  • Prognosis in oriental medicine gathers information by four examination methods. It provides important information to understand the degree of deficiency - excessiveness of a patient to treat properly. To generalize the degree of deficiency - excessiveness can be found by seeing the patient's muscle response and pain perception to the palpitations. The theoretical basis to generalize deficiency - excessiveness, oriental and western medical understanding of pain perception and the elasticity of muscle were discussed. The usual symptoms for the excessiveness could include Pain (dislikeness to the palpitation), Stiffness of nape and limbs, Contracture of the limbs, Clonic convulsion and Fast pain. The symptoms for the deficiency could include Pain (likeness to the palpitation, Gastrocnemius muscle spasm, Flaccid paralysis of limbs and Slow pain. More theoretical bases for generalization of deficiency - excessiveness are needed along with the simplifying the complex clinical symptoms. In this way, we can discuss about deficiency - excessiveness with the regard to western medicine to help its generalization.

  • Parkinson's disease is a slowly progressive degenerative disorder of the central nervous system. It is characterized by tremor when muscles are at rest, increased muscle tone, slowness of voluntary movements, and difficulty maintaining balance. In oriental medicine, these symptoms are diagnosed as yin(陰)-deficiency of liver and kidney, deficiency of qi(氣) and blood, retention of phlegm(痰), qi-stagnation and blood stasis. In this case, we diagnosed patients as deficiency of qi(氣) and blood type according to symptoms and treated by herbs that strengthen yang and benefiting yin for two weeks, while maintaining existing parkinson's western medication. After treatment, clinical symptoms were improved, while UPDRS (Unified Parkinson's Disease Rating Scale) score was decreased. These cases suggest that oriental medicine therapy maybe effective in the treatment of Parkinson's disease.

  • Yang deficiency pattern is a representative syndrome differentiation. This article is a study on to which categories of modern diseases yang deficiency is assigned by reference to modern clinical papers and the meaning of yang deficiency interpreted with a perspective of Korean Medicine and a modern perspective. Yang deficiency, yang qi deficiency, lack of yang qi and yang qi debilitation are the words found in "Nei Ching" and yang qi can be interpreted as something to warm, drive and arouse. Zhangzhongjing considered recovery or loss of Yang as the key to life in "Shanghanlun". Danxi proposed "Yang being liable to hyperactivity, Yin being insufficient" and emphasized pathological ministerial fire of Yang exuberance rather than physiological ministerial fire of Yang deficiency. Zhangjingyue proposed "Yang not being in excess, Yin being often deficient" and understood growth and decline of yin qi are all led by yang qi and put emphasis on true yin in addition to yang qi. Diseases of yang deficiency pattern are related with decline of metabolic level, hypofunction of internal secretion, disorder of immune function, disorder of automatic nerve system, sympathetic nerve inhibition, metabolic disorder of microelements, increase of cGMP, change of microcirculation, low speed of blood stream, kidney malfunction. Diseases related with kidney are sterility, polycystic ovary syndrome, spinal stenosis, edema, renal failure, IgA nephropathy, erectile dysfunction, nephritis, prostatitis, benign prostatic hyperplasia, decrease of adrenal cortical hormone by nephrotic syndrome, myelodysplastic syndrome. Disease related with heart are heart failure, arrhythmia, cardiomyopathy, atherosclerosis heart disease, hypertension, hyperlipidemia, pulmonary heart disease. Diseases related with spleen are irritable bowel syndrome, ulcerative colitis. Diseases related with liver are hypothyroidism, liver cirrhosis ascites, hepatitis B, chronic hepatitis, hepatic diabetes. Diseases related with lung are allergic rhinitis, cough variant asthma, bronchial asthma, pulmonary emphysema. And diabetes mellitus, metabolic syndrome, aplastic anemia, headache, encephalatrophy, Alzheimer's disease are also related with yang deficiency.

  • Background : It was not enough to apply three kinds of syndrome differentiation in our oriental medical textbook to tremor's treatment according to reports of Korea and traditional medical textbook of China. Objective : To investigate syndrome differentiations by types of diseases related to tremor through Chinese journals review and to suggest adding possible syndrome differentiations. Methods : Literature search was performed using China Academic Journal (CAJ), the search engine of China National Knowledge Infrastructure (CNKI) from January 1994 to December 2009. Searching key words were Chinese characters in combination meaning tremor, paralysis agitans, and syndrome differentiation. We included all types of articles that explained or referred to definite syndrome differentiations. The symptoms and oriental medications by syndrome differentiation in selected articles were extracted and summarized. Results : 56 Chinese journals were ultimately selected. 37 kinds of syndrome differentiations about tremor were investigated, which included dual deficiency of qi and blood (氣血兩虛) quoted 31 times, liver-kidney yin deficiency (肝腎陰虧) 23 times, liver-kidney deficiency (肝腎不足) 21 times, and phlegm-heat stirring wind (痰熱動風) 20 times. 37 kinds of syndrome differentiation could by group into eight types, such as liver-kidney yin deficiency (肝腎陰虧), dual deficiency of qi and blood (氣血兩虛), phlegm-heat stirring wind (痰熱動風), heart deficiency and spirit weakness (心虛神弱), blood stasis due to qi stagnation (氣滯血瘀), sea of marrow deficiency (髓海不足), liver qi depression (肝氣鬱結), and liver yang transforming into wind (肝陽化風). Conclusion : We suggest that the syndrome differentiations of tremor, such as heart deficiency and spirit weakness (心虛神弱), blood stasis due to qi stagnation (氣滯血瘀), sea of marrow deficiency (髓海不足), liver qi depression (肝氣鬱結), and liver yang transforming into wind (肝陽化風), can be added to liver-kidney deficiency (肝腎不足), dual deficiency of qi and blood (氣血兩虛), and phlegm-heat stirring wind (痰熱動風) of the textbook. Further systematic research will be needed on them.

