A Study on Knowledge, Self-efficacy and Quality of Life in Rheumatic Arthritis Patients (류마티스 관절염 환자의 지식, 자기효능감 및 삶의 질과의 관계 연구)

  • The purpose of this study was to analyze the relationship between knowledge, self-efficacy and quality of life of patients suffering from rheumatic arthritis. The data were collected from 100 rheumatic arthritis patients 1 university hospital, K rheumatic clinic in kwang-ju city for 20 days from March 2 to March 22, 1999, by means of questionnaires. The instruments used in this study were the knowledge scale developed by Kim, the self-efficacy scale developed by Long et al and the quality of life scale developed by Rho. The data were analyzed by t-test, ANOVA, Pearson's Correlation Coefficient, and Stepwise Multiple Regression by using the SAS program. The results of this study were summarized as follows : 1. The total mean score of knowledge was 15.01(SD${\pm}$3.08), In self-efficacy, the total mean score was 1030.40(SD${\pm}$233.42), In quality of life, the total mean score was 135.19(SD${\pm}$11.07). 2. The relationship between general characteristics and knowledge were significant difference in sex(t=19.03, p=.00), occupation(F=2.34, p=.03), types of exercise (F=2.95, p=.0.3), and time of exercise(F=3.20, p.=02). 3. The relationship between general characteristics and self-efficacy were significant difference in religion(F=2.75, p=.04), in monthly salary(F=3.64, p=.01), in occupation(F=2.30, p=.03), in period of rheumatic arthritis(F=2.70, F=.03), in time of exercise(F=3.77, P=.01), and in effectiveness of exercise (F=4.56, p= .02). 4. The relationship between general characteristics and quality of life were significant difference in age(F=3.36, p=.01), monthly income(F=3.11, p=.02), types of housing(t=4.93, p=.02), arid time of exercise(F=3.03, p=.03). 5. There was not significant correlation between the subjects knowledge and quality of life. 6. There was significant correlation between the self-efficacy and quality of life(r=462, p<.011). 7. The main factor influencing on quality of life was self-efficacy(21.4%).

Current Status and Management of Congregate Meal Service Program for the Elderly at Community Centers (사회복지관의 노인 경로식당 급식 서비스 현황)

  • To investigate current status of the elderly meal service program, community centers nationwide were surveyed for congregate meal services. The survey was conducted during the month of December 2002 by mail using self-developed questionnaire on administration, staffs, funding, and meal service management. Only 95 out of 356 community centers (27%) answered and returned the questionnaires, and congregate meal menus were also collected from 20 centers for 492 days. Results were analyzed using SAS package program. In more than half of the elderly congregate meal service programs, following criteria were used in multiple form to allow participation; 65 years old or older, low income, or living alone. All centers served lunch, most of them free of charge, and 88.4% of them provided service 5 days a week. About 79% of the total cost occurred on the congregate meal service was supported by the government. Most of the menus served at congregate meal service were Korean style dish with rice. On the average, each meal provided 3 side dishes including kimchi, in addition to soup or stew. Only 19% of the centers have employed dietitian working for meal service programs; meal menus were prepared by social welfare workers or other non-dietetic personnels in more than 80% of the centers. Food purchasing and food hygiene control was also practiced mainly by social welfare workers or cooks. This study suggests that dietetic professionals are needed to better manage meal service programs for the elderly in both aspects, food hygiene and nutrition. Development of more nutritious menus and determination of appropriate serving sizes for the elderly meal service programs are necessary to meet the RDAs and dietary action guide for the elderly.

A Study on the Informal Cost Burden of the Patients Admitted to the Hospital (입원환자의 비공식적 의료비용 부담에 관한 연구)

