The purpose of this study is to determine the inhibiting factors of dying well for people who want to have a good death. The final respondents in this study were sampled using stratified proportional allocation using a stratified random sampling method, and 1,000 adults aged between 19 and 75 years were selected. The questionnaire used consisted of four items on general characteristics and 20 items related to the inhibiting factors of dying well scored on a 7-point Likert scale. Analysis was conducted using descriptive statistics, correlation analysis, and decision tree analysis. Results showed that, among the inhibiting factors of dying well, "degenerative diseases (such as dementia)" and "loss of control (mental / physical)" scored 5.502 and 5.268 points, respectively; the highest significant positive correlation was found between "bad marital relationship" and "bad relationship with children," followed by "did not receive death education" and "lack of medical policy promotion (dying well)" and "bad relationship with children" and "indifference of others." Considering these findings, it appears that the whole society will make efforts to improve the perception and practice of good death, and life and death education will be expanded if death education for dying well is organized and implemented.

Background : Febrile neutropenia (FN) is one of the side effects in the patients treated with chemotherapy, and the patients who have FN generally need immediate treatment with extended-spectrum antibiotics and hospitalization. Pegfilgrastim and pegteograstim, which are used for the prevention of FN as a granulocyte-colony stimulating factor (G-CSF), have been granted insurance coverage in the Republic of Korea for certain breast cancer patients using doxorubicin and cyclophosphamide (AC) from September 2016. Methods : The data of the patients with breast cancer using AC regimen and G-CSF were collected retrospectively. This study involves cost-utility analysis of pegfilgrastim and pegteograstim. In this study, we constructed a simple decision tree model for short-term observation and calculated quality-adjusted life year (QALY) and the direct medical costs from the medical provider's perspective. Results : From September 2016 to May 2017, 15 patients were treated with pegfilgrastim and 15 patients were treated with pegteograstim. As a result of dividing the average cost by QALY for each treatment group, it was observed that pegfilgrastim and pegteograstim were consumed 24,923,384 won and 22,808,336 won per 1QALY, respectively. Consequently, incremental cost effectiveness ratio (ICER) showed 2,115,048 won more per pegfilgrastim than pegteograstim per 1QALY, and the cost per 1QALY of both the drugs was lower than 30,500,000 won; the Koreans were willing to pay this amount. Conclusions : This study suggests that pegfilgrastim and pegteograstim can be used to improve the quality of life of breast cancer patients undergoing AC therapy. Among the two drugs, pegteograstim seems to be more cost-effective. However, since this study was conducted as a retrospective observation method on a small scale, it is associated with many limitations. Therefore, a long-term prospective cohort study is needed to supplement the present findings.

Purpose: Traditional markets often have irregular space utilization patterns because the spaces are created and divided as time passed. Internally, there is high risk of fire due to problems such as aging facilities and high-density of stores and externally, there is high risk of fire spread since it is often adjacent to deteriorated residential and commercial facilities. Method: In this paper, on-site investigations were carried out to check fire risk factors and fire spread risk, and fire occurrence and expansion risk factors were investigated for traditional markets in Hwanghak-dong and Dong-daemun by using large-scale fire data from existing traditional markets. Result: As a result of the analysis, there are likely to be various problems such as high fire load and lack of safety awareness due to aging facilities and high-density of stores. In particular, it is necessary to prepare countermeasures because deteriorated residential facilities with narrow alleys around traditional markets have high fire spread. It is situation that while traditional markets mainly are managing for fire and disaster centering on the merchant association, the surrounding residential areas are not properly managed. Conclusion: It is necessary to manage deteriorated residential facilities with traditional markets, also to be linked early warning system and information to evacuate rapidly in case of fire there.

