Orthognathic surgery with removal of lipoma in the asymmetric mandibular prognathism of a patient with a mandibular bone defect due to intramuscular lipoma on the medial aspect of the ramus: a case report

  • Lipomas, the most common soft-tissue mesenchymal neoplasms in adults, are characterized by the proliferation of mature white adipocytes without cytologic atypia. Lipomas are rarely observed in the head and neck region. We present a case of resection and orthognathic surgical removal of an intramuscular lipoma of the mandible with involvement of the mandibular ramus and condylar head and neck. An 18-year-old female patient was referred to our hospital for orthognathic surgery for the management of facial asymmetry and mandibular prognathism. The patient did not present with facial swelling, pain, or temporomandibular dysfunction; however, on radiographic examination, including cone-beam computed tomography and magnetic resonance imaging, an infiltrative fatty lesion was observed in the masticator space inside the right mandible, and the adjacent mandible exhibited bone thinning and deformity. Resection of the lipoma was performed along with orthognathic surgery, including a Le Fort I osteotomy for the maxilla and bilateral sagittal split ramus osteotomy (BSSRO). In this case, because the ramus was split using BSSRO, accessing the lipoma intraorally was easy. Consequently, aesthetic scarring was avoided, and no complications, such as unfavorable splitting or pathologic fracture, occurred. Although recurrence has not been observed about 1 year, long-term follow-up should be performed.

악교정수술 후 발생한 호르너 증후군 (HORNER'S SYNDROME AFTER ORTHOGNATHIC SURGERY)

  • Horner's syndrome as a complication of orthognathic surgery is given little attention of head and neck surgery and is a relatively benign and transient condition. A 18-year-old male referred to our department with long and anteriorly projected chin. The cephalometric evaluation revealed a skeletal Class III relationship. A 10-mm setback of the mandible to eliminate Class III relationship and 4-mm vertical reduction genioplasty were performed. Three weeks after operation, the patient was recognized anhidrosis in left face and the head, and ptosis of left eye. The trauma to cervical sympathetic nerve during left sagittal split ramus osteotomy was thought to be the cause of Horner's syndrome. Patient was treated by dermatologic and opthalmologic care. Follow-up examination 8 months later, he was recovery of horner's symptom.

  • This is a case report and review of literature that deal with a surgical correction of mandibular deficiency. Patient, 38 years old male, had visited to treat mandibular deficiency. On the basis of clinical and radiographic examination, he was diagnosed as a micrognathia. Surgical method. 1. Intraoral Bilateral sagittal split osteotomy 2. Augmentation of genioplasty - double step. Patient was satisfied with final esthetics by gnathosurgery.

하악전돌증에 있어서 전산화단층촬영을 이용한 하악지의 형태학적 평가 (MORPHOLOGICAL EVALUATION OF MANDIBULAR RAMUS IN MANDIBULAR PROGNATHISM BY COMPUTED TOMOGRAPHY)

  • Sagittal split ramus osteotomy (SSRO) is widely used in treatment of dentofacial deformities. But, many complications can occur including unfavorable fractures during osteotomy. To prevent these complications, it is necessary to understand comprehensively the anatomy of the mandiular ramus. The purpose of this study was to evaluate the morphology of the madibular ramus in manibular prognathism patients by computed tomography comparing with normal control group. The study group consisted of 33 skeletal class III patients (20 males, 13 females) and the control group consisted of the 52 patients without dentofacial deformities (32 males, 20 females). The mean age of study group was 22.0-year old, and that of control group was 37.1-year. For the CT examination, following scan parameters was used: 1mm slice thickness, 0.5 second scan time, 120kV and 100mA/s. The axial scans of the head were made parallel to the mandibular occlusal plane. The anteroposterior length of the ramus, the distance from anterior border of the ramus to lingula, the relative distance from the anterior border of the ramus to lingula compared to the anteroposterior length of the ramus, the thickness of anterior and posterior cortical plate, the thickness of medial cortical plate of the ramus at lingula level, the thickness of cancellous bone of the ramus at lingula level were measured. The skeletal class III mandibular prognathism patients exhibited shorter anteroposterior length of the ramus, thicker anterior and posterior cortical plate, thinner mediolateral cancellous bone thickness. The lingula has a relative stable anteroposterior position in ramus in all groups. There was higher possibility of fusion of medial and lateral cortical plate at lingula level in the mandibular prognathism group. In conclusion, the mandibular prognathism patients have narrow rami with scanty cancellous bone, which means that careful preoperative examination including CT scan can prevent undesirable fractures during osteotomy.