  • To estimate total burden of hospital admission over patient of gamily, we need to know the unofficial private expenses in addition to explicit hospital admission fees. This study was conducted from June 29, 2,000 to August 10, 2,000. Subjects were 104 patient at university hospital located at chungnam province. After thorough explanation of purpose and procedures, notebooks are given to each patient or guardian. They are requested to recorded all relevant expenditures occurred during hospital stay. Incomplete records were filled-up by direct personal contact or phones. Datas were summarized and analyzed using SAS statistical package. P-value less than 0.05 was considered significant. The results of the study are as follows: 1. In 96.1% of the patient, guardians stayed at hospital to take care of patients. In 38,8% one of the family members get work-leave or temporary resting from job. Average date of leave was 7.5days. 2. Average informal cost burden per patient was 204,467 won (14,330 won${\sim}$1,594,870 won). Average hospital cost paid by the patient was 1,061,807 won. The ratio of informal cost burden to hospital cost paid by the patient was 0.327. 3. According to the regression analysis, the relevant factors affection informal cost burdens were distance from home to hospital(p=0.018), and duration of hospitalization(p=0.0001). 4. Informal cost burden was composed of expenses for personal expense of care giver (126, 720 won/patient), meal (86,924 won/patient), transportation (77,648 won/patient), necessaries of life (18,789 won/patient), tests and treatments not covered by insurance (17,289 won/patient), medical supplies not covered by insurance (15,280 won/patient), treat for visitors (14,757 won/patient), TV coin (8,247 won/patient), and others (7,582 won/patient). In addition to the hospital cost paid by the patient for hospital admission, the informal cost burdens should be recognised explicitly because it is not small. Significant proportion of informal cost burden is composed of care-giver's personal expense, transportation, meal. It is suggested that some polices are to be devised and implemented enabling that this portion of informal expenses be directed to formal professional nursing care. Thus we can improve the quality of care and decrease discomfort of patient's relatives.

Effects of Home Nursing Intervention on the Quality of Life of Patients with Nasopharyngeal Carcinoma after Radiotherapy and Chemotherapy

  • Background: The effects of home nursing intervention on the quality of life in patients with nasopharyngeal carcinoma (NPC) after radiotherapy and chemotherapy are unclear. According to the characteristics of nursing home patients with nasopharyngeal carcinoma, we should continuously improve the nursing plan and improve the quality of life of patients at home. Materials and Methods: We selected 180 patients at home with NPC after radiotherapy and chemotherapy. The patients were randomly divided into experimental and control groups (90 patients each). The experimental group featured intervention with an NPC home nursing plan, while the control group was given routine discharge and outpatient review. Nursing intervention for patients was mainly achieved by regular telephone follow-up and home visits. We use the quality of life scale (QOL-C30), anxiety scale (SAS) and depression scale (SDS) to evaluate these patients before intervention, and during follow-up at 1 month and 3 months after the intervention. Results: Overall health and quality of life were significantly different between the groups (p<0.05), Emotional function score was significantly higher after intervention (p<0.05), as were cognitive function and social function scores after 3 months of intervention (p<0.05). Scores of fatigue, nausea and vomiting, pain, appetite and constipation were also significantly different between the two groups (p<0.05). Rates of anxiety and depression after 3 months of intervention were 11.1%, 22.2% and 34.4%, 53.3%, the differences being significant (p<0.05). Conclusions: NPC home nursing plan could effectively improve overall quality of life, cognitive function, social function (after 3 months) of patients, but improvement regarding body function is not suggested. Fatigue, nausea and vomiting, pain, appetite, constipation were clearly improved. We should further pursue a personalized, comprehensive measurements for nursing interventions and try to improve the quality of life of NPC patients at home.

Isokinetic Evaluation of Trunk Flexors and Trunk Extensors in Normal Adult Subjects and Patients with Post-operative Herniated Intervertebral Lumbar Disc (정상 성인남자와 요추간판탈출증 수술후 성인남자의 요추부 굴곡근 및 신전근의 등속성 근력평가)