  • Purpose : This study investigated the breast-feeding period, the milk bottle-using period, the age of cow's milk, introduced and the amount of cow's milk consumed in relation to anemia. Methods : Over the course of three years, 930 children(12 months to 36 months) who went to the Presbyterian Medical Center, Chonju, Korea were tested for anemia and their parents were surveyed for a history of their children's milk consumption. Results : Anemia appeared more likely between 30 months and 36 months, however, iron-deficiency anemia appeared more likely between 18 months and 23 months. Anemia, low serum ferritin levels and iron-deficiency anemia appeared more likely in children breast fed less than 6 months and greater than 12 months. Although there were survey reports of side effects with cow's milk, including constipation, diarrhea and skin rash, the milk bottle-using period, age of cow's milk introduced and amount of cow's milk consumed had no connection with anemia, serum ferritin levels and irondeficiency anemia. Conclusion : The data showed no correlation between the cow's milk, milk bottle-using period and iron deficiency. But the data revealed that iron deficiency anemia is more likely in children who are breast fed for less than 6 months and over 12 months, so we suggest careful attention during this period to prevent iron deficiency anemia.

  • Isolated 3-methylcrotonyl-CoA carboxylase deficiency is an autosomal recessive disorder affecting leucine metabolism; it is one of the most common inborn metabolic diseases detected in newborn screening. Mutations in the genes MCCC1 or MCCC2 cause a defect in the enzyme 3-methylcrotonyl-CoA carboxylase, with MCCC2 mutations being the form predominantly reported in Korea. The majority of infants identified by neonatal screening usually appear to be asymptomatic and remain healthy; however, some patients have been reported to exhibit mild to severe metabolic decompensation and neurologic manifestations. Here we report the clinical features of a patient with asymptomatic 3-methylcrotonyl-CoA carboxylase deficiency and novel heterozygous MCCC1 mutations.

  • This study covers pattern differentiation based on Korean medical references, research trend and modern clinical applications about Sleep-Wake disorders of Diagnostic and Statistical Manual of Mental Disorders(DSM-V) published by American Psychiatric Association. Insomnia disorder is mostly caused by yin deficiency of liver-kidney or liver qi depression and main patterns are heart-kidney non-interaction, deficiency-excess complex pattern containing phlegm-heat due to qi stagnation and blood stasis. Hypersomnolence disorder is more due to yang deficiency rather than yin deficiency and it's major pattern is spleen-kidney yang deficiency. Cataplexy is main feature in narcolepsy and corresponds to depressive psychosis or fainting in terms of Korean Medicine and narcolepsy is assumed to be relevant to liver wind. Breathing-related sleep disorders are related with phlegm-fluid retention brought on spleen deficiency with dampness encumbrance. Pattern of circadian rhythm sleep-wake disorders is combined with yin deficiency of liver-kidney or liver qi depression of insomnia disorder and spleen-kidney yang deficiency or dampness-phlegm of hypersomnolence disorder. Yin deficiency with effulgent fire brought on drugs or alcohol is one of main patterns of substance/medication-induced sleep disorder and combined patterns with yin deficiency of liver-kidney and blood stasis or dampness-phlegm-heat are mostly applied clinically. This study drew major and frequently applied patterns of sleep-wake disorders based on Koran medical literature and modern clinical applications. And that can be the groundwork for the task ahead like clinical practice guideline of sleep-wake disorders containing pattern differentiation, diagnosis and prescriptions.

  • The objective of this study was to investigate the diagnosis and treatment of Sjogren's syndrome(SS) such as syndrome differentiation and herbal medicine by reviewing Chinese traditional medicine's journals. The journal search was carried out using China National Knowledge Infrastructure(CNKI) and PubMed from January 2007 to July 2012. Searching key words were the various combination of "Sjogren's syndrome", "Traditional Chinese Medicine", "herbal medicine", and "syndrome differentiation". The final selection of 57 studies were extracted and summarized by two researchers independently. The syndrome differentiation was classified as yin deficiency with fluid depletion, yin deficiency with dryness heat, dryness toxin with yin damage, internal obstruction of static blood, dual deficiency of qi and yang, dampness-heat obstructing, wind with external contraction, liver qi depression, blood deficiency and wind-dryness, dual deficiency of yin and yang, and internal obstruction of phlegm-blood stasis. Liriope platyphylla(麥門冬), Rehmania glutinosa(生地黃), and Scrophularia buergeriana(玄蔘) were primarily prescribed to tonify yin, engender fluid and moisten dryness.
