Retrospective analysis of intensity-modulated radiotherapy and three-dimensional conformal radiotherapy of postoperative treatment for biliary tract cancer

  • Purpose: This study was conducted to compare the outcome of three-dimensional conformal radiotherapy (3D-CRT) and intensity-modulated radiotherapy (IMRT) for the postoperative treatment of biliary tract cancer. Materials and Methods: From February 2008 to June 2016, 57 patients of biliary tract cancer treated with curative surgery followed by postoperative 3D-CRT (n = 27) or IMRT (n = 30) were retrospectively enrolled. Results: Median follow-up time was 23.6 months (range, 5.2 to 97.6 months) for all patients and 38.4 months (range, 27.0 to 89.2 months) for survivors. Two-year recurrence-free survival is higher in IMRT arm than 3D-CRT arm with a marginal significance (25.9% vs. 47.4%; p = 0.088). Locoregional recurrence-free survival (64.3% vs. 81.7%; p = 0.122) and distant metastasis-free survival (40.3% vs. 55.8%; p = 0.234) at two years did not show any statistical difference between two radiation modalities. In the multivariate analysis, extrahepatic cholangiocarcinoma, poorly-differentiated histologic grade, and higher stage were significant poor prognostic factors for survival. Severe treatment-related toxicity was not significantly different between two arms. Conclusions: IMRT showed comparable results with 3D-CRT in terms of recurrence, and survival, and radiotherapy toxicity for the postoperative treatment of biliary tract cancer.

Prevalence and Characteristics of Antimicrobial-Resistant Staphylococcus aureus and Methicillin-Resistant Staphylococcus aureus from Retail Meat in Korea

  • This study was to investigate the prevalence and characteristics of antimicrobial-resistant Staphylococcus aureus and methicillin-resistant S. aureus (MRSA) from 4,264 retail meat samples including beef, pork, and chicken in Korea between 2013 and 2018. A broth microdilution antimicrobial susceptibility testing was performed for S. aureus. Molecular typing by multilocus sequence typing (MLST), spa typing, and pulsed-field gel electrophoresis (PFGE), was performed on mecA-positive S. aureus strain. S. aureus was isolated at a rate of 18.2% (777/4,264), of which MRSA comprised 0.7% (29 strains). MLST analysis showed that 11 out of the 29 MRSA isolates were predominantly sequence type (ST) 398 (37.9%). In addition, ST72, ST692, ST188, ST9, and ST630 were identified in the MRSA isolates. The spa typing results were classified into 11 types and showed a high correlation with MLST. The antimicrobial resistance assays revealed that MRSA showed 100% resistance to cefoxitin and penicillin. In addition, resistance to tetracycline (62.1%), clindamycin (55.2%), and erythromycin (55.2%) was relatively high; 27 of the 29 MRSA isolates exhibited multidrug resistance. PFGE analysis of the 18 strains excluding the 11 ST398 strains exhibited a maximum of 100% homology and a minimum of 64.0% homology. Among these, three pairs of isolates showed 100% homology in PFGE; these results were consistent with the MLST and spa typing results. Identification of MRSA at the final consumption stage has potential risks, suggesting that continuous monitoring of retail meat products is required.

STRESS ANALYSIS AT SUPPORTING TISSUE OF ABUTMENT TEETH AND RESIDUAL RIDGE ACCORDING TO DENTURE DESIGN WITH REMAINING UNILATERAL POSTERIOR TEETH (편측 후방 치아 잔존시 의치 설계에 따른 지대치지지 조직과 잔존 치조제의 응력 분석에 관한 연구)

  • This study was peformed to investigate the distribution and magnitude of stress at supporting tissue of abutment teeth and residual ridge tissue with remaining unilateral posterior teeth. Four types of removable partial dentures that included clasp retained removable partial denture, attachment retained removable partial denture, telescopic removable partial denture, and swing-lock partial denture were designed, and strain gauge was used for stress analysis. Each prosthesis was subjected to simulated vertical and oblique load. The following conclusions were drawn from this study. 1. The clasp retained removable partial denture generally distributed simulated vertical force more evenly to the supporting structure. 2. The stress at buccal side of 1st premolar was the lowest in swing-lock partial denture and that was highest in attchment retained removable partial denture. The stress at lingual side of 1st premolar was the lowest in telescopic partial denture. 3. In clasp retained removable partial denture, stress was lower at load site and ridge crest at mid-line, but it was higher at 1st premolar area on vertical load. 4. In attachment removable partial denture, stresses at buccal side of 1st premolar. lingual side of 1st premolar on vertical load, and ridge crest at midline on oblique load were higher. 5. In telescopic removable partial denture, stress at lingual side of 1st premolar was the least in all removable partial dentures, but the stress at load site was higher. 6. In swing-lock removable partial denture, stress at buccal side of 1st premolar was the lowest, and stresses at load site and distal end of residual ridge crest were higher.

