Reducing Irrational Beliefs and Pain Severity in Patients Suffering from Non-Cardiac Chest Pain (NCCP): A Comparison of Relaxation Training and Metaphor Therapy

  • Background: Patients suffering from non-cardiac chest pain (NCCP) can interpret their chest pain wrongly despite having received a correct diagnosis. The objective of this study was to compare the efficacy of the relaxation method with metaphor therapy for reducing irrational beliefs and pain severity in patients with NCCP. Methods: Using a randomized controlled trial, 33 participants were randomly divided into a relaxation training group (n= 13), a metaphor therapy group (n = 10), and a control group (n = 10), and were studied for 4 weeks. The two tools used in this research were the Brief Pain Inventory (BPI) index for determining the degree of pain and the short version of the Jones Irrational Belief Test. Metaphor therapy and a relaxation technique based on ${\ddot{O}}$st's treatment were used as the interventions. The collected data were analyzed with a multivariate analysis of covariance (MANCOVA), a Chi-square test, and the Bonferroni procedure of post-hoc analysis. Results: The relaxation training method was significantly more effective than both metaphor therapy and the lack of treatment in reducing the patients' beliefs of hopelessness in the face of changes and emotional irresponsibility, as well as the pain severity. Metaphor therapy was not effective on any of these factors. In fact, the results did not support the effectiveness of metaphor therapy. Conclusions: Regarding the effectiveness of the relaxation method as compared with metaphor therapy and the lack of treatment in the control group, this study suggests that relaxation should be paid greater attention as a method for improving the status of patients. In addition, more studies are needed to determine the effectiveness of metaphor therapy in this area.

  • The purpose of this study was investigation to examine the prevention diabetic mellitus and treatment effect on Gastrodia Elata Blume(GEB) dieted at prior and after induced diabetic application. Prior induced diabetic 3 weeks ago application GEB dieted. It is to analysis changes in body weight, blood glucose, SOD, CAT and histopathological findings. For the fingding significantly concentration diabetic rats were divided into 3 different experimental groups and each groups were induced diabetic. Experimental group Ⅰ (STZ-induced diabetic rats; n=10), experimental group Ⅱ (after induced DM and GEB dieted rats; n=10), experimental group Ⅲ (Prior GEB dieted thereafter DM induced; n=10). Prior and After GEB dieted application was that body weight, blood glucose were increase in experimental group Ⅱ, Ⅲ. Specially, group Ⅲ was significantly change than group Ⅱ at 1st weeks. and the level of CAT were significantly decrease in experimental group Ⅱ, Ⅲ than group Ⅰ. but SOD level was increase in experimental group Ⅱ, Ⅲ than group Ⅰ. In histological observation; group Ⅰ showed decrease in the intensity and incidence of vacuolations, cellular infiltration and hypertrophy of in liver and kidney. The Gomori's stain result, group Ⅰ showed disruption ${\beta}$-cell in pancrease.

Effects of Fall Prevention Education on the Variables Related to Using Orthosis and Fear of Falling in Fracture Patients Wearing the Leg Orthosis (하지보조기 사용 골절환자를 위한 낙상예방교육이 보조기 사용관련 변수 및 낙상공포감에 미치는 효과)

  • Purpose: The purpose of this study was to test the change of study variables (knowledge, efficacy, and fatigue) related to using orthosis and fear of falling in fracture patients wearing the leg orthosis after fall prevention education in terms of educational method and frequency. Methods: Participants were 87 fracture patients wearing the leg orthosis. Experimental I group (n=30) and experimental II group (n=27) received the fall prevention education once and three times respectively with leaflets. Experimental III group (n=30) received video training once. Results: The level of the subjects' knowledge was significantly increased in experimental I and II groups rather than in experimental III group. In case of experimental I and experimental II group, fear of falling was decreased when compared to experimental III group. However, there were no significant changes in efficacy and fatigue related to using orthosis among three groups. Conclusion: The fall prevention education using leaflets was more effective than video training method. Only one education with leaflets was effective enough. Therefore, it is recommended that the education with leaflets or pamphlets should be developed systematically according to the characteristics of fracture patients wearing the leg orthosis.

Effect of Meridian Muscle Release and the Graston Technique on Pain and Functional Movement in Patients with Myofascial Pain Syndrome of the Shoulder Joint (경근 이완 기법과 그라스톤 기법이 어깨 관절 근막 통증 증후군 환자의 통증 및 기능 향상에 미치는 효과)

  • PURPOSE: This study examined the effects of the meridian muscle release technique on the pain and functional movement of patients with myofascial pain syndrome (MPS) of the shoulder joint. METHODS: The subjects of this study included 45 patients with MPS of the shoulder joint. The subjects were divided randomly into the following groups; the meridian muscle release technique group (n=15), the Graston technique group (n=15), and the control group (n=15). Both the meridian muscle release group and the Graston technique group received conventional therapy for 35 minutes initially and were then treated using the meridian muscle releases technique and Graston technique for 10 minutes, respectively. The control group received only conventional therapy for 35 minutes. All three groups underwent treatments three times a week for four weeks. Each subject was evaluated randomly using the VAS, PPT, SPADI and ROM both before and after treatment. RESULTS: The Graston technique group showed a significantly more substantial increase in functional movement (p<.05) than the meridian muscle release technique and control groups. The meridian muscle release technique group had significantly less pain (p<.05) compared to the Graston technique and control groups. CONCLUSION: These findings suggest that the meridian muscle release technique can be useful for decreasing pain and increasing the functional movement of patients with MPS of the shoulder joint.

