혈관여포양 임파선 증식증 -1예 보고- (Angiofollicular Lymph Node Hyperplasia (= Castleman's Disease) -Report of A Case -)

  • 혈관여포양 임 파선증식 증 (angiofollicular lymph node hyperplasia)은 원인 미상의 드문 양성 질환으로서 대부분 흉곽내에 발생한다. 진단은 임상증상, 이학적 경사 그리고 X-선 촬영을 비롯한 각종 영상술에 의하며 여러 다른 질환과 감별진단이 어렵다. 대개 수술적 적출에 의한 조직학적 검색으로 확진이 된다. 조직학적으로는 두 가지로 분류되며 하나는 초자양혈관형으로서 여포내에 혈관이 증식되고 초자양화(hyalinization)가 일어나는 것이 특징이다. $90\%$가 이에 속하고 다른 하나는 형질세포형으로서 $10\%$를 차지한다. 후자는 여포간 조직내에 형질세포가 미만성으로 증식되고 신증후군, 발열, 빈혈, 적혈구침강속도 증가, 고감마글로블린혈증, 저알부민혈증 등의 변화가 있는 점이 전자와 다른 점이다. 치료는 주로 수술적 제거술에 의하며 수술로서 절제되지 못하는 것은 방사선 치료에 의하게 된다. 저자들은 경도의 흉부불쾌감과 경부 임파절 종대를 보인 61세 남자에서 전상종격동 종괴를 수술적 부분제거 후 외부 방사선 치료를 실시하여 완치된 1예를 경험하였기에 보고하는 바이다.

Subtype Distribution of Blastocystis in Thai-Myanmar Border, Thailand

  • Blastocystis sp. is a common zoonotic intestinal protozoa which has been classified into 17 subtypes (STs). A cross-sectional study was conducted to determine the prevalence and subtype distribution of Blastocystis in villagers living on the Thai-Myanmar border, where the risk of parasitic infection is high. A total of 207 stool samples were collected and DNA was extracted. PCR and sequencing using primers targeting small-subunit ribosomal RNA (SSU rRNA) gene were performed. The prevalence of Blastocystis infection was 37.2% (77/207). ST3 (19.8%; 41/207) was the predominant subtype, followed by ST1 (11.6%; 24/207), ST2 (5.3%; 11/207), and ST4 (0.5%; 1/207). A phylogenetic tree was reconstructed using the maximum likelihood (ML) method based on the Hasegawa-Kishino-Yano + G + I model. The percentage of bootstrapped trees in which the associated taxa clustered together was relatively high. Some sequences of Blastocystis positive samples (TK18, 39, 46, 71, and 90) were closely related to animals (pig and cattle) indicating zoonotic risks. Therefore, proper health education in parasitic prevention for the villagers should be promoted to improve their personal hygiene. Further longitudinal studies are required to monitor the prevalence of parasitic infections after providing health education and to investigate Blastocystis ST in animals living in these villages.

전방골간신경 증후군: 수술적으로 치료한 7예에 대한 임상적 고찰 (The Anterior Interosseous Nerve Syndrome: Clinical Investigation of Surgically Treated 7 Cases)

  • Purpose: The etiology and treatment strategy of the anterior interosseous nerve (AIN) syndrome are still controversial. Seven patients with the AIN syndrome who were treated by surgical exploration and neurolysis were reviewed at a mean of 35.9 months follow up period. Materials & Methods: There were six men and one woman. The mean age was 37.3 years, ranging from 26 to 59. No patient was related to trauma and associated neurological lesion. Surgical exploration was performed at 7.7 months after onset of paralysis. Results: All except one patients experienced pain around the elbow region before the onset of the palsy. On 7 patients, only the flexor pollicis longus was paralysed in 1, only the index flexor digitorum profundus in 2, and none had paralysis of the middle. The most common compression structures were fibrous bands within flexor digitorum sublimis arcade. However there was no demonstrable abnormality in three. Recovery was complete in all cases within 12 months after surgery. Conclusion: We recommended surgical exploration and neurolysis in patients who have shown no improvement after 6 months of conservative treatment. And careful preoperative examination is essential to avoid misdiagnosis and inappropriate surgery, especially in incomplete AIN syndrome.