  • Isokinetic evaluation of trunk flexors and trunk extensors was performed at $60^{\circ}/sec\;and\;120^{\circ}/sec$ of angular velocity by using cybex 6000TEF Unit on 31 healthy male white workers and 15 post-operative HILD patients with no significant difference in mean age and mean body weight between two groups, and compared each other. The purpose of this study is to obtain the isokinetic normative strength values and endurance latins for Dunk extensors and trunk flexors, and is to provide a guideline for rehabilitation program of post-operative HILD patients. The collected data were analyzed by ANOVA, Duncan's Nyktuoke Range Test, and Pearson correlation coefficiency in PC-SAS program, The results obtained were as follow ; 1. Post-operative subjects has lower isokinetic values than normal subjects in peak torque, peak torque $\%$ by body weight, total work, total work $\%$ by body weight, average power, average power $\%$ by body weight, TAAE of trunk flexors and trunk extensors, and there are significant differences with statistic value in trunk extensors at $60^{\circ}/sec$ and in trunk flexors and trunk extensors at $120^{\circ}/sec$ between two groups(p<0.05). 2. Pest-ooperative subjects has lower values for angle of peak torque than normal subjects in trunk extensors, and there are significant differences with statistic value at $60^{\circ}/sec$ and $120^{\circ}/sec$ between two groups. 3. Post-operative subjects has higher values for endurance ratios than normal subjects in trunk extensors and flexors, but there are no significant differences with statistic value between two groups. 4. Post-operative subjects has higher values than normal subjects in peak torque ratios, total work ratios, average power ratios of trunk flexors to trunk extensors, and there are significant differences with statistic value between two groups(p<0.01). 5. There is significant positive-correlation with statistic valve between peak torque and height and body weight in normal subjects(p<0.05), but Thjere is significant negative-correlation between peak torque of trunk extensor at $120^{\circ}/sec$ and age (p<0.05). 6. There is significant positive-correlation with statistic value between peak torque of trunk flexors and body weight in post-operative subjects (p<0.05), but There is significant negative-correlation between peak torque of trunk extensor add age (p<0.05). In conclusion, post-operative subjects have greater weakness in trunk musculature than normal subjects, especially there is more significant weakness in trunk extensors than in trunk flexors

Comparative Study on the Job Satisfaction of Part-time Nurses and Internship Nurses (시간제 간호사 및 인턴간호사의 직무만족도에 관한 비교연구)

  • Recently, Korean health care industry managers are paying more attention to customer oriented service, the rationalization of business administration, and quality control of service to adjust their business to outer environment since IMF bailout program. They are focusing on the cost reduction through remodeling the size and structure of man power, viz. labor flexibility. Nurses are not free from this juncture and contingent nurses are increasing fast. This article intends to verify the actual condition of two types of contingent nurses -part-time nurses and internship nurses-and to compare their job satisfaction to provide basic resource for efficient management of nurse man power. The concrete goals of this article are; Firstly, to verify the actual condition of their employment. Secondly, to compare their job satisfaction. And Thirdly, to the relation between employment condition and job satisfaction. To accomplish these research goals, a statistical survey was executed, in which 384 questionnaires - 66 for manager nurses, 318 for contingent nurses-were given to nurses working at 66 general hospitals-which have at least 100 beds-in Seoul. Among them, 121 questionnaires-of 28 general hospitals-were returned. Then, the data coded and submitted to mean, standard deviation, T-test, variance analysis (ANOVA), correlation analysis, multiple regression analysis with SAS program. The research results of the contingent nurses are followings: 1. Two types of contingent nurses shows similar age spans: they are mostly 21 - 30 years old and unmarried. But internship nurses have high level of educational career. Part - time nurses spread at general beds, out -patient part, intensive care part, operation part, etc, but internship nurses work mostly at general beds. 2. Two groups shows difference in actual employment condition: average employment career of part -time nurses is 7.0 months but internship nurses' is 2.0 months: average duty-on days per month of part - time nurses are 23.7 days but internship nurses' are 24.8 days. But there are little difference in average working time per day: 7.7 hours for part -time nurses and 0 hours for internship nurses. 3. The average wage per month for part -time nurses is 836,026 won but for internship nurses is 557,428 won-66.7% of part-time nurses'. Both groups are enjoying little additional pay. 4. Both groups are getting job not so much through advertisement of newspaper or hospitals as through acquainted person or college. 5. Both groups show very high level of job satisfaction: 3.2195 for part -time nurses and 3.2881 for internship nurses. But they show very low satisfaction on payment level compared with other categories and two groups show meaningful difference(P<.0001). 6. The multiple regression test reveals the factors related with job satisfaction: wage level, working part(OR or ICU), age, job career, and motive of contingent job-taking('because I can take care of family duties at the same time') influence positively: motive of contingent job-taking('because I can work regularly without alternation') influences negatively.

