents supporter. This means that we should consider not only to the elder people but also to the family which contained the old people as the elderly welfare service. As a remedy, we can find the cooperation between elder welfare service at the social welfare organization and counsel, education at the family strengthen center. To improve the quality of the elderly welfare service in Korea, the elderly welfare service should focus not only on the physical and spiritual health maintenance but also active understanding living environment and growing ability to arbitrate between individual and living.

  • Dental caries is one of the main factors to cause the teeth to be lost. Diet is the important factor in the development of dental caries. Today, Dental plaque control, Fluoride to pical application, Fissure sealing and Diet control are used to prevent dental caries. By the five day diet diary, the author surveyed the food in take of 600 infants aged from 10 to 12 in the subures of Seoul. Using the cariogenic food intake analysis form, the detergent food intake analysis form and the basic food intake analysis form, the data were collected, analysed and discussed. After discussing the results, the author concluded as follows : 1. The intake frequency of meals per day was 4.91 times, of which 2.74 times taken at meals and 2.17 times at between meals. Girls(5.00) had taken more times than boys(4.69) at meals an between meals. 2. The intake frequency of cariogenic food per day was 1.93 times, Liquid cariogenic food was taken 0.05 times at meals, and 0.58 times at between meals. Solid cariogenic food was taken 0.05 times at meals, and 0.08 times at between meals. Girls(1.67) had taken cariogenic food more times than boys(1.46). 3. The percentage of intake without detergent food of each intake time per head per day was 71.62% at meals, and 85.7% at between meals. The highest percentage was at evening meals. Boys(44.00%) had taken more detergent food than girls(56.71%). 4. Both boys and girls had the basic food intake taken enough only in 2nd group of basic food, lacking in the other 4 groups. Girls had taken the basic food comparatively more times than boys.
