Objectives The purpose of this study was to review and evaluate the clinical evidence of the efficacy and safety of treatment based on Sasang constitutional medicine (SCM) for post-stroke patients by systematic review and meta-analysis. Methods Randomized controlled trials (RCTs), published in 10 electronic databases up to December 2020, were searched. For the included studies, Cochrane's risk of bias assessment was performed to analyze the methodological quality. The strength of evidence was evaluated using the grading of recommendations assessment, development, and evaluation system based on the results of analyses. All review processes were performed by two independent researchers. Results Five RCTs were finally included. All included RCTs were conducted for one month on post-stroke patients in 60-80s, four studies on Tae-Eum patients and one study on So-Yang patients. Four types of constitution-specific herbal medicine (Chungpyesagan-tang, Cheongsimsanyak-tang, Yeoldahanso-tang, and Yangkyuksanhwa-tang) and constitution-specific acupuncture therapy were identified as interventions. More than half of the included studies were evaluated as low quality due to the high-risk of bias in selection, performance, and detection. The combination of constitution-specific herbal medicine, acupuncture, and conventional treatment was more effective in improving the patients' motor impairment, dysphagia, aphasia, and depression than conventional treatment alone. No serious adverse events by SCM treatment were reported. Conclusions SCM treatment may improve the sequelae of post-stroke patients safely in combination with conventional treatment. Since the quality of clinical evidence included in this study was low, higher quality clinical evidence obtained in well-designed clinical studies will be needed.
The elderly population is rapidly increasing in South Korea, and interest in food development considering the Sasang constitution theory from oriental medicine and balanced nutrition is increasing. We developed the oriental medicinal porridges based on the Sasang constitution for the elderly. By Sasang constitutional medicine, Taeyangin has a large lung and small liver, and Soyangin has a large stomach and small kidney. Taeeumin has a small lung and large liver, and Soeumin has a small stomach and a large kidney. In this study, proper oriental medicine and food ingredients were identified, and a total of 12 oriental medicinal porridges were developed for 3 items by 4 Sasang constitution types. A single portion was developed based on about 600±66kcal, and the food ingredients were chopped. After cooking the menu developed based on the Sasang constitution, a sensory test was conducted. Five items, such as taste, appearance, aroma, texture, and overall preference, were evaluated on a 7-point scale. Sewage omegaenggul porridge among porridges for Taeyangin had the highest overall acceptance (6.17±0.7 points). Sukjihwang abalone black sesame porridge for Soyangin had the highest score(5.83±0.9 points). Sanyakyulmu hwangtae porridge for Taeeumin had the highest score(5.90±0.6 points). Ginseng chicken curry porridge among porridges for Soeumin had the highest overall preference in taste, appearance, aroma, texture, and overall acceptability (6.53±0.7 points). A limitation of this study was that the clinical trial could not be conducted on the elderly classified as Sasang constitution. In the future, the elderly will be able to have oriental medicinal foods according to the classification of Sasang constitution.
1. Objective: This study was designed to analyze the characteristics of each Sasang constitutional type using the Temperament and Character Inventory (TCI). 2. Methods: A total of 135 healthy subjects (age 19-30) participated in this study, consisting of 35 Soyang-type, 66 Taeeum-type, 30 Soeum-type, and 4 Taeyang-type subjects. A Sasang Constitutional Medicine specialist determined each subject's Sasang constitutional type based on the Two Step Questionnaire for Sasang Constitution Diagnosis (TS-QSCD) and comprehensive constitutional assessment. The subjects were also asked to complete the TCI. 3. Results: Among the four temperament dimensions, the Soyang- and Taeeum-type subjects showed significantly higher scores than the Soeum-type subjects in Novelty Seeking (NS). The Soeum-type subjects showed significantly higher scores than the Taeum- and Soyang-type subjects in Harm Avoidance (HA) (Soeum type > Taeeum type > Soyang type). The Soyang-type subjects showed significantly higher scores than the Soeum-type subjects in Reward Dependence (RD). The Soyang- and Taeeum-type subjects showed significantly higher scores than the Soeum-type subjects in Persistence (P). Among the three character dimensions, the Soyang-type subjects showed significantly higher score than the Taeeumand Soeum-type subjects in Self-Directedness (SD) (Soyang type > Taeeum type > Soeum type). The Soyang-type showed significantly higher scores than the Taeeum-type in Cooperativeness (C). 4. Conclusions: Differences in temperament and character were discovered across the Sasang constitutional types. The Sasang constitutional characteristics should be investigated in larger studies in the future. Similar studies could add to the growing knowledge of Sasang constitutional typology and contribute to a more accurate Sasang constitutional diagnosis.
