노인간호요양시설의 QMI 개발과 현장 실태분석 (Development of Nursing Home Quality Management Index & Application in the Field)

  • Purpose: Expecting a new system of nursing security for elderly in operation in 2007, a substantial expansion of nursing facilities for elderly and quality management of those facilities are imperative now. Therefore. specific quality management measures or schemes which could be used for the operation of small-scale nursing homes are in urgent need. The purpose of this study was to develop a Quality Management Index(QMI) to guide Nursing Home management. Method: This study developed a QMI for small-scale nursing homes, which focused on quality management in structural dimensions such as environment, staffs, local community network, atmosphere, and quality management in process dimensions such as nursing, communication, resident satisfaction. The QMI developed in this study is based on extensive review of literature and the actual experiences of nursing home operation. It consists of 7 dimensions, 39 components and 148 indexes, which have been verified by three times validity tests of experts group. Then the QMI was used to evaluate the quality management levels of nursing homes in Korea. Result: In sum, after analyzing all nursing homes in Korea operated by nurses, this study found that there were significant differences of quality management level among facilities in several dimensions such as environment, staffs, local community connections, communication. In dimensions such as nursing, atmosphere, resident satisfaction, however, little differences of quality management level among facilities were reported. It is considered that this result has to do with several factors like the size of facilities, operation expenses, operation forms, the disposition of professional human resources, and philosophy and management policy of the owners. As far as generalization is concerned, however, it needs to be noted here that the sample size for this study is not statistically big enough to generalize the results. Conclusion: Having Developed a QMI for small-scale nursing home with 7 dimensions, 39 components and 148 indexes, this study is expected to be used in developing more elaborated quality evaluation tools for nursing homes, and also function as a practical guide of quality management for those who are opening and managing nursing homes. I hope this quality index could lead to further development of a standardized quality management index, and eventually contribute to quality improvement of nursing homes.

재가 뇌졸중 환자를 위한 자조관리프로그램이 일상활동, 근육 강도, 우울 및 삶의 만족도에 미치는 효과 (The Effects of Community based Self-help Management Program on the Activity of Daily Life, Muscle Strength, Depression and Life Satisfaction of Post-stroke Patients)

  • Stroke is one of the leading causes of death in Korea. Because of their sequelae, strokes are categorized as a sudden-onset, constant course chronic illness which needs continuous efforts for rehabilitation. Unfortunately, there are few community based rehabilitation program for post-stroke patients who stay at home. The authors developed a community based selp-help management program for post-stroke patients to enhance their rehabilitation process. The program consists of five sessions and each session contains health education. ROM exercise, ADL training, and stress management like foot reflexology. A professor and two graduate students of nursing college coordinated the program. To test the effects of the program we conducted a 5 week program to the 10 conveniently selected post-stroke patients who were living in Kang-buk district of Seoul. The Questionnaires about ADLs, IADLs, depression and life satisfaction were asked to the all subjects before and after program. The hand grisp power and muscle strength of four limbs were measured at the end of each sessions. The analysis of data revealed that the program was effective to increase the ADLs, IADLs, and muscle strength and to decrease the depression levels of subjects. However, there was no significant difference between pre and post hand grisp power and life satisfaction. Because the program was effective to Improve the physical and psycholocial function of subjects, we suggest continual development and Implementation of community based self-help management programs.

상급종합병원 간호·간병통합서비스병동과 일반병동 간호사의 시점별 직무만족도 및 삶의 질 비교연구 (A Comparative Study on the Nurses Job Satisfaction and Quality of Life in Integrated Nursing Care Service and General Wards in a Tertiary Hospital)