  • As a society develops and a standard of living raises, the orthoganthic operation, the purpose of which is the harmony of oral and surrounding tissue, the improvement of the basal bony relations for stable tooth-support, and the improvement of facial appearance, generalized in our society. This is a clinical study on 135 patients who underwent orthognathic operation at the Dept. of Oral and Maxillofacial surgery, College of Dentistry, Kyung-Hee University during the last 5 years. The results were as follows. 1. The ratio of female to male was 1.4 : 1. 2. The age distribution of most patients was the period of 20 years through 24 years. 3. The major reasons for seeking orthagnathic surgery were ethetic complanints(94.8%), and additional reason was functional complaints such as masticatory disturbances, phonetic disturbances, and TMJ discomfort. 4. In order to consult about orthognathic surgery, the patients first visited to the Dept. of oral and maxillofacial surgery at the ratio of 24.0%. 5. The ratio of the patients who underwent the presurgical orthodontic treatment was 79.5%, and the treatment usually required the period from 7 months to 18 months. 6. The setback amount was less than 10mm at the ratio of 85.8%. 7. The orthognathic operations which were practiced were usually composed of ramus osteotomy such as sagittal split ramus osteotomy and vertical ramus osteotomy and additional surgery on mandible at the ratio of 86.7%, and SSRO was done at the ratio of 62.5%, and EVRO was done at the ratio of 37.5%.

Radiologic assessment of bone healing after orthognathic surgery using fractal analysis

  • Purpose : To evaluate the radiographic change of operation sites after orthognathic surgery using the digital image processing and fractal analysis. Materials and Methods : A series of panoramic radiographs of thirty-five randomly selected patients who had undergone mandibular orthognathic surgery (bilateral sagittal split ramus osteotomy) without clinical complication for osseous healing, were taken. The panoramic radiographs of each selected patient were taken at pre-operation (stage 0), 1 or 2 days after operation (stage 1), 1 month after operation (stage 2), 6 months after operation (stage 3), and 12 months after operation (stage 4). The radiographs were digitized at 600 dpi, 8 bit, and 256 gray levels. The region of interest, centered on the bony gap area of the operation site, was selected and the fractal dimension was calculated by using the tile-counting method. The mean values and standard deviations of fractal dimension for each stage were calculated and the differences among stage 0, 1, 2, 3, and 4 were evaluated through repeated measures of the ANOVA and paired t-test. Results : The mean values and standard deviations of the fractal dimensions obtained from stage 0, 1, 2, 3, and 4 were 1.658±0.048, 1.580±0.050, 1.607±0.046, 1.624±0.049, and 1.641 ±0.061, respectively. The fractal dimensions from stage 1 to stage 4 were shown to have a tendency to increase (p < 0.05). Conclusion: The tendency of the fractal dimesion to increase relative to healing time may be a useful means of evaluating post-operative bony healing of the osteotomy site.

Surgery-early approach combined with condylectomy for correction of severe facial asymmetry with mandibular condylar hyperplasia: a case report

  • In patients with unilateral mandibular condyle hyperplasia, whether to perform condylectomy and orthognathic surgical procedures at the same time or orthognathic surgery in two stages for remains controversial. Reported here is a case of facial asymmetry with mandibular condyle hyperplasia, for which condylectomy and orthognathic surgery procedures were performed at the same time. A 28-year-old woman was presented to our department with chief complaints of left deviation of the mandible and right temporomandibular joint (TMJ) noise. Findings obtained in several imaging examinations led to a diagnosis of facial asymmetry associated with right mandibular condyle hyperplasia. Following 3 months of preoperative orthodontic treatment, in October 2018 under general anesthesia the patient underwent a right mandibular condylectomy, Le Fort I osteotomy, right mandibular sagittal split ramus osteotomy, and left mandibular inverted L ramus osteotomy. In examinations up to 3 years after surgery, good results were noted. For this case of severe facial asymmetry with mandibular condyle hyperplasia, early surgery and condylectomy were performed simultaneously to significantly shorten the total treatment time. The effectiveness of a surgery-early approach was confirmed by no postoperative findings indicating abnormalities in the TMJ or retroversion.

골격성 III급 부정교합자에서 전진이부성헝술을 동반한 하악 후퇴술 후 연조직 외형의 변화 (Soft tissue changes associated with advancement genioplasty in skeletal class III individuals receiving mandibular set-back surgery)

  • 본 연구는 전진이부성형술을 동반한 하악지 시상분할골절단술을 시행한 하악 전돌증을 가진 환자에서 하안면부, 특히 이부의 경조직과 연조직의 변화를 예측하는 데 있어서, 전후관계와 수직적 계측치를 통하여, 경조직의 변화에 따른 연조직의 상대적 변화량에 대해 비교, 분석하여 한국인의 골격성 III급 부정교합자의 악교정 수술시 전진이부성형술이 연조직 외형에 미치는 영향을 알아보고자 시행하였다. 교정치료와 악교정수술을 시행한 골격성 III급 부정교 합자 40명을 전진이부성형술을 시행하지 않고 하악지 시상분할골절단술만 시행한 환자 20명(BSSO group, 남자 10 명, 여자 10명)과 하악지 시상분할골절단술과 동시에 전진이부성형술을 시행한 환자 20명(Genie group, 남자 10명, 여자10명)으로 분류하여 수술직전(T1)과 수술직후(T2), 수술 최소 6개월 후(T3)의 측모 두부 방사선계측사진을 채득하여 계측 항목의 변화량을 분석하였다. BSSO군에서는 수술후 경조직 B점, Pogonion, Menton에 대하여 연조직 B점, Pogonion, Menton이 각각 수평적으로 0.997, 0.965, 1.022의 변화율을 보였으며, Genie군에서는 각각 0.824, 0.602, 0.887의 수평적 변화율을 보여서, Genie 군에서 연조직 B점, 연조직 Pogonion의 연조직 두께의 감소량이 더 크게 나타났다. 경조직 Pogonion(Pg)의 악교정수술 전과 후의 수평적 변화에 따른 하순의 하방점(Li), 연조직 B점 (B')의 변화간의 상관관계계수는 BSSO군에서는 각각 0.833, 0.922인 반면에, Genie 군에서는 각각 0.775, 0.799로 BSSO군에 비해서 상관관계가 다소 작은 것으로 나타났다. 이상의 결과에서 하악지 시상분할골절단술을 시행하는 환자에서 전진이부성형술을 동반할 경우에는 골격의 변화에 대해서 연조직의 두께 변화가 다소 적게 나타나므로 수술시에 이를 고려하여 전진이부성형술을 시행하여야 할 것이다.