The medical informatics for clustering Sasang types with collected clinical data is important for the personalized medicine, but it has not been thoroughly studied yet. The purpose of this study was to examine the usefulness of neural network data mining algorithm for traditional Korean medicine. We used Kohonen neural network, the Self-Organizing Map (SOM), for the analysis of biomedical information following data pre-processing and calculated the validity index as percentage correctly predicted and type-specific sensitivity. We can extract 12 data fields from 30 after data pre-processing with correlation analysis and latent functional relationship analysis. The profile of Myers-Briggs Type Inidcator and Bio-Impedance Analysis data which are clustered with SOM was similar to that of original measurements. The percentage correctly predicted was 56%, and sensitivity for So-Yang, Tae-Eum and So-Eum type were 56%, 48%, and 61%, respectively. This study showed that the neural network algorithm for clustering Sasang types based on clinical data is useful for the sasang type diagnostic test itself. We discussed the importance of data pre-processing and clustering algorithm for the validity of medical devices in traditional Korean medicine.
In this study, we suggested indirect evidence of whether Sasang constitution(SC) could be a risk factor for Post-traumatic stress disorder(PTSD) among soldiers who participated in Vietnam war. The number of 199 subjects joined this study. We surveyed SC with KS-15 and PTSD with Korean Version of Impact of Event Scale-Revised questionnaires. A Chi-square test and a one-way ANOVA were performed according to SC type, and multiple logistic regression was used to produce odds ratios(ORs). Significant p was .05. The number(rate) of Taeeumin(TE), Soeumin(SE), and Soyangin(SY) types were 131(65.8%), 33(16.6%), and 35(17.6%) respectively. The score of PTSD in SE type(8.78±8.61) was significantly higher than those of SY(4.00±6.5) and TE types(3.65±6.78)(p=.001, SE>TE, SY, Scheffe). SE type was associated with increased PTSD prevalence compared with TE [OR 4.338; 95% CI 1.525-12.335, p<.01], and it was still associated with increased PTSD prevalence [OR 10.658; 95% CI 1.296-87.661, p<.05] after adjusting for age and weight. This study suggests that SC, particularly the SE type, might be significantly associated with PTSD and could be considered as a risk factor for PTSD.
The Sasang constitutional medicine classifies the mankind into four constitutional types according to the individual psychological and physical traits. Differences in the Sasang constitutional medicine may be explained by the genetic factors. In order to determine the association of Sasang constitutional classification and interleukin-1 receptor antagonist (IL-1Ra) in genetic susceptibility to ischemic stroke, we classified the four constitutional types in ischemic stroke patients (n = 125) and the healthy control subjects (n = 107), and genotyped for IL-1Ra polymorphism by polymerase chain reaction (PCR) methods. The distribution of the $IL1RN^*1/IL1RN^*2$ genotype in the ischemic stroke patients was significantly different from the healthy controls (OR = 6.09; P =0.0134). And the prevalence of $IL1RN^*1/IL1RN^*2$ genotype was increased in Taeum-in ischemic stroke patients, as compared to Taeum-in healthy controls (OR = 14.71; P = 0.0144). These results suggest that $IL1RN^*1/IL1RN^*2$ genotype in Taeum-in might be associated with the increasing risk for ischemic stroke. Furthermore, this relationship could provide the basis for a new approach in the investigation of the etiology of ischemic stroke.
Objectives : This study aimed to investigate the circadian variation in stroke types, ischemic stroke subtypes, and Sasang constitutions. Methods : 295 patients with acute stroke within 14 days onset were included, who were admitted to Kyunghee Oriental Medical Center from October 2005 to May 2007. The stroke onset time was subdivided into four groups of 6-hour intervals in a day. Stroke types, ischemic stroke subtypes, and Sasang constitutions in four groups were examined. Results : Most ischemic stroke occurred between 6:01-12:00 hours (30.2%). For ischemic stroke subtypes, the peak period of small-vessel occlusion was between 6:01-12:00 hours (33.2%), large-artery atherosclerosis was most common between 12:01-18:00 hours (39.5%), and cardiac embolism was most frequent between 18:01-24:00 hours (50%). In terms of Sasang constitution, Soyeumin and Taeyeumin were most common between 6:01-12:00 hours (36.4% versus 41.5%, respectively), but the peak time of Soyangin was between 12:01-18:00 hours (35.2%). Conclusion :Most ischemic stroke events occurred in the mid-to-late morning hours in the present study and there was a circadian variation of onset in ischemic stroke subtypes and Sasang constitution.
