In reliability engineering, the bathtub-shaped hazard rates play an important role in survival analysis and many other applications as well. For the bathtub-shaped, initially the hazard rate decreases from a relatively high value due to manufacturing defects or infant mortality to a relatively stable middle useful life value and then slowly increases with the onset of old age or wear out. In this paper, we present a new two-parameter lifetime distribution function, called the Loglog distribution, with Vtub-shaped hazard rate function. We illustrate the usefulness of the new Vtub-shaped hazard rate function by evaluating the reliability of several helicopter parts based on the data obtained in the maintenance malfunction information reporting system database collected from October 1995 to September 1999. We develop the S-Plus add-in software tool, called Reliability and Safety Assessment (RSA), to calculate reliability measures include mean time to failure, mean residual function, and confidence Intervals of the two helicopter critical parts. We use the mean squared error to compare relative goodness of fit test of the distribution models include normal, lognormal, and Weibull within the two data sets. This research indicates that the result of the new Vtub-shaped hazard rate function is worth the extra function-complexity for a better relative fit. More application in broader validation of this conclusion is needed using other data sets for reliability modeling in a general industrial setting.

Safety performance of a new car is evaluated through USNCAP and their results in the star rating are provided to the consumers. It is very important to obtain high score of USNCAP to appeal their performance to consumers. Therefore the car companies have made the effort to improve their car safety performance. These efforts should satisfy the demand not only to get high score but also to pass the FMVSS, NHTSA regulations on safety. Huge numbers of car crash tests have been conducted on these bases by car companies. However physical tests spend too much cost and time, as an alternative way, the simulation on the car crash could be a solution to reduce the cost and time. Therefore the simulations have been widely conducted in car industry and various researches on this have been reported. In this study, restraint system had been optimized to minimize the injury of female passenger. Belted $5^{th}%ile$ female frontal crash test was selected from various test methods of USNCAP for the study. Initial velocity of the test was 56km/h. The combination injury probability of USNCAP was selected as an objective function and the injury limit value, which was defined in FMVSS, was set to an optimization constraint. Many researches that were similar to this study had been conducted, however most of them had limitation that interaction between airbag and safety belt had not been considered. Contrary to these researches, the interaction was considered in this study.
This paper presents an adaptive feedback based actuator fault detection and tolerant control algorithms for longitudinal functional safety of autonomous driving. In order to ensure the functional safety of autonomous vehicles, fault detection and tolerant control algorithms are needed for sensors and actuators used for autonomous driving. In this study, adaptive feedback control algorithm to compute the longitudinal acceleration for autonomous driving has been developed based on relationship function using states. The relationship function has been designed using feedback gains and error states for adaptation rule design. The coefficients in the relationship function have been estimated using recursive least square with multiple forgetting factors. The MIT rule has been adopted to design the adaptation rule for feedback gains online. The stability analysis has been conducted based on Lyapunov direct method. The longitudinal acceleration computed by adaptive control algorithm has been compared to the actual acceleration for fault detection of actuators used for longitudinal autonomous driving.
To meet the growing needs from a variety of stakeholders, the development of modern systems is getting more complex and thus, the systems failure in the actual operations can potentially become more serious. This is why several international or military standards on systems safety have been published. In spite of the importance of meeting those standards such as IEC 61508 and ISO 26262 in the systems development, the associated practical methods seem deficient since those standards do not provide them. The objective of this paper is to present a method to identify potential hazards in fulfilling the requirements of the safety standards. In particular, the approach taken here is based on applying the functional analysis that covers several levels of the system under development. Note, however, that in the most of the conventional methods for hazards identification, the analysis has been focused on the failure at or underneath the component level of the system. The hazards identification method in this paper would cover the level up to the system by utilizing the functions-oriented approach. The case study of the safety enhancement for locomotive cabs is also discussed.
This report was prepared to introduce the developed Internet of Things (IoT)-based Smart Food Safety Management System (iMEAL) for children's foodservices registered in the Center for Children's Foodservice Management and report the results of hygiene monitoring through microbial analysis of two foods simulated and distributed based on this system. The program consisted of three menu screens: a foodservice management and meal inspection function, a refrigerator/freezer monitoring function, and a sanitary/safety inspection log function. Data such as cooking temperature, refrigerator and freezer temperature, salinity, and chlorine concentration were collected using IoT sensors or terminals, and hygiene safety inspection diary results (recorded by cooks) were transmitted to the Internet and stored. The APCs (3.78±0.07 log CFU/g) and E. coli (not detected) in stir-fried pork teriyaki sauce and the heating process met cooking standards. Similarly, the APCs (4.05±0.05 log CFU/g) and E. coli (not detected) in cucumber/chomuchim, which was not heated, also met cooking standards. APCs increased over time when cooked food was left for 1 hour, 1.5 hours, or 2 hours but remained acceptedable. Based on hygiene monitoring results of these two foods, using the i-MEAL system resulted in the safe production and distribution of children's food.
Korean industry has achieved dramatic growth in a short time through continuous technical innovations. However, safety consciousness and management have not kept pace with industry growth. The characteristics of industrial accidents in socio-technical systems have become so complicated that it has been challenging for safety professionals to implement effective preventive measures because it is difficult to determine what is happening in the complex workplace. Thus far, they have focused on technical improvements and the requirements of OSH regulations to avoid legal responsibility. Accordingly, this study has used the systemic analysis method to explain the emergence of something unexpected in complex and multi-layered business structures, which is usually related to the variabilities of humans and organizations. This study chose a shipyard where a few fatal accidents had occurred because the shipbuilding industry includes numerous variabilities, including obtaining orders, manpower supply, procurement, etc. Systemic analysis progressed using FRAM based on two accident cases related to the truss platform used in LNG vessels. The outcome shows that each function within the system has its variabilities and has become coupled or dependent on other functions, increasing the possibility of accidents. This analysis method can provide OSH professionals with practical techniques for explaining what is happening in a complex socio-technical system and how to take proper measures.
In this study, we propose a three-dimensional simplified slope stability analysis using a hybrid-type penalty method (HPM). In this method, a solid element obtained by the HPM is applied to a column that divides the slope into a lattice. Therefore, it can obtain a safety factor in the same way as simplified methods on the slip surface. Furthermore, it can obtain results (displacement and strain) that cannot be obtained by conventional limit equilibrium methods such as the Hovland method. The continuity condition of displacement between adjacent columns and between elements for each depth is considered to incorporate a penalty function and the relative displacement. For a slip surface between the bottom surface and the boundary condition to express the slip of slope, we introduce a penalty function based on the Mohr-Coulomb failure criterion. To compute the state of the slip surface, an r-min method is used in the load incremental method. Using the result of the simple three-dimensional slope stability analysis, we obtain a safety factor that is the same as the conventional method. Furthermore, the movement of the slope was calculated quantitatively and qualitatively because the displacement and strain of each element are obtained.

