위전절제술 후 재건술식에 따른 환자의 영양상태와 삶의 질 비교 (Comparison of Reconstructive Techniques after Total Gastrectomy as Determined by Patient Quality of Life and Nutritional Status)

  • 목적: 위전절제술 후 이용될 수 있는 재건 술식에 대해서는 많은 방법들이 보고되어 왔지만 지금까지도 이상적인 술식에 대해서는 논쟁의 여지가 있고 장기간에 걸친 비교 연구는 더욱이 부족한 실정이다. 이에 저자는 본원에서 실시한 4가지 술식들인 Roux-en-Y 술식(R-Y), 공장간치술(JI), 공장낭간치술(JPI), Double-tract 재건술(DT)간의 경과 관찰을 통해 보다 나은 재건 술식을 찾고자 하였다. 대상 및 방법: 본원에서 2000년 8월부터 2004년 1월까지 위암으로 근치적 위전절제수술을 받고 재발이 없고 추적이 가능한 환자 중 80세 이상과 병기 IV기를 제외하고 재건 술식에 따라 RY군(n=15), JI군(n=8), JPI군(n=8), DT군(n=10)으로 분류한 총 41명의 환자를 대상으로 수술 후 증상, 수술 후 1년, 3년째의 혈액 및 생화학적 검사의 변화, 체중 변화를 포함한 영양학적 변화 및 삶의 질을 평가하기위한 GSRS score를 분석하였다. 또한 내시경 소견을 통해 위식도 역류 여부를 확인하였다. 결과: 검사실소견은 3년째 총단백과 알부민에서 DT군이 유의하게 낮은 결과를 보였고(P=0.006, P=0.033), 체중감소는 JI군이 1년, 3년째에서 수술 전 체중의 84.50%, 86.50%로 가장 많았다(P=0.013, P=0.011). GSRS score에서 JPI군이 수술 후 1년, 3년 모두에서 유의하게 더 좋은 결과를 보였다. (P=0.028, P=0.003). 결론: 공장낭간치술은 위전절제술 후 환자의 삶의 질을 높일 수 있는 유용한 재건 방식으로 생각된다.

조기위암에서 복강경보조 위전절제술 및 췌장보존식 비전절제술 1예 (Laparoscopy-assisted Total Gastrectomy with Pancreas-preserving Splenectomy for Early Gastric Cancer: A Case Report)

  • 저자들은 조기위암 환자에서 복강경보조 위전절제술 및 췌장보존식 비장절제술을 경험하였기에 이를 보고 하고자 한다. 62세 남자 환자로 내시경 조직검사에서 위상부와 위하부에 2개의 병변을 가진 조기위암으로 진단되어 복강경보조 위전절제술을 계획하고 수술을 진행하였다. 기복을 형성 후 5개의 투관을 삽입하여 위의 대만을 초음파 소작기로 박리 후 비장의 하단으로 박리를 진행하던 중 비장문부의 림프절 종대가 관찰되어 동결절편검사를 시행한 결과 암전이가 확인되어 위전절제술과 함께 췌장보존식 비장절제술을 시행하였다. 검상돌기 직하부에 수직으로 4 cm의 절개창을 형성 후 위장과 비장을 꺼낸 후 Roux-en-Y 식도공장문합술을 시행하였다. 환자는 수술 후 3일째 첫 번째 가스배출과 함께 유동식을 시작하였으며 별다른 문제없이 수술 후 9일째 퇴원하였다. 저자들은 비장문부의 림프절 암전이가 발견된 근위부 조기위암의 경우에도 복강경을 통한 제2군 림프절 절제가 가능함을 알 수 있었으며 복강경 보조 위전절제술 및 췌장보존식 비장절제술을 안전하게 시행할 수 있다고 생각한다.

Total Gastrectomy in Gastric Conduit Cancer

  • We report a very rare case of surgery on gastric conduit cancer. A 67-year-old male patient underwent esophagectomy and intrathoracic esophagogastrostomy for squamous cell carcinoma of the lower thoracic esophagus 27 months ago. Upon follow-up, a gastric carcinoma at the intra-abdominal part of the gastric conduit was found on an esophagogastroduodenoscopy. We performed total gastrectomy and esophagocolonojejunostomy in the manner of Roux-en-Y anastomosis. The postoperative course was not eventful and an esophagogram on the 10th postoperative day showed no leakage or stenosis of the passage. The patient was discharged on the 17th day with no complications.