심한 교합 부조화를 보이는 환자에서 악교정수술 및 교정치료를 동반한 구강회복: 증례 보고 (Rehabilitation with orthognathic surgery and orthodontic treatment in patient with severe occlusal disharmony: A case report)

  • 교합의 부조화가 있고 중심위와 최대 교두간 접촉위의 불일치가 과도하며 이와 관련된 병적인 징후가 관찰되는 경우, 반복 재현이 가능한 안정된 교합을 얻고 상실된 저작기능을 회복하기 위해 교합치료를 고려해야 한다. 면밀한 진단을 통하여 중심위를 기준으로 한 악간 및 교합관계를 분석하여 그 부조화의 정도에 따라 교합조정이나 보철수복, 교정치료, 악교정 수술 등을 시행할 수 있다. 본 증례의 환자는 수 년 전부터 발생한 교합 불안정 및 저작기능 상실을 주소로 본원에 내원하였으며 내원 당시 최대 교두간 접촉위에서 상악 우측 제2대구치만 접촉이 이루어지는 심한 교합부조화를 보였다. 중심위를 기준으로 한 교합분석 결과, 교합조정이나 보철수복을 통한 치료만으로는 문제를 해결하기 힘든 상,하악간 골격성 2급의 심도가 매우 깊었다. 따라서 임시의치수복을 통한 중심위를 확보하고 심한 골격성 2급을 해결하기 위한 술 전,후 교정치료를 동반한 양측 하악골 상행지 시상분할 골절단술(bilateral sagittal split ramus osteotomy, BSSRO)을 시행하였다. 이후, 전악에 걸친 세밀한 교합조정을 시행하여 교합을 더욱 안정화시켰으며, 무치악 부위는 임플란트를 식립하고 임시수복 하였다. 경과관찰 동안, 주기적인 교합평가 및 추가적인 교합조정을 통하여 안정적인 중심위와 조화로운 전방 및 측방유도를 얻었다. 이에 최종 보철수복을 완료하여 환자의 저작기능을 회복하였기에 이를 보고하는 바이다.

하악전돌증환자에서 하악지시상분할골절단술 후 전산화단층촬영을 이용한 근심골편의 횡적인 변화에 관한 연구 (TRANSVERSE CHANGE OF THE PROXIMAL SEGMENT AFTER BILATERAL SAGITTAL SPLIT RAMUS OSTEOTOMY IN MANDIBULAR PROGNATHISM USING COMPUTED TOMOGRAPHY)

  • Purpose: This study was performed to evaluate transverse changes of proximal segment after bilateral sagittal split ramus osteotomy (BSSRO) using 3-D CT in mandibular prognathism. Patients and methods: Twenty-two patients who underwent BSSRO for mandibular set-back in class III malocclusion without facial asymmetry were examined. Miniplates were used for the fixation after BSSRO. Facial CT was taken before and after BSSRO within 3 months. Frontal-ramal inclination (FRI), inter-gonial width (IGW) and intercondylar width (ICW) were measured in 3-D CT images using V-works $4.0^{TM}$ program. Student t-test was used to compare the changes between pre- and post-operative measurements using SPSS 10.0 program. Results: 1. Mean peroperative FRI value ($12.87{\pm}3.43^{\circ}$) was slightly increased to postoperative value ($14.13{\pm}3.72^{\circ}$) (p < 0.05). The average FRI increased 10.42% and the range was from $-2.46^{\circ}$ to $3.34^{\circ}$. 2. Mean peroperative IGW ($99.01{\pm}5.36$) was slightly decreased to postoperative IGW ($96.51{\pm}5.88mm$) (p < 0.05). The average IGW decreased 2.52 % and the range was from $-6.61^{\circ}$ to 0.91 mm. 3. Mean preoperative ICW ($125.01{\pm}5.30mm$) was slightly decreased to postoperative ICW ($125.40{\pm}5.45mm$) (p < 0.05). There is no significant difference between pre- and post-operative ICW. 4. There was significant correlationship between FRI difference and IGW difference (p < 0.05). Conclusions: These results indicate that the lower ramus of the proximal segment is moved inward after BSSRO procedure for mandibular set-back.
