Correlation between The Parents' Character of Orthodontic Patients and Patients Cooperation (치과 교정 환자 부모의 인성(SCL-90-R)과 환자 협조도의 상관관계)

  • The study on correlation between the parents' character of orthodontic patients and patients cooperation. Although dentists have excellent skill and technique, patients' cooperation is always demanded. Patients' poor cooperation results in poor treatment result. Cooperation between psycho-social state of parents and patient's cooperation was studied. The parent's psychosocial state is evaluated with scl-90-r. The cooperation is measured by appointment time compliance, appliance taking rate, breakage of appliance, and fee paying. Parents' psychosocial contents are somatization, obsessive-compulsive, interpersonal sensitivity, depression, anxiety, hostility, phobic-anxiety, paranoid ideation, psychoticism, and additional item. Statistics was done by SPSS WINDOW version 12.0 program. The result showed no correlation between patients' cooperation and their parents' psychosocial state. There is no correlation between father's psychosocial state and patients' cooperation except psychoticism. There is no correlation between mother's psychosocial state and patients' cooperation. However, there was correlation among cooperation variables: appointment time compliance, appliance taking rate, breakage of appliance, cost paying. Appliance taking rate and cost paying showed positive correlation. Our findings suggest that patients' cooperation does not followed their parents' pychosocial state. Other combined factors should be considered like pychosocial state of patient and doctor-patient relationship.

Alexithymia in Somatoform Disorder and Diabetes Mellitus (신체형장애 환자와 당뇨병 환자에서의 Alexithymia)

  • Objectives : "Alexithymia" mean literally "no word for mood(or emotion)". It is not only a marked constriction in emotional functioning but a deficit in their cognitive processing. We designed this study to investigate the level of alexithymia, psychopathology and personality factors of patients with somatoform disorder and with diabetes mellitus. Methods : The subjects were consisted of patients with somatoform disorder(N=20), patients with diabetes mellitus(N=20), and normal control(N=20). The level of alexithymia, psychopathology and personality factors were assessed by the Toronto Alexithymia Scale(TAS), the Symptom Checklist 90-Revision(SCL 90-R), and the Sixteen Personality Factor Questionnaire(16-PF). And we compared demographic characteristics, psychopathology and personality factors among three groups, and assessed the relationship between alexithymia and psychopathology, and between alexithymia and personality factors. Results : The results were as follows. 1) Patients with somatoform disorder showed significantly higher TAS scores compared to patients with diabetes mellitus and the normal control group. 2) Patients With somatoform disorder showed significantly higher scores of somatization, anxiety scales than patients with diabetes mellitus and the normal control group, and showed significantly higher scores of obsessive-compulsive, depression, phobic anxiety, psychoticism scales than the normal control group by the SCL-90-R. 3) The normal control group showed high intelligence scores only as compared to patients with somatoform disorder by the 16-PF. 4) A significant relationship was found between TAS scores and psychopathology in patients with somatoform disorder. 5) All three groups did not shown any correlation between TAS and 16-PF. Conclusion : Patients with somatoform disorder showed higher TAS scores and more multiple psychopathology than patients with diabetes mellitus and the normal controls. A significant relationship was found between TAS scores and psychopathology in patients with somatoform disorder. We suggest that the therapeutic approach to patients with somatoform disorder to express emotions and manage psychopathology, and that the treatment methods of patients with diabetes mellitus aims to improve firstly physical conditions are more helpful.

Mental Health and its Correlates of Marriage-Migrant Women in a City (도시거주 결혼이민여성의 정신건강 현황과 영향 요인)

  • Objective : The objective of this study was to examine the mental health status and its correlates of the marriage-migrant women in Seoul, Korea. Methods : One hundred and seventy marriage-migrant women and one hundred and sixteen married Korean women were recruited from community to complete Symptom Check List-90-Revision (SCL-90-R). Marriage-migrant women were also asked for their socio-demographic factors, acculturative stress, family-relationship stress, and social support. The scores on the SCL-90-R were compared between marriage-migrant women and married Korean women, and the correlates of marriage-migrant women's mental health were investigated. Results : Compared to married Korean women, marriage-migrant women showed lower levels of mental health problems including somatization, obsession-compulsion, interpersonal sensitivity, depression, anxiety, hostility, paranoid ideation, and psychoticism. Longer length of residence in Korea, lower family income, social support, higher acculturation stress, and family relationship stress were all associated with mental health problems of marriage-migrant women Results : Our results suggest that mental health of marriage-migrant women is not necessarily bad, and several factors may affect their mental health. However, further studies are required in a larger representative sample to confirm the study findings.

