Predictors of Mammography Participation Among Rural Korean Women Age 40 and Over

  • Purpose. The study was conducted to identify predictors of mammography screening for rural Korean women according to 'Stage of Change' from the Transtheoretical Model which, along with the Health Belief Model, formed the theoretical basis for this study. Methods. A cross-sectional descriptive design was utilized. Through convenience sampling 432 women were selected from 2 rural areas. Data were collected by survey. Health beliefs constructs were measured with Champion's HBM Scale-Korea version. Mammography participation was measured using the Stage of Mammography Adoption Scale developed. by Rakowski, et al.(1992). Results. The most frequent stage of mammography adoption was 'contemplation' ($40.5\%$). Predictors of stage of mammography adoption included 'mammogram recommended by health professional' (beta==0.59, t=16.12, p=.000), 'perceived benefits' (beta=0.09, t=2.21, p=.050), 'perceived susceptibility' (beta=0.09, t=1.98, p=.050), and 'perceived barriers' (beta=-0.07, t=-2.05, p=.041). 'Mammogram recommended by health professional' demonstrated the greatest association with having a mammogram. Conclusion. Health professionals play key roles in improving mammography participation and should recognize the importance of their role in cancer prevention and be more actively involved in education and counseling on prevention of breast cancer.

Study on Suicidal Ideation and Associated Factors in Rural Elderly (농촌 노인의 자살의도와 관련요인)

  • Purpose: The purpose of this study was to investigate differences in suicidal ideation according to general characteristics, and to identify the associated factors of suicidal ideation in rural elderly. Method: We surveyed 168 elderly people using structured questionnaires from March through April, 2013. Data analy was performed using descriptive statistics, ${\chi}^2$ test, Fisher's Exact test, and multiple regression using the SPSS program. Results: Significantly differences in suicidal ideation score were observed according to drinking state(p=.036), sleep state(p=.013), health state(p=.020), and stress state(p=.049). The associated factors identified in this study significantly explained 13.2% of suicidal ideation. Smoking and sleep state were the most significant factors of suicidal ideation. Conclusion: Based on these results, development of programs for promotion of psychological health in order to reduce suicide rates of the elderly is urgent. In particular when focusing on relations between sleep disturbance and suicide, it is considered that nursing intervention will be required to promote quality of sleep in clinical practices and communities.

Effect of Auricular Acupressure Therapy on Smoking Cessation for Male Adults in Rural Areas (흡연 남성에게 적용한 이압요법이 흡연량, 니코틴 의존도, 흡연욕구에 미치는 효과)

  • Purpose: This study is to examine the effect of auricular acupressure therapy on smoking cessation for male adult in rural areas. Method: Research design was nonequivalent control pretest-posttest design. The samples were 90 male adults(Experimental: 40, Control: 40) in a community. Measures were the number of cigarette smoking per day, dependency on nicotine, and need for smoking. Auricular acupressure therapy, experimental treatment was applied for 2 weeks, 2 times/week, 3 day/time. Data were collected from June 2005 to December 2005. Data were analyzed using SPSS PC+ 12 version. Descriptive statistics, independent t-test, ${\chi}^2-test$, were used for each aim of this study. Results: The number of cigarette smoking per day (t= 13.230, p=.000), dependency on nicotine (t=-29.743, p=.000), and need for smoking (t=-19.799, p=.000) were significantly decreased in the experimental group by application of the auricular acupressure therapy. Conclusion: Auricular Acupressure Therapy can be a better effective primary nursing intervention on smoking cessation for male adults in rural areas through reverification by a study of repetition.

Case Report of Terminal Cancer Patient by Community Health Practitioner (보건진료소에서의 말기 암 환자 간호사례)

  • Purpose: This study was done to describe a community health practitioner's nursing case management for a terminal cancer patient registered in the public health post. Methods: For this purpose, data were collected through the patient and family through home visits, health clinic offices, and phone calls. The nursing process was carried out from August to November 2019. Results: The patient suffered the most from anorexia and lack of energy. Also he expressed psychologically uncertainty about disease and death anxiety caused by long-term treatment. In order to reduce the death anxiety, Community Health Practitioner (CHP) asked him to express his life stories and listened to him. CHP provided information of appropriate medications and alternative foods for symptoms such as gastrointestinal disorders and anorexia to the patient and family. Observing the situation of the patient and family, CHP guided the patient and family to prepare for death and has confirmed to them that the process was not with the patient alone. Conclusion: CHP's this experience has shown the possibility for CHP to help the terminal cancer patient and family to prepare peaceful death in their communities.

Suicidal Ideation and Associated Factors of the Elderly According to Residence Area - Focusing on the Comparisons between Urban and Rural Areas - (거주지역별 노인의 자살생각과 관련요인 - 도시와 농촌의 비교 -)

  • Purpose: This study was conducted in order to investigate the incidence and associated factors of suicidal ideation among the elderly according to residence area. Methods: This cross-sectional study used secondary data from the 2011 Korean National Health and Nutrition Examination Survey (KNHANES V-2). A representative sample of 1,464 men and women aged 65 and over was selected. Rao-scott $X^2$-test, multiple logistic regression models based on sampling scheme of the data were used. Results: Of the 1,464 respondents, 22.4% had experienced suicidal ideation during the past year. Higher incidence of suicidal ideation was observed in rural seniors (23.7%) than in urban seniors (21.1%). The incidence of suicidal ideation showed correlation with age, education level, depression, stress, lying in a sick bed, and current smoking status among elderly living in urban areas. However, among elderly living in rural areas, the incidence of suicidal ideation showed correlation with household income, depression, stress, and daily activity. Conclusion: Based on the results, nurses should manage effective and individualized nursing interventions for elders in planning suicide prevention programs with consideration for residence areas, because there were differences in the factors affecting suicidal ideation among elders according to residence area.

