Research Trends in Articles of Sasang Constitutions Studies on Children and Adolescents (소아·청소년 대상 사상의학 연구 동향 분석)

  • Objectives The aim of this study was to review the current Sasang Constitutions studies on children and adolescents and to provide basic data for developing in a better direction of research. Methods Research articles on children and adolescents of the Sasang Constitutions studies were extracted from six databases. Published year, published journals, types of studies were classified. Subjects of research and clinical methods for the Sasang type classification were summarized, especially in clinical studies. Results A total of 26 selected research articles were published from 1990 to 2013 and 14 articles (54%) have been published in Journal of Sasang Constitutional Medicine and Journal of Pediatrics of Korean Medicine. 96% of the total researches were based on clinical studies. Psychological and physical characteristics by Sasang Constitutional type accounted for most of the clinical research studies subjects. But these studies were not performed with diagnostic tools only for children and adolescents. Discussion In this study, the necessity of developing methods of Sasang constitutional type classification for children and adolescents was confirmed. If accompanied by additional research based on this study, it is expected to be used in diverse fields with active collaboration or education.

Clinical Practice Guideline for Sasang Constitutional Medicine: Prevention and Risk Fators of Sasangin Disease (사상체질병증 임상진료지침: 예방 및 위험인자)

  • Objectives This research was proposed to present Clinical Practice Guideline (CPG) for Prevention of Sasangin disease pattern of Sasang Constitutional Medicine (SCM) and diseases closely related with Sasang constitution. Each CPG was developed by the national-wide experts committee consisting of SCM professors. Methods At first, we searched the literatures related to SCM such as "Dongeuisusebowon", Textbook of SCM and Clinical Guidebook of SCM. Also we searched the articles related to the studies about risk factors for Sasangin disease pattern of both at home and abroad. Finally, we selected leading risk factors of Sasangin disease pattern and developed CPG for prevention of Sasangin disease pattern of SCM. And then, we searched the literatures related SCM such as "Dongeuisusebowon" and the articles on the correlation between disease and Sasang constitution using case-control studies, observational studies or cross sectional studies of both at home and abroad. Next, we selected diseases closely related with Sasang constitution on the basis of articles including prevalence rate and odds ratio between disease and Sasang constitution and finally developed CPG for these diseases. Results and Conclusions We categorized risk factors of Sasang disease pattern into 2 types: non-modifiable and potentially modifiable. 3 items (age, sex and genetic factors) were classified as non-modifiable risk factors of Sasang disease pattern. 6 items (original symptom, stress, diet and nutrition, physical activity, alcohol and drug misuse) were classified as less well-documented or potentially modifiable risk factors of Sasangin disease pattern. We found out Sasang constitution is more likely to develop some diseases. It was proven that Sasang constitution increase the risk of hypertension, diabetes mellitus, metabolic syndrome, stroke, nonalcoholic fatty liver and obstructive sleep apnea. And there is high probability of Sasang constitution being potential risk factor for obesity, hyperlipidemia, allergy and cancer. Also, we found out Taeeumin is independent risk factor for hypertension, diabetes mellitus, metabolic syndrome, stroke, nonalcoholic fatty liver and obstructive sleep apnea. Therefore we recommend that Taeeumin need to prevent these disease by regular checkups and aggressive management.

A Morphological Study of Fingerprints and Palm Prints in Sasang Constitution (사상체질(四象體質) 유형(類型)과 지문(指紋), 손바닥문(紋)의 관련성(關聯性)에 대(對)한 연구(硏究))

  • 1. Background : Sasang Constitutional Medicine divides mankind into Taeyangin(太陽人), Taeumin(太陰人), Soyangin(少陽人) and Soeumin(少陰人) according to appearance, moral nature and characteristic of constitutional symptoms, and it is a medical science to study about different treatment for each constitution. In Sasang Constitutional Medicine, diagnosis of constitution was so important that we had the necessity of studying constitutional diagnostic method which had presented wholly and intuitively in Dongyi Soose Bowon(東醫壽世保元) with objective and analytic thinking. There were several attempts to classify Sasang Constitutional Types through morphological investigation of each parts of body. The one of them was to use measuring the body. In this study, we purposed to make clear whether the analysis of fingerprints and palm, one of the physical anthropologic methods would be helpful to classify Sasang Constitution. 2. Method : After practicing Questionnaire I and QSCC(Questionnaire of Sasang Constitution) II from 347 healthy Korean adults(242 males and 106 females), we analyzed fingerprints and palm prints of 208 people(142 males and 66 females) who were classified same constitution from two questionnaires. We analyzed the types of fingerprints according to the number of triradius and counted the number of dermal ridges from fingerprint center to triradius. We found triradius in the lower part of each finger and analyzed the courses of dermal ridges and counted the number of dermal ridges between each triradius. We mesured angle atd, and then researched relationship between fingerprints, palm prints and Sasang Constitution. 3. Result and Conclusion : The results of Sasang Constitution analysis of 208 Korean adults showed 76 Taeumin(36.5%), 81 Soeumin(39.0%) and 51 Soyangin(24.5%). The analytic results of fingerprints and palm prints are as follows : The types of fingerprints were helpful to sort Taeumin and the types of palm prints were helpful to sort Soyangin and Soeumin especially. In addition, the number of dermal ridges in fingerprints and palm prints were helpful to sort Soyangin. Fingerprints and palm prints have characteristic in each constitutions, so the investigation of fingerprints and palm prints seems to helpful to classify Sasang Constitution. We are planning to report fingerprints and palm prints of more subjects.