Objectives The purpose of this study was to reveal that Sasang constitution(SC) was associated with hypertension and pre-hypertension and could be a risk factor. Methods We introduced this study to educational personnel in D university in Daejeon, and 275 subjects joined this study. The SC classification was conducted with KS 15 questionnaire. The subjected measured the blood pressure with Jawon medical device automatically after 10 minute rest. The hypertension and pre-hypertension was classified by the guide of the Seventh Report of the Joint National Committee on Prevention, Detection, Evaluation, and Treatment of High Blood Pressure. The frequency analysis and T-test was used in general characteristics, and chi-square test was also used between SC and pre-hypertension and hypertension. Logistic regression was used to calculate the odds ratios (ORs) and 95% confidence interval (95% CI) for pre-hypertension and hypertension. Results The number of Taeeumin(TE), Soeumin(SE), and Soyangin(SY) was 142, 71, and 61 respectively. There was significantly different in systolic and diastolic blood pressure among SC types(p<.001). The distribution of the normal group, pre-hypertension and hypertension group by SC types was significantly different (p<.001). The ORs of TE was significantly increased (ORs 4.039, 95% CI=2.019-8.082 in pre-hypertension and ORs 4.235, 95% CI=1.581-11.348 in hypertension) compared with SE(p<.001), and after adjusting gender and smoking habit, it was still significantly different(p<.001). Conclusions It is possible that SC, especially TE could be a risk factor both pre-hypertension and hypertension.
1. Backgrounds and Aims Human being is divided into 4 constitution types(Taeyangin, Soyangin, Taeumin, and Soumin). According to Sasang Constitutional Medicine[SCM] Each constitution has its own incidence, treatment and prevention of disease. The purpose of this study is to find the relation of dietary habits, life styles and Psychosocial stress with obesity in each Sasang Constitution. 2. Methods 975 subjects who have received health examinations and diagnosis of Sasang Constitution at Kyung-hee Medical Center were divided into each constitutional group. Each constitutional group was then divided into obese group and normal group. We evaluated risk factors of obesity such as dietary habits, life styles, and Psychosocial stress. These data were statistically analysed to investigate the relations between risk factors and obesity. And then significant factors were analysed by multinomial logistic regression analysis to get each odds ratio. 3. Results In a multinomial logistic regression analysis adjusted for age and sex, promotion of appetite and overeating or excessive diet elevated risks of obesity in Taeumin, promotion of appetite and high speed of diet elevated risks of obesity in Soyangin, and Psychosocial stress elevated risks of obesity in Soeumin. 4. Conclusions The results suggest that promotion of appetite and overeating or excessive diet may be major risk factors for obesity in Taeumin, promotion of appetite and high speed of diet in Soyangin and Psychosocial stress and dyschezia in Soeumin.
Objectives: The purpose of this study was to identify the tendency toward depression and fatigue in early postpartum women and to determine the relationship between the symptoms of these conditions, according to the women's Sasang constitutions. Methods: In this study, 73 healthy postpartum women were analyzed according to Sasang constitution: 33 Taeeumin, 22 Soyangin, and 18 Soeumin. Symptoms of depression and fatigue were assessed using the Edinburgh Postnatal Depression Scale-K(EPDS-K) and the Fatigue Continuum Form(FCF), respectively, on the fourth(time 1), seventh(time 2), and fifteenth(time 3) days postpartum. Sasang constitutional types were determined by a medical specialist, using results from the Two Step Questionnaire for Sasang Constitution Diagnosis(TS-QSCD). Results: 1. The overall depressive symptoms and fatigue scores decreased significantly over time in early postpartum women. 2. The mean scores for the EPDS and FCF were higher in the Soeumin group than in other groups. The scores for EPDS and FCF dropped sharply in comparison to a former level in the Soyangin group. And the mean EPDS score was lower in the Soyangin group than in other groups. The EPDS and FCF scores in the Taeeumin group changed slightly over time. However, these results were not statistically significant according to Sasang constitution. 3. The relationship between postpartum depression and fatigue showed a significant positive correlation. Conclusions: These results suggest that, in early postpartum women, depressive symptoms and fatigue differ according to Sasang constitution.
