  • Objectives: This study was conducted to evaluate the impact of a School-based Depression Awareness Education Program on depression knowledge and help-seeking orientation for adolescent. Methods: The students of two high schools were allocated to the intervention (n=221) or control group (n=223) in this cluster-randomized trial. The school-based depression awareness education was a 3-hour program. The intervention group received pre, post and follow up tests while the control group received pre and follow up tests after two months. Depression knowledge, attitude to friends'depression, and help-seeking for professional assistance was evaluated at each test. Differences in the two month change between the intervention and control groups were analyzed by t-test or chi-square test. Results: There were positive changes in depression knowledge, response to friend's depression, and attitude to professional help-seeking in the post test compared to the pretest among intervention group. Improvement of depression knowledge was greater in the intervention group than in the control group (P=0.011), but there were no significant differences in response to friend's depression (P=0.268) or help-seeking orientation (P=0.263) after two months. Conclusions: School-based Depression Awareness Education resulted in significant positive changes in depression knowledge in high school students, but made no attitudes difference to professional help-seeking.

학령기 아동을 위한 신체활동-행동수정 비만관리 프로그램이 대사증후군 위험인자에 미치는 효과 분석 (The Effects of a Physical Activity-Behavior Modification Combined Intervention(PABM-intervention) on Metabolic Risk Factors in Overweight and Obese Elementary School Children)

  • Purpose: The purpose of this study was to identify the effects of a physical activity-behavior modification combined intervention(PABM-intervention) on metabolic risk factors in overweight and obese elementary school children. Methods: Thirty-two participants ($BMI\geq85$ percentile or relative $obesity\geq10$) were allocated to the PABM-intervention group and behavior modification only intervention group. The PABM -intervention was composed of exercise intervention consisting of 50 minutes of physical activity(Hip-hop dance & gym-based exercises) twice a week and the behavior modification intervention consisted of 50 minutes of instruction for modifying lifestyle habits(diet & exercise) once a week. Effectiveness of intervention was based on waist circumference, BP, HDL-cholesterol, TG, and fasting glucose before and after the intervention. Results: The proportion of subjects with 1, 2, 3 or more metabolic risk factors were 28.1, 43.8, and 15.6%, respectively. After the 8-week intervention, waist circumference, systolic BP, diastolic BP, and HDL-cholesterol changed significantly(p<.01) in the PABM group. Conclusion: This provides evidence that a PABM-intervention is effective in changing metabolic risk factors such as waist circumference, systolic BP, diastolic BP, and HDL-cholesterol in overweight and obese elementary school children.

Effects of the Instrument Pilates Exercise Based on the Schroth Exercise on the Cobb's Angle, Angle of Trunk Rotation and Low Back Pain in Patients with Idiopathic Scoliosis: A Single Subject Study

  • Purpose: This study examined the effects of the application of Schroth exercise-based instrument Pilates exercise on the Cobb's angle, angle of trunk rotation, and low back pain of female patients with idiopathic scoliosis. Methods: Three patients with idiopathic scoliosis at a Cobb's angle of 20° or more participated in this study. Among the single-subject experimental research designs, a reversal (ABA) design was performed. In particular, Schroth exercise-based instrument, Pilates exercise, was performed for 10 weeks, consisting of five weeks between the baseline and intervention one period and five weeks between the intervention 1 and intervention 2 periods, and then followed up for five weeks. Results: After the Schroth exercise-based instrument, Pilates exercise, was applied, the Cobb's angle and the angle of trunk rotation decreased compared to the baseline in all subjects, and the follow-up period also showed a continuous decline. After Pilates exercise was performed, low back pain in subjects 1 and 2 was decreased in the intervention 1 period compared to the baseline. The level of low back pain in the intervention 2 period increased compared to the intervention 1 period, but a reduction was noted in the follow-up period. The low back pain in the subject was decreased in all intervention periods and the follow-up period. Conclusion: Schroth exercise-based Pilates exercise improves the Cobb's angle and the angle of trunk rotation for female patients with idiopathic scoliosis in their teens and 20s, and an effective intervention method is proposed for low back pain.