The Effect of Garden Activities in Apartments on Changes in Mental Health and Quality of Life of Parenting Women (아파트 내 정원 활동이 육아 여성의 정신건강 및 삶의 질 수준 변화에 미치는 영향)

  • Through psychological evaluation, this study attempted to verify the effect of quality of life improvements from gardening activities in apartment landscaping spaces. The study was conducted on 32 mothers raising children. Psychological valuation was conducted using world Health Organization Quality of Life questionnaire(WHOQOL-Bref), the Satisfaction with Life Scale(SWLS), the Life Satisfaction Experience Scale(LSES), and the Symptom Checklist-90-Revised(SCL-90-R). The psychological analysis results showed that, engaging in gardening activities for two weeks had, a positive effect on mothers' environmental of quality of life evaluation. The LSES indicates that satisfaction with oneself significantly increased after starting the gardening activities. The activity resulted in a positive change in attitude toward life. The SCL-90-R results showed changes in the mother's psychological state. There was a significant amelioration of obesessive-compulsive behaviors, hostility, and phobic anxiety. In conclusion, this study was found that, for mothers in apartments, outdoor gardening activities can significantly affect their quality of life. Moreover, it can help prevent mental health deterioration, Further study should be conducted on the beneficial effects of gardening on the variety of age groups and activities.

A study for diagnosis and pattern identification of Hwa-Byung (화병의 진단 및 변증유형에 관한 연구)

  • Objective : This empirical research is performed to recognize diagnostic concept, pattern identification, and clinical features of Hwa-byung. In other words, the aims of this research are to examine the differences of the diagnosis between Hwa-Byung and the other psychiatric disorders, and to find out pattern identification, and clinical characteristics of Hwa-Byung for prescriptions of this syndrome. Method : In the experiment, there were participated 30 patients who were met for our criterions according to HBDIS (Hwa-Byung Diagnostic interview Schedule). These patients were diagnosed as Axis1 according to criterions of DSM-IV with administering SCID-I. OMS-prime was utilized for finding out pattern identification of oriental medicine. Symptom Check List-90-Revision(SCL-90-R), Hemilton rating Scale for Depression(HRSD), Heart Rate Variability(HRV), and Digital Infrared Thermographic imaging(D.I.T.I.) were also utilized to discover clinical characteristics of Hwa-Byung Patients. Results : 1. Regarding Sex-ratio, male subjects were 3(10%), and female subjects are 27(90%). The age of subjects ranged from 22 year old to 75 $(51.87{\pm}11.04;\:Mean{\pm}SD)$ 2. In the results of diagnosis on the basis of DSM-IV, the 17(56.67%) patients were MOD (Major Depressive Disorder), the 5(16.67%) patients were USD (Undifferentiated Somatoform Disorder), the 4(13.33%) patients were Dysthymic Disorder, the 3(10%) patients were GAD (Generalized Anxiety Disorder), and the 1(3.33%) was Panic Disorder. Two of the patients who diagnosed as MOD were diagnosed as Panic Disorder too, and one of them was diagnosed as Pain Disorder too. 3. Regarding pattern identification, Hwa-Byung is positively correlated to deficiency of Heart(心). and then to stagnancy of Liver-Gall bladder. Hwa-Byung is correlated deficiency symptom-complex rather than excessiveness symptom-complex. That is also correlated positively to Pathological heat and fire. 4. In SCL90-R, the mean of PSDI was $(75.3{\pm}10.7;\:Mean{\pm}SD)$. The each mean of the other 11 factors was distributed between50-70. 5. The mean of HRSD was $(17.9{\pm}5.6;\:Mean{\pm}SD)$ in the entire subject's group. Then the group of MDD was $20.9{\pm}4.4$ and the group of USD was $12.0{\pm}4.8$ 6. In the results of HRV. the mean of TP is $972.4{\pm}1174(Mean{\pm}SD)$, this is lower than normal range 1000-200. The other factors were within normal range. Then, there were no significant differences between them (p<0.05). 7. The temperatures of each acupoint have significant differences between HNl(印堂) and PC6(內關), between CV17(顫中) and PC6(內關), between HN1(印堂) and CV8(神闕), between CV17(顫中) and CV8(神闕) in comparison with the average of body temperature in the use of D.I.T.I. (p<0.01) 8. In the analysis of correlation between SCL-90-R, HRSD, HRV. and D.I.T.I. there were no significant results. According to results that the correlation was analyzed with only the MDD group as subjects, there was negative correlation between RMSSD of HRV and HRSD, between LF of HRV and PDSIof SCL-90-R, and between LF/HF of HRV and ANX, PSY, and PDSI of SCL-90-R. Conclusion : In the observation of clinical features of 30 cases of Hwa-Byung patients by using diverse structured tests, there could make diverse diagnosis as depressive disorder, anxiety disorder, and Somatoform Disorder. Particularly. MDD was highly distributed. Considering oriental medicine's pattern identification of Hwa-Byung, this syndrome is related strongly to Heart, and there were demonstrated deficiency symptom-complex, and Pathological heat and fire. One of the limits of this study is lack of control subject's group, therefore, in the future study, it requires reexamination through a comparative research with these data to complete this study.

