조현병 외래환자의 회복에 대한 견해와 회복 의지 (Opinions on the Recovery of Outpatients with Schizophrenia and Their Wills to Recover)

  • Objectives: To examine the opinions regarding recovery and assess their will to recover in outpatients with schizophrenia. Methods: Total 98 stable outpatients with schizophrenia were enrolled. To investigate the outpatients' opinions regarding recovery, we asked the subjects to write down the ways they could think they had recovered from their current disease. We classified their answers and chose final 39 statements. Using this questionnaire, they were asked to what extent they agreed with each statement. Moreover, the Recovery Assessment Scale (RAS) was used to measure the subjects' will to recover. Results: Many of the 39 subjects who were surveyed about their opinions on recovery agreed on 11 statements about taking medicine, social life, daily life, social functioning, positivity about life (happiness, hope, confidence, and tranquility), symptoms, occupation, and self-control. The RAS score showed a significant correlation with age (r=-0.26, p<0.01), age at onset (r=0.26, p<0.01), duration of illness (r=-0.52, p<0.01), and number of hospitalization (r=-0.46, p<0.01). Conclusion: This study shows that positivity about life is an important part of the patients' subjective recovery criteria in schizophrenia. Therefore, if clinicians pay more attention to this part of subjective recovery, it would strengthen the patients' will for recovery.

조현병 외래 환자의 아동기 외상과 대사증후군 및 신체 건강과의 관계 (Childhood Trauma, Metabolic Syndrome, and Physical Health among Outpatients with Schizophrenia)

  • Objectives Childhood trauma increases substantial risks for later developing not only mental health issues including psychotic illnesses such as schizophrenia but also physical illness. In this study, possible associations of childhood trauma with metabolic syndrome and physical heath indices were tested among outpatients with schizophrenia. Methods A final sample of 46 adult outpatients with schizophrenia was recruited from an outpatient psychiatric unit of Hanyang University Guri Hospital. Participants completed the Childhood Trauma Questionnaire-Short Form (CTQ-SF), laboratory tests and physical measurement including Body Mass Index (BMI) and Waist to hip ratio (WHR). The Clinical Global Impression (CGI) scale and the Global Assessment of Functioning scale (GAF) were also administered. Results We did not find significant correlations between total scores of childhood trauma and any of these variables, but physical neglect was negatively associated with BMI (r = -0.329, p = 0.026) and waist circumference (r = -0.304, p = 0.040). Conclusions In this preliminary study, we noted that subtypes of childhood trauma could contribute to physical health status separately. Clinicians need to consider the possibility that childhood trauma may affect physical health as well as psychological aspect of schizophrenic illness.

조현병 외래 환자에서 병식 모순 (The Insight Paradox among Outpatients with Schizophrenia)

  • Objectives: The primary objective was to test the phenomenon known as the insight paradox, which refers to the association between higher levels of insight and lower self-esteem, higher hopelessness, and a higher perception of social prejudice among patients with schizophrenia. Methods: A total of 216 outpatients with schizophrenia according to the DSM-IV diagnostic criteria were recruited. All participants were receiving ongoing outpatient treatment and were in a stable phase of the disorder. The participants were divided into a good-insight (N=109) and a poor-insight (N=107) group by the direct interview and the mean score of questionnaire. The parameters used for analysis and comparison were demographic variables (age, gender, education), clinical variables (age at onset, number of hospitalizations), self-esteem, hopelessness, self-stigma, and quality of life. Results: Compared with the poor-insight group, the good-insight group was found to have a lower number of hospitalizations, lower self-esteem, higher hopelessness, lower quality of life, and a higher level of internalized stigma. Conclusion: The insight can cause the negative consequences in patients with schizophrenia. Therefore, clinicians should pay attention to the negative aspects of insight for improving the quality of life in the treatment of patients with schizophrenia.

조현병 환자의 범죄에 대한 대중의 관심과 조현병 환자의 정신의료서비스 이용과의 상관관계 (Public Attention to Crime of Schizophrenia and Its Correlation with Use of Mental Health Services in Patients with Schizophrenia)

  • Objectives: This study was performed to examine the effects of the public attention to 'crime of schizophrenia' on the use of mental health services in patients with schizophrenia using big data analysis. Methods: Data on the frequency of internet searches for 'crime of schizophrenia' and the patterns of mental health service utilization by patients with schizophrenia spectrum disorders by month were collected from Naver big data and the Health Insurance Review and Assessment Services in Korea, respectively. Their correlations in the same and following month for lagged effect were examined. Results: The number of outpatients correlated negatively with public attention to 'crime of schizophrenia' in the same month. The lagged relationship between public attention and the number of admissions in psychiatric wards was also found. In terms of sex differences, the use of outpatient services among female patients correlated negatively with public attention in the same month while the number of male patients' admissions in both same and following month correlated positively with public attention. Conclusion: These findings suggested that public attention to 'crime of schizophrenia' could negatively affect illness behavior in patients with schizophrenia.