In this study, the seismic safety of containment building is assessed using response surface method. The structural analyses considering random variables such as load, resistance and analysis by ABAQUS are performed to obtain the structural response. The structural response is represented by polynomial of random variables, and the reliability analysis is performed by Level II method. Drucker-Prager failure criterion is applied as limit state function to take bi-axial stress states into account in the concrete. The lifetime probability of failure is evaluated by considering the lifetime of containment building, the annual occurrence rate of earthquake and the conditional probability of failure. Also the sensitivity analysis on the selection of sampling points is performed to obtain the steady results from response surface method.

Among the various functions of ADAS (Advanced Driver Assistance System), the most important and representative function to the safety of vehicle passengers is AEB (Autonomous Emergency Braking system). In South Korea, laws are in progress from 2022 for making it mandatory for passenger vehicles to be installed. And as AEB-equipped vehicles continues to increase in the future, the demand for accident analysis related to the AEB function is expected to increase in the future. In order to find out the operating limits of AEB, it is necessary to consider the situations exceeding the standards covered by EuroNCAP. Therefore we have performed four experiments in this study, including situations encountered in real-word traffic conditions, i.e., an oblique stop of Global Vehicle Target (GVT) and ADAS sensor failures. These experimental results are expected to be of great help in accurate and reliable accident analysis by considering them when analyzing traffic accidents for ADAS vehicles.