  • 본 연구는 간호·간병통합서비스병동(INCSW) 및 일반병동(GW) 간호사들의 직무만족도와 삶의 질을 비교, 분석하여 간호·간병통합서비스 제도를 도입한 의료기관의 간호 질 향상과 고품질의 간호환경조성을 위한 기초자료를 제공하고자 시도되었다. K 병원 INCSW와 GW에서 근무하는 간호사 88명 대상으로 총 3회에 걸쳐 6개월 동안 추적 조사하였다. 연구결과 INCSW와 GW는 6개월 동안 직무만족도에 유의미한 차이가 없는 반면, 삶의 질은 '전반적인 삶의 질'(1차 p=.033, 2차 p=.030)과 '전반적인 건강상태'(1차 p=.049) 항목에서 유의미한 차이가 있었다. 직무만족도 차수별 비교에서는 '조직별 인정 및 전문적 성취'(2/3차 p=.037)와 '존중과 인정의 인간관계'(1/3차 p=.005, 2/3차 p=.006) 항목에서 통계적으로 유의미한 차이가 있었다. INCSW의 삶의 질을 차수별로 비교한 결과 '전반적 삶의 질' 항목의 통계적으로 유의한 차이가 있었다(1/3차 p=.025, 2/3차 p=.010). 본 연구 결과를 토대로 간호 인력의 숙련도와 환자 요구도를 반영한 인력배치, 배치 전 직무 교육 및 조직문화 개선 등의 기간별 다양한 간호중재전략을 개발한다면 INCSW와 GW 간호사 직무만족도와 삶의 질 향상에 도움이 될 것이다.

중환자실 간호사의 의사소통능력과 인문학적 소양이 전문직 삶의 질에 미치는 영향 (Effects of Communication Skills and Humanistic Knowledge on Professional Quality of Life in Intensive Care Unit Nurses)

  • Purpose : The purpose of this study was to identify relation of the communication skills, humanistic knowledge and professional quality of life and to investigate the enhancing ways of professional quality of life in intensive-care unit nurses. Methods : The study design was a descriptive survey from 152 intensive-care unit nurses, from December 1 to 30, 2019. The data analysis was with ANOVA, Scheffé test, Pearson's correlation coefficient and Multiple regression using the SPSS/WIN 23.0 program. Results : The factors affecting compassion satisfaction were communication skills (β=0.42, p=.001), satisfaction for working (β=0.61, p=.001), marriage (β=0.23, p=.005), humanistic knowledge (β=0.47, p=.008) and these factors explained 35.6%. The factors affecting secondary traumatic stress were job stress (β=0.19, p=.011), number of patients per nurse (β=-0.36, p=.004) and these factors explained 9.4%. The factors affecting burnout were job stress (β=-0.25, p=.001), communication skills (β=-0.29, p=.001), working period(β=-0.18, p=.010), satisfaction for working (β=-0.31, p=.001), self care (β=-0.11, p=.033) and these factors explained 36%. Conclusion : Therefore, it is necessary to develop solution to improve professional quality of life in intensive-care unit nurses. It needs to develop programs to improve communication skills and humanistic knowledge in order to enhance compassion satisfaction and to improve communication skills to prevent burnout.

국내 아동전문간호사의 근무지 역할에 따른 직무수행과 직무만족도 및 직무스트레스 비교 (Comparison of Job Performance, Job Satisfaction and Job Stress of Child Health Nurse Practitioners by Roles in the Work Place)

  • Purpose: Child Health nurse practitioners (CHNPs) in Korea have important roles in disease management and health promotion for children and adolescents. Yet, practices of CHNPs licensed and employed in hospitals have not been adequately identified. Thus, in this study the scope of practice by CHNPs and job satisfaction and stress were investigated and compared according to the CHNPs' position in the working place. Methods: A descriptive cross-sectional survey design was used. All 53 licensed CHNPs participated in the mail survey which included a 71-item questionnaire on job performance and job satisfaction and a job contents questionnaire. Descriptive statistics, ${\chi}^2$ test, independent t-test and Mann-Whitney U test were used to compare CHNPs employed as nurse practitioner (NP) and CHNPs employed as staff nurses. Results: Compared to CHNPs employed as staff nurses, CHNPs employed as NPs more frequently provided education, environment management, coordination and research in their practice areas. No significant difference was found in job satisfaction between the two groups except for the administration and income subdomains. Only the physical exertion subdomain in job stress was stressful to CHNP employed as staff nurse. Conclusions: Job performance of CHNPs in Korea needs to be revised to include more practical practice in education, coordination, and research related areas.