Background and Objectives Tinnitus retraining therapy (TRT) is one of the most effective treatment modalities of tinnitus based on the neurophysiological model proposed by Jastreboff and Hazell. This study was performed to evaluate the effect of counselor factor on treatment outcomes of TRT. Subjects and Method The total of 78 patients who had TRT from three different counselors in a tinnitus clinic of tertiary referral center from Jan 2015 to Dec 2015 were included in this study. Their medical records were retrospectively reviewed to evaluate the therapeutic response to TRT. Results Among 78 patients who were followed-up for more than 6 months, 47, 20, and 11 patients were treated by counselors A, B, C (all ENT specialists), respectively. Counselor A had 15-year-experience of TRT counseling, whereas counselor B and C were well trained but beginners of TRT counseling. Initial clinical characteristics including Tinnitus Handicap Inventory (THI) and tinnitus Visual Analogue Scale (VAS) scores of the patients among three groups were not significantly different. Treatment responses evaluated via THI and most of the tinnitus VAS scores after at least 6 months after TRT were significantly improved in all three groups (p<0.05) with no significant difference between the senior (A) and junior (B, C) group. Conclusion TRT seems to be an effective treatment modality of tinnitus even in this short term follow-up study. Treatment outcomes of TRT may not depend on the counselors once they are well trained and follow the same protocol.

The experiments in coastal engineering are very complex and a lot of components should be concerned. The experience has an important role in the successful execution. Hydraulic model experiments have been improved with the development of the wave generator and the advanced measuring apparatus. The hydraulic experiments have the advantage, that is, the stability of coastal structures and the hydraulic characteristics could be observed more intuitively rather than the numerical modelings. However, different experimental results can be drawn depending on the model scale, facilities, apparatus, and experimenters. In this study, two-dimensional hydraulic experiments were performed to suggest the guide of the test wave(random wave) generation, which is the most basic and important factor for the model test. The techniques for generating the random waves with frequency energy spectrum and the range for the incident wave height [(HS)M/(HS)T = 1~1.05] were suggested. The proposed guide for the test wave generation will contribute to enhancing the reliability of the experimental results in coastal engineering.