Evaluation of Antioxidant Potential and UV Protective Properties of Four Bacterial Pigments

  • In the present study, four distinctly colored bacterial isolates that show intense pigmentation upon brief ultraviolet (UV) light exposure are chosen. The strains are identified as Micrococcus luteus (Milky yellow), Cryseobacterium pallidum (Yellow), Cryseobacterium spp. (Golden yellow), and Kocuria turfanensis (Pink) based on their morphological and 16S rDNA analysis. Moderate salinity (1.25%), 25-37℃ temperature, and pH of 7.2 are found to be the most favorable conditions of growth and pigment production for all the selected isolates. The pigments are extracted using methanol: chloroform (1:1) and the purity of the pigments are confirmed by high-performance liquid chromatography (HPLC) and thin-layer chromatography (TLC). Further, Fourier transform infrared (FTIR) and UV-Visible spectroscopy indicate their resemblance with carotenoids and flexirubin family. The antioxidant activities of the pigments are estimated, and, all the pigments have shown significant antioxidant efficacy in 2,2'-azino-bis(3-ethylbenzothiazoline-6-sulfonic acid) (ABTS), 2,2-diphenyl-1-picryl-hydrazyl (DPPH), and ferric reducing antioxidant power (FRAP) assays. The UV protective property of the pigments is determined by cling-film assay, wherein, at least 25% of UV sensitive Escherichia coli survive with bio-pigments even after 90 seconds of UV exposure compared to control. The pigments also hold a good sun protective factor (SPF) value (1.5-4.9) which is calculated with the Mansur equation. Based on these results, it can be predicted that these bacterial pigments can be further developed into a promising antioxidant and UV-protectant for several biomedical applications.

Toxic Effect of Cadmium on the Amphibian Axis Formation (카드뮴이 양서류의 체축 형성에 미치는 독성 효과)

  • Effect of cadmium on the early amphibian development was analyzed through FETAX (Frog Embryo Teratogenesis Assay: Xenopus). Embryos manifested concentration-dependent mortality and realformations; shortage of anterior-posterior axis gut realformation, ocular anomalies, bent notochord, misshapen dorsal fin, and derreal blisters. The treatment with 1.5ppm cadmium solution caused 100% mortality and concentration of lppm did not kill the embryos that caused 100% anomaly. The teratogenic index (TI = LC50 /EC50) was 2.8 indicating that $CdCl_2$ is teratogenic for Xenopus laevis. Embryos that were pulsetreated with at early to late blastula stage (St. 3-9) and mid to late blastula stage (St. 6-10) showed relatively strong resistance to cadmium, but the embryos treated at gastrula stage (St. 10-13) showed high mortality. And the embryos treated at tailbud stage (after St. 25) showed highest mortality of any other early stages. Effects of temperature were studied through pulse- treatment during gastrula stage at $20^{\circ}C$ and $30^{\circ}C$. The embryos treated with 7.5ppm at $30^{\circ}C$ and 15ppm at $20^{\circ}C$ caused 100% mortality respectively, indicating that higher temperature had more severe toxic effect. One of the most peculiar effect of cadmium at gastrulation was distortion of the tail. The probable cause of toxic effect of Cd was discussed.

Idiopathic Hypertrophic Cranial Pachymeningitis Misdiagnosed as Acute Subtentorial Hematoma

  • A case of idiopathic hypertrophic cranial pachymeningitis (IHCP) misdiagnosed as an acute subdural hematoma is reported. A 37-year-old male patient presented with headache following head trauma 2 weeks earlier. Computerized tomography showed a diffuse high-density lesion along the left tentorium and falx cerebri. Initial chest X-rays revealed a small mass in the right upper lobe with right lower pleural thickening, which suggested lung cancer, such as an adenoma or mediastinal metastasis. During conservative treatment under the diagnosis of a subdural hematoma, left cranial nerve palsies were developed (3rd and 6th), followed by scleritis and uveitis involving both eyes. Magnetic resonance imaging (MRI) revealed an unusual tentorium-falx enhancement on gadolinium-enhanced T1-weighted images. Non-specific chronic inflammation of the pachymeninges was noticed on histopathologic examination following an open biopsy. Systemic steroid treatment was initiated, resulting in dramatic improvement of symptoms. A follow-up brain MRI showed total resolution of the lesion 2 months after steroid treatment. IHCP should be included in the differential diagnosis of subtentorial-enhancing lesions.