Comparative Study on the Effectiveness of a Health Promotion Program Using School Forest and a Traditional School-based Health Promotion Program in Elementary Students

  • Purpose: This study compared the effects of two six-week school-based intervention programs - a health promotion program using a school forest and a traditional school health promotion program (TSHPP) - on physical and mental health among elementary school students. Methods: A total of 73 students participated in the study: 21 students in the 6-week school forest program conducted in a rural area and 52 students in the 6-week TSHPP conducted in an urban area. Children's health promotion behavior, depression and hyperactivity were measured using a self-report questionnaire. To assess children's physical health, body mass index (BMI), body fat percentage, and heart rate variability (HRV) were used. Results: Overall, both intervention programs improved participants' physical and mental health. Both programs significantly decreased the body fat percentage; this effect was more prominent in the TSHPP group. Only the TSHPP significantly decreased the participants' BMI after the intervention. The school forest group showed significantly improved relaxation and diminished hyperactivity; the TSHPP group showed significantly improved health promoting behavior and social relationship after the intervention. Comparing the two groups'post-pre difference scores, the two groups significantly differed only in social relationship. Both group showed significantly improved depression after the intervention. Conclusion: These findings support the effectiveness of these 6-week school-based health promotion programs in improving physical and mental health among school-aged children.

병원기반 금연중재가 뇌경색 환자의 퇴원 후 12개월 지속 금연율에 미치는 효과 (The effect of a hospital-based smoking cessation intervention on the continuous abstinence rate from smoking over 12 months in patients with cerebral infarction)

  • Objectives: The aim of this study was to investigate the effectiveness of a hospital-based smoking cessation intervention for increasing continuous abstinence rate from smoking in patients with cerebral infarction. Methods: One-hundred and two smokers with cerebral infarction who decided to quit smoking were enrolled in the smoking cessation intervention from December 2012 to February 2015. The smokers underwent six consecutive times of individual intervention with nurse specialist on smoking cessation including education on behavioral modification, counseling for withdrawal symptoms, and anti-smoking advice over a 12-month period. Results: Among the total participants, the continuous abstinence rate from smoking changed from 79.4% at 1 month to 60.8% at 12 months after discharge. The continuous abstinence rate from smoking after 12 months was 88.5% in participants who completed the entire program (6 times), while 51.3% in participants who did not complete the entire program (${\leq}5$ times) (P=0.001). After adjustment for general and smoking-related characteristics, complete implementation of hospital-based smoking cessation intervention was significantly associated with continuous abstinence from smoking after 12 months (odds ratio: 5.93; 95% confidence interval: 1.45-24.22). Conclusions: The hospital-based smoking cessation intervention might be effective for smoking cessation in patients with cerebral infarction, especially when the intervention was implemented thoroughly.

Process Evaluation of a Mobile Weight Loss Intervention for Truck Drivers

  • Background: In a cluster-randomized trial, the Safety and Health Involvement For Truck drivers intervention produced statistically significant and medically meaningful weight loss at 6 months (-3.31 kg between-group difference). The current manuscript evaluates the relative impact of intervention components on study outcomes among participants in the intervention condition who reported for a post-intervention health assessment (n = 134) to encourage the adoption of effective tactics and inform future replications, tailoring, and enhancements. Methods: The Safety and Health Involvement For Truck drivers intervention was implemented in a Web-based computer and smartphone-accessible format and included a group weight loss competition and body weight and behavioral self-monitoring with feedback, computer-based training, and motivational interviewing. Indices were calculated to reflect engagement patterns for these components, and generalized linear models quantified predictive relationships between participation in intervention components and outcomes. Results: Participants who completed the full program-defined dose of the intervention had significantly greater weight loss than those who did not. Behavioral self-monitoring, computer-based training, and health coaching were significant predictors of dietary changes, whereas behavioral and body weight self-monitoring was the only significant predictor of changes in physical activity. Behavioral and body weight self-monitoring was the strongest predictor of weight loss. Conclusion: Web-based self-monitoring of body weight and health behaviors was a particularly impactful tactic in our mobile health intervention. Findings advance the science of behavior change in mobile health intervention delivery and inform the development of health programs for dispersed populations.

학령기 과체중, 비만 아동 중재에 관한 통합적 문헌고찰 (An Integrative Literature Review of Interventions for School-aged Overweight and Obese Children)

  • Purpose: This study aimed to identify the characteristics of an intervention program for school-aged overweight and obese children through an integrated literature review. Methods: We used five databases (MEDLINE [PubMed], CINAHL, Embase, DBpia, and RISS) to search for studies published between January 1, 2013, and April 18, 2018. The studies were reviewed according to specific inclusion and exclusion criteria. Results: Finally, nine studies were selected and classified into three types: school-based, community-based, and home-based. Specific intervention methods were identified, including exercise, diet, exercise and diet, and cognitive behavioral therapy. Conclusion: Most studies have shown the effectiveness of obesity intervention programs. However, the variables, tools, and methods of measuring the effectiveness of intervention varied, making it difficult to compare the intervention studies. Therefore, future studies should develop objective and integrated measurement tools and evaluate the effects with long-term follow-up.