High Throughput 프로세스에서 품질혁신의 성능평가를 위한 Z-Factor의 적용방안 (Implementation of Z-Factor Statistics for Performance Evaluation of Quality Innovation in the High Throughput Process)

  • The purpose of this study is to introduce the limit of previously used six sigma quality process evaluation metrics, $Z_{st}$ and $P_{pk}$, and a solution to overcome this drawback by using a metric based on performance evaluation of Z-factor quality innovation. Case analysis on projects from national six sigma contest from 2011 to 2012 is performed and literature review on new drug development HTS (High Throughput Screening) is used to propose innovative performance evaluation metrics. This research shows that experimental study on six sigma evaluation metric, $Z_{st}$ and $P_{pk}$, have no significance difference between industrial type (Manufacturing, Semi-Public Institute, Public Institute) and CTQ type (Product Technology Type CTQ, Process Technology Type CTQ). Following discovery characterize this quality improvement as fixed target type project. As newly developed moving target type of quality innovation performance metric Z-Factor is used for evaluating experimental study, hypothetical analysis suggests that $Z_{st}$ and $P_{pk}$ share different relationship or even show reciprocal relationship. Constraints of the study are relatively small sample size of only 37 projects from past 2 years and conflict on having interview and communication with six sigma quality practitioner for qualitative experimental study. Both moving target type six sigma innovation project and fixed target type improvement project or quality circle enables efficient ways for a better understanding and quality practitioner use by applying quality innovation performance metric. Downside of fixed target type quality performance evaluation metric, $Z_{st}$ and $P_{pk}$, is presented through experimental study. In contrast, advantage of this study is that high throughput requiring product technology, process technology and quantum leap typed innovation effect is evaluated based on precision and accuracy and Z-Factor that enables relative comparison between enterprises is proposed and implemented.

교통량이 다른 고속도로변 생태계의 영양단계별 납과 카드뮴 함량의 차이 (Content Differences of Pb and Cd by Trophic Levels of the Ecosystem near Highways with Different Traffic Volumes)

  • The content of Pb and Cd in the soil, producer, 1st consumer and 2nd consumer by the distance from two highway with different construction years and traffic volumes was measured from 1984 through 1986. The result was compared with that of a control in Yangsu-ri near Mt. Unak, Namyangju-gun, Kyunggi-do. The producers in the area of Yangjae-done are composed of 27 species of floras with Glycine soja being the dominant species while those in Shingal 20 species, with Leesia japonica being the dominant species. The faunas in the former area include 37 families and 69 species, with insects being dominant. In the area of Yangjae-dong, the biomass by trophic levels increased from the ration of 1/400 between producer and 1st consumer to the ratio of 1/10 between 1st consumer and 2nd consumer. The Pb content of soil, producer, 1st consumer and 2nd consumer in Yangjae-done near the Kyungbu highway with traffic jam has the sigingacant difference of 1% level at the vicinity of 10 meters and it is the same with that of Shingal area nearthe Youngdong highway with relatively sparse traffic except for the 2nd consumer. In case of cadmium content, the 1st and 2nd consumer has 1% signigicant difference near the Kyungbu highway and other trophic levels have no diference. Besides, the content of Pb and Cd at the site 1, 2 and 3 within the range of 100 meters from the two highways has 1% significant difference from that of a control while the site 4 has no difference.

2004년 하계 한국 남해에 있어서 저층 트롤 어획물의 종조성 (Species Composition of the Catches collected by a Bottom Trawl in the Southern Waters of Korea in Summer, 2004)

  • 본 연구는 트롤 어획물의 종조성을 조사한 것으로 2004년 6월 24일부터 26일까지 우리나라 남해의 5개 조사 지점에서 저층 트롤 어구를 사용하여 시험조업이 이루어졌으며, 예망 속력은 3.4${\sim}$3.7k't 였다. 어획물은 어류가 2강 10목 37과 46종이었으며, 연골어류가 2목 2과 2종, 경골어류가 8목 35과 44종이었따. 어획된 어류중에서는 농어목 어류가 18과 24종으로 가장 많았다. 두족류는 1강 2목 3과 3종이 어획되었고, 살오징어목 오징어과의 화살꼴뚜기가 가장 많이 어획되었다. 갑각류는 1강 1목 5과 7종이 어획되었고, 십각목 가시발새우과의 가시발새우가 가장 많이 어획되었다. 각 조사지점에 있어서 어획마리수 및 생체량은 ST-1에서 44마리 376.0kg, ST-2에서 908마리 240.3kg, ST-3에서 666마리 90.1kg, ST-4에서 2,050마리 300.kkg 그리고 ST-5에서 561마리 24.7kg이었다. 조사지점의 종다양성지수는 1.49${\sim}$2.32, 종풍부도지수는 2.13${\sim}$3.48, 종균등도지수는 0.48${\sim}$0.77, 우점도지수는 0.43${\sim}$08의 범위였다. 주요 어종(3지점이상에서 어획되고, 개체수가 100마리 이상인 것)의 체장 범위는 전갱이 9${\sim}$32cm(가랑이체장), 살오징어 7${\sim}$24cm(외투장), 화살꼴뚜기 9${\sim}$43cm(외투장), 말쥐치 23${\sim}$36cm(전장), 황돔 10${\sim}$28cm(가랑이체장), 달고기 10${\sim}$36cm(전장), 참돔 8${\sim}$35cm(가랑이체장)로 나타났다.

