The Philosophical View of Nature on the Sasang Constitutional Medicine (사상의학으로 본 철학적 자연관)

  • 1. The traditional view of nature in oriental medicine originated with a horizontal relation between nature and human. But the Sasang Constitution Medicine is based on the thought that ethical value is due to nature and at the same time nature could be transformed by ethical inclination. 2. The Sasang Constitution Medicine is not only a clinical medicine, but also a nature-philosophical system which implies the korean general view of nature. 3. We must examine a thought critically, four seperated physical structure were indeed deterministic. A deterministic understanding is false, because it is based on the recipiency of western philosophical ontology without reflection. 4. Korean view of nature is not kept in the fixed structure, and then can be opened and transformed by personal doings (practical acts). It is important that Sasang medicine is the pathology centering on man which is embodiment of ethical judgements.

  • After studying the theory of JangBu(臟腑), that is a physiology of Sasang Constitutional Medicine, in Sasang Constitutional Medicine, the author could draw a conclusion as these. 1. The basic thought of Sasang Constitutional Medicine is emphatic on the human beings itself unlike the idea of traditional Oriental Medicine, that accentuate the universe.(The traditional Oriental Medicine has a concept that the human beings follows the order of universe, but Sasang Constitutional Medicine has a different idea that human beings has the free will to the universe and newly comprehend the relationships between human beings and universe.) 2. Like the preceding, the theory of JangBu(臟腑), the physiology of Sasang Constitutional Medicine, is based on the autonomic mechanisms of human body itself. 3. The medical philosophy of Sasang Constitutional Medicine is Sasang(四象), that is Sa(事), Sim(心), Sin(身) and Mul(物). This Sasang is a classification of universe include human beings.(Sa(事) is event, Sim(心) is mind, Sin(身) is body, and Mul(物) is things. these four elements express the universe and human.) 4. The JangBu(臟腑) theory of Sasang Constitutional Medicine regard mind condition as important. The mind condition is divided into two factors. The one is Seong(性) the other is Jeong(情). The Seong(性) is an attitude toward the world, and the Jeong(情) is response to the stimulation from the world. 5. By the actions of Seong(性) and Jeong(情), the human body has different JangBu(臟腑) function, so the human body can be grouped in four constitutions. 6. Because of emphasis on mind condition, the Sasang JangBu(臟腑) theory has activism of human beings itself. 7. This activismor practical philosophy of Sasang JangBu(臟腑) theory, that is a physiology of Sasang Constitutional Medicine, gave birth to ethics or morality in Medical philosophy, that the human practice of ethics or morality bring a health and well being of human body.

A Case Report of Taeeumin with Post COVID-19 Syndrome (코로나19 후유증을 호소하는 태음인 환자에 대한 치험 1례)

  • Objectives The purpose of this case study was to report a patient with Post COVID-19 syndrome treated with Seunggeumjowi-tang based on Sasang Constitutional Medicine. Methods The patient was hospitalized after being diagnosed with COVID-19 and was suffering with post from COVID-19 syndrome such as fatigue and loss of appetite after discharge. The patient was diagnosed as Taeeumin and treated with Seunggeumjowi-tang. The progress was evaluated with the global assessment scale (GAS). Results and Conclusions Fatigue and anorexia were significantly improved, and other symptoms were restored as before. This case shows the possibility of Sasang Constitutional medication treatment for the sequelae of COVID-19.

The Preventive Thoughts of Sasang Constitutional Medicine (사상의학의 예방의학사상)

  • Objectives On the point of modern preventive medicine, Sasang Constitutional Meidicine(SCM) has the thoughts of preventive medicine. Modern preventive medicine has the three dimensional prevention; primary, secondary and tertiary prevention. The preventive thoughts of SCM concerning three dimensional prevention have been investigated in this article. Method Materials and references were collected about the prevention of SCM. Lee Jema's books such as Donguisusebowon, Gyeokchigo and Donguisusebowon Chobonguen were included. Result and Conclusions As the primary prevention of SCM, Regimen according to ages, explanation according to health index as Myeongmaeksilsu(命脈實數), moderation of four ruinous causes such as intoxication, seduction, opulence and authority, adequate social life and control of 4 kinds of emotions(sorrow, anger, joy and pleasure) are included. As the secondary prevention of SCM, explanation according to disease index, fragile Organs and frequently acquired diseases according to constitutions are included. As the tertiary prevention of SCM, rehabilitation for the stroke patients according to constitutions is included. In conclusion, SCM has all of 3 dimensional prevention methods although that might be applied with modern considerate modification.

Bibliographic Study on Dongeuisasang Geumnangbogyeol (동의사상 금낭보결(東醫四象 錦囊寶訣)에 대한 문헌적 고찰)

  • Objectives This paper was to compare and analyze the contents of 『Dongeuisasang Geumnangbogyeol(東醫四象錦囊寶訣)』 with previously published books on Sasang medicine. Methods 『Dongeuisasang Geumnangbogyeol(東醫四象錦囊寶訣)』 was compared and analyzed with the previously published 『Dongeuisasangsinpyeon(東醫四象新編)』 and 『Dongeuisusebowon(東醫壽世保元)』. Results and Conclusions 『Dongeuisasang Geumnangbogyeol(東醫四象錦囊寶訣)』 is a book related to Sasang medicine, written by the author Park Byeong-Gil in 1961 and published by Haenglim Seowon in 1961. Mainly, the contents of 『Dongeuisasangsinpyeon(東醫四象新編)』 were transcribed, but it is meaningful in that he recorded the body shape of Sasang constitution and the prescriptions he experienced clinically. 18 Taeumin prescriptions, 43 Soeumin prescriptions, and 13 Soyangin prescriptions were recorded, and it could be seen that in the Taeeumin prescriptions, the brain-nervous system disease was recorded, and in Soeumin prescriptions and Soyangin prescriptions, digestive system diseases were mainly recorded.

