학년에 따른 간호학생의 찔림사고 현황 조사 및 관련요인 (Needlestick and Sharps Injuries of Nursing Students)

  • Purpose: The purpose of this study was to find factors influencing needlestick and sharp injuries (NSI). Methods: This study was a descriptive research for current situation of NSI, recognition and implementation for standard precaution, and factors influencing NSI exposures. The subjects were 450 nursing students of 2 universities in Seoul and Kyunggi-do. The data were collected from June to November, 2008. The gathered data were analyzed by descriptive statistics and regression analysis. Results: The 33.8% of 450 nursing students experienced NSI during 2 weeks clinical training. NSI events were more common among third-year students than second-year students. The most common causative device causing needlestick injuries was blood glucose lancet as 12.4%. Frequencies influencing NSI were students' year and experience of needlestick injuries. Conclusion: The results suggest development of education guidelines for nursing students and implementation of continual systematic education and training in clinic to control blood-borne infections efficiently.

반도체 직업병 10년 투쟁의 법·제도적 성과와 과제 (Legal and Institutional Outcomes from the 10-year Struggle against Occupational Diseases of Semiconductor workers)

  • 시민단체 '반올림'을 중심으로 지난 10년간 이루어진 반도체 직업병 투쟁은 법 제도적 측면에서 상당한 성과를 이루었다. 먼저 직업병 인정 투쟁에서 총 24명의 재해노동자가 10개 질환으로 법원과 근로복지공단의 직업병 인정을 받았다. 특히 각 사건에 대한 법원 판결들이 대상 사업장과 질병을 확장했을 뿐 아니라 인정 논리면에서도 발전하는 모습을 보여 왔다. 그 정점에는 2017년 8월에 나온 대법원 판결이 있다. 직업병 예방 대책으로 가장 중요시된 '노동자 알권리'와 관련해서도 의미 있는 법안과 판결, 정부 지침이 나왔다. 안전보건자료 공개, 영업비밀 사전 심사제 등을 도입하는 산업안전보건법 개정안이 국회에서 논의 중이고, 최근 법원은 반도체 공장에 관한 정부의 안전보건 진단 결과를 공개하라고 판결했다. 고용노동부도 최근 안전보건자료를 적극적으로 공개하도록 하는 내부 지침을 마련했다. 이 연구는 이러한 판결과 법안, 지침 등이 나오게 된 구체적인 경위와 각각의 내용들을 정리하고 그 의미를 분석함으로써, 앞으로 계속될 '전자산업 노동건강권' 운동의 밑거름이 될 수 있도록 하고자 한다.

손가락보호대의 적용이 간호대학생의 유리앰플 개봉 시 찔림사고, 불안 및 사용자만족도에 미치는 효과 (Effects of a Finger Guard while Opening the Glass Ampoule by Nursing Students)

  • Purpose: The purpose of this study was to identify the effects of a finger guard developed to prevent sharp injuries in nursing students. Methods: This study was an equivalent control group posttest design. Seventy nursing students were randomly allocated to either the experimental group (n= 35) or the control group (n= 35). The finger guard was used whileopening the glass ampoule in the experimental group. The outcome variables such as sharp injuries, anxiety and user satisfaction were measured. Results: Sharp injuries were 0 in the experimental group and 2 in the control group (p= .160). Anxiety in the experimental group was significantly lower than the control group (p< .001). User satisfaction was 4.33 score in the range from 1 to 5, the highest item was the weight (4.63), followed by effectiveness (4.51). Conclusion: Using a protective device while opening the glass ampoule was observed to be effective in reducing anxiety among the nursing students, and exhibited protection of skin.

