Characteristic Radial Pulse Properties of Elderly Females according to Sasang Constitution-based Health Level (고령자 여성의 체질건강수준에 따른 맥파 특성 연구)

  • In Sasang constitutional medicine, doctors diagnose and treat patients according to their Sasang consitutition (SC) type. In this work, by a clinical test with a radial pulse tonometer, we investigated radial pulse properties which were significant in distinguishing unhealthy subjects from healthy subjects for each SC type. We measured radial pulse properties on left and right Gwan locations with a pulse tonometer for 299 elderly female subjects of age of 50 years old or above. We used a newly developed SCAT system to determine subjects' SC types. Subjects' health levels of either healthy or unhealthy were determined independently by two Korean medical doctors. To investigate the statistical differences, we used either of Student's t-test or Mann-Whitney U test depending on the normality of distribution of test statistic. For TE type, unhealthy subjects were characterized with significant increases in heart rate, systolic to diastolic period, pulse depth, while they showed significant decreases in width of pulse in normal direction, pulse area in diastolic period, and high harmonic components (6th, 7th) of power spectral density. For SE type, unhealthy subjects were characterized with significant increases in pulse pressure and pulse depth, while no significant differences were found for SY type. We investigated the characteristic differences in radial pulse properties due to change in health levels on each SC type for elderly females. Pulse variables which were significantly different between healthy subjects and unhealthy subjects were found to vary between SC types. For TE type subjects, many variables were found significant at the left Gwan location, which is in support of the theory of hyperactive liver functioning for TE type. Irrespective of the constitution, the related changes in the pulse properties due to worsened health level were in support of elevated blood flow amount in compensation with weakened blood circulatory function.

The Constitutional Ideas in the 『Huangdi Neijing』 (『황제내경(黃帝內經)』의 체질론(體質論))

  • One's constitution is formed congenitally and is also acquired, and is relatively stable in its individually manifested function, structure and temperament. It has characteristics of universality, complexity, generality and continuity. It is also manifested in physiological responses and expresses pathologic tendencies including susceptibility. Attempts at understanding constitution has had a long history throughout the world. In Oriental Medicine, the constitution had been acknowledged from the "Huangdi Neijing", which has been a bible in Oriental Medicine for about two thousand years. In many aspects, the "Huangdi Neijing" provides the basis for both basic and practical fields. In order to derive the ideal structure from the "Huangdi Neijing" to Lee Je-ma's(Lee Jayma's) Sasang Constitutional ideas, which can strengthen the understanding of ideal core of Oriental Medicine's constitutional characteristics, the author launched his analysis of the constitutional understandings in the "Huangdi Neijing" as a first step and came to the following: 1. The constitutional understanding in the "Huangdi Neijing" was done both via the physiological responses to heat, pain and acupuncture treatment and the pathologic tendencies via body structure, strength, body heat, courage and obesity. 2. It was recognized that the constitution was formed congenitally and also acquired factors like living conditions including food customs, residents and geographical conditions were explained in detailed. 3. The constitutional typology was suggested both by the general and systematic criterion according to the Yin-yang and the Five phase theories and by single criterion such as obesity, courage and the response to acupuncture treatment. 4. As diagnosis is related to the constitution, courage was adopted for the disease caused by emotions, obesity was for its manifestations of qi and blood, the quantities of qi-blood and Yin-yang in the typology by the Yin-yang and Five phase theories were suggested. 5. In the case of obesity, Yin-yang and Five phase theory, treatment according to the constitutional ideas was mentioned, and their goals were concluded as "being the balancing between Yin and Yang" which means the Yin-yang theory is more practical than the Five phase theory in Oriental Constitutional Medical field. According to the above understandings, the author would like to suggest that the constitutional ideas in the "Huangdi Neijing" based on the Yin-yang theory blossomed via the practical spirit of Zhang Zhong-jing's "Shanghanlun" and at last from Lee Je-ma's(Lee Jayma's) Sasang Constitutional Medicine which realized "the balancing between Yin and yang" through the practical applications of herbal treatments.

A Statistical Study on the Result Analysis of CaPSPI, a Diagnostic System for Climacteric and Postmenopausal Syndrome Pattern Identification (CaPSPI(Diagnostic System for Climacteric and Postmenopausal Syndrome Pattern Identification) 업그레이드를 위한 검진용 치료용 진단 결과 분석에 대한 통계 연구)

  • Objectives: It is a statistical analysis study to examine the results of CaPSPI (Diagnostic System for Climacteric and Postmenopausal Syndrome Pattern Identification), developed for objective defecation of climacteric and postmenopausal syndrome. Methods: Total 341 people's questionnaire responses were statistically analyzed. 275 people involved in developing CaPSPI 2018 (E) and 146 people involved in 2019-2020 study of research1,3). Results: The frequency of diagnosis for examination was the highest at liver depression, 93.8% for 320 times, the lowest at heartheat, 62.8% for 214 times. The frequency of treatment for examination was the highest at liver depression, 54.3% for 185 times, and the lowest at dual deficiency of heart-spleen, 16.7% for 57 times. The diagnosis ratio was the lowest at dual deficiency of heart-spleen, 19.72%, and the highest at liver depression, 57.81%. As a result of comparing these diagnoses with the Kupperman's index, all showed significant differences. As a result of comparing these disease elements, all showed significant differences. The correlation between diagnosis and dialectic elements was found to have similar results with the korean medical pathology, and in 7 dialectics except for heartheat, the treatment version was more severe or progressing to perjury than for examination. Conclusions: The CaPSPI shows the characteristics of korean medicine well, and it is needed to utilize the high correlative disease elements to upgrade the system.