Exploring the Mechanisms and Target Diseases of Sasang Constitutional Prescription based on Multiscale Interactome (다계층 상호작용 네트워크 기반 사상처방의 작용 기전과 대상 질환 탐색 연구)

  • Objectives The aim of this study is to explore the mechanism of action and target diseases of Sasang constitutional prescriptions using a multiscale interactome approach. Methods The compound and target information of Sasang constitutional prescriptions were retrieved from various databases such as the TM-MC, STITCH, and TTD. Key targets for Sasang constitutional prescriptions were identified by selecting the top 100 targets based on the number of simple paths within the constructed network. Diffusion profiles for Sasang constitutional prescriptions and diseases were calculated based on a biased random walk algorithm. Potential diseases and key mechanisms of Sasang constitutional prescriptions were identified by analyzing diffusion profiles. Results We identified 144 Sasang constitutional prescriptions and their targets, finding 80 herbs with effective biological targets. A cluster analysis based on selecting up to 100 key targets for each prescription revealed a more cohesive grouping of prescriptions according to Sasang constitution. We then predicted potential diseases for 62 Sasang constitutional prescriptions using diffusion profiles calculated on a multiscale interactome. Finally, our analysis of diffusion profiles revealed key targets and biological functions of prescriptions in obesity and diabetes. Conclusions This study demonstrates the effectiveness of a multiscale interactome approach in elucidating the complex mechanisms and potential therapeutic applications of prescriptions in Sasang constitutional medicine.

A Case Report of Dyspnea and Arrhythmia as Sequelae of Aspiration Pneumonia in a Soeumin Patient (흡인성 폐렴 후유증의 호흡곤란 및 부정맥 소음인 치험 1례)

  • Objectives To report the improvements after applying modified Sibjeondaebo-tang and Hyangsayangwi-tang on Soeumin patient with dyspnea and arrhythmia as sequelae of aspiration pneumonia. Methods The 88-year-old female patient had the symptoms of dyspnea, sputum, cough, dysphagia, arrhythmia and general weakness after antibiotics for an aspiration pneumonia. Soeumin Sibjeondaebo-tang and Hyangsayangwi-tang were applied to treat these symptoms. Results The respiratory symptoms improved and the cardiac abnormalities, including arrhythmia showed improvement on the electrocardiogram and in cardiac-related biomarkers. Conclusions This case report suggests that Sasang constitutional herbal medicine could improve respiratory and cardiac symptoms as sequelae of aspiration pneumonia.

A Systematic Review on the Treatment of Post-Stroke Patients Based on Sasang Constitutional Medicine (사상의학적 중풍 후유증 치료에 대한 체계적 문헌 고찰)

  • Objectives The purpose of this study was to review and evaluate the clinical evidence of the efficacy and safety of treatment based on Sasang constitutional medicine (SCM) for post-stroke patients by systematic review and meta-analysis. Methods Randomized controlled trials (RCTs), published in 10 electronic databases up to December 2020, were searched. For the included studies, Cochrane's risk of bias assessment was performed to analyze the methodological quality. The strength of evidence was evaluated using the grading of recommendations assessment, development, and evaluation system based on the results of analyses. All review processes were performed by two independent researchers. Results Five RCTs were finally included. All included RCTs were conducted for one month on post-stroke patients in 60-80s, four studies on Tae-Eum patients and one study on So-Yang patients. Four types of constitution-specific herbal medicine (Chungpyesagan-tang, Cheongsimsanyak-tang, Yeoldahanso-tang, and Yangkyuksanhwa-tang) and constitution-specific acupuncture therapy were identified as interventions. More than half of the included studies were evaluated as low quality due to the high-risk of bias in selection, performance, and detection. The combination of constitution-specific herbal medicine, acupuncture, and conventional treatment was more effective in improving the patients' motor impairment, dysphagia, aphasia, and depression than conventional treatment alone. No serious adverse events by SCM treatment were reported. Conclusions SCM treatment may improve the sequelae of post-stroke patients safely in combination with conventional treatment. Since the quality of clinical evidence included in this study was low, higher quality clinical evidence obtained in well-designed clinical studies will be needed.

Literature Review on Sasang Constitutional Medicine for Tumors through Database Search (데이터베이스 검색을 통한 종양의 사상체질의학 관련 연구 문헌 고찰)

  • Objectives The purpose of this review was to investigate constitutional medical research on tumors and to propose information for further research. Methods The literature was investigated from Oriental medicine advanced searching integrated system, China national knowledge infrastructure, and PubMed. The searching terms were cancer, tumor, neoplasm, Sasang constitution, and so on. And studies were collected and analyzed into the three parts of distributions, treatments, and preventions. Results The distribution and type of tumors were different depending on the type of Sasang constitution. The symptoms of a tumor can occur variously in all types of diseases or patterns. Preventing tumors and cultivating health should be considered all aspects of physical, mental, and social health in the Sasang constitutional concept of securing requisite energy of minor organs. Conclusions The Sasang constitutional medicine for treating tumors should be focused on strengthening the constitutional healthy energy considering personal characteristics.

  • Through the survey of related studies for last ten years, it was known that accurate sample data of sasang constitution are the most important condition in sceintific research of sasang constitutional medicine. So we can propose three step for scientific sasang constitution medicine: firstly, the construction of SCIM(Sasang Constitutional Information Bank); secondly, the development of instruments for constitution diagnosis; thirdly, the new diagnosis methods based on the organic definition of sasang constitution.