아동병동 간호사의 병원감염 표준주의에 대한 인지도와 이행도 (Study on Pediatric Nurses' Attitudes and Compliance with Hospital Infection Standard Precautions)

  • Purpose: The purpose of this study was to identify pediatric nurses' attitudes to, and compliance with, standard precautions in the prevention of hospital infections by enhancing their practice of standard precautions. Methods: The participants were 206 nurses who worked in pediatric nursing departments of a general medical institution in Seoul. The questionnaire was a modification of Kim (2008) and consisted of 18 questions about hand washing, personal protective equipment, sharps, linen and patient care equipment. Collected data were processed using SPSS 15.0 WIN. Results: Mean scores for attitudes to standard precautions and for compliance with standard precautions were 4.43 (${\pm}0.83$) and 4.22 (${\pm}0.44$) respectively. This difference was statistically significant (t=3.368, p=.001). The nurses' compliance with standard precautions differed significantly according to the general characteristics of age (F=8.705, p<.001), total clinical experience (F=9.426, p<.001), current department experience (F=6.555, p<.001), and education experience (t=0.616, p<.043). There was a positive correlation between attitudes to, and compliance with, standard precautions (r=.156, p=.025). Conclusion: The results of this study indicate that educational programs and policy on infection control and standard precautions for pediatric nurses are needed. Also these results should contribute to baseline data for establishing appropriate clinical policy on infection control.

Current Status of Infection Prevention and Control Programs for Emergency Medical Personnel in the Republic of Korea

  • Objectives: Emergency medical personnel (EMPs) are pre-hospital emergency responders who are at risk of exposure to infections and may also serve as a source for the transmission of infections. However, few studies of infection control have specifically addressed EMPs in the Republic of Korea (hereafter Korea). The goal of this study was to assess the current status of infection prevention and control programs (IPCPs) for EMPs in Korea. Methods: A cross-sectional survey was conducted to quantitatively assess the resources and activities of IPCPs. A total of 907 EMPs in five metropolitan cities completed a structured questionnaire from September 2014 to January 2015. The data were analyzed using descriptive statistics, multi-response analysis, and the chi-square test. Results: The mean age of the participants was $34.8{\pm}15.1years$. IPCPs were found to have weaknesses with regard to the following resources: the assignment of infection control personnel (ICP) (79.5%), hand hygiene resources such as waterless antiseptics (79.3%), the use of paper towels (38.9%), personal protective equipment such as face shields (46.9%), and safety containers for sharps and a separated space for the disposal of infectious waste (10.1%). Likewise, the following activities were found to be inadequately incorporated into the workflow of EMPs: education about infection control (77.5%), post-exposure management (35.9%), and the decontamination of items and spaces after use (88.4%). ICP were found to have a significant effect on the resources and activities of IPCPs (p<0.001). The resources and activities of IPCPs were found to be significantly different among the five cities (p<0.001). Conclusions: IPCPs for EMPs showed some limitations in their resources and activities. IPCPs should be actively supported, and specific IPCP activities for EMPs should be developed.

Bending 30-gauge needles using a needle guide: fatigue life evaluation

  • Background: Dentists bend needles prior to certain injections; however, there are concerns regarding needle fracture, lumen occlusion, and sharps handling. A previous study found that a 30-gauge needle fractures after four to nine 90° bends. This fatigue life study evaluated how many 90° bends a 30-gauge dental needle will sustain before fracture when bent using a needle guide. Methods: Two operators at Element Materials Technology, an independent testing, inspection, and certification company tested 48 30-gauge needles. After applying the needle guide, the operators bent the needle to a 90° angle and expressed the anesthetic from the tip. The needle was then bent back to a 0° angle, and the functionality was tested again. This process was repeated until the anesthetic failed to pass through the end of the needle due to fracture or obstruction. Each operator tested 24 needles (12 needles from each lot), and the number of sustained bends before the needle fracture was recorded. Results: The average number of sustained bends before needle failure was 40.33 (95% confidence interval = 37.41-43.26), with a minimum of 20, median of 40, and a maximum of 54. In each trial, the lumen remained patent until the needle fractured. The difference between the operators was statistically significant (P < 0.001). No significant differences in performance between needle lots were observed (P = 0.504). Conclusion: Our results suggest that using a needle guide increases the number of sustained bends before needle fracture (P < 0.000001) than those reported in previous studies. Future studies should further evaluate the use of needle guides with other needle types across a variety of operators. Furthermore, additional opportunities lie in exploring workplace safety considerations and clinical applications of anesthetic delivery using a bent needle.