한국산 고구마잎과 고구마줄기 에탄올 추출물의 in vitro 항산화, 항알레르기 및 항염증효과 (In vitro Antioxidant, Anti-allergic and Anti-inflammatory Effects of Ethanol Extracts from Korean Sweet Potato Leaves and Stalks)

  • 고구마뿌리에 비하여 많은 양이 폐기되는 고구마 줄기와 잎의 이용을 활성화시키고 기능성식품 소재로의 가능성을 알아보기 위하여 잎이 달려 있는 보라색 고구마줄기를 재래시장에서 구입하여 고구마잎(SL), 고구마줄기(ST), 껍질 제거한 고구마줄기(PST)의 총 폴리페놀과 총 플라보노이드 함량을 측정하고, in vitro 시스템에서의 항산화효과, 항알레르기 및 항염증효과를 검색하고자 하였다. 고구마잎은 데치고, 고구마줄기는 생으로 동결건조한 후 에탄올 추출물을 얻어 실험하였다. 총 폴리페놀 함량은 SL(11.03 mg tannic acid/g)> ST(0.87 mg tannic acid/g)> PST(0.37 mg tannic acid/g)이었고, 총 플라보노이드 함량은 SL(9.01 mg rutin/g)> ST(0.50 mg rutin/g)> PST(0.25 mg rutin/g)이었다(p<0.001). DPPH 라디칼을 50% 제거시키는 에탄올 추출물의 농도($IC_{50}$)는 SL($43.6{\mu}g/mL$)< ST($308.4{\mu}g/mL$)$1,631.3{\mu}g/mL$)로 고구마잎이 가장 우수한 효과를 나타내었다. 양성대조시약으로 사용한 BHA의 $IC_{50}$값은 $8.30{\mu}g/mL$이었으며, 처리 농도 $1,000{\mu}g/mL$에서의 환원력은 SL($59.72{\mu}g$ ascorbic acid eq./mL)> ST($12.56{\mu}g$ ascorbic acid eq./mL)> PST($2.18{\mu}g$ ascorbic acid eq./mL)로 고구마잎이 가장 좋았다(p<0.001). 한편 염증반응에 관여하는 xanthine oxidase(XO) 활성저해율을 측정한 결과 처리 농도 $250{\mu}g/mL$에서 SL(13.06%)> ST(5.05%)> PST(0.0%)로 고구마잎이 줄기에 비하여 우수하였으며(p<0.01), 처리 농도 $50{\mu}g/mL$에서의 5-lipoxygenase(LOX) 활성저해율은 SL(91.16%)> ST(33.38%)> PST(14.93%)로(p<0.001) 고구마잎의 효과는 양성대조시약인 EGCG의 저해율(94.42%)과 비슷한 정도로 매우 우수하였다. 또한 $250{\mu}g/mL$ 농도로 처리 시 cyclooxygenase(COX)-2 활성저해율은 SL(55.34%)> ST(2.18%)> PST(0.0%)로 XO 활성저해율과 비슷한 패턴을 보였다(p<0.001). 측정변수들 간의 상관관계를 분석해 본 결과 총 폴리페놀 함량은 플라보노이드 함량($r^2$=0.9988, p<0.001), 환원력($r^2$=0.9982, p<0.001), XO 활성저해율($r^2$=0.8322, p<0.05), COX-2 활성저해율($r^2$=0.9950, p<0.001), 5-LOX 활성저해율과 양의 상관관계($r^2$=0.9823, p<0.001)를 나타내었으며, 플라보노이드 함량은 환원력($r^2$=0.9946, p<0.001), XO 활성저해율($r^2$=0.8392, p<0.05), 5-LOX 활성저해율($r^2$=0.9749, p<0.01), COX-2 활성저해율과 유의한 양의 상관관계($r^2$=0.9937, p<0.001)를 보였다. 또한, 환원력은 XO 활성 저해율($r^2$=0.8384, p<0.05), 5-LOX 활성저해율($r^2$=0.9883, p<0.001) 및 COX-2 활성저해율과 유의한 양의 상관관계($r^2$=0.9954, p<0.001)를 나타내었으며, XO 활성저해율은 5-LOX 활성저해율과 유의한 양의 상관관계를 보였으나($r^2$=0.8786, p<0.05) COX-2 활성저해율과는 상관성을 보이지 않았다. 5-LOX 활성저해율은 COX-2 활성저해율과 유의한 양의 상관관계($r^2$=0.9815, p<0.01)를 나타내었다. 이상의 결과들로부터 고구마잎은 폴리페놀 및 플라보노이드 함량이 매우 높았고 우수한 항산화효과를 보였으며, 알레르기 및 염증반응과 관련이 있는 효소인 XO, 5-LOX 및 COX-2의 활성도 모두 억제하는 효과도 우수하였지만 특히 5-LOX 활성 억제효과는 EGCG와 비슷한 정도로 매우 우수하였다. 따라서 건강을 위하여 고구마잎의 섭취를 증대시킬 수 있는 다양한 방안을 강구할 필요가 있으며, 고구마 줄기를 섭취할 경우에는 가능한 껍질을 모두 섭취하는 것이 좋겠다.