Effect of nutrition education and traditional game-based physical activity interventions, on lipid profile improvement in overweight and obese children in West Java Indonesia

  • BACKGROUND/OBJECTIVES: One ongoing effort to curtail dyslipidemia in school children is through lifestyle intervention. This study analyzes outcomes of the intervention of nutrition education and Javanese traditional game-based physical activity, on lipid profiles of overweight and obese children. SUBJECTS/METHODS: The experimental study consisted of three steps: data collection (October to December 2016), intervention processes (January to March 2017), and final report preparation (April to May 2017). This was a split-plot block study, enrolling a total of 72 subjects. Traditional game intervention (12 meetings) and nutrition education (9 meetings) was carried out within 3 months with material related to obesity. RESULTS: Our results showed no decrease in the levels of total cholesterol in the group receiving nutrition education (rise by 1.56 mg/dL), but when compared to subjects without intervention, total cholesterol levels experienced was increased. The group which did not receive physical activity showed increase in the total cholesterol, whereas the traditional game intervention group showed a decrease of 0.05 mg/dL total cholesterol at the end of the intervention. Nutrition education and physical activity interventions did not influence any reduction in the low-density lipoprotein cholesterol (LDL-C) levels, but rather showed a tendency to increase at the end of the intervention. However, the increase in LDL-c levels in the traditional game group was lower than in other intervention groups. Duncan's test indicated that the effect of nutrition education and physical activity interventions did not differ from the cut-off values of LDL-C and high-density lipoprotein cholesterol. CONCLUSIONS: Total cholesterol and triglyceride levels in the traditional game group tended to decrease at the end of the intervention, but the results did not differ much from other intervention groups. We deduce that nutrition education and traditional game-based physical activity interventions are not capable of improving blood lipid profiles in overweight and obese children within 3 months. It is necessary to increase the time of physical activity intervention to maintain the lipid profile in another study.

모바일폰을 이용한 초등학생 비만관리 복합지원의 잠재적 이로움 : 프로그램 제공자 측면에 대한 질적 연구 (Leveraging Multimodal Supports using Mobile Phones for Obesity Management in Elementary-School Children: Program Providers' Perspective from a Qualitative Study)

  • Objectives: This study was conducted to investigate providers' perspectives on current challenges in implementing a program for prevention and management of childhood obesity and adoption of mobile phone as a potential solution of leveraging multimodal delivery and support in a school setting. Methods: The qualitative data were collected through face-to-face in-depth interviews with 23 elementary-school teachers, 6 pediatricians, and 6 dieticians from community health centers and analyzed using a qualitative research methodology. Results: Current challenges and potential solutions of obesity-prevention and -management program for obesity program for elementary school children were deduced as two themes each. Lack of tailored intervention due to limited recipient motivation, lack of individualized behavioral intervention, and different environmental conditions can be solvable by mobile technology-based personalized intervention which brings about interactive recipient participation, customized behavioral intervention, and ubiquitous accessibility. Lack of sustainable management due to stigmatization, limited interactions between program providers and inconsistent administrative support can be handled by multimodal support based on school setting using mobile platform providing education of health promoting behaviors toward larger scale and interactive networking between program participants, and minimizing administrative burden. Conclusions: Adoption of mobile-based health management program may overcome current limitations of child obesity program such as lack of tailored intervention and sustainable management via personalized intervention and multimodal supports although some concerns such as increased screen time need to be carefully considered in a further study.

A Systematic Review on Oral Health Care Programs for the Elderly in Korea (2009~2020)

  • Background: Various oral health management programs in Korea affect the oral health improvement in the elderly Several studies have been conducted to date; however, those studies have not shown uniform results due to the differences in research methods or designs. Hence, this study aimed to review the overall research trends of the reported oral health care programs for the elderly in Korea, verify their effects, and clarify them based on the systematic literature review. Methods: The literature search selected intervention studies that applied the oral health care program for the elderly in Korea from 2001 to 2020. Following the COre, Standard, and Ideal (COSI) models presented by the US National Library of Medicine, we selected databases including Korean studies Information Service System (KISS), ScienceOn, Research Information Sharing Service (RISS), DBpia, PubMed, and Google Scholar. Of the 1,335 studies searched using keywords, titles, and abstracts, 21 were finally selected based on primary and secondary exclusion criteria. Results: The most frequent intervention period was 4 weeks, and the number of interventions varied between 2 and 90 times. As for the type of intervention, 14 studies that conducted both theory and practice were the most frequent. Significant differences in the clinical indicators, such as calculus, halitosis, salivation rate, swallowing function, and dry mouth, were found in most oral health care programs. Conclusion: Based on the results of this study, the intervention program needs further verification using multiple indicators in future studies. In addition, a study extending the intervention period and the number of samples is considered necessary for verifying continuous effectiveness of the intervention program.