Characteristics of Horizontal Community Distribution and Nutrient Limitation on Growth Rate of Phytoplankton during a Winter in Gwangyang Bay, Korea (동계 광양만에서 식물플랑크톤 군집구조의 수평적 분포특성과 성장에 미치는 영양염 제한 특성)

  • To estimate the effects of limitation nutrients for phytoplankton growth and its influences on short-term variations of a winter phytoplankton community structure, we investigated the abiotic and biotic factors of surface and bottom waters at 20 stations of inner and offshore areas from 6 to 7 February in Gwangyang Bay, Korea. Also, several algal bio-assay studies were conducted to identify any additional nutrient effects on phytoplankton assemblage using surface water for the assay. The dominant species in the bay was diatom Skeletonema costatum, which occupied more than 70% of total species in most stations (St.1-16) of the inner bay. According to a cluster and multidimensional scaling (MDS) analysis based on phytoplankton community data from each station, the bay was divided into three groups. The first group included stations from the south-western parts of Myodo lsland, which can be characterized as a semien-closed eutrophic area with high phytoplankton abundance. The second group included most stations from the north-eastern part of Myodo lsland, influenced indirectly by surface water currents from offshore of the bay. The standing phytoplankton crops were lower than those of the first group. The other cluster was restricted to samples collected from offshore of the bay. In the bay, silicon (Si) and phosphorus (P) were not a major limiting factor for phytoplankton production. However, since the DIN: DIP and DSi: DIN ratios clearly demonstrated that there were potential stoichiometric N limitations, nitrogen (N) was considered as a limiting factor. Based on the algal bio-assay, in vivo fluorescence values in N (+) added experiments were higher compared to control and P added experiments. Our results suggested that nitrogen may act as one of the most important factors in controlling primary production during winter in Gwangyang Bay.

On Ordination, Clustering and Neighbourhood Effects in the Semi-natural Pine Stands in Central Korea (반자연 소나무 숲에 있어서의 Ordination 미분류 및 인근 효과 ( 경쟁 ) 에 대하여)

  • To discern general tendency in relatively pure even-aged pine stands, to group the stands and to perceive neighbourhood effects a total of 39 sites of pine stand was surveyed from nearby Seoul (12 sites), Chunsung, Kangwon (13 sites) and Sosan, Chungnam (14 sites), for herb and shrub species 32, 19; 37, 19 and 41, 14 in the respective areas from September 1987 to July 1988. In terms of detrended correspondence analysis (DECORANA), the stands were subjected to ordinate with 16 physical variables and the vegetational variables. The resource ratio (N:P, N:K, P:K) as physical variables also was tried out in the DECORANA as well as independent variable (N.P.K). The outcome did not show any meaningful difference. It is suggested that there seems to be no apparent interaction among the elements in the study. Three vertical vegetation componeent, that is, tree layer, shrub layer, herb layer were subjccted to DECORANA independently, pairwisely and as a whole (a total 7 combinations). Of those analysis herb layer trial alone seems to indicate relatively clearer differences among the physical variables. In the stands nearby Seoul first axis indicated soil field capacity and exchangeable cations (K, Ca and Na) and second axis did not show any tendency. For the Chunsung stands first axis also revealed soil field capcity and amount of arganic matter and second axis showed amount of exchangeable cation (K, Ca and Na), In the Seosan 1st axis indicated pH and exchangeable cations (K, Ca and Na). For the 39 sites 4 clusters in terms of herb layer might be defined: Peucedanum terebinthaceum-Cymbopogon tortilis-Polygala japonica-Festuca ovina (1); Atractylodes japonica-Patrina scabiosaefolia (2); Potentilla fragarioides-Atractylodes (3); and Cymbopogon tortilis (4). In the neighbourhood effects study in terms of the basal area distribution, Thiessen polygon area and Gini coefficient for the Pinus thunbergii stands of Seosan the Thiessen polygon area approach seems to indicate earlier (30 years old) neighbourhood effect than the others (45 years).