총담관낭의 임상적 고찰 (Clinical Study of Choledochal Cyst)

  • Congenital dilatation of the common bile duct (choledochal cyst) is an uncommon disease. Although the etiology is unknown yet, various theories such as distal obstruction of the common bile duct, congenital weakness of the duct and anomalous union of the pancreaticobiliary duct have been offered to explain the occurrence of choledochal cyst. Thirty - six cases of choledochal cyst over 22 years were analyzed clinically and classified according to Todani's classification and Kimura's anomalous union of pancreaticobiliary duct type. Todani type 1 consisted with 22 cases which were subdivided into 19 cases of type Ia, 1 case of type Ib and 2 of type Ic. Type IVa consisted with 14 cases including one case of Caroli's disease. There were 25 type BP cases and 10 type PB cases and 1 normal pancreatobiliary junction. Serum alkaline phosphatase was increased significantly in almost all cases. Seven patients (19.4%) had associated congenital anomalies such as double gallbladder, left - sided gallbladder, common bile duct web, biliary atresia, accessory hepatic duct, heterotopic pancreas, cleft lip and 2 cases of intestinal malrotation. All patients underwent cyst excision and Roux - en - Y hepaticojejunostomy and cholecystectomy. There was one death due to methicillin resistant Staphylococcus aureus sepsis.

소아 담관낭종의 로봇수술의 초기경험 (Initial Experience of Robot-assisted Resection of Choledochal Cyst in Children)

  • Although laparoscopic surgery for hepatobiliary disease in children is possible, it is technically challenging. In an attempt to overcome these difficulties, the da Vinci Robotic Surgical System$^{(R)}$ was used to facilitate the minimally invasive treatment of choledochal Cyst in six children. In early consecutive three cases, we experienced three complications; a case of laparotomy conversion, a case of late stenosis of the hepaticojejunostomy, and a case of leakage from a hepaticojejunostomy. However, in the last three cases the complete resection of the choledochal cyst and Roux-en-Y hepaticojejunostomy were performed using the robotic surgical system without complication. We think robot-assisted choledochal cyst resection in children appears safe and feasible, and may increase the variety of complex procedures in pediatric surgical fields.

Modified Puestow Procedure for Chronic Pancreatitis in a Child Due to Annular Pancreas and Duodenal Duplication: A Case Report

  • An 18-year-old woman with annular pancreas and duodenal duplication presented with recurrent acute pancreatitis and underwent a resection of duodenal duplication. However, the patient experienced recurrent abdominal pain after resection. Abdominal computed tomography and magnetic resonance imaging showed a dilatation of the peripheral pancreatic duct and stenosis and malformation of both the Wirsung's and Santorini's duct due to multiple stones. The modified puestow procedure was performed. The main pancreatic ducts in the body and tail were opened, and the intrapancreatic common bile duct was preserved. A Roux-en-Y pancreatico-jejunostomy was performed for reconstructing the pancreaticobiliary system after removing the ductal protein plug. The patient experienced no abdominal pain, no significant elevation of the serum amylase and lipase levels, and no stone formation during the 2 years of follow-up. This procedure is considered to be beneficial for pediatric patients with chronic pancreatitis due to annular pancreas and duodenal duplication.

최근 9년간의 담도폐쇄증 경험 - 새로운 초음파 진단법 소개 - (Recent 9-year Experience With Biliary Atresia Utilizing a New Ultrasonographic Diagnostic Technique)

  • This paper reports our 9-year experience treating 34 infants with biliary atresia utilizing a new non-invasive diagnostic method, ultrasonographic "triangular cord"(TC) sign. The TC sign is present when there is visualization of a triangular or a band-like echogenicity just cranial to the portal vein. The ultrasonographic TC sign appears to be a simple, non-invasive, time-saving and useful tool in the diagnosis of biliary atresia. Sensitivity is 84 %. Active bile excretion was restored in 90 % of the patients who were treated between 31-60days, 78 % of those between 61-90 days, and 33 % of those being 91days or older. The incidence of postoperative cholangitis was 36 %. Construction of an antireflux valve in the Roux-en-Y loop did not affect the incidence of postoperative cholangitis(p=0.18). Among 34 infants with biliary atresia, 23(68 %) are alive for 2-102 months after operation, and 12 are alive for more than 5 years. Five-year estimate survival by Kaplan-Meier method was 66 %.

