발의 과도한 회내 상태가 슬관절 근력과 보행에 미치는 영향 (Effects of excessive Pronation of the Foot on Knee joint Strength and Gait)

  • BACKGROUND: This study aimed to determine the changes in muscle strength and walking ability in patients who complained of knee instability due to excessive pronation of the foot. METHODS: Twenty patients (ten men and ten women) who complained of instability of the knee joint due to excessive pronation of the foot participated in the experiment. In the experimental group, the internal rotation of the tibia caused by excessive adduction of the foot was maintained as external rotation, and the joint state was to recognize the movement of the joint position changed through maintenance of the muscle. This exercise was performed five times for each patient, and the muscle strength maintenance was performed for 20 seconds. In the control group, stretching and range of motion (ROM) exercises were performed. For the stretching exercise, one specific motion was performed for 20 second, and the ROM exercise was performed to confirm the change in muscle strength in the knee joint area and walking ability. RESULTS: The knee flexion and extension strength in the patients with excessive pronation of the foot differed significantly from those in the subjects from the control group (p<.05). Further, the before-after comparison of the step time and length in the evaluation of walking ability, which affects overall postural movement due to knee joint instability, revealed a significant difference between the experimental and control groups (p<.05). CONCLUSION: The patients that were subjected to manual therapy and ROM exercise for the knee joint showed improved knee joint muscle strength and walking ability compared to the subjects from the control group.

해상 광대역 통신을 위한 LTE/Wi-Fi용 이중대역 안테나 설계 (Dual-Band Antenna Design for LTE/Wi-Fi for Maritime Broadband Communication)

  • 본 논문에서는 해상에서 사용할 수 있는 소형 광대역 통신용 안테나를 위해 2.65 GHz, 5 GHz 대역에서 동작하는 LTE/Wi-Fi 통신용 안테나를 설계하였다. 대역폭의 개선을 위해 마이크로스트립 패치 안테나를 선택하였고 각 단계마다 이론적인 근거에 의한 수식을 이용하여 슬롯의 폭, 길이, 전송선로의 폭 등을 계산하였다. 또한 3D 설계가 가능한 CST Microwave Studio 2014 프로그램을 이용하여 마이크로스트립 안테나를 설계하였고, 설계된 안테나를 시뮬레이션한 결과 반사손실이 2.65 GHz에서 -12.712 dB이고 5 GHz에서 -16.583 dB의 값을 보여주었다. 이득은 2.65 GHz에서 1.738 dBi의 값과 5 GHz에서 3.284 dBi의 수치를 보여주었다. 육상에서 사용하는 안테나와 비교하였을 때 통신속도, 통신의 안정성 등 차이가 나타나기 때문에 육상통신보다 열악한 환경인 해상에 사용 가능한 LTE/Wi-Fi용 이중대역 안테나를 설계하고자 한다.

  • 해상에서 사용하고 있는 통신용 안테나는 눈에 띄게 계속 개발되고 있다. 하지만 해상통신용 안테나의 발전속도는 사용자의 요구와 비교했을 때 많이 부족한 것을 느끼고 있다. 그래서 해상에서 도움이 되는 안테나를 개발하고자 하였고 안테나의 소형화, 이득 및 방사패턴을 개선한 고속 통신망 시스템을 효과적으로 사용하기 위해 3 [GHz], 5.72 [GHz] 대역에서 동작하는 UWB/Bluetooth용 안테나를 설계하였다. 대역폭의 개선을 위해 마이크로스트립 패치 안테나를 선택하였고 3D 설계가 가능한 CST Microwave Studio 2014 프로그램을 이용하였다. 프로그램을 통해 각 단계마다 이론적인 근거에 의한 수식을 이용하여 슬롯의 폭, 길이, 전송선로의 폭 등을 계산하여 결과값을 확인하였다. 또한 시뮬레이션을 통해 제작 기준에 적합한 지 확인하는 과정을 거쳤다. 해상에서 근거리 통신을 위한 고속 무선 통신용인 UWB와 해당 기술을 쉽고 편하게 접할 수 있고 각각의 기기와 연결을 통해 근거리의 정보교환을 강조한 Bluetooth를 추가하여 사용자들에게 많이 활용될 수 있을 것이다.