The Psychological Characteristics of Women in the Obesity Clinic (비만클리닉에 내원한 여성의 심리적 특성)

  • Introduction: This research was performed to contract the attitude of dietary restriction and the psychological problems such as depressive mood and perceived stress and to investigate the relationship of these and obesity in women who visited the obesity clinic. Methods: During May 2001, sociodemographic variables, physical characteristics, Three Factor Eating Questionnaire(TFEQ), Symptom Check List-90-R(SCL-90-R) and Perceived Stress Scale were assessed from 150 female who visited the obesity clinics which were located at downtown, Seoul and the Hospital of Ajou University, Medical College. Hamilton depression rating scale(HDRS) was estimated by author. And then 116 female cases who filled up the questionnaire faithfully were included. Results: Obese group more than Body Mass Index(BMI) $25.0kg/m^2$ was 50% of the total subjects. BMI was increased as the age goes up(p<0.001). The frequency of unmarried cases in the under normal weight group was high rate of 48.8% while it in the obese group was 13.8%(p<0.001). There was no significant difference in the rate of smoking and alcohol drinking among subjects by BMI. There was no significant difference of TFEQ among subjects by BMI and the percent of body fat. Factor 2(r=0.27, p<0.01) and Factor 3(r=0.24, p<0.01) were significantly correlated with Global Severity Index(GSI). Only the paranoia scale among each estimated mean value of T scores of SCL-90-R by BMI was the significant difference between the overweight group and the obese group(p<0.05). T scores of scales of SCL-90-R were less than 50, but T scores of the under normal weight group and the obese group were higher than overweight group. GSI was significantly correlated with HDRS(r=0.75, p<0.01) and Perceived Stress Scale(r=0.32, p<0.01). Depressive mood in the obese group was significantly higher than non-obese group that HDRS was compared to two groups by the percent of body fat(p<0.05). Perceived Stress Scale was no significant correlation with BMI and the percent of body fat. All of the subject were in trouble of high stress. Stress affected dietary restriction owing that perceived stress had a relation with Factor 2(r=0.29, p<0.01) and Factor 3(r=0.37, p<0.01). Also, it affected psychological characteristics owing that perceived stress had a relation with the depression scale, GSI and HDRS(r=0.33, r=0.32, r=0.34, p<0.01). Conclusion: Obese women have more psychological difficulties including depression and high perceived stress, which closely related with the attitude of dietary restriction. Psychiatric intervention and aggressive assessment of psychological problems will be needed to the people who visit the obesity clinic in the future.

  • Personality characteristics were studied by means of SCL-90-R test, Behavioral problem and pain site was studied Isleep disturbance, irregular eating habit, decreased physical activity, decreased social activity, increased emotional tension) by examination sheets. This study was conducted to examine the mature and extent of psychological difference between the students who failed the college-entrance examination and freshment and to determine whether psychological distress acts as a precipitate for craniocervical pain. 96students who failed the college-entrance examination (examination-failed student) and 86 freshmen were included in this study. The obtained results were as follows : Mean T-score of SCL-90-R primary scaled and global Indies in examination-failed students were significantly higher than the freshmen group. In comparison with examination-failed student group, the prevalence of neck and shoulder pain was significantly higher than the freshmen group. When the study and control group were compared in behavioral problem the study group had not more sound sleep than the control group. The study group had more irregular eating habit than control group. The study group had the lower physical activity than control group. The study group had poor social activity than control group. There was no significant difference in emotional tension.