Review of Research Topics on Consumptive Disease and Chronic Fatigue (허로에 대한 국내 연구동향 분석 및 연구방향 제안)

  • Exhaustion syndrome(虛勞) became broadly experienced symptoms in Korean population. In this work, we carried out a systematic literature review on exhaustion syndrome(ES) and chronic fatigue. We searched through the databases Koreanstudies Information Service System, Oriental Medicine Advanced Searching Integrated System, DataBase Periodical Information Academic for the articles published between 1994 and 2013, with the keywords 'exhaustion syndrome(虛勞)', 'consumption(虛損)', 'overexertion syndrome(勞倦)', 'fatigue', 'chronic fatigue' and 'degree of fatigue'. Among the first-run rough-searched 602 articles, we narrowed down the scope within the field of Oriental medicine (126 articles), and finally selected 28 articles appropriate to the intended research field; the selected articles were categorized by literature study(7 papers), clinical treatment (7), clinical diagnosis (5), treatment effects of herbal medicine (2), diagnosis in Sasang medicine and treatment effect of dry cupping therapy (2), and questionnaire-based diagnosis (5). We found that the overall research level on ES remained in the preliminary stages, and more efforts are needed in the field of terminology definition and standardization of diagnosis, and treatment efficacy validation beyond muscle fatigue. Finally, to develop reliable diagnostic devices on ES, we proposed a study design that included the development of objective ES diagnostic indicators and a clinical validation procedure.

A Case Study of Six Sigma Project for Improving Measurement Method of Skin Elasticity (피부탄성 측정 문제점 개선을 위한 6시그마 프로젝트)

  • The usefulness of constitutional diagnoses based on skin measurements has been established in oriental medicine. According to Sasang constitutional medicine, humans can be distinguished based on properties of skin's friction, thickness and elasticity. To quantify and standardize skin diagnosis, the present study designed an equipment for measuring skin elasticity of hand. But there were some noises in measuring skin elasticity such as measurement method, environment, operator and conditions of patient. So we considered a six sigma project for reducing the measurement errors. The project was followed to discipline process of five macro phases: Define, Measure, Analyze, Improve and Control(DMAIC). So, we could find the major factors which should be controled for stabilizing measurement system and we revised the SOP(Standard Operating Procedure) of the skin elasticity measurement.

Study on the Development of a Questionnaire Software for Health Examination in Oriental Medicine (한방건강검진 소프트웨어 개발에 관한 연구)

  • General health exanimation comes into operation to focus on physical inspection for industrial workers and doesn't contain oriental health care. Thus we need information of health status and disease prevention, so develop a questionnaire software for health examination in oriental medicine. Items of this soft ware consists of personal information and symptoms to could check oneself , pulse and tongue diagnosis by oriental medical doctor. Symptoms are made up of syndrome differentiation about Qi and blood, Yin and Yang, body fluid, five Zang organs, Sasang Constitutional Medicine. And we reconstruct 116 items by whole body, chest and abdomen, urine and feces, head, limbs, waist and back, five sensory organs, objective signs. A subject enter symptoms and a oriental medical doctor input diagnosis of pulse and tongue, then this software return the result of health index and explanation for oriental health care. This software would be used as tool not only health examination but also clinical research.

  • Objectives : The aim of the study was to investigate the principal symptoms and a syndrome differentiation in the obesity using surveys from Oriental medical doctors and obese patients. Methods : Seventy three Oriental medical doctors who participated in the 2006 autumn annual conference of Korean Oriental Association for Study of Obesity and 243 obese patients responded to the survey. Results : Twenty nine percent of Oriental medical doctors replied that the syndrome differentiation is the most important diagnosis index, and 21 percent of them replied they use Sasang Constitution classification during diagnostic process. The syndrome differentiations used were mainly phlegm-fluid, blood stasis, spleen vacuity, food accumulation, damp phlegm, and Gi deficiency order. In the response of doctors and patients about principle symptoms of 6 syndrom differentiation belong inside 5 place except phlegm fluid and liver stasis Conclusions : We should develop syndrome differentiation questionnaire about obese symptoms.