두강내 출혈 및 출혈성 경향을 동반한 선천성 담관 낭종 -1예 보고와 문헌보고- (A Choledochal Cyst Associated with Hemorrhagic Tendency and a Cerebral Hematoma -Report of a case and review of literature-)

  • 저자들은 3개월 된 남아에서 생긴 선천성 담관낭종으로 인해 vitamin K 흡수 장애로 출혈성 경향이 생기면서 두강내 혈종을 동반한 1예를 치험하였기에 문헌고찰과 함께 보고하는 바이다. vitamin K를 주사하여 출혈성 경향을 교정하고 두강내혈종을 제거한 뒤 담관낭종 절제 및 담관공장 문합술을 시행하였으며, 술후 경과는 양호 했다.

Esophagojejunal Anastomosis after Laparoscopic Total Gastrectomy for Gastric Cancer: Circular versus Linear Stapling

  • Purpose: No standard technique has been established for esophagojejunal anastomosis during laparoscopic total gastrectomy (LTG) for gastric cancer owing to the technical difficulty and high complication rate of this procedure. This study was performed to compare the short-term outcomes of circular and linear stapling methods after LTG. Materials and Methods: A total of 106 patients treated between July 2010 and July 2018 were divided into 2 groups according to the following anastomosis procedures: hemi-double-stapling technique (HDST; circular stapling method; group C, n=77) or overlap method (linear stapling method; group L, n= 29). The clinicopathological features and postoperative outcomes, including complications, were analyzed. Multivariate analysis was performed using a logistic regression model to identify the independent risk factors for anastomotic complications. Results: The incidence of anastomotic complications was significantly higher in group C than in group L (28.0% vs. 6.9%, P=0.031). The incidence of anastomosis leakage did not differ between the groups (6.5% vs. 6.9%, P=1.000). However, anastomosis stricture occurred only in group C (13% vs. 0%, P=0.018). Multivariate analysis showed that the anastomosis type was significantly related to the risk of anastomotic complications (P=0.045). Conclusions: The overlap method was superior to the HDST with respect to anastomotic complications, especially anastomosis stricture.

Combined Hepatocellular-Cholangiocarcinoma in Extrahepatic Bile Duct with Co-existing of Scirrhous Type of Hepatocellular Carcinoma

  • We report a patient with combined hepatocellular-cholangiocarcinoma confined in the common hepatic duct and scirrhous type of hepatocellular carcinoma in the caudate lobe of liver simultaneously. The patient was a 55-yearsold Korean man with hepatitis B virus (HBV) carrier who was referred from a local hospital due to detected liver mass on abdominal computed tomography (CT). He has presented jaundice and weight loss for the previous 3 weeks. Laboratory examination showed AST/ALT elevation and hyperbilirubinemia. HBsAg was positive. The tumor marker study showed elevated AFP and DCP, not CEA and CA 19-9. Abdominal CT disclosed an about 2.1×0.9 cm sized soft tissue density in hilum with both intrahepatic duct (IHD) dilatations and an about 3×2.1 cm sized arterial enhancing lesion at segment 8 of the liver. Patient received 15 cycles of Gemcitabine/Cisplantin chemotherapy from February 27, 2013 to December 31, 2013. Caudate lobectomy of liver, segmental resection of bile duct and Roux-en-Y hepaticojejunostomy was performed on February 10, 2014. The final pathologic report showed double primary liver cancer, combined hepatocellular-cholangiocarcinoma in common hepatic bile duct and scirrhous type of hepatocellular carcinoma in segment 1 of the liver. This is a very unusual case in which combined hepatocellular-cholangiocarcinoma confined in the large bile duct and two rare hepatic cancers coexisted.