A Study on Health Behavior Experience of Middle-aged Women in Rural Area (농촌 중년여성의 건강행위 경험)

  • The purpose of this study is to describe the health behavior experience of middle-aged Korean women in rural areas, and to help understand their health practice, perceive their nursing needs and provide guidelines to developing appropriate nursing interventions. Method: The participants were 18 healthy middle-aged women living in rural areas, with no serious illness that require hospitalization. The data was collected through interviews and participant observation, analyzed by the grounded theory methodology of Strauss & Corbin(1997). The data collection period was from April to November of 2001. Result: Depending on the paradigm model, the central phenomenon was family-oriented pursuing of yangsaeng. The causal condition was less confidence on one's own health, responsibilities in caring for family. The contexts were cultural system. The intervening condition was information system, support system, limitation of approaching a medical institution. The action/ interaction strategies were yangsaeng through dietary practice, yangsaeng through promoting clothing and housing, yangsaeng through exercise, practice of folk therapy, yangsaeng through mental hygiene, and use of medical institution. The consequences were stabilization of body and mind, and stabilization of family. Conclusion: It is recommended for nurses to understand health behavior experience of middle-aged women, and provide nursing intervention with theoretical scheme and practical principles so that these women can pursue the family-oriented process of yangsaeng.

Clonorchis Sinensis Research Reported in Korea from 2001 to 2007 (간흡충 관련 국내 연구논문 분석)

  • Purpose: The purpose of this study was to analyze the trend of research on clonorchis sinensis in Korea. Method: The study design was a literature analysis in which research studies with 'Clonorchis' in the title were collected from 2001 to 2007. A total of 115 research studies were analyzed based on the year of publication, study design, major or institute of researcher, study sample, and research theme. Results: 10-19% of the research studies were published each year from 2001 to 2007. Experimental study design was the most frequent at 54.8%. Medicine or parasitology as the major of researchers were 43.2% and 41.2% retrospectively, but no nursing studies were found. For study samples, 65.9% were cyst larvae and animals, followed by human beings (29.5%). In research theme, the structure and gene analysis of clonorchis sinensis was the most frequent (32.1%), followed by animal experimental studies (15.7%), epidemic studies of clonorchis sinensis prevalence and changes (13.0%), and methods of diagnosis of clonorchis sinensis and effectiveness (12.2%). Conclusion: Although positive rate of clonorchis sinensis is still high, intervention studies for managing clonorchis sinensis were not found. So, methodological research developing clonorchis sinensis management program and intervention study is needed for high risk groups in rural communities.

A Study on the Factors Affecting Self-Regulation Program for the Hypertensives in Rural Area (농촌지역 고혈압 환자를 위한 자기조절 프로그램 관련 요인 연구)

  • This study was conducted to identify the changes of the relationship among general characteristics, self-efficacy. and self-care of the hypertensives in rural area. For this purpose, self-regulation program was carried at two Community Health Practitioner Post located in the suburbs of Daejeon City from September to November. 1999. Data were analyzed with non-parametric statistics, t-test, pearson correlation. The results were as follows; 1. The level of self-care was significantly different according to sex, status of smoking, and the level of self-efficacy of non-smoker was significantly higher than smoker. 2. After self-regulation program, there were significant positive relationship among self-efficacy, self-care, internal locus control, and negative relationship between perceived barrier and self-care. From these results, it can be concluded that the self-regulation program is an effective nursing strategy to promote self-care performance of hypertensives in rural area, and this program can be recommended for the management of hypertensives.

Perceived Health Status, Body Image, Self-esteem of Women in Rural Area (농촌여성의 지각된 건강상태, 신체상 및 자아존중감)

  • Purpose: The purpose of this study was to examine the relationship among perceived health status, body image, and self-esteem of women in rural area. Methods: This cross-sectional descriptive design was used. A total of 90 women in the K rural area completed a questionnaire, including perceived health status, body image, and self-esteem. Data were analyzed with independent t-test, ANOVA, Pearson's correlation coefficients. Results: The mean score of perceived health status was 3.00 out of 5.00, body image was 2.50 out of 4.00, and self-esteem was 2.80 out of 4.00. There were statistically positive correlations between perceived health status and body image (r=.41, p<.001), perceived health status and self-esteem (r=.34, p=.001), and body image and self-esteem (r=.48, p<.001). Conclusion: It is necessary to develop educational and manageable program regarding to body image and self-esteem to improve perceived health status of women in rural area.

Effects of an Integrated Stress Management Program(ISMP) for the Elders in a Rural Community (농촌 재가노인을 위한 통합적 스트레스관리 프로그램 적용효과)

  • Purpose: This study was done to develop and evaluate the Integrated Stress Management Program (ISMP) for elders in rural communities. Method: ISMP consists of 6 educational programs to improve self-management behavior and therapeutic recreational activities and was developed by analyzing the published reports and through multidisciplinary cooperation. Effectiveness of the ISMP was evaluated by implementing the program with 53 rural elders living at home, one session a week of, 2 hours per session over, 6 weeks from March 14 to April. 22, 2005. Results: After participating in the program, participants' perceived levels of stress decreased(t=2.940, p<.05), and mood state (t=4.229, p<.001), life satisfaction(t=-4.911, p<.001), and perceived social support (t=-2.891, p<.05) increased significantly. Conclusion: The results of this study demonstrated that the ISMP is an effective program for relieving stress level, and increasing positive mood, life satisfaction, and social support for elders in a rural community.