Models Describing Growth Characteristics of Holstein Dairy Cows Raised in Korea

  • The objective of the present study was to determine the best model to describe and quantify the changes in live body weight, height at withers, height at rump, body length and chest girth of Holstein cows raised under Korean feeding conditions for 50 months. The five standard growth models namely polynomial linear regression models, regression of growth variables on the first and second-order of ages in days (model 1) and regression of growth variables on age covariates from first to the third-order (model 2) as well as non-linear models were fitted and evaluated for representing growth pattern of Holstein cows raised in Korean feeding circumstances. Nonlinear models fitted were three exponential growth curve models; Brody, Gompertz, and von Bertalanffy functional models. For this purpose, a total of 22 Holstein cows raised in Korea used in the period from April 2016 to May 2020. Each model fitted to monthly growth curve records of dairy cows by using PROC NLIN procedure in SAS program. On the basis of the results, nonlinear models showed the lower root mean square of error (RMSE) for live body weight, height at withers, height at rump, body length and chest girth (12.22, 1.95, 1.55, 4.04, 2.06) with higher correlation coefficiency (R2) values for live body weight, height at withers, height at rump, body length and chest girth (0.99, 0.99, 0.99, 1.00, 1.00). Overall, the evaluation of the different growth models indicated that the Gompertz model used in the study seemed to be the most appropriate one for standard growth of Holstein cows raised under Korean feeding system.

A Comparative Study on Regional Obeserving State of Korean Holiday's Traditional Festive Foods (Centering Around KANGREUNG, DAEJEON, JEONJU and DAEGU Regions) (한국절식(韓國節食)의 여행실태(旅行實態)에 관(關)한 지역별(地域別) 비교연구(比較硏究) -강릉, 대구, 대전, 전주지역을 중심으로-)

  • This study was conducted to research on the observing state of Korean traditional holidays and the traditional foods related to those days through the questionnaires completed by 578 housewives who live in Kangreung, Daejeon, Jeonju, and Daegu area. For data processing, SAS program was employed, and all data was analyzed by frequency, percentage, and $x^2-square$. The results are as follows; 1. In observing the traditional holidays, more than 90% of housewives who answered the inquiry keep up SEOLNAL, DAEBOREUM, and CHUSEOK, whithout regarding regions. There are larger number of people who keep up IPCHUN, JUNGWHAJEAL, and DANOH in Kangreung and SAMBOK in Daegu than that of other regions (p<.001). Compared to general circumstances, CHOPAIL is more commonly celebrated by older class as well as lower educated class (p<.001) Also, the consequence shows that Buddhists observe IPCHUN, SAMJITANL, CHOPAIL, DANOH, SAMBOK, and DONGJI more commonly compared with other classes (p<.001), IPCHUN, SAMJITNAL, (p<.01). DAEBOREUM, YUDU, SAMBOK, JUNGYANGJEOL, OHIL, and DONGJI (p<.001) are celebrated more generally among extended. 2. On inheritance of the traditional holidays, most of the housewives answered that the SEOLNAL, DAEBOREUM, CHUSSEOK, and DONGJI are to be inherited. Compared to general circumstance CHOPAIL is suggested to be inherited by older people (p<.001). The IPCHUN, CHOPAIL, SAMBOK (p<.001), and DANCH (p<.01) are suggested by Buddhists that YUDU and SAMBOK are by nuclear. 3. In preparaing festive foods, DEOKGUK on SEOLNAL, OHKOKBAP and MUKEUNNAMUL on DAEBOREUM, SONGPYEON and GATEUNNAMUL on CHUSEOK, and PATJUK on DONGJI are being made very generally. KANGJEONG on SEOLNAL, BUREUM on DAEBOREUM, KALGUKSU and SAMGYAETANG on SAMBOK, DARKJIM on CHUSEOK, SUJEONGGWA and DONGCHIMI on DONGJI appeared comparatively high rated of making. 4. In normal times, DEOKGUK, MANDU, INJEOLMI, SIKHYAE, SUJEEONGGWA, PYEONYUK, JEONYUEO, SIRUDDEOK, BOKSSAM, MUKEUNNAMUL, SONGPYUN, MINARINAMUL, YUKGAEJANGGUK, KALGUKSU, SAMGYAETANG, HOBAKJIJIM, TORANTANG, GATEUNNAMUL, NUREUMJEOK, DAKJIM, KALBIJIM, PATJJUK, and DONGCHIMI is usully made. 5. The source to learn about traditional foods is mostly by her mother and the rest orders are husband's mother, cooking books, mass media (including T.V), school education, and cooking instituse, etc.