A study on the correlation between Sasang Constitution and HLA type (사상체질(四象體質)과 HLA type과의 상관성(相關性)에 관(關)한 연구(硏究))

  • Exact constitutional classification is especially important in Sasang Constitutional Medicine. Therefore, most studies on the Sasang Constitutional medicine have been focused on how accurately to classify Sasang Constitution. Recently, there have been reported on the studies about relationship between genetic polymorphism and Sansang Constitution. The purpose of this study is to investigate whether there is any correlation between Sasang Constitution and HLA type. Polymorphism of HLA genes have been known to be important role in transplantation, autoimmune disease, immune response against infection. This study was concentrated on a relationship of HLA-A, -C, -DR, and -DQ types with Sasang Constitution. Results are as following. 1. In the case of HLA-A, there was no significant difference. Only $HLA-A^*31$ has a tendency which Soyangin is less frequent than Soeumin and Taeumin. 2. In the case of HLA-C, there was significant difference between Sasang Constitution and HLA type. In persons having $HLA-C^*04$ allele, Taeumin is more frequent than Soyangin and Soeumin. On the other hand, Soyangin is more frequent than Taeumin and Soeumin in $HLA-C^*07$ allele. $HLA-C^*14$ has a tendency which Soeumin is more frequent than Soyangin and Taeumin. 3. In. the case of HLA-DR type and HLA-DQ type, there was no significant difference among Sansang Constitution. On the study of correlation between Sasang Constitution and HLA type, there was little statistical significance and tendency. This study was the first try about presentation of frequency on the HLA alleles corresponding with Sasang Constitution. I think that more study on the relationship between HLA gene and Sasang Constitution is necessary by method of large case research and various statistical analysis.

An Evaluation of the Statistical Techniques Used in Papers of the Journal of Sasang Constitutional Medicine (2000-2009) (최근 10년간(2000-2009) 사상체질의학회지 게재논문의 통계분석방법에 대한 연구)

Objectives The purpose of this study is to assess the statistical methods used in the Journal of Sasang Constitutional Medicine and to identify the types of errors in the statistical analysis. 2. Methods We have reviewed quantitative articles published in the Journal of Sasang Constitutional Medicine from 2000 to 2009. Except for the case report, a total of 505 articles was reviewed by year. We focused on the methods of statistics in these articles. We have analysed the number of thesis used the statistical methods, the number of the statistical methods, the types of the statistical methods, and the errors of the statistical methods with the object of 202 thesis used the statistical method. 3. Results 1. Of the 370 articles, 202 used inferential statistics. 2. The most dominant was the thesis with one type of the statistical method forming 61.9% (125 thesis) of the total. 3. The ANOVA was the statistic of choice overall (40.1%), followed by the t-test (35.6%). 4. By examining the errors of the statistical methods, there were 49 thesis with errors among 81 thesis using ANOVA, 25 thesis among 72 thesis using t-test, and 18 over 62 thesis using chi-squared test. 4. Conclusions We have found various mistakes or misuses in the applications of statistical methodologies in the articles published in the Journal of Sasang Constitutional Medicine. It is necessary to present the cases of inappropriate statistical methods in order to improve the quality of academic researches. So long as Sasang constitutional medicine uses the statistical methods, researchers should fully examine and be aware of the statistical methods which they wish to use through other research articles, and statistic publications.

Association between cold-heat symptoms and sleep disturbances according to the Sasang constitution: a cross-sectional community study

  • Objectives : Evidence supporting the cold-heat symptom and sasang constitution type, which are diagnostic items of traditional Korean medicine, is needed to manage sleep disturbances, which is a typical symptom of mibyeong (subhealth). This study examined the association between each cold-heat symptom and sleep disturbances according to each sasang constitution type. Methods : This research was a cross-sectional study of 5,793 subjects from the Korean Medicine Data Center (KDC) community cohort survey. The association between each cold-heat symptom and sleep disturbances was analyzed by logistic regression analysis adjusted for several demographic variables. Subgroup analysis was then performed for each type of sasang constitution. Results : The soeum and soyang types were 1.53 and 1.26 times more likely to have sleep disturbances than the taeum type. Sleep disturbances were associated with 'coldness of the abdomen', 'watery mouth' in the cold domain items, and 'body feverishness', 'flushed face and eye', 'thirst', and 'scanty dark urine' in the heat domain items. The soeum and soyang types were 1.55 and 1.39 times more likely to sleep less than five hours per night than the taeeum type. In addition, the associations of those showed a different pattern for each sasang constitution type. Conclusions : Sleep disturbances are associated with specific cold-heat symptoms, and the associated cold-heat symptoms differ according to the sasang constitution type. These results may help traditional medicine specialists select customized interventions for patients with sleep disturbances.