병동 간호사의 가정용 인공호흡기 적용 환자 간호 스트레스 영향요인 (Factors Influencing Stress of Nurse who care for patients using a Home Mechanical Ventilator in General Ward)

  • Purpose: The purpose of this study was to identify factors associated with stress related to home mechanical ventilator (HMV) care in general ward nurses. Methods: The study participants were 110 general ward nurses. Data on participant characteristics, level of knowledge, education needs, coping ability in emergency situations, confidence, and stress were collected from August 1 to 30, 2018 using a structured questionnaire by web-based surveys. Data were analyzed using SPSS/WIN 20.0 for descriptive statistics and independent t-test, one-way analysis of variance, Pearson's correlation coefficient, and multiple regression analysis. Results: Significant factors associated with stress related to HMV care were ward career, intensive care unit (ICU) career, intensive care room (ICR) career, education experience, and satisfaction level of HMV education. Stress had negative correlations with confidence and positive correlations with education needs. The determining factors affecting stress related to HMV care in the general ward were confidence (${\beta}=-.31$, p=.004), ICR career (${\beta}=-.27$, p<.001), education needs (${\beta}=.24$, p=.005), education frequency (${\beta}=-.18$, p=.040), and ICU career (${\beta}=-.18$, p=.025); their explanation power was about 41.8%. Conclusions: It is necessary to develop HMV care training manuals and guidelines and consider ICU or ICR careers for patient safety.

일 무료요양시설 거주 여성 노인의 심리사회적 욕구에 대한 구성주의 연구 (The Constructivist Study on Psychosocial Needs of Elderly Female Residents living in Nursing Home)

  • 본 연구의 목적은 요양원 거주 노인들의 심리사회적 욕구를 탐색하고 이들을 돌보는 일에 효과적으로 개입하기 위한 것이다. 본 연구의 참여자는 지방 소도시의 한 요양원에 거주하는 노인들 중, 인지적으로 건강하고, 의사소통이 가능하며, 거주한지 1년이 넘는 65세 이상의 여성노인 8명으로 의도적 표집방법을 통해서 선정되었다. 본 연구는 구성주의 접근(constructivist inquiry)을 바탕으로 한 사례연구로서 자료 수집은 심층면접을 통하여 이루어졌으며, 자료 분석은 지속적인 비교방법(constant comparison method)이 이용되었다. 참여자들의 심리사회적 욕구는 충족된 욕구와 충족되지 못한 욕구, 숨겨진 욕구로 분류되었으며, 욕구 표현의 소극성이 또 다른 범주로 드러나, 연구결과는 총 4개 영역으로 범주화되어졌다. 먼저, 충족된 욕구는 편안함과 종교생활과 관계된 욕구였으며, 충족되지 못한 욕구는 음식 문제와 의미 있는 활동에 대한 결핍으로 나타났다. 참여자들이 구체적으로 인식하지 못하지만, 충족될 수 없는 숨겨진 욕구로는 정서적인 관계 욕구였다. 마지막으로, 욕구 표현의 소극성은 참여자들 대부분이 요양원내에서 그들의 욕구를 표현하는데 소극적이어서 분류된 것인데, 이는 스스로를 불쌍하게 여기는 부정적인 자기인식과 적극적인 욕구표현은 비난을 받을 수 있다는 암묵적 동의, 참여자의 독립과 의존사이의 갈등이 그 원인으로 분석되었다. 본 연구의 결과는, 요양원내 노인거주자들의 심리사회적 복지를 향상시키기 위한 사회 복지적 개입을 실천하는데 중요한 토대가 된다. 또한, 시설 생활에 대한 부정적인 인식이 현존하는 가운데, 자칫 감춰질 수 있는 시설노인들의 목소리에 귀 기울이고 이들의 경험을 드러냄으로써, 요양원이 노인들의 대안적인 삶의 공간으로서 사회 속에 자리매김하는 것을 돕는다.

암환자의 우울, 고통의 의미와 삶의 만족 (Depression, The Meaning of Suffering and Life Satisfaction in Cancer Patients)