Isokinetic Evaluation of the Trunk Flexors and Extensors for the White Collar Workers in Adult Males (사무직근로자의 요추부 굴곡근 및 신전근의 등속성 근력평가 서울시 일부지역의 21세 $\sim$ 49세 남자직장인을 중심으로)

  • After warming-up exercise for 20 minutes, Isokinetic measurement of trunk strength for flexor and extensor was done by using Cybex 6000 TEF Unit on 91 healthy male white workers from 22years old to 49 years old, and compared each other. 20 repetitions of trunk extension-flexion were done at $120^{\circ}$/sec angular velocity. After resting for 1 minutes, Four repetitions at two different angular velocities($60^{\circ}$/sec, $120^{\circ}$/sec) were done with 30 seconds of resting interval between each angular velocity. The purpose of this study is to obtain the isokinetic normative strength values for trunk extensors and flexors, and is to know the correlation between age, height, weight of subjects and data from isokinetic trunk strength measurement, and is to provide a guideline for exercise program of male white collar workers The collected data were analyzed by ANOVA, Duncan's Multiple Range Test, and Pearson correlation coefficiency in PC-SAS program. The results obtained were as follow; 1. There is significant positive-correlation with the statistic value between weight and peak torque of trunk muscles at two different angular velocities($60^{\circ}$/sec, $120^{\circ}$/sec)(p<01), between height and peak torque of trunk muscles at two different angular velocities($60^{\circ}$/sec, $120^{\circ}$/sec) except peak torque of trunk flexor at $60^{\circ}$/sec(p<01). 2. There is nagitive-correlation between age and peak torque of trunk muscles at two different angular velocities($60^{\circ}$/sec, $120^{\circ}$/sec), there is significant differences with statistic value between age and peak torque of trunk extensor at $120^{\circ}$/sec(p<.01). 3. Mean peak torque and mean peak torque % by body weight of trunk extensor is 1.1 times higher values than trunk flexor at $60^{\circ}$/sec. 4. There is the increase in peak torque angle of trunk flexor with increasing of age, and the decrease in peak torque angle of trunk flexor with increasing of age at two different angular velocities($60^{\circ}$/sec, $120^{\circ}$/sec). there is significant differences with statistic value in peak torque angle of trunk flexor at $120^{\circ}$/sec(p<.01). 5. There is significant decrease in endurance ratio of trunk extensor with increasing of age at $120^{\circ}$/sec(p<.01). In conclusion, peak torque of trunk extensor is 1.1 times higher values than trunk flexor in healthy male white collar workers.

Evaluation of Current Coding Practices in 3 University Hospitals (3개 대학병원의 주 진단 코딩사례 평가)

  • Background : Coding of principal diagnosis is essential component for producing reliable health statistics. We performed this study to evaluate the current practice of principal diagnoses determination and coding, and to give some basic data to improve coding of principal diagnosis. Method : Nineteen medical record administrators (MRAs) of 3 university hospitals participated in coding principal Dx. from August 1, 2001 to August 31, 2001. From each hospital, 10 medical records of patients with high frequency disease were selected randomly. Each 10 medical records were grouped into three (A. B, C). Then, these 30 medical records were given to each MRAs for coding. At the same time questionnaire was given to each of them. Questions were to prove how they decide and code the principal diagnosis among many current diagnoses; how they decide and code the principal diagnosis when they see irrelevant diagnosis recorded as the principal diagnosis in medical record, when only tentative diagnoses were recorded without final diagnosis, and when different diagnoses were recorded in different sheets of same record. Agreement of coding among 3 hospitals were compared and survey results were analysed with SAS 6.12. Results : Agreement of coding was found in medical records 5-6 of each 10 medical records. Causes of disagreement were as follows. Difference of clinician's opinion from each hospital; mixed use of guideline from KCD-3 and guideline from DRG; difference in 4th digit classification according to the absence of pathology report in the medical record; difference of abbreviations among hospitals. 57.9% of MRAs selected the principal diagnosis recorded by physician, 42.1% of MRAs decided principal diagnosis after consulting to KCD-3 guideline. When there were difficulties in determining the principal diagnosis, 42.1% of MRAs decided principal diagnosis after discussion with the physician, 26.3% after discussion with fellow MRAs. Conclusion : There were differences in codings among hospitals. To minimize the difference, we suggest the development of disease-specific guidelines for coding in addition to the current general guideline such as KCD-3. To do this, Coding Clinic which can produce guidelines is needed.
