Gardner syndrome has head and neck manifestations that may be recognized during dental visits. Features such as multiple gnathic osteomas, impacted supernumerary teeth, and multiple foci of idiopathic osteosclerosis can be easily identified on dental radiographs, prompting the clinician to refer the patient for further investigation. A dental examination and routine radiographs play a vital role in revealing the extracolonic presentation of Gardner syndrome, which facilitates timely screening and detection of colorectal cancer and other malignancies associated with this condition. This report discusses the case of a 50-year-old Caucasian man who presented with a hard swelling of the left angle of the mandible and was diagnosed with Gardner syndrome based on abnormal findings from an oral examination, dental imaging, and medical and family history.
Purpose : This study was designed to evaluate the efficacy and utility of panoramic radiograph for the improvement of the periodic oral examinations. Materials and Methods : Clinical examinations and panoramic examinations were done for the 242 subjects of oral examinations. The results of panoramic radiograph interpretation were compared with the clinical findings. Two questionnaires were created. One was carried out before the panoramic examination and the other done afterwards, to find out the subjects' cognition and satisfaction for the clinical and panoramic examinations. Results : 1. Panoramic findings showed a higher detection rate of 31.9% for periodontal diseases, and 23.1% for dental caries than clinical findings. 2. The additional abnormalities detected through panoramic examinations were impacted tooth in 81 subjects (33.6%), maxillary sinus abnormalities in 28 subjects (11.6%), condylar abnormalities in 5 subjects (2.1%), congenital and acquired dental anormalies in 59 subjects (24.5%), and other miscellaneous abnormalities in 34 subjects (14.1%). 3. 164 subjects (67.8%) were satisfied with the current periodic oral examination, and 75 subjects (31.1%) hoped for better accuracy. 4. In the first and second questionnaire, 154 subjects (67.0%) and 163 subjects (70.6%) responded respectively that panoramic examination was necessary, and 193 subjects (83.2%) responded that it actually helped. Conclusion : The panoramic examination was revealed to improve the effectiveness of the periodic oral examination and to increase the satisfaction of the subjects of examination.
Purpose: The practical examination portion of the National Dental Licensing Examination (NDLE) is slated to be administered in the latter half of 2021 in the form of a clinical performance examination that comprehensively evaluates the patient-dentist interaction using standardized patients (SPs). The SPs should be equipped with the basic qualities and capacity as evaluators for a fair and reliable administration of the test. Materials and Methods: In this study, we analyzed the existing training materials for SPs who participated in domestic and overseas practical tests for the development of training materials for SPs through seminars and surveys of 11 dentistry schools and colleges. Result: First, SPs should be selected according to the basic quality criteria and capacity, which they must possess, and the preliminary basic training about the details which they must have knowledge of and be provided through videorecorded cases before the implementation of the preliminary field training. Second, the roles of SPs and the calibration process of the evaluation result forms are needed when conducting the preliminary field training for SPs. After watching video-recorded scenario cases, the SPs participate in discussions about the watched videos before proceeding to calibration practices of evaluation result forms. Third, because the Type A questionnaire of the practical examination of the NDLE is dependent on the SPs' capacity and training, the fairness of the practical test is largely dependent on the SPs. Therefore, practicing the roles as evaluators and evaluation training should be provided using practical test items that can improve the reliability of the test and show a high level of reproducibility about the same case. Conclusion: The findings of this study will be utilized for the development of training materials for SPs, so they can participate in the administration of a fair and reliable practical examination of the NDLE.
Purpose: The purpose of this study is to investigate the improvement measure of dental technician practical examination. Methods: A survey was performed on 111 dental technicians who were self-written questionnaire. The results were analyzed by SPSS 12.0. The collected data was analyzed by frequency and chi-square test. Results: Questions most necessary work tools with the articulator was the highest with 55.9%, Electric wax carver(16.2%), heating clear(14.4%), CAD/CAM program(9.0%) was in the order. The need for additional practical test in response to actions included maxillary and mandibular comments occlusal work that you have to wax up the highest rate of 63.8%. Items most in need of improvement was strengthen of clinically oriented(48.0%), separation of written and practical(14.6%), control the degree of difficulty(11.4%) were the next order. Conclusion: Based on the results of this study, the current dental technician did not show major problems in the practical test. However, clinicians should be strengthened mainly. To do this, the use of the articulator is essential.
The purpose of this study is to provide the general history of fostering dentists in Japan and introducing their new roles. This research was conducted based on the government policy report on dentists and the information published by each educational institution. Based on the collected data, the official websites were used to represent the latest statistics of the institutions. The number of dentists in Japan has increased. The government established the National Examination for Dentists to guarantee the quality of dentists. After the standards for developing questions for the national examination were established in 1985, the contents of the examination have been appropriately improved by revising the standards every four years. This improvement has required dental students to study a variety of subjects for six years at dental school. Since dentists in Japan are required to respond to various demands from the nation; the Model Core Curriculum for Dental Education was developed to teach medical ethics and abilities to ensure that dentists conduct themselves professionally. Recently, the roles of dentists have been changing in Japan. When providing dental services to older patients over the age of 65, dentists and other dental professions focus on maintaining oral functions, such as saliva secretion, bite force, tongue movement, and masticatory/swallowing functions. However, oral function-related services for children are different. In addition to providing essential dental services, dental practitioners also provide special treatment, such as oral muscle training, myofunctional therapy, health guidance, and space retainers to the child patients with developmental insufficiency in oral functions. Dentistry in Japan has undergone numerous changes over the years and has continued to offer high-quality dental health services. Thus, information gained from the Japanese experience may be helpful to dental professions in other developed countries for planning oral health measures.