A Study on the Association between Sasang Constitutions and Body Composition in Teenagers (청소년의 사상체질과 체성분과의 상관성에 관한 연구)

  • Purpose In Sasang Constitution Medicine, persons are divided into four types such as Taeyangin, Taeumin, Soyangin and Soeumin according to their appearance, moral nature and characteristic of constitutional symptoms. In teenager group, diagnosis of constitution based on their appearance was so difficult that we adopted new diagnostic method which measures and compares body compositions. Methods 180 middle school students who live in Pusan were studied from 20th of March, 1999 to 20th of March, 2000. After practicing Questionnaire(I), QSCC(Questionnaire of Sasang Constitution) I , QSCCII, we have measured the height and body compositions with Inbody 3.0, five times. Results 1. The results of Sasang Constitution analysis showed 91 students were classified as Taeumin(50.3%), 75 as Soyangin(41.4%), 14 as Soeumin(7.7%), and 1 as Taeyangin(0.6%). 2. The level of total body water, muscle area, BMR(Basal Metabolic Rate), BCM(Body Cell Mass), and AMC(Arm Muscle Circumference) were increased during spring and summer. That of body weight, fat body weight, fat body weight ratio, waist hip ratio, and BMI(Body Mass Index were increased during fall and winter. 3. In Taeumin and Soyangin groups, the changed capacity of total body water, muscle area and BMR were increased with statistical significance. 4. Waist hip ratio of Soeumin group was reduced more than those of Taeumin, Soyangin groups with statistical significance.

Systematic Review on the Sasang Type-specific Pathophysiological Symptoms of Sleep (사상체질별 수면 소증(素證)에 대한 체계적 고찰연구)

  • Objectives: The purpose of this study was to review previous clinical studies on underlying mechanisms of sleep-related type-specific pathophysiological symptoms among the Sasang types. Methods and Procedure: We reviewed seven research databases from December 2003 to August 2015 with the keywords Sasang typology, constitution and sleep. The Sasang type-specific sleep-related symptoms were analyzed based on seven categories, including subjective sleep quality, sleep latency, sleep duration, habitual sleep efficiency, sleep disturbances, use of sleep medications and daytime dysfunction. Results: Total of twelve studies were included in the analysis. The Tae-Yang type showed low subjective sleep quality, long sleep latency and frequent awakening, and the So-Yang type showed long sleep latency. The Tae-Eum type presented high subjective sleep quality, short sleep duration, frequent snore, toss and turn during the sleep, and insomnia and low frequency of dream. The So-Eum type showed long sleep latency in male and high frequency of dream. The Eum type combining Tae-Eum and So-Eum types had higher subjective sleep quality, longer sleep duration and higher frequency of dream than the Yang type combining Tae-Yang and So-Yang types.Conclusions This study reviewed type-specific sleep-related characteristics and discussed possible pathophysiological mechanisms involved. Differences in sleep characteristics among the Sasang types might stem from type-specific temperaments and require further study.

Study on the Meaning of Yin-Yang and Sasang in the "Huangdineijing" ("황제내경(黃帝內經)"의 '음양(陰陽)'과 '태양(太陽).소양(少陽).소음(少陰).태음(太陰)'의 의미 고찰)

  • The purpose of this study was to examine how the term Yin-Yang and Sasang categorized in the book "Huangdineijing". In order to investigate how the terms are used, we reviewed the text including the terms expressed in the manner of [(Yin/Yang) within (Yin/Yang)] and [Sasang]. We found three forms of expressions; [(Yin/Yang) within (Yin/Yang)], [Sasang], [(Sasang) within (Yin/Yang)]. Two paragraphs of [(Yin/Yang) within (Yin/Yang)] was found in one chapter, two paragraphs of [(Sasang)] was found in two chapters, and three paragraphs of [(Sasang) within (Yin/Yang)] was found in three chapters. We found five types of relation between [(Yin/Yang) within (Yin/Yang)], [Sasang], and five phases in "Huangdi neijing" as follows; (1) Yang within Yin, lesser Yang, and wood (2) Yang within Yang, greater Yang, and fire (3) ( ), ( ), and extreme Yin (4) Yin within Yang, lesser Yin, and metal, and (5) Yin within Yin, greater Yin, and water. And, as for the [(Yin/Yang) within (Yin/Yang)] and [(Sasang) within (Yin/Yang)], the classification criteria for Yin-Yang were brightness, abdomen/back or lumbar. The order of Sasang with the description form of [Sasang] or [(Sasang) within (Yin/Yang)] in "Siqi Tiaoshen Dalun" and "Liu Jie ZangXiang Theory" was lesser Yang, greater Yang, greater Yin, and lesser Yin, which is based on the meridian system or a plant-shaped change order. We discussed the results and its implication for the analysis of medical classics with the consideration of previous studies on Yin-Yang theories in "Huangdi neijing".