퇴원한 정신분열병 환자와 가족에 대한 지역사회복지관에서의 단기가족교육 프로그램의 효과 (The Effectiveness of Brief Family Education Programs for Schizophrenic Outpatients and their Families in a Community Social Welfare Center.)

  • 본 연구에서는 퇴원한 정신분열병 환자와 가족에 대한 지역사회복지관에서의 단기가족교육 프로그램의 효과를 알아보기 위하여 전형적인 실험연구인 전-후 시험 통제집단설계를 이용하여 그 개입효과를 검증하였다. 본 연구의 결과, 지역사회복지관에서의 단기가족교육 프로그램이 정신분열병 환자 가족의 정신분열병에 대한 병적 지식, 가족의 정신분열병 환자에 대한 거부감과 불안 그리고 정신분열병 환자가 인지하는 가족의 정서적 지지에서 긍정적인 개입효과가 있는 것으로 검증되었다. 이러한 연구결과들을 볼 때, 지역사회복지관에서의 단기가족교육 프로그램이 정신분열병 환자의 치료와 재활 그리고 환자 가족의 안정에 긍정적인 영향을 미치는 지역사회 자원으로서 적극 활용될 수 있을 것이다. 이에 본 연구에서는 전문 인력의 확보, 보다 다양한 프로그램 내용의 개발, 프로그램 종료 후의 가족모임에 대한 지원 등을 지속적인 연구와 함께 제안하였다.

조현병 환자의 아동기 외상과 정신작용약물 사용 경향에 대한 예비 연구 (A Preliminary Study of Childhood Trauma and Patterns of Psychotropic Medication Use in Patients with Schizophrenia)

  • Objectives Experience of early childhood abuse elevates the risk of developing schizophrenia in later period of life, incidence of psychiatric comorbidity, symptomatic severity and complexity. In this context, we hypothesized that the pattern of psychotropic medication used would reflect this; those with childhood trauma will received more types and higher doses of psychotropic medication. Methods From our database of 102 outpatients diagnosed with Diagnostic and Statistical Manual of Mental Disorders, Fourth Edition (DSM-IV) schizophrenia, we analyzed experiences of childhood trauma measured by the Childhood Trauma Questionnaire-Short Form and types and dose of prescribed psychotropic medication. Results We found significant positive correlations between child sexual abuse and the number of psychotropic medications (p = 0.029) and between child emotional neglect and the number of psychotropic medications other than antipsychotics (p = 0.045). Conclusions This preliminary study suggests that the pattern of psychotropic use may be affected by types of childhood trauma. Further studies will have to shed light on mediating factors such as symptoms or comorbid conditions that lead to prescription of certain psychotropic class.

정신과 외래 환자에서의 신경 심리 검사 (Neuropsychological Tests in Psychiatric Outpatients)

  • Clinical neuropsychological tests were developed originally for the diagnosis of neurological and neuro-surgical diseases. Recently, these tests are being introduced to psychiatric patients. Authors had the experience to use these tests in pychiatric outpatient clinic. Results were as follows. There was a significantly increase in language and attentional function in residual schizophrenia compared to normal control. In chronic neurosis, as visuospatial function was reduced, language and attentional functions were enhanced. With these results, authors suggest that application of neuropsychological tests in psychiatric patients may be very helpful in classifying the subgroups of disease, in selecting the modality of treatment, and in expecting prognosis.