  • 저자는 1990년부터 1993년 사이에 부산대학병원 구강내과에 내원하여 두 개하악장애로 진단되어 보존적 치료를 시행받은 884명의 환자를 대상으로 Solberg의 악관절장애조사 설문지를 작성케한 후, 성, 연령, 병력기간, 진단명, SCL-90-R, 치료에 대한 반응에 따라 환자군을 세분하여 각 환자군의 설문지 문항별 응답양태 및 이에 따른 예후예측을 분석한 바 다음과 같은 결론을 얻었다. 1. 악기능 및 예후악화요인 문항에서는 여성이 높은 응답수를 보인 반면, 기여요인 및 습관문항에 서는 남성이 높은 응답수를 나타내었다. 2. 고령층의 환자에서 행동 및 예후악화요인이 두드러졌다. 3. 만성군이 급성군에 비해 전 문항에서 높은 응답수를 나타내었다. 4. 혼합군 및 근육장애혼자가 관절장애환자에 비해 설문지 전 문항에서 많은 응답을 하였다. 5. SCL-90-R에서 비정상인 환자가 정상군의 환자에 비해 악기능을 제외한 전 문항에서 높은 응답수를 보였다. 6. 치료에 무반응인 환자가 성공한 환자에 비해 습관요인 문항을 제외한 설문지 전체에서 많은 응답을 하였다.

Personality Profile of TMD Patients from the Viewpoint of Pain (통증을 중심으로 본 악관절장애 환자의 인성적 경향에 대한 고찰)

  • Personality characteristics of third decade female TMD patients was studied by means of SCL-90-R test. 112 TMD patients and 96 dental controls were subjected at the department of Oral medicine, Kyungpook National University Hospital. All the patients were divided into 3 groups according to their pain profile namely, MPDS, ID with Pain and ID with out Pain, and dental controls were divided into 2 groups manely, Dental Controls with Pain and Dental Controls without Pain for comparison. The obtained results were as follows : 1. Mean T-scores of SCL-90-R primary scales and global indexes in the patient and control group were within normal range. 2. When compared between groups with pain, MPDS group were significantly higher in scales of SOM, 0-C, I-S, HOS and PHOB than ID with Pain group and in scales of SOM, I-S. DEP, HOS and PHOM than Dental Controls with Pain group. 3. When compared with ID and Control groups in same pain condition, there were no significant differences between ID without pain and Dental Controls without Pain group. 4. When compared between groups with pain and groups without pain, there were significant differences in scales of SOM and O-C between Dental Controls with Pain and Dental controls without Pain group.

Effects of Orofacial Pain Experience on Psychological Symptoms under Emotional Stress (스트레스하에서 구강안면동통 경험이 심리적 증상에 미치는 영향에 관한 연구)

  • The author studied the prevalence and sites of orofacial pain during the latest 6 months, and psychologic symptoms under the emotional stress through SCL-90-R in thirty-five dental students(nineteen men and sixteen women) of school of Dentistry, Chonbuk National University. 26 students(74.3%), 12 men and 14 women, experienced the orofacial pain; headache, toothache, facial & buccal pain, TMJ pain and intraoral pain were occurred in 51.4%, 37.1%, 25.7%, 17.1% and 17.1% respectively. In psychological scores in before-, during- and after examination, there was no significant difference between men and women. On the other hand, when these scores were compared each other in one group with orofacial pain experience and the other group without pain experience, the former showed significantly higher values in somatization, depression, hostility, and paranoid ideation dimension before examination, interpersonal sensitivity and anxiety dimension during examination, and somatization dimension after examination than those of the later group(P<0.05). The changes of psychologic symptoms in before-, during-, and after examination were observed in one group with pain experience and the other group without pain experience, respectively; in the former group the significantly higher values of anxiety dimension in during examination than after examination, and the significantly higher values of interpersonal sensitivity dimension in before examination than after examination(P<0.05) were evaluated.