도로교통에 있어서 운전자 주시특성분석과 그 적용성에 관한 연구 (Study on Analysis of Driver's Visual Characteristics in Road Traffic and its Applications)

  • The Subject of this research work is to study the driver's vision and eye-movement ch-aracteristics under the diffrent condiction of road traffic and driving. The analysis of this investigation was conducted spatially or temporaly into three parts'eye-mark distribution, viewing-time percentage and fixation duration. This dissertation focuses on analysis of dr-iver's visual characteristics to improve road circumstamces. In this study driver's ch-aracteristics are measured with eye-mark recorder and analyzed statistically The main features of this study are : 1st Duration distribution of fixation point is significant in 87% at 5% of the significant level in Gamma Distribution. The average of fixation duration by road are 0.33sec on streets 0.45sec on Roads and 0.86sec on highways. The average of fixation duration by visual objects are 0.4sec on road surface 0.26sec on road shoulder 0.49sec on traffic sign 0.37sec on warning sign and 0.67sex on gwide sign. 2st Moving anglrs of a fixation point are fit in the Exponential Distribution. The average moving angle is appeared to be 3.85。 on streets 2.81。 on roads 2.73。 on highway and 5 。 on intersecyion. 3st As a result of examining alignment of guide and warning sign in traffic signs cxisting foundation methods are less affected by lane than by apeed of a vehicle.

pH조절이 음식폐기물의 유기산발효에 미치는 영향 (Effect of pH on the $VFA_s$ fermentation in the anaerobic treatment of food waste)

  • The purpose of this study was to investigate the created liquid post-acid fermentation of usability of denitrification as exterior carbon sources by pH control. The time of acid fermentation of food waste, the slower loading capacity of organic matter, the faster decomposition rate, but the density of generation Volatile Fermentation Acids(VFAs), was weak and, $SBOD_{5}$ : ST-N rate and $SBOD_{5}/SCOD_{Cr}$ rate was low. Between TS and VS, VS was decreased to 4.5th day fast, and then was decreased slowly. 1.5 days after stating the experiments, $SCOD_{Cr}$, $SBOD_{5}$, STOC and $VFA_{s}$ was decreased of increased slowly, and then increased fast. And after showing the highest density, it was tended to decreased fast. At the time of $SBOD_{5}$ with the highest density, at $SBOD_{5}$ : ST-N ratio, $R_1$ was 303:1, $R_2$ was 319:1, $R_3$was the highest. After studying $SBOD_{5}$ : ST-N ratio and $SBOD_{5}/SCOD_{Cr}$ ratio, as a carbon source of biological denitrification it was profitable composition ratio.

Kudo형 주관절 전치환술에서 골용해 (Osteolysis in Kudo Type Total Elbow Arthroplasty)

  • This study evaluated the development of osteolysis of Kudo total elbow arthroplasties according to the fixation with or without cement. Twenty-four elbows of nineteen patients of rheumatoid arthritis underwent total elbow arthroplasty from 1986 to 1993. The patients were followed for an average of 42 months ranging from 36 months to 59 months. In cemented humeri, 1 of 13 (7.7%) showed osteolysis. In the humeri of cementless fixation, 5 of II (45.5%) revealed osteolysis, representing higher rate compared to cement fixation (P<0.05). In the ulna, osteolysis was identified in none of 16 (0%) of cement fixation, which was significantly less than the incidence of osteolysis in cement less fixation (3 of 8,37.5%) (P<0.01). It was concluded that the cement fixation result in less osteolysis than the cementless fixation in Kudo type total elbow arthroplasty.