한국의 소득수준 간 의료이용 차이의 계량적 분석: 2015 (Econometric Analysis of the Difference in Medical Use among Income Groups in Korea: 2015)

  • Background: The purpose of this study is to estimate empirically whether there is a difference in medical use among income groups, and if so, how much. This study applies econometric model to the most recent year of Korean Medical Panel, 2015. The model consists of outpatient service and inpatient service models. Methods: The probit model is applied to the model which indicate whether or not the medical care has been used. Two step estimation method using maximum likelihood estimation is applied to the models of outpatient visits, hospital days, and outpatient and inpatient out-of-pocket cost models, with disconnected selection problems. Results: The results show that there was the inequality favorable to the low income group in medical care use. However, after controlling basic medical needs, there were no inequities among income groups in the outpatient visit model and the model of probability of inpatient service use. However, there were inequities favorable to the upper income groups in the models of probability of outpatient service use and outpatient out-of-pocket cost and the models of the number of length of stay and inpatient out-of-pocket cost. In particular, it shows clearly how the difference in outpatient service and inpatient service utilizations by income groups when basic medical needs are controlled. Conclusion: This means that the income contributes significantly to the degree of inequality in outpatient and inpatient care services. Therefore, the existence of medical care use difference under the same medical needs among income groups is a problem in terms of equity of medical care use, so great efforts should be made to establish policies to improve equity among income groups.

Towards the Saturation Throughput Disparity of Flows in Directional CSMA/CA Networks: An Analytical Model

  • Using directional antennas in wireless Ad hoc networks has many superiorities, including reducing interference, extending transmission range, and increasing space division multiplexing. However, directional transmission introduces two problems: deafness and directional hidden terminals problems. We observe that these problems result in saturation throughput disparity among the competing flows in directional CSMA/CA based Ad hoc networks and bring challenges for modeling the saturation throughput of the flows. In this article, we concentrate on how to model and analyze the saturation throughput disparity of different flows in directional CSMA/CA based Ad hoc networks. We first divide the collisions occurring in the transmission process into directional instantaneous collisions and directional persistent collisions. Then we propose a four-dimensional Markov chain to analyze the transmission state for a specific node. Our model has three different kinds of processes, namely back-off process, transmission process and freezing process. Each process contains a certain amount of continuous time slots which is defined as the basic time unit of the directional CSMA/CA protocols and the time length of each slot is fixed. We characterize the collision probabilities of the node by the one-step transition probability matrix in our Markov chain model. Accordingly, we can finally deduce the saturation throughput for each directional data stream and evaluate saturation throughput disparity for a given network topology. Finally, we verify the accuracy of our model by comparing the deviation of analytical results and simulation results.

Lateral Symmetry of Center of Pressure During Walking in Patients With Unilateral Knee Osteoarthritis

  • Background: Although symmetry of spatio-temporal parameter and center of pressure (COP) shift during walking is associated with knee adduction moment, research on clinical association with knee osteoarthritis (OA)-related knee pain and functional scores is lacking. Objects: The aims were 1) to compare symmetry of gait parameters and COP-shift in patients with unilateral knee OA and pain and matched controls, and 2) to investigate the relationship between symmetry of gait parameters and COP-shift, and clinical measures. Methods: Female subjects (n = 16) had with unilateral radiological knee OA and pain. Healthy controls (n = 15) were age-matched to OA group. Symmetry of foot rotation, step length, stance and swing phase, lateral symmetry of COP and anterior/posterior symmetry of COP during walking was assessed. To assess the clinical variables, pain intensity, pain duration and function using Knee Osteoarthritis Outcome Survey (KOOS) subscales were collected. We compared symmetry between groups using Mann-Whitney U-test or independent t-test. Relationships between clinical measures and symmetry index measured using Spearman's correlation test. Statistical significance was set at α = 0.05. Results: Knee OA group showed significantly greater values of only lateral symmetry of COP (p < 0.01) than healthy group. Values of lateral symmetry of COP had moderate or strong correlation significantly with the intensity of knee pain, pain duration, and scores of all KOOS subscales (p < 0.01). Conclusion: Patients with unilateral knee OA and pain showed more asymmetry of lateral COP-shift during walking compared with matched healthy controls. In addition, larger asymmetry of lateral COP-shift has the moderate or strong association with worse of knee pain, worse in KOOS scores and longer duration of knee pain. Asymmetry of lateral COP-shift during walking may be one of the characteristics of unilateral knee OA as the compensatory strategy response to unilateral OA of the knee.

Shoreline Changes and Erosion Protection Effects in Cotonou of Benin in the Gulf of Guinea

  • Coastal erosion has been a threat to coastal communities and emerged as an urgent problem. Among the coastal communities that are under perceived threat, Cotonou located in Benin, West Africa, is considered as one of the most dangerous area due to its high vulnerability. To address this problem, in 2013, the Benin authorities established seven groynes at east of Cotonou port, and two additional intermediate groynes have recently been integrated in April 2018. However, there is no quantitative analysis of groynes so far, so it is hard to know how effective they have been. To analyze effectiveness, we used optical satellite images from different time periods, especially 2004 and 2020, and then compared changes in length, width and area of shoreline in Cotonou. The study area is divided into two sectors based on the location of Cotonou port. The difference of two areas is that Sector 2 has groynes installed while Sector 1 hasn't. As result of this study, shoreline in Sector 1 showed accretion by recovering 1.20 km2 of area. In contrast, 3.67 km2 of Sector 2 disappeared due to coastal erosion, although it has groynes. This may imply that groynes helped to lessen the rate of average erosion, however, still could not perfectly stop the coastal erosion in the area. Therefore, for the next step, we assume it is recommended to study how to maximize effectiveness of groynes.