  • Though modern medicine has taken rapid strides, varieties of intractable maladies and diseases go on increasing more and more. And so medical technologies and academic achievements related to diagnoses and prognoses are being carried on. As the progress of genetics, all sorts of diseases have proved to be hereditary. This makes efficient use of the prevention of diseases. According to Sasang-Constitution medicine, each person inherently possesses a unique constitution different from that of any other person. The shapes and sizes, temperaments and characters of people have enormous variations that must affect our health and happiness. Without understanding our particular constitution, we must fall into poor health and disease. The food and exercise that are good for one person's constitution may not be good for another. No standardized medicine can adequately deal with our individual variations. Only a system that can discern our different constitutional types has this capacity. This paper is to study Life-Regulating Science that has taken firm root in our culture from time immemorial, so that I suggest the methods of diagnoses and prognoses by using Life-Regulating Science. In the medical texts, psychological imbalances of Eum-Yang and Five Phases generate all kinds of diseases. Accordingly visceral manifestation and diseases as a future possibility may be known by the Four Pillars.

  • Objectives : This study examined the reliability of disease mechanism diagnosis, to evaluate items of questionnaires and inquire about the relationships between disease mechanisms and 'diagnosis program' questionnaires used for the objective diagnosis of Oriental medicine in the department of Oriental OB&GYN, Oriental Medical Hospital of Dongeui University. Method : We analyzed the results of questionnaires from 3504 outpatients of OB&GYN disease at the Oriental Medical Hospital of Dongeui University from April 2000 to April 2005. Results & Conclusions : 1. The research questionnaire had 188 questions, the summary questionnaire 137, and the diagnosis questionnaire 80. 2. The reliability of all questionnaires shows above 90% in deficiency of qi, deficiency of Yin, insufficiency of Yang coldness heat syndrome liver and spleen kidney in all, 8 case disease mechanisms. These are higher in the diagnosis questionnaires than in the research questionnaires and the summary questionnaires, except for kidney disease mechanism. 3. Cronbach a of the questionnaires decreased, especially blood deficiency, phlegm, heat syndrome, and insufficiency of Yang; these 4 case disease mechanisms were lower than 0.6. 4. For degree of correspondence of meeting points, both. the diagnosis and the summary questionnaires were above 80% with the exception of the 2 case disease mechanisms heart and blood deficiency. The meeting points of both the diagnosis and research questionnaires were above 80% in the to case disease mechanisms deficiency of qi blood stasis deficiency of Yim insufficiency of Yang damp dryness liver spleen kidney phlegm. 5. The change in the result values of questionnaires was a decreased level of deficiency of qi heat syndrome phlegm damp kidney and raised level of coldness heart disorder of qi dryness 6. The computation degree of disease mechanism in DSOM(r) D.1.1 was much lower on phlegm deficiency of qi heat syndrome disorder of blood, somewhat lower on insufficiency of Yang and higher on coldness than in the two different questionnaires.

  • In spite of recent remarkable recent development in both western and oriental medical sciences, there is still only a shallow understanding of individual differences for various prognoses of incurable diseases and immunopathy diseases. Nevertheless, the care, cure and prevention methods of Sasang Constitutional Medicine are broadly used as an effective treatment of incurable diseases like immunopathy diseases and stress-related diseases and diseases due to aging. In this sense, the establishment of classification norms is urgent and essential for the worldwide application of Sasang Constitutional Medicine(SCM). This study began with the confirmation process of whether Sasang Constitutional types exist in Americans. To accomodate for cultural differences, the distinguishing tool was readjusted so that Sasang Constitutional Types in Americans could be determined. Hence, the selected tool is the new QSCCII+, which is a newly revised English version of the QSCCII. QSCCII was made and standardized by Dept. of SCM in Kyung Hee Medical Center and Dr. Kim7). The evaluation methods of the old version were improved in the new QSCCII+ through necessary statistical manipulation. The original QSCCII was officially authorized by the Korean Society of Sasang Constitutional Medicine as the only computerized version of Sasang diagnostics. This study is the first attempt to design a new diagnostic tool for the classification of Sasang Constitutional types in North Americans with the revision of QSCCII. The subjects of this study were selected from the cooperative people among the students and staffs of the University of Bridgeport and the patients who visited the Clinic in the Health Science Center. This study takes for about 1 year from 1998. 8 to 1999. 8 The conclusions of the study can be summarized as follows: 1. Sasang constitutional types also exist in Americans. It can also naturally be inferred that Sasang Constitutional types exist in all human beings, for there are many different human races in America. 2. There are more So-Yang In's than any other types in American white people. This result confirms the hypothesis that there also exist Sasang Constitutional types in westerners. 3. The result of repetitive tests suggests that the new QSCCII+ is an effective diagnostic tool for westerners when we consider the constant diagnostic results of the QSCCII+. 4. Sasang Constitutional types exit in the sample group regardless of racial difference. 5. The question items that were not often checked by Americans need to be modified into more understandable expressions. 6. The standardization of diagnosis for Americans should be established by use of the QSCCII+ 7. It can be guessed that there are many Tae-yang In's among the 71 persons who could not be clearly classified by the QSCCII+. Due to the scarcity of Tae-yang-In in general, it is important to improve upon the discernability of the QSCC II+. 8. The results of the Sasang Constitutional distribution in North Americans are as follows: The percentage of So-yang In distribution in the sample group is 36.25%(87persons), that of Tae-eum In is 13.75%(33persons), and that of So-eum In is 20.41%(49persons).
