  • The facilities of domestic elderly people are getting higher in the facility due to the rapid increase of the elderly population. Elderly people ability to evacuate the facility such as cognition and walking ability is very low. Therefore, it is necessary to secure the safety of evacuation in case of fire, but the evacuation safety measures in case of domestic fire accidents are insufficient. This study is to identify the problems of the safety of evacuation through the fire case analysis and to propose the measures against it.

  • Airworthiness authorities specify the technical standards of airworthiness that propose minimum requirement of the commercial transport category and apply the rules in the certification process to ensure the safety of the aircraft. The Federal Aviation Administration and other national airworthiness authorities define the fatal accident risk levels for the safety assessment of the aircraft system and establish standard procedures to apply both qualitative and quantitative analysis techniques. However, an accident or incident may occur by the combination of various factors, although the aircraft is designed in accordance with the strict standards and approval by the Airworthiness Authorities. There are some key factors, such as human error, unpredictable complex system failures, degradation of the components reliability, improper maintenance task and intervals. Risk can be reduced by reflecting aircraft operational experience with similar types of aircraft in the process of aircraft development and safety assessment. Result of the root cause analysis for the Airbus A300-600 incident in which the aircraft engine reverser was deployed in the air have been introduced to reflect the design of system and related components. Also, this paper suggests to create a big-database in order to provide a feed-back to the FAR Part 25 transport category design and safety assessment of the operational experience.

  • Chemical plants with high-technological equipments and complex processes are continually increasing potential hazards that may result in huge industrial accidents. If an incident breaks out, it will have a hazardous effect on in-plant workers, neighboring inhabitants or the surrounding environment. Therefore, it is necessary to develop a safety management system that can pre-evaluate the safety and reduce the risk of a chemical plants. The safety management evaluation program developed in this study can implement an effective loss control management in chemical plants. It integrates important lists in safety management lists and legal measures to analyze elements systematically. It also includes incident case analysis and the required components in domestic laws.

  • In an effort to decrease aviation accident worldwide, development of aviation safety management has been sought through aviation index and standardization, both by establishing SMS(Safety Management System). It also needs to be done in the domestic. both study on SMS which ICAO recommends and setting the top priority safety goal, each differently classified by nations. Accordingly, defining safety index and developing continuous monitoring approach. Aircraft accident reporting system in Air Force does not operate in a open approach method due to its uniqueness related to mission achievement. Therefore, limits of utilizing the recent worldwide aircraft data sharing and analyzing results to prevent accident is inevitable. This paper introduces an overview of ECCAIRS 5 which become the standard for the recent worldwide aviation safety reporting and data exchange system. Also using ECCAIRS 5, aircraft accident cases of the Air Force are classified such as accident type, year, month, occurrence category, and flight phase. The result of the study will provide a guide line for utilizing the civil system in prevention of future military aviation accidents.

  • Recently, in an effort to decrease aviation accident worldwide, there has not been any significant decline of the aviation accident rate. Therefore, in the international society, it is more focused on removing risk factors based on data collected and analyzed, in order to improve the aviation safety. This paper introduces on the characteristic of ECCAIRS 5 which become the standard for the worldwide aviation safety reporting and data exchange system. Also, using domestic aviation accident data collected during last 10 years(from 2003 to 2012), we analyze the distribution of the aviation accidents/incidents, annual and monthly aviation accident rate, flight phase, and occurrence category according aircraft type(Fixed/Rotary wing). The analyzation regarding the tendency of aviation accident/serious incident will give the direction to approach the quantitative safety management.

  • The International Electrotechnical Commission (IEC) has regulated the definition and requirements of the exposure index (EI). In this study, we calculated the EI of several image receptors in digital radiography system of two different manufacturers according to the method as per IEC, and evaluated the relationship with incident air kerma. To calculate the EI, w e obtained the characteristics curve of each image receptor by increasing the incident air kerma at RQA 3, 5, 7 and 9, respectively. As a result, there was no significant difference in the EI values between different image receptors of the same manufacturer, but EI values of different manufacturer was different despite the same air kerma was incident. Therefore, understanding the characteristics of the digital radiography systems is important in order to use EI as a tool for measuring and managing the radiation dose.