  • Despite the advances in cancer therapy in the last 20 years, cancer continues to be a life-threatening illness, and the newly diagnosed individual faces a crisis that emphasized his or her mortality. Patients who suffer from cancer may have psychological problems, especially depression. Most tend to seek the meaning of suffering when continuing pain was experienced. Then the search for meaning, which is one of the primary needs of humans, begins. This meaning is "unique and specific" to the individual, and it must be fulfilled by the individual alone. This study was conducted to provide a basis of data for a nursing intervention program to minimize a cancer patient's suffering and to understand the relationship between life satisfaction, depression, and the meaning of suffering in cancer patients. The sample was composed of 160 cancer patients who were inpatients or outpatients of three general hospitals in Seoul. Data collections were carried out from February 25th to April 20th of 2000. The data was analyzed using a SAS program for descriptive statistics, Pearson Correlations, ANOVA, and Duncan tests. The results were as follows: 1. The scores on the depression scale ranged from 20 to 65 with a mean of 40.76 (SD 9.6) The mean score on the suffering scale was 97.72 (SD 12.7), and the score of the life satisfaction ranged from 15 to 37 with a mean of 25.51 (SD 5.2). 2. There were significant correlations between the amount of life satisfaction and depression (r=-.61, P=.00), the life satisfaction and the meaning of suffering (r=.30, p=.00), and the depression and the meaning of suffering (r=-.24, p=.00). 3. The factors influencing the depression in patients with cancer are age (F=2.52, p=.04) and education level (F=3.98, p=.00). The level of the meaning of suffering in cancer patients differed by education level (F=4.13, p=.00). Also, the level of the life satisfaction in cancer patients differed by education level (F=2.72, p=.04). In conclusion, the correlation between the depression, the meaning of suffering, and life satisfaction can be used as a concrete and practical datum for the development of nursing intervention. This may assist patients with cancer, overcome their suffering and lead to a hopeful life by understanding the meaning of suffering.

일개 공공병원의 의료서비스 만족도와 재이용의사에 관한 연구 (The patient satisfaction of medical services and the revisiting intention in a public hospital)

  • 본 연구는 서울에 소재한 일개 공공병원을 이용하는 환자 330명을 대상으로 의료서비스 만족도와 재이용의사를 파악하여 공공의료 서비스를 개선시키기 위한 기초자료를 제공하고자 시행되었다. 본 연구결과 병원 의료서비스 만족도는 평균 4.11점, 재이용의사는 평균 3.94점을 나타냈다. 의료서비스 만족도가 재이용의사에 미치는 영향은 통계적으로 유의한 결과를 보였고 설명력은 32.9%였다(t=11.335, p<.001). 결론적으로 의료서비스에 대한 이용자들의 만족도가 높을수록 다시 의료기관을 찾을 의사가 높은 것으로 나타났다. 따라서 만족도를 높이기 위해서는 공공의료기관을 찾는 대상자들의 요구를 파악할 필요가 있으며 병원 인근 지역주민과의 친화력 강화 및 편의 시설 확충 등이 필요하다.

임상간호사의 자율성과 직무만족 관련요인의 인과관계 분석 (The Causal Relationships among Nurses' Perceived Autonomy, Job Satisfaction and Realated Variables)

  • The present study examined the causal relationships among nurses' perceived autonomy, job satisfaction, work environment (work overload, role conflict, situational support, head nurses' leadership), personal aspects(experiences, need for achievement, professional knowledge and skill) by constructing and testing a theoretical framework. Based on literature review nurses' perceived autonomy and job satisfaction were conceived of as outcomes of the interplay among work environment and personal characteristics. Work environment factors involved work overload, role conflict, situational support, and head nurses' leadership (task oriented leadership, relation oriented leadership). Personal charateristics included experiences, need for achievement, and professional knowledge and skill. Three large general hospital in Chonbuk were selected to participate. The total sample of 516 registered nurses represents a response rate of 92 percent. Data for this study was collected from July to September in 1998 by Questionnaire. Path analyses with LISREL 7.16 program were used to test the fit of the proposed conceptual model to the data and to examine the causal relationship among variables. The result showed that both the proposed model and the modified model fit the data excellently. It needs to be notified, however, that path analisis can not count measurement errors; measurement error can attenuate estimates of coefficient and explanatory power. Nevertheless the model revealed relatively high explanatory power. 42 percent of nurses' perceived autonomy was explained by predicted variables; 32 percent of nurses' job satisfaction was explained by by predicted variables. Tn predicting nurses' perceived autonomy the findings of this study clearly demonstrated the work overload might be the most important variable of all the antecedent variables. Head nurses' relation oriented leadership, situational supports, need for achievement, and role conflict were also found to be important determinants for nurses' perceived autonomy. As for the job satisfaction, role conflict, situational supports, need for the achievement, and head nurses' relation oriented leadership were in turn important predictors. Unexpectedly the result showed perceived autonomy have few influence on job satisfaction. The results were discussed, including directions for the future research and practical implication drawn from the research were suggested.
