자율형 고등학교의 자기주도적 학습전형 운영 실태와 개선방안 (Improvement of Screening System of Self-initiated Learning in Self-governing High Schools)

  • The purpose of this study is to explore improving ways of screening of self-directed learning in self-governing high schools for expanding self-initiated learning screening system. To achieve the above aim, it was used survey method to 99 admission teachers of 12 self-governing high schools and analyzed responses with SPSS program. The research findings are as follows. To expand self-initiated learning screening system in self-governing high schools, it needs to obtain admission members of entrance committee, pooling of external screening members, quick decision of school admission members, increasing payment, training and understanding of the system and consideration of admission documents and study plan. Some improving ways of the expanding of self-initiated learning screening may be listed as follows: 1) development of screening model for self-governing schools 2) policy consideration for self-governing public high school 3) expanding autonomy of school 4) supporting budget 5) sharing of operation case and reinforcement of public relations.

Breast, Cervical, and Colorectal Cancer Screening Status of a Group of Turkish Women

  • Introduction: The aim of the study was to determine the breast, cervical, and colorectal cancer screening rates and the influencing factors in a group of Turkish females. Methods: This descriptive study was conducted in a School of Nursing. The study sample consisted of 603 females who were the mothers/neighbors or relatives of the nursing students. Data collection forms were developed by the investigators after the relevant literature was screened and were used to collect the data. Results: Of the women aged 30 and over, 32.8% had undergone a pap smear test at least once in their life. Of those aged 50 and over, 48.2% had undergone mammography at least once and FOBT had been performed in 12% of these women in their life. Having heard of the screening tests before, knowing why they are done, and having information on the national cancer screening program were important factors influencing the rates of women having these tests done. Discussion: The results of this study show that the rates of women participating in national cervical, breast, and colorectal cancer screening programs are not at the desired levels. It is suggested that written and visual campaigns to promote the service should be used to educate a larger population, thus increasing the participation rates for cancer screening programs.

Socioeconomic Inequalities in Cervical and Breast Cancer Screening among Women in Korea, 2005-2015

  • Purpose: Consistent evidence indicates that cervical and breast cancer screening rates are low among socioeconomically deprived women. This study aimed to assess trends in cervical and breast cancer screening rates and to analyze socioeconomic inequalities among Korean women from 2005 to 2015. Materials and Methods: Data from the Korean National Cancer Screening Survey, an annual nationwide cross-sectional survey, were utilized. A total of 19910 women were finally included for analysis. Inequalities in education and household income status were estimated by slope index of inequality (SII) and relative index of inequality (RII), along with calculation of annual percent changes (APCs), to show trends in cancer screening rates. Results: Cervical and breast cancer screening rates increased from 54.8% in 2005 to 65.6% in 2015 and from 37.6% in 2005 to 61.2% in 2015, respectively. APCs in breast cancer screening rates were significant among women with higher levels of household income and education status. Inequalities by household income in cervical cancer screening uptake were observed with a pooled SII estimate of 10.6% (95% CI: 8.1 to 13.2) and RII of 1.4 (95% CI: 1.3 to 1.6). Income inequalities in breast cancer screening were shown to gradually increase over time with a pooled SII of 5.9% (95% CI: 2.9 to 9.0) and RII of 1.2 (95% CI: 0.9 to 1.3). Educational inequalities appeared to diminish over the study period for both cervical and breast cancer screening. Conclusion: Our study identified significant inequalities among socioeconomically deprived women in cervical and breast cancer screening in Korea. Especially, income-related inequalities were greater than education-related inequalities, and these were constant from 2005 to 2015 for both cervical and breast cancer screening.

Adaptive Recommendation System for Health Screening based on Machine Learning

  • As the demand for health screening increases, there is a need for efficient design of screening items. We build machine learning models for health screening and recommend screening items to provide personalized health care service. When offline, a synthetic data set is generated based on guidelines and clinical results from institutions, and a machine learning model for each screening item is generated. When online, the recommendation server provides a recommendation list of screening items in real time using the customer's health condition and machine learning models. As a result of the performance analysis, the accuracy of the learning model was close to 100%, and server response time was less than 1 second to serve 1,000 users simultaneously. This paper provides an adaptive and automatic recommendation in response to changes in the new screening environment.