High-performance computing for SARS-CoV-2 RNAs clustering: a data science-based genomics approach

  • Nowadays, Genomic data constitutes one of the fastest growing datasets in the world. As of 2025, it is supposed to become the fourth largest source of Big Data, and thus mandating adequate high-performance computing (HPC) platform for processing. With the latest unprecedented and unpredictable mutations in severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), the research community is in crucial need for ICT tools to process SARS-CoV-2 RNA data, e.g., by classifying it (i.e., clustering) and thus assisting in tracking virus mutations and predict future ones. In this paper, we are presenting an HPC-based SARS-CoV-2 RNAs clustering tool. We are adopting a data science approach, from data collection, through analysis, to visualization. In the analysis step, we present how our clustering approach leverages on HPC and the longest common subsequence (LCS) algorithm. The approach uses the Hadoop MapReduce programming paradigm and adapts the LCS algorithm in order to efficiently compute the length of the LCS for each pair of SARS-CoV-2 RNA sequences. The latter are extracted from the U.S. National Center for Biotechnology Information (NCBI) Virus repository. The computed LCS lengths are used to measure the dissimilarities between RNA sequences in order to work out existing clusters. In addition to that, we present a comparative study of the LCS algorithm performance based on variable workloads and different numbers of Hadoop worker nodes.

도플러 추정을 적용한 직접수열 대역확산 전송 기반 수중 이동통신 방법 및 가상 이동신호를 이용한 해상시험 결과 (Underwater mobile communication scheme based on the direct sequence spread spectrum transmission using Doppler estimation and its sea trial results with the pseudo-moving transmission)

  • 본 논문에서는 수중통신 장치가 수중에서 이동시 발생하는 도플러(Doppler) 편이를 극복하기 위한 도플러 편이 추정방법과 직접수열 확산대역 전송의 신호처리 방법을 제시한다. 제안한 방법은 도플러 편이에 강인하고, 자기 상관 특성이 우수한 64길이의 Frank 신호열을 두 번 반복하는 프리앰블을 사용하여 2단계로 도플러 편이를 추정한다. 또한, 제안한 방법을 직접수열 대역확산 전송을 사용하는 수중통신방식에 적용하여 수중이동통신용 패킷 설계와 RAKE 수신기를 설계한다. 설계한 확산대역 수중이동통신 변조방식은 데이터 심볼을 Differential-Quadrature Phase Shift Keying(DQPSK) 변조하여 수신시 위상추적이 필요 없고, RAKE 수신기에서 동위상 결합이 용이하게 할 수 있는 장점이 있다. 제안한 도플러 편이 추정방법을 포함하는 RAKE 수신방법은 약 17.5 m/s의 이동속도까지 가상으로 이동하는 송신기로부터 수신한 직접수열 대역 확산 신호를 성공적으로 수신한다.

뇌졸중 환자의 마비쪽에 따른 골반의 기울임과 보행의 상관관계 분석 (Correlation Analysis of Pelvic Tilt and Gait according to the Paralytic Side of Stroke Patients)

  • Objective: This study investigated the effect of pelvic tiltng according to the paralytic side on gait in stroke patients during a 10 m functional movement timed up and go (TUG) test. Method: In this study, gait parameters were measured using a gait analyzer for 20 stroke patients and their gait was analyzed during a 10 m TUG test. For statistical analysis, an independent sample t-test were performed for age, height, and weight among general characteristics of subjects and homogeneity was tested by performing a chi-square test for gender, paralysis side, period of onset, and K-MMSE score. In order to understand the relationship between each variable, Pearson correlation analysis was performed on the variables. Results: First, the right-hand paralyzed group showed correlations in cadence and gait velocity in the up and down tilt of the pelvis, and the left-hand paralyzed group showed correlations in cadence and step length in the anterior and posterior tilt of the pelvis. Second, the tilt of the pelvis was correlated with the Sit to stand, walk forward, walk backwards, turn around at the end point, sit on a chair and the total TUG time in the right hemiplegic group compared to the left hemiplegic group. Conclusion: In this study, a significant correlation was confirmed as a result of gait analysis of right-handed stroke patients divided into a right paraplegic group and a left paraplegic group. In the future, it is suggested that treatment for improving gait of stroke patients should be treated differently for the right and left paralyzed side.
