The Effectiveness of Smoking Cessation Program in Adults: Systematic Review of Randomized Controlled Trials (성인 흡연자의 금연 프로그램 효과: 무작위대조군 실험연구의 체계적 문헌고찰)

  • Objectives: The purpose of this study was to determine if smoking cessation programs (SCPs) are effective for adults through a systematic review of the results of such programs in large randomized controlled trials (RCTs). Methods: The PICO (Patient, Intervention, Comparator, Outcome) strategy was established, 1,160 literature from domestic and foreign electronic databases was reviewed, and 22 references were selected based on the inclusion and exclusion criteria. The quality of each reference was evaluated using the Scottish Intercollegiate Guidelines Network tool, and meta-analysis was carried out. Results: The SCPs were significantly effective for adult smokers. Smoking cessation counseling, education, and smoking cessation medications such as nicotine patch were more effective than the other interventions. However, the results showed short-term effects (within six months), and differences were observed among the SCPs. For the outcome measures for SCPs, the abstinence rate of seven days was mainly used, but differences were identified between the CO level and the cotinine-verified abstinence rate of smoking cessation. Conclusions: For a smoking cessation program for adult smokers, the strength of the evidence of the program's effectiveness in RCTs that provide the identified intervention strategies should be considered.

Effects of Goal-Oriented Functional Tasks on Gross Motor Function and Activities of Daily Living in Children with Cerebral Palsy -A Single Case Study-

  • Purpose: The study sought to determine whether goal achievement, performance, satisfaction, activities of daily living, and gross motor function in children with spastic cerebral palsy are positive affected by goal-oriented functional task training. Methods: In a single case study, two 7-year-old girls diagnosed with spastic cerebral palsy were given functional task training based on individualized goals for 24 60-minute sessions over a period of 6 months. The individualized goals reflected the needs of both children and parents. The Canadian occupational performance measure (COPM) and the goal attainment scale (GAS) were used to measure goal performance and satisfaction before and after intervention. Independence of activities of daily living before and after intervention were assessed using the functional independence measure for children (Wee-FIM). And change of gross motor function were assessed using gross motor function measure (GMFM). Results: Clinically significant changes were observed in COPM and GAS, as well as positive changes in independence of gross motor function and activities of daily living. Conclusion: The findings indicate that goal-oriented functional task training addresses the needs of children with spastic cerebral palsy, which increases treatment satisfaction and has a positive effect on independent activities and participation in daily life.

A Concept Analysis of the Quality of Life for the Children (아동의 삶의 질에 대한 개념 분석)

  • Purpose: The purpose of this study was to attempt an accurate understanding of the quality of life for the children and provide basic materials for application to nursing research, intervention and further theoretical development. Method: Data obtained from such research methods as the lexical and literature review were analysed according to Walker and Avant's(1995) process of concept analysis. Results: As a result of analysis, it was found that the attributes of the concept of the quality of life for the children included value/acceptance, family relationship, safety, good relationship with friends, right of children, school life, parent relationship, leisure, economical stability, not child abuse, and health. There was the coexistence between antecedent element and the outcomes of subjective judgement - health, family, economic strength, education, friends, welfare, social-cultural environment which indicates the coexistence of positive and negative emotions. Conclusion: nursing intervention should be provided for children by concentrating focus on the coordination of internal and external situations in order to prompt them to express their life with positive emotions. Experiential reference was used to suit the purpose of this study based on the use of objective and subjective indicators, and the development of the instrument considering the children were proposed because of the absence of the unified measurement instrument.

  • 학생들의 기초 기본학습 능력의 신장이 강조되고 있는 교육상황에 비추어 볼 때 지금까지 간과되어 왔던 학습장애에 대한 재인식과 학습장애아들을 위한 대책이 필요하다. 그 대책으로 최근 특수교육분야에서 기존의 지능-불일치 교육이론에 대한 대안으로 제시되고 있는 중재-반응 (Responsiveness to Intervention : RTI) 이론을 기반으로 학습장애아들에 대한 교육이 보다 효율적으로 적용될 수 있다. 하지만 실제 우리의 교육에 적용을 위해서는 많은 걸림돌들이 존재하고 있다. 실제 교육 현장의 교사들이 RTI 이론 적용의 걸림돌로 제기하는 점들을 해결하기 위한 대안으로 모바일 시스템설계를 통해 실제 현장에서의 RTI 이론 적용 및 실행 가능성을 높이고자 한다. 본 시스템의 특징은 첫째, 실제 국내의 교육현장에서 기존 교사들이 제시한 RTI 이론 적용가능성이 낮은 항목들을 중점으로, 이론 적용의 문제점을 해결하기 위한 설계에 중점을 두고 있다. 둘째, 시스템의 구성을 기존의 행정중심이 아닌 실제 수행자 중심 설계를 위하여 기존의 학교행정가를 제외한 일반교사, 특수교사, 학부모, 학습장애 학생으로 설계하였다. 셋째, 일반교사, 특수교사, 학부모, 학습장애 학생의 의사소통 및 진전도 체크 등을 모바일 기기의 편이성과 결합하여 학습장애아의 학습능력을 향상시키고자 한다.