정신분열병 환자의 인지 기능 재활에 대한 보드게임 요법의 효과 분석 (The Effects of Board-Game Therapy on Cognitive Rehabilitation in Patients with Chronic Schizophrenia)

  • Objectives : In order to explore effects of board-game therapy for cognitive rehabilitation in patients with schizophrenia, we investigated the change of executive cognitive function over a 2-month period of board-game therapy in patients with schizophrenia. Methods : Two groups of chronic schizophrenic inpatients were participated in this study. One group(n=21) were treated with board-game therapy for 2 months and the other control group(n=19) were not treated. For the evaluation of the executive cognitive function, a Wisconsin Card Sorting Test(WCST) was administered before and after the introduction of the board-game therapy. PANSS score change was also evaluated. Result : At the beginning of this study, there was no significant difference in performance of cognitive function tests, demographical data or clinical severity between both patient groups. After 2 months of treatment with the board-game therapy, the board-game therapy group showed significant improvements of executive cognitive function without any significant change of their schizophrenic symptoms. On the contrary, there was no change in control group. Conclusion : This study showed that a board-game therapy is effective for the enhancement of executive cognitive function in patients with chronic schizophrenia. A board-game therapy could be introduced with ease into psychiatric fields, such as inpatients' or outpatients' clinic wards and day hospital. Our result indicates that the board-game therapy is a promising tool for the enhancement of cognitive function, especially executive cognitive function and helpful for cognitive rehabilitation for schizophrenic patients.

정신과 외래 환자가 지각하는 낙인과 자기효능감 및 삶의 만족도에 관한 연구 (Self-perceived Stigma, Self-efficacy and Quality of Life in Psychiatric Outpatients)

  • Purpose: The purpose of this study was to explore self-reported perceived stigma, self-efficacy, and quality of life among psychiatric outpatients Methods: In the present study, researchers analyzed the survey responses of 195 patients recruited in the S hospital psychiatry outpatient clinic. The measurement tools used in this study were the "Devaluation & Discrimination" scale to determine perceptions of stigmatization, general and social efficacy scales to measure self-efficacy, and the "General Well-Being Index" to measure quality of life in psychiatric outpatient. Statistical analysis included means with standard deviation, t-test, ANOVA and Pearson correlation coefficients to identify relationships between the variables. Results: There was a significant difference in self-perceived stigma and quality of life in psychiatric outpatients (r=-.233, p=.001) and a negative correlation between self-efficacy and quality of life (r=-.424, p=.000). Correlation analysis results support the hypothesis that higher self-perceived stigma scores were related to reduced quality of life among patients with schizophrenia (r=-.231, p=.021), while there was a positive correlation between self-perceived stigma scores and self-efficacy scores among patients with bipolar disorder (r=.362, p=.013). Conclusion: The findings suggest that nursing imtervention strategies should include education programs to reduce stigmatization and enhance self-efficacy and quality of life among patients with chronic psychiatric illnesses.

급성기 정신분열병의 치료로서 단기적인 안구운동 민감소실 및 재처리요법에 대한 예비연구 (A Pilot Study of Brief Eye Movement Desensitization and Reprocessing(EMDR) for Treatment of Acute Phase Schizophrenia)

  • Objectives : Eye movement desensitization and reprocessing(EMDR) is a novel, time-limited psychotherapy originally developed for treatment of psychological trauma. The effectiveness of this therapy has been validated only for posttraumatic stress disorder ; however, EMDR is often applied to other psychiatric illnesses, including other anxiety disorders and depression. This pilot study tested the efficacy of EMDR added to the routine treatment for individuals with acute stage schizophrenia. Methods : This study was conducted in the acute psychiatric care unit of a university-affiliated training hospital. Inpatients diagnosed with schizophrenia were randomly assigned to either three sessions of EMDR, three sessions of progressive muscle relaxation(PMR) therapy, or only treatment as usual(TAU). All the participants received concurrent typical treatments(TAU), including psychotropic medication, individual supportive psychotherapy and group activities in the psychiatric ward. The Positive and Negative Syndrome Scale(PANSS), the Hamilton Depression Rating Scale and the Hamilton Anxiety Rating Scale were administered by a clinical psychologist who was blinded to the patients' group assignment. Results : Forty-five patients enrolled and forty patients(89%) completed the post-treatment evaluation. There were no between-group differences in the withdrawal rates of patients during the treatment or at the three-month follow-up session. All three groups improved significantly across each of the symptomatic domains including schizophrenia, anxiety, and depressive symptoms. However, a repeated measures ANOVA revealed no significant differences among the groups over time. Effect size for change in total PANSS scores was also similar across treatment conditions, but effect size for negative symptoms was large for EMDR(0.60 for EMDR, 0.39 for PMR and 0.21 for TAU only). Conclusion : These findings supported the use of EMDR in treating the acute stage of schizophrenia but the results failed to confirm the effectiveness of the treatment over the two control conditions in three sessions. Further studies with longer courses of treatment, more focused target dimensions of treatment, and a sample of outpatients are necessary.