Cost Effectiveness of Interventions to Promote Screening for Colorectal Cancer: A Randomized Trial

  • Objectives: Screening for colorectal cancer is considered cost effective, but is underutilized in the U.S. Information on the efficiency of "tailored interventions" to promote colorectal cancer screening in primary care settings is limited. The paper reports the results of a cost effectiveness analysis that compared a survey-only control group to a Centers for Disease Control (CDC) web-based intervention (screen for life) and to a tailored interactive computer-based intervention. Methods: A randomized controlled trial of people 50 and over, was conducted to test the interventions. The sample was 1224 partcipants 50-70 years of age, recruited from Kelsey-Seybold Clinic, a large multi-specialty clinic in Houston, Texas. Screening status was obtained by medical chart review after a 12-month follow-up period. An "intention to treat" analysis and micro costing from the patient and provider perspectives were used to estimate the costs and effects. Analysis of statistical uncertainty was conducted using nonparametric bootstrapping. Results: The estimated cost of implementing the web-based intervention was $40 per person and the cost of the tailored intervention was $45 per person. The additional cost per person screened for the web-based intervention compared to no intervention was $2602 and the tailored intervention was no more effective than the web-based strategy. Conclusions: The tailored intervention was less cost-effective than the web-based intervention for colorectal cancer screening promotion. The web-based intervention was less cost-effective than previous studies of in-reach colorectal cancer screening promotion. Researchers need to continue developing and evaluating the effectiveness and costeffectiveness of interventions to increase colorectal cancer screening.

Breast and Colorectal Cancer Screening and Associated Correlates among Chinese Older Women

  • Objective: To explore the participation rates for breast and colorectal cancer screening and identify associated correlates among elderly women. Methods: Logistic regressions were conducted using data collected in 2006 from 1,533 elderly women aged 60 years or above who had completed a screening instrument, the Minimum Data Set-Home Care, while applying for long-term care services at the first time in Hong Kong. Results: The participation rates for breast and colorectal cancer screening among frail older Chinese women were 3.7% and 10.8% respectively. Cognitive status was inversely associated with the likelihood of participation in screening (breast: OR = 0.66, 95%CI = 0.47-0.94; colon: OR = 0.81, 95%CI = 0.66-0.99), as was educational level with the likelihood of participation in breast cancer screening (no formal education: OR = 0.20, 95%CI = 0.06-0.61, some primary education: OR = 0.31, 95%CI = 0.10-1.00). Conclusion: The delivery of cancer preventive health services to frail older women is less than ideal. Cognitive status and educational level were important factors in cancer screening behaviour. Tailor-made strategic promotion programmes targeting older women with low cognitive status and educational levels are needed to enhance awareness and acceptance within this vulnerable group.

Information Engineering and Workflow Design in a Clinical Decision Support System for Colorectal Cancer Screening in Iran

  • Background: Colorectal cancer is a major cause of morbidity and mortality throughout the world. Colorectal cancer screening is an optimal way for reducing of morbidity and mortality and a clinical decision support system (CDSS) plays an important role in predicting success of screening processes. DSS is a computer-based information system that improves the delivery of preventive care services. The aim of this article was to detail engineering of information requirements and work flow design of CDSS for a colorectal cancer screening program. Materials and Methods: In the first stage a screening minimum data set was determined. Developed and developing countries were analyzed for identifying this data set. Then information deficiencies and gaps were determined by check list. The second stage was a qualitative survey with a semi-structured interview as the study tool. A total of 15 users and stakeholders' perspectives about workflow of CDSS were studied. Finally workflow of DSS of control program was designed by standard clinical practice guidelines and perspectives. Results: Screening minimum data set of national colorectal cancer screening program was defined in five sections, including colonoscopy data set, surgery, pathology, genetics and pedigree data set. Deficiencies and information gaps were analyzed. Then we designed a work process standard of screening. Finally workflow of DSS and entry stage were determined. Conclusions: A CDSS facilitates complex decision making for screening and has key roles in designing optimal interactions between colonoscopy, pathology and laboratory departments. Also workflow analysis is useful to identify data reconciliation strategies to address documentation gaps. Following recommendations of CDSS should improve quality of colorectal cancer screening.