With the recent occurrence of large-scale earthquakes in Korea, the importance of seismic design has greatly increased. Seismic design standards stipulate that dynamic time history analysis be performed for important or special structures. In the seismic analysis and design of such structures, determining a rational design input seismic wave is a very important factor in ensuring the reliability of the analysis and design. In the seismic design standards, rational design seismic waves must reflect the characteristics of the area (fault) and satisfy the design response spectrum for each seismic performance level. This requirement can be partially satisfied by modifying the actual seismic wave measured in the area (fault) according to the design response spectrum. In this study, a method of correcting and generating seismic wave time histories according to the design response spectrum based on actual measured seismic waves using the harmonic wavelet transform was proposed. To examine the applicability of the proposed technique, the technique was applied to earthquakes of magnitude 5.8 and 5.4, respectively, that occurred in Gyeongju (2016) and Pohang (2017), and the seismic wave time histories corresponding to the design response spectrum were modified and generated.
With the aim of preventing safety accidents at construction sites, the company aims to create safe behaviors intended through variables called smart safety and health activities to help reduce industrial accidents. Purpose: It analyzes how smart safety and health activities affect accidents caused by unsafe behavior and changes in worker behavior, which is the root cause, and verifies the hypothesis that it helps prevent safety accidents and protect workers' lives. Method: Smart safety and health activities were selected as independent variables (X), and intended safety and anxiety, which are workers' behavioral intentions, were set as dependent variables (Y), attitude and subjective norms, and planned behavioral control as parameters (M). Exploratory factor analysis, discriminant validity analysis, and intensive validity analysis of safety and health activities were used to analyze the scale's reliability and validity. To verify the hypothesis of behavior change, the study was verified through Bayesian model analysis and MC simulation's probability density distribution. Result: It was found that workers who experienced smart safety and health activities at construction sites had the highest analysis of reducing unstable behavior and performing intended safety behavior. The research hypothesis that this will affect changes in worker behavior has been proven, the correlation between variables has been verified in the structural equation and path analysis of the research analysis, and it has been confirmed that smart safety and health activities can control and reduce worker instability. Conclusion: Smart safety and health activities are a very important item to prevent accidents and change workers' behavior at construction sites.
Lake Biwa Canal was a dream project that reminds us the passion and innovation of Kyoto Citizens more than water supply. It is a modernization project combining engineering knowledge and scientific technology, which started transportation by ship through big-scale civil construction as well as supplying electricity as the first waterpower plant in Japan, and it overcame the physiographic limit through adopting unique method of waterway transportation. Lake Biwa Canal, which has tangible and intangible culture heritage value as the traditional space of Kyoto, the Old Capital of 1200 years, conceives a cultural meaning that is connected through various mutual relation as well as scientific technologic factor. Lake Biwa Canal is not only the function for supplying water to gardens and temples of Kyoto region, but it is also a cultural fruit that is formed by complex causal relationship of various contents such as geographical and environmental background, the phases of the times, local development policy, political circumstances and religions. This study is aimed at interpreting the value of Lake Biwa Canal multilaterally by the convergence of cultural and technological aspects through the view of the world which the age tried to pursue, focusing on the construction of Lake Biwa Canal which was accomplished in the process of promoting the modernization of Kyoto.
Purpose: In order to reduce forklift industrial accidents, it is necessary to classify them into institutional, technical, and educational factors and conduct research on whether each factor affects disaster reduction. Method: Descriptive statistical analysis, validity analysis, reliability analysis, and multiple regression analysis were conducted using SPSS 18 program based on an offline questionnaire based on a 5-point Likert scale. Result: As a result of multiple regression analysis, it was found that institutional, technical, and educational factors, which are independent variables for disaster reduction, explain about 62.5% of the variance in disaster prevention, which is the dependent variable. The regression model verification was found to be statistically significant with F=118.775 and significance probability p<0.01. Conclusion: First, there is a need to prevent disasters by including electric forklifts weighing less than 3 tons in the inspection system. Second, there is a need to make it mandatory to install front and rear cameras and forklift line beams to prevent forklift collision disasters. Third, there is a need to conduct special training related to forklifts every year, and drivers and nearby workers need to be included in the special training for forklifts.

Objective : Recently, robotic-assisted spine surgery (RASS) has been considered a minimally invasive and relatively accurate method. In total, 495 robotic-assisted pedicle screw fixation (RAPSF) procedures were attempted on 100 patients during a 14-month period. The current study aimed to analyze the accuracy, potential risk factors, and learning curve of RAPSF. Methods : This retrospective study evaluated the position of RAPSF using the Gertzbein and Robbins scale (GRS). The accuracy was analyzed using the ratio of the clinically acceptable group (GRS grades A and B), the dissatisfying group (GRS grades C, D, and E), and the Surgical Evaluation Assistant program. The RAPSF was divided into the no-breached group (GRS grade A) and breached group (GRS grades B, C, D, and E), and the potential risk factors of RAPSF were evaluated. The learning curve was analyzed by changes in robot-used time per screw and the occurrence tendency of breached and failed screws according to case accumulation. Results : The clinically acceptable group in RAPSF was 98.12%. In the analysis using the Surgical Evaluation Assistant program, the tip offset was 2.37±1.89 mm, the tail offset was 3.09±1.90 mm, and the angular offset was 3.72°±2.72°. In the analysis of potential risk factors, the difference in screw fixation level (p=0.009) and segmental distance between the tracker and the instrumented level (p=0.001) between the no-breached and breached group were statistically significant, but not for the other factors. The mean difference between the no-breach and breach groups was statistically significant in terms of pedicle width (p<0.001) and tail offset (p=0.042). In the learning curve analysis, the occurrence of breached and failed screws and the robot-used time per screw screws showed a significant decreasing trend. Conclusion : In the current study, RAPSF was highly accurate and the specific potential risk factors were not identified. However, pedicle width was presumed to be related to breached screw. Meanwhile, the robot-used time per screw and the incidence of breached and failed screws decreased with the learning curve.