  • As workplace incidents has been being declining in Korea, there is criticism of the effectiveness of occupational safety policy implementation. It is unknown that which policy target group needs to be targeted to yield effective injuries prevention. The purpose of this paper is to analyze and reveal the policy intervention group with a high priority in terms of industrial incident prevention and the related investment cost. A Policy Priority Model(PPM) is composed of 6 indicators regarding influences of both the incident reduction and the cost reduction. Z-score analyses are used to confirm the high policy priority area or policy target group. Overall, workplace with worker below 50 persons, construction site with the sales of more than a hundred million won, workplace with relatively small percentage of female employees and relatively higher percentage of older worker should be prioritized to reduce workplace injuries. This paper provides an analytic way that can be used to decide the policy priority workplace in order not only to reduce work-related injuries&illnesses and the related investment cost but to further lessen the related societal costs.

Seismic failure analysis and safety assessment of an extremely long-span transmission tower-line system

  • Extremely long-span transmission tower-line system is an indispensable portion of an electricity transmission system, and its failures or collapse can impact on the entire electricity grid, affect the modern life, and cause great economic losses. It is therefore imperative to investigate the failure and safety of the transmission tower subjected to ground motions. In the present study, a detailed finite element (FE) model of a representative extremely long-span transmission tower-line system is established. A segmental damage indicator (SDI) is proposed to quantitatively assess the damage level of each segment of the transmission tower under earthquakes. Additionally, parametric studies are conducted to investigate the influence of different ground motions and incident angles on the ultimate capacity and weakest segment of the transmission tower. Finally, the collapse fragility curve in terms of the maximum SDI value and PGA is plotted for the exampled transmission tower. The results show that the proposed SDI can quantitatively assess the damage level of the segments, and thus determine the ultimate capacity and weakest segment of the transmission tower. Moreover, the different ground motions and incident angles have a significant influence on the SDI values of the transmission tower, and the collapse fragility curve is utilized to evaluate the collapse resistant capacity of the transmission tower subjected to ground motions.

Integration of Laser Scanning and Three-dimensional Models in the Legal Process Following an Industrial Accident

  • Background: In order to obtain a deeper understanding of an incident, it needs to be investigated to "peel back the layers" and examine both immediate and underlying failures that contributed to the event itself. One of the key elements of an effective accident investigation is recording the scene for future reference. In recent years, however, there have been major advances in survey technology, which have provided the ability to capture scenes in three dimension to an unprecedented level of detail, using laser scanners. Methods: A case study involving a fatal incident was surveyed using three-dimensional laser scanning, and subsequently recreated through virtual and physical models. The created models were then utilized in both accident investigation and legal process, to explore the technologies used in this setting. Results: Benefits include explanation of the event and environment, incident reconstruction, preservation of evidence, reducing the need for site visits, and testing of theories. Drawbacks include limited technology within courtrooms, confusion caused by models, cost, and personal interpretation and acceptance in the data. Conclusion: Laser scanning surveys can be of considerable use in jury trials, for example, in case the location supports the use of a high-definition survey, or an object has to be altered after the accident and it has a specific influence on the case and needs to be recorded. However, consideration has to be made in its application and to ensure a fair trial, with emphasis being placed on the facts of the case and personal interpretation controlled.

The Trend Analysis about Aviation Accident and Incident in Korea Using the ECCAIRS Data (ECCAIRS Data에 의한 한국의 항공사고·준사고에 대한 경향 분석)

  • Recently, despite of the development of aviation safety, there has not been any significant decline of the aviation accident rate. Therefore, in the international society, it is more focused on removing risk factors based on data collected and analyzed, in order to improve the aviation safety. This research introduces ECCAIRS, which is a program developed by European Union to collect and analyze data regarding risk factors. This is used in Korea since 2010. Moreover, using national aviation accident data collected through ECCAIRS, this research analyzes the distribution of the aviation accidents/incidents, annual and monthly aviation accident rate, flight phase, and division system. The analyzation regarding the tendency of aviation accident/incident will give the direction to approach the quantitative safety management.