Carpal Tunnel Syndrome Caused by Lipofibromatous Hamartoma of the Median Nerve

  • Lipofibromatous hamartoma (LFH) is a rare tumor of the peripheral nerves, which usually involves the median nerve. The authors reported on two rare cases of carpal tunnel syndrome due to LFH of the median nerve. A 49-year-old female patient complained of the mass and symptoms consistent with LFH. Magnetic resonance imaging (MRI) showed typical LFH findings. The symptoms were successfully ameliorated with carpal tunnel release and external neurolysis. A 37-year-old female patient complained of weakening thumb abduction and the mass where the MRI showed atypical findings. Opponensplasty and debulking operations were performed after which thumb abduction was improved; however, neurological sequelae remained. LFH of the median nerve is managed on a case-by-case basis as treatment guidelines are not very clearly defined yet. However, the less invasive treatment such as carpal tunnel release and external neurolysis than more aggressive surgical treatment should be recommended as a treatment option.

A Study of Morphology of Maxillary Sinus by using the Orthopantomograms (Orthopantomogram을 이용한 상악동의 형태에 관한 연구)

  • This study was performed in order to analyse the morphology of maxillary sinus on orthopantomogram. The author anlaysed the anterior-posterior width of mesial wall, the extension of anterior margin in maxillary sinus and the interrelation between the root apex of maxillary 1st molar and sinus floor. The films consisted of 514 orthopantomograms divided into three groups ranging the 3rd decade, 4th and 5th decade and 6th decade. The obtained results were as follows: 1. The mean dimensions of the anterior - posterior width of mesial wall were 43.81±4.55㎜ for male and 43.80±4.49㎜for female in the right of maxillary sinus, 43.75±74㎜ for male and 43.37±3.92㎜ for female. 2. With age, the dimensional change of the anterior - posterior width of mesial wall in maxillary sinus was not observed. 3. The extension order of anterior margin of the maxillary sinus was distal to canine side, 1st premolar mesial side, canine mesial side, and 1st premolar distal side. 4. In the distance between the root apex of upper 1st molar and the maxillary sinus floor, the degree of closeness was more increased in the older-aged group than in the younger-aged group.

  • The author has taken intraoral standard films by the paralleling technique of 6414 teeth in 838 patients ranged from early twenties to early sixties and analysed the change of pulp cavity in the mandibular posterior teeth by ageing. The obtined results were as follows : 1. There was no significant difference between the right and left teeth. 2. The degree of change of pulp cavity in female was higher than that in male. 3. The change of pulp cavity was increased by ageing. 4. Studies on the Changes in the Alliinase Activity during the Aging of Pickled Garlic (마늘장아찌 숙성 중 Alliinase 활성 변화에 관한연구)

  • Changes in the alliinase activity during the aging of pickled garlic samples prepared by the several methods were investigated. The activity of alliinase in raw garlic was 8,37 units/mg protein. The ac-tivity in the garlic pickled with swoy sauce was reduced to 4.57 units/mg with 52% remaining by 1st week of pickling and to 1.05 units/mg protein with 12% remaining by 2nd week of pickling. The ac-tivity of alliinase in the garlic pickled with vinegar was 2.79 units/mg protein with 32% remaining by 1st week of pickling and was 0.26 units/mg protein only with 3% remaining by 2nd week of pickling. The activity of alliinase in the garlic pickled with 10% salt solution was 5.06 units/mg protein with 58% remaining by 1st week of pickling. After one week pickling the juice of pickled garlic was removed. Then garlics were pickled again with vinegar. The allinase acting in was reduced to 0.85 units/mg pro-tein with 10% remaining by 2nd week of pickling. The activity of alliinase in the garlic pickled with vin-egar was 2.79 units/mg protein with 32% remaining by 1st week of pickling. The juice of pickled garlic was removed after one week. Then the galics were again pickled with saysauce. The allinase activity in the garlic the garlic pickled again with soy sauce was reduced to 0.43 units/mg protein with 5% remain-ing by 2 week of pickling.