Repeat Colonoscopy Every 10 Years or Single Colonoscopy for Colorectal Neoplasm Screening in Average-risk Chinese: A Cost-effectiveness Analysis

  • Background: The appropriate interval between negative colonoscopy screenings is uncertain, but the numbers of advanced neoplasms 10 years after a negative result are generally low. We aimed to evaluate the cost-effectiveness of colorectal neoplasm screening and management based on repeat screening colonoscopy every 10 years or single colonoscopy, compared with no screening in the general population. Methods and materials: A state-transition Markov model simulated 100,000 individuals aged 50-80 years accepting repeat screening colonoscopy every 10 years or single colonoscopy, offered to every subject. Colorectal adenomas found during colonoscopy were removed by polypectomy, and the subjects were followed with surveillance every three years. For subjects with a normal result, colonoscopy was resumed within ten years in the repeat screening strategy. In single screening strategy, screening process was terminated. Direct costs such as screening tests, cancer treatment and costs of complications were included. Indirect costs were excluded from the model. The incremental cost-effectiveness ratio was used to evaluate the cost-effectiveness of the different screening strategies. Results: Assuming a first-time compliance rate of 90%, repeat screening colonoscopy and single colonoscopy can reduce the incidence of colorectal cancer by 65.8% and 67.2% respectively. The incremental cost-effectiveness ratio for single colonoscopy (49 Renminbi Yuan [RMB]) was much lower than that for repeat screening colonoscopy (474 RMB). Single colonoscopy was a more cost-effective strategy, which was not sensitive to the compliance rate of colonoscopy and the cost of advanced colorectal cancer. Conclusion: Single colonoscopy is suggested to be the more cost-effective strategy for screening and management of colorectal neoplasms and may be recommended in China clinical practice.

폐암의 조기 암검진 여부에 미치는 요인: 폐암 검진 사업대상자를 중심으로 (Factors Affecting Early Cancer Screening for Lung Cancer: Focusing on Lung Cancer Screening Subjects)

  • Objectives: The purpose of this study is to examine the current status of cancer screening among subjects in the lung cancer screening cycle and to analyze the factors affecting the cancer screening of subjects in the lung cancer screening cycle. Methods: This study used the 'National Health and Nutrition Survey 7th Year (2017)' surveyed nationwide as the main data. The subjects are lung cancer screening projects, the dependent variable is early cancer screening, the independent variables are gender, age, marital status, household income level, education level, national health insurance type, private health insurance, The number of chronic diseases, general health examination, smoking status, drinking status, moderate intensity physical activity, stress perception rate, and weight control efforts were determined. Results: The results of this study showed that factors affecting early cancer screening of lung cancer screening subjects were gender, age, marital status, education level, national health insurance, smoking status, drinking status, moderate physical activity, and weight. Irrespective of the control effort, it was found that the private medical insurance, the number of chronic diseases, the medical examination, and the stress perception rate were affected. Conclusion: If the lung cancer screening subjects recognize the importance of early cancer screening themselves and create a social environment to increase their participation rate, lung cancer screening patients and their families will help them to live a healthy life.

고등학생 청각선별 결과 (Results of Hearing Screening in Senior High School Students)

  • 본 연구는 경산지역 남녀 고등학생을 대상으로 하고, 재검대상자 비율을 통해 청각 선별의 필요성을 고찰하고자 한다. 연구 대상은 여자 고등학생 359명, 남자 고등학생 205명 등 모두 564명으로 하였다. 청각선별은 고막운동성계측(tympanometry)과 자동화 이음향방사(automated otoacoustic emission; AOAE)를 이용하였다. 재검대상자 중 위양성 소견을 보인 여학생 8명(2.228%) 11귀와 남학생 9명(4.39%) 10귀를 제외하면 최종 재검대상자는 여학생이 9명(2.5%), 남학생이 19명(9.268%)으로 각각 관찰되었다. 고막운동도와 AOAE는 남녀 사이 차이가 나타나지 않았다. 결론적으로 청소년 청각선별은 이관 성장 발달에도 불구하고 순음청 각선별로 확인하기 어려운 난청을 선별하기 위하여 고막운동성계측을 시행할 필요가 있고, 위양성 결과를 낮추기 위한 노력이 필요하다.