A sham moxibustion device and Masking test (가짜뜸 개발 및 Masking test)

  • TObjectives: Develop a sham moxibustion and determine whether subjects can distinguish a sham moxibustion from a real moxibustion. Design: Single-blinded, randomized, placebo-controlled clinical trial Methods: Sham and real moxibustion resemble each other in appearance, burning procedure, but the base of the sham moxibustion isolates the moxa-producing heat and smoke. This device was tested in a clinical trial in which subjects received moxibustion at Zhongwan(CV12), Mingmen(GV4), Quchi(LI11), Zusanli(ST36), Taichong(LR3)), Hegu(LI4). Volunteers(n=32) were given pre-treatment questionnaire to assess their experience in getting moxibustion therapy and performing it. They randomized into treatment(n=16) or sham controlled group(n=16), received moxibustion according their groups. After treatments, the effectiveness of blinding was assessed. Results: There were no significant differences between two groups in sex, age, moxibustion experience. In the treatment group and the sham group, the number of subjects who believed they received real moxibustion or sham is not different significantly.(P=.668) The consistency of a moxibustion type which subjects received actually and the their guess about that, isn't different significantly in two groups.(P=.465) Conclusion: The sham moxibustion was successfully validated in this study, The results demonstrate that this sham moxibustion blinds subjects and can be used as effective placebo-control in moxibustion clinical trials.

  • Spinel $LiMn_{2-y}$$M_{y}$ $O_4$powder was prepared solid-state method by calcining the mixture of LiOH - $H_2O$, Mn $O_2$, ZnO and MgO at 80$0^{\circ}C$ for 36h. To investigate the effect of substitution with Mg, Zn cation, charge-discharge experiments and initial impedance spectroscopy performed. The structure of $LiMn_{2-y}$$M_{y}$ $O_4$crystallites was analyzed from powder X-ray diffraction data as a cubic spinel, space group Fd3m. all cathode material showed spinel phase based on cubic phase in X-ray diffraction. Ununiform which calculated by (111) face and (222) face was constant in spite of the change of y value, except PUf\ulcorner LiM $n_2$ $O_4$. The discharge capacities of the cathode for the cation subbstitUtes $LiMn_{2-y}$$M_{y}$ $O_4$/Li cell at the 1st cycle and at the 40th cycle were about 120~124 and 108~112mAh/g except LiM $n_{1.9}$Z $n_{0.1}$ $O_4$/Li cell, respectively. This cell capacity is retained by 93% after 40th cycle. AC impedance of $LiMn_{2-y}$$M_{y}$ $O_4$/Li cells revealed the similar resistance of about 65~110$\Omega$ before cycling. before cycling.g.g.

Development of Design and Field Quality Control Techniques of Shallow Foundation Resting on Intermediate Geomaterials (중간토에 지지된 직접기초의 설계 및 현장품질관리기법 개발)

  • To suggest a modified shallow foundation design method which can be considered the scale effect of foundation on IGM(intermediate geomaterial) soil layer, the weathered soil layer that is uniformly formed up to 8m(2B) with over 50 N-value is selected and 3 times field loading tests are performed on several sized square-shaped shallow foundations with 30, 75, 150, 240 and 400cm in width respectively. Because the soil modulus of elasticity(Es) calculated by soil investigation and 1st field test(PBT) results showed an underestimated tendency, a modified correlation is required for the reasonable estimation of Es on the weathered soil. Also, the N-value was increased with an increasing in depth. However, the N-values around the test foundations showed the different values even though the foundations on the same level because the test site was arranged by excavation. Therefore, the more detail soil investigations are required for the each test foundations respectively. Since Es based on elasticity theory is determined by the stress distribution shape of the foundation and elasticity modulus of the soil, the scale effect considered pressure-settlement curve can be clearly derived from the correlation on stress distribution shape and the variation of soil elasticity modulus with depth. Therefore, the modified correlation will be suggested to estimate a reasonable Es on the weathered soil, and the scale effect considered shallow foundation design method is also developed based on the elastic theory and field tests in this research.

Effects of EA Application to GV20 and LI4 on BAX and NF-kB Expression of the SD-Rat's Hippocampus with Global Ischemia

  • This study aims to reveal how EA affects BAX and NF-kB involved in cell deaths from global ischemia, and to do this, observes the changes of BAX and NF-kB caused by EA application after transient global ischemia. The experimental method is to give rise to global ischemia and apply EA to 27 SD rats with the particulars of being six-week-old, male, around-300 gram-weighing, and adapted to laboratory environment for more than a week, and divide them into three groups, that is, GV20 EA group(n=9), L14 EA group(n=9), no-treatment GI group(n=9), and then observe their changes of BAX and NF-kB at the time lapse of 6 hours, 9 hours and 12 hours after ischemia, using western blotting. The numerical decrease of BAX expression at the time lapse of 9 hours after EA application, though not statistically significant, was observed in GV20 EA group and L14 EA group, and the NF-kB expression appeared statistically significant decrease in GV20 EA group and L14 EA group, but the expression was higher in the group with EA application. Therefore, EA application at the early phase of global ischemia is considered to affect BAX and NF-kB and play a positive role in decreasing apoptosis and cell deaths by inflammation.