Relationship between Postural Balance Training and Fall Risks for Elderly: a Systematic Review of Randomized Controlled Trials

  • Objective: Falling is one of main accident to facilitate the physical injuries in order adults. The purpose of the systematic review was to determine the effects of postural balance training whether the recovery of falls in elderly with normal physical function or not throughout summing the selected studies quantitatively. Design: A systematic review Methods: MEDLINE and other four databases were searched up to April 20, 2021 and randomized controlled trials (RCTs) evaluating postural balance approaches on fall risks in elderly. The researched studies excluded the double studies, titles and abstract, and finally full-reported study. The selected RCTs studies were extracted characteristics of the studies and summary of results based on PICOS-SD (population, intervention, comparison, outcomes, and setting- study design) model to synthesize the papers qualitatively. Results: The review involved 22 RCT reports with 4,847 community older adults aged 65 years or over. Nineteen of the selected RCT studies reported dual or multimodal exercises show the beneficial effect for older adults compared to one-type treatment or no intervention. All of selected showed low risk in the selection, attrition, and reporting bias. However, detection bias showed low risk at 75% records of the involved RCTs and performance bias was low risk at only three records. Conclusions: The results of the systematic review propose that a standardized therapeutic approach and the intensity are needed for improving risk of falls in older adults.

Development and Evaluation of Integrated Management Program for Hemodialysis Patients (혈액투석 환자를 위한 통합적 관리 프로그램의 개발 및 효과)

  • Purpose: This study aimed to develop and evaluate an integrated management program to enhance self-efficacy, compliance with sick-role behaviors, symptom management, and biomarker indication in hemodialysis patients. Methods: The integrated management program was developed through a systematic review of literature, analysis of relevant online data, and expert validation. It comprised 480 min of video-based education delivered eight times over four weeks, supplemented by weekly phone consultations and text message support from a nurse. To evaluate the program's effectiveness, it was administered to 44 patients with hemodialysis in a single group in a pre-post test experimental study. Changes in self-efficacy, sick-role behavior compliance, dialysis symptom index, and biomarkers were assessed. Results: The program yielded statistically significant improvements in self-efficacy (t=-7.13, p<.001), sick-role behavioral compliance (t=-7.35, p<.001), dialysis symptom index (t=4.32, p<.001), and blood urea nitrogen levels (t=2.55, p=.014) among the participants. Conclusion: The integrated management program is an effective intervention for improving hemodialysis patients' self-efficacy, compliance with sick-role behaviors, and experience of symptoms. Additionally, it is considered an intervention with high clinical applicability and efficiency through video reproducibility.

Development of tailored nutrition information messages based on the transtheoretical model for smartphone application of an obesity prevention and management program for elementary-school students

  • BACKGROUND/OBJECTIVES: Easy access to intervention and support for certain behaviors is important for obesity prevention and management. The available technology such as smartphone applications can be used for intervention regarding healthy food choices for obesity prevention and management in elementary-school students. The transtheoretical model (TTM) is comprised of stages and processes of change and can be adopted to tailored education for behavioral change. This study aims to develop TTM-based nutrition contents for mobile applications intended to change eating behaviors related to weight gain in young children. SUBJECTS/METHODS: A synthesized algorithm for tailored nutrition messages was developed according to the intake status of six food groups (vegetables, fruits, sugar-sweetened beverages, fast food and instant food, snacks, and late-night snacks), decision to make dietary behavioral changes, and self-confidence in dietary behavioral changes. The messages in this study were developed from December 2014 to April 2015. After the validity evaluation of the contents through expert consultation, tailored nutrition information messages and educational contents were developed based on the TTM. RESULTS: Based on the TTM, stages of subjects are determined by their current intake status, decision to make dietary behavioral changes, and self-confidence in dietary behavioral changes. Three versions of tailored nutrition messages at each TTM stage were developed so as to not send the same messages for three weeks at most, and visual materials such as figures and tables were developed to provide additional nutritional information. Finally, 3,276 tailored nutrition messages and 60 nutrition contents for applications were developed. CONCLUSIONS: Smartphone applications may be an innovative medium to deliver interventions for eating behavior changes directly to individuals with favorable cost-effectiveness. In addition, using the TTM for tailored nutrition education for healthy eating is an effective approach.

An Association between Menarche and Nutritional Status of Female Students of A Rural Primary School

  • The association of menarche and nutritional status was studied in 116 female students of the 6 th grade in a rural primary school. Participants were divided into two groups based on menarcheal status. The anthropometric data showed that mean heights and weights of menarcheal group on two occasions were significantly higher than those of the other group (p<0.01). Neither hemoglobin levels nor hematocrit values for determination of anemia were not associated with menarche. Twenty four hour dietary recall revealed that young females with menarche consumed less energy and Ca compared to the other group. Ca intake was 34.8% of RDA in menarcheal group. It might be suggested that effective intervention strategies need to be developed and include education programs for nutritional needs and food sources of Ca, targeting rural residents.

  • The purpose of this study is to understand smartphone addiction of adolescent by SEM analysis. This study was analyzed relations of self-esteem, negative parenting attitudes, school adjustment and peer attachment based on the KCYPS 2014. As a result of negative parenting attitudes and peer attachment is a factor that increases the smartphone addiction, self-esteem and school adjustment factors were analyzed by reducing the smartphone addiction. So it suggested clinical and policy intervention for reducing the smartphone addiction in adolescents.

  • Oral mucositis or stomatitis produced by stomatotoxic chemotherapy and/or radiation therapy are painful, restrict oral intake and, importantly, act as sites of secondary infection and potals of entry for the endogenous oral microflora often leading to bacteremias or sepsis. A number of clinical observations and studies of animal model suggests a pathophysiological complexity in the development of mucositis. The condition appears to represent a sequential interaction of the oral mucosal cells and tissues, pro-inflammatory cytokines, and local environmental factors in the mouth. This article discussed and reviewed biological process of the mucositis and, the role of cytokines as initiators and amplifiers of the process. The recognition that the pathophysiology of mucositis is a multifactorial process has presented opportunities for intervention based upon biological attenuation.