Oral examination ensures early detection and treatment of oral diseases and improvement of quality of health-related life. This is imperative as it reduces individuals' dental medical expenses and social costs caused by diseases. However, as the low oral examination acceptance rate continues to be a problem, this study was conducted to identify the factors that influence the oral examination of an economically active population. In this study, 4,836 economically active individuals between the ages of 20 and 65 years were studied using data from the 7th National Health and Nutrition Survey in 2018. Logistic regression analysis was performed to determine the factors influencing demographic and sociological characteristics on oral examination after adjusting for complaints of mastication and speech discomfort. As a result of confirming the factors influencing the oral examination, increase in age tended to be inversely proportional to the oral examination. Income level, type of job, and job position were identified as factors that influenced oral examinations. Compared to the group with a high socioeconomic status, which is represented by a group with a high income level or a stable job type or job position, the group with low economic status was found to have a negative effect on oral examination. Oral management of vulnerable groups, who might not be considered while devising oral health management policies, should be promoted through the development and provision of oral health management policies that consider employment status and environment.

Orthognathic surgery often requires intraoral orthodontic appliances that are fixed directly to the bone, which can complicate nasotracheal intubation if the devices protrude into the nasal cavity. This case report describes a 19-year-old man scheduled for elective orthognathic surgery who experienced recurrent cuff tears of the endotracheal tube during nasotracheal intubation due to protruding orthodontic screws in the palate. Despite initial attempts at nasotracheal intubation through the left nostril, the cuff of the 7.0 mm internal diameter (ID) Ring, Adair & Elwyn (RAE) tube repeatedly ruptured, with identical rupture patterns observed. Facial CT revealed that the orthodontic screws had protruded into both nasal cavities with significant visibility in the sagittal, coronal, and transverse views. Fiberoptic examination of the left nasal passage identified a firm protrusion below the inferior turbinate, corresponding to the location of the screw, which likely caused the cuff tears. Intubation was successfully performed via the right nostril during the fiberoptic examination. This case highlights the critical importance of evaluating intraoral corrective devices using comprehensive craniofacial imaging before anesthesia induction, as well as conducting fiberoptic examinations during intubation to avoid complications and ensure patient safety.
Purpose : To evaluate the efficacy of panoramic radiography by comparing the results of clinical examination with radiographic findings. Materials and Methods: We studied 190 patients (20 men and 170 women; mean age, 40 years; range, 22 to 68 years) who visited the health promotion center of Korea Medical Science Institute and were examined both clinically and by panoramic radiography. We compared results from both examinations. Treatment options by clinical examination were described as "no treatment indicated", "treatment of dental caries", "removal of calculus", "treatment of periodontal disease", "prothodontic treatment" and "extraction of the third molar". Findings taken from the panoramic radiography were: dental caries, periapical lesion, alveolar bone loss, calculus deposition, retained root, impaction of the third molar, disease of maxillary sinus, bony change of mandibular condyle, etc. Results: The prevalence of panoramic findings were: 37.9% of dental caries, 17.4% of periapical lesions, 44.7% of alveolar bone losses, 62.6% of calculi deposition, 7.9% of retained roots, 26.8% of third molar impactions, 6.3% of diseases of maxillary sinus, 2.1 % of bony changes of mandibular condlye and 35.8% of miscellaneous lesions. Abnormal conditions revealed by panoramic radiography which had not been discovered on clinical examination were: 24.2% of the patients had dental caries, 17.4% had periapical lesions, 7.4% had calculi deposition, 5.3% had retained roots, 15.3% had third molar impactions. The opposite cases were: 5.2% had dental caries, 12.6% had calculi deposition, and 9.5% had third molar impactions. Conclusion: The use of panoramic radiography as a supplement to the clinical examination might be a valuable screening technique.
Objectives : This study was to prepare basic data about middle and high school students' school loss due to oral diseasea and to investigate the relevant factors. Methods : The survey is conducted for 575 middle school students in Busan and Gyeongnam. School loss was investigated based on the experiences of absences and leaving school early, which had happened from oral diseasea. The independent variables were oral health behaviors and sociopeconomic factors such as sex, age of their parents, educational level of their parents, family income. Factors related with school loss was analyzed by the multiple logistic regression method. Results : The experience ratio of leaving school early to the dental clinic or having difficulty in studying was higher in the case of high school student than in middle school student case. The parameters related with absence or leaving school early for oral disease were the education level, the distinction of sex, fear about medical examination and the standard of living. The reasons of absence or leaving early for visiting the dental clinic were related with education level, the distinction of sex, fear about medical examination, distrust of oral care and the satisfaction of oral health. The parameters having effect on school performance were education level, sex, fear about medical examination, distrust of oral care, cost burden, interests in oral health by the parents and school record. Conclusions : The oral health promotion should be developed to decrease school loss for students.
Purpose: The purpose of this study is to investigate the improvement measure of dental technicians practical examination system. Methods: The subjects in this study were 290 dental technicians who were self-written questionnaire. The results were analyzed by SPSS 16.0. The collected data was analyzed by frequency and chi-square test. Results: Dental technician practical exam feasibility of survey were largely negative views. Articulators work required in practical exam as a tool used (28.7%), electric wax carver (19.5%), heating clear (17.8%), handpiece (16.2%) had a response. Dental Technician positions in accordance with practical exam in comparison to satisfaction, the professor is satisfied, but the clinical dental technicians were generally dissatisfied (p<0.05). Conclusion: Based on the results of this study, Current is necessary to analyze the practical test. It also should be compared with foreign institutions. Long-term and short-term improvement examination system that can be applied to the improvement of the dental technician to draw practical test it is necessary to improve the system.
