관절경적 회전근 개 봉합술 : 1 - 4년 추시 결과 (Arthroscopic Rotator Cuff Repair : Outcome of 1 to 4 years follow up)

  • Purpose : To compare and analyze the results of arthroscopic subacromial decompression and rotator cuff repair between partial rotator cuff tear and complete rotator cuff tear. Material and Methods : The authors studied 42 patients of rotator cuff tear with operation and followed over one year at Dankook university hospital from September, 1998 to March, 2001 The patient average age is 53 years and follow up period is 23 months (12-42mon1hs). We obtained 22 cases in the partial rotator cuff tear group and 20 cases in the complete rotator cuff tear group. In all cases, we used ASES methods to evaluated pain scale and function. Result : In the last follow up patients, the pain scale is decreased from 7.2 to 0.9 (ASES method : 34 to 91) in the partial rotator cuff tear group and from 7.6 to 1.2 (ASES method . 29 to 88) in the complete rotator cuff tear group, but there was no evidence of statistical difference between two groups (P>0.05). The range of motion after operation were increased in two groups. Excellent to good results were obtained 93% and 95% patients had pain relief and satisfied function. Conclusion : Arthroscopic subacromial decompression and arthroscopic rotator cuff repair showed good results for both of (ult-thickness and partial thickness rotator cuff tear patients in pain relief and improving the function.

충돌 증후군과 회전근 개 파열을 위한 이학적 검사들의 진단적 가치에 대한 분석 (Diagnostic Accuracy of Physical Examinations in Impingement Syndrome and Rotator Cuff Tear)

  • Purpose: The aim of this study was to investigate the diagnostic accuracy of Neer, Hawkins provocative tests and supraspinatus manual muscle test for the assessment of impingement syndrome, partial tear and small complete tear of De rotator cuff. Materials and Methods: Seventy-one female and 115 male patients were included in the study. Patients were divided into four groups of no impingement, impingement without tear, partial tear and small complete tear of the rotator cuff, which were confirmed by sonogram, magnetic resonance imaging and surgery. Neer and Hawkins provocative tests and supraspinatus manual muscle test were performed respectively. SAS 6.12 version was used in statistical analysis. Results: We found that Neer test had 94% sensitivity, 54% specificity for impingement without tear and 89% sensitivity, 78% specificity for partial tear and 96% sensitivity, 23% specificity for small tear. Hawkins test revealed 95% sensitivity, 54% specificity for impingement without tear and 93% sensitivity, 78% specificity for partial tear and 100% sensitivity, 23% specificity for small tear. Supraspinatus manual muscle test revealed 27% sensitivity, 94% specificity for impingement without tear and 29% sensitivity, 82% specificity for partial tear and 48% sensitivity, 82% specificity for small tear. Conclusion: Neer and Hawkins tests have high sensitivity, low specificity for impingement syndrome, partial and small tear. Supraspinatus manual muscle test had low sensitivity and high specificity. However this test was not effective to differentiate the partial and small rotator cuff tear. We thought that more effective provocative test should be designed to detect the partial and small rotator cuff tear.

Acute Rotator Cuff Tears due to Low Voltage Electrical Injury: A Case Report

  • Since shoulder have a higher proportion of muscle which would have low electrical resistance, there could be more electrical damage to the rotator cuff muscles. We present a patient with acute rotator cuff tear by sudden uncontrolled jerking contractions caused by an electrical shock. A case of 42-year-old man with acute rotator cuff tear due to electrical injury to the shoulder was presented. Magnetic resonance imaging showed a full thickness tear and an undulating appearance of the peripheral end of the torn supraspinatus and infraspinatus muscle, suggesting an acute complete rupture. By arthroscopic surgery, the torn rotator cuff tendons were repaired with a suture bridge technique. At the final follow-up, the patient had a full, pain-free range of motion and had fully recovered shoulder muscle power.

Longitudinal Supraspinatus Tear Associated with Antegrade Humeral Intramedullary Nailing: A Case Report and Literature Review with Focus Placed on Nail Entry Point

  • Iatrogenic damage of the rotator cuff followed by postoperative shoulder function loss is a potential complication after antegrade intramedullary nailing (AIN) for a humeral fracture. The authors present a case of arthroscopic rotator cuff repair and subacromial decompression of a non-healed rotator cuff tendon (mainly supraspinatus) and secondary impingement syndrome caused either by the tear or a proud nail after AIN for a mid-shaft humeral fracture. At presentation, the patient complained of right shoulder pain and 'snapping', especially during forward elevation and abduction of the shoulder, of 4 years duration. Right shoulder pain started sometime after pain due to the humeral shaft fracture, operation had subsided, and persisted after nail removal. Arthroscopic findings showed a longitudinal rotator cuff tear at the nail entry point that had not healed and severe fibrous hypertrophy on the acromion underspace, which is a unique finding since most longitudinal splits of tendon fibers are more likely to heal than conventional rotator cuff tears detached from bone. The torn rotator cuff was repaired after debridement and placing side-to-side sutures. At his 34-month follow-up after rotator cuff repair, the patient showed complete recovery and had excellent clinical scores.

관절경적 견봉하 감압술에 영향을 미치는 예후 인자 (Prognostic Factors for Arthroscopic Subacromial Decompression)

  • Purpose: The purpose of this study was to analyze the prognostic factors for arthroscopic subacromial decompres­sion and rotator cuff debridement in impingement syndrome. Materials and Methods : Arthroscopic subacromial decompression with or without rotator cuff debridement was performed in 46 cases of 44 consecutive patients with either stage n or stage ill impingement syndrome. The patients were classified by Neer's stage and size of tear according to the criteria of Cofield. The results were assessed with UCLA rating scale. We used repeated measures ANOVA and Chi-square test to assess correlation between the results and six variables including stage, rotator cuff tear size, age at the operation, duration of symptom, throwing sports activity, and trauma history. The follow-up period averaged 53 months(range, 27 to 92 months). Results: Lower stage by Neer's stage was correlated with higher postoperative scores and with significant difference between preoperative and postoperative scores of UCLA rating scale. However, other factors did not show significant influence upon the results. The patients with complete rotator cuff who showed satisfactory results after procedures were older and had shorter symptom duration, small cuff size. Conclusions : In patients with impingement syndrome treated by arthroscopic debridement and subacromial decompression, superior results were obtained when belonged to a lower Neer stage and when the rotator cuff was only partially tom. In cases with complete rotator cuff tear, higher success rates were obtained with smaller tear sizes. Age at operation, duration of symptoms, throwing athlete, traumatic tear did not affect the results.

대결절에 1 cm 이상의 잔여 건이 부착된 회전근 개 실질 내 완전 파열 - 2례 보고 - (Intrasubstance Complete Rotator Cuff Tear with more than 1 cm of Remnant Attached to the Greater Tuberosity: 2 Case Reports)

  • 회전근 개의 전층 파열에서 건은 대결절 또는 소결절의 골 부착부에서 주로 분리된다. 저자들은 상완골 대결절에 1cm 이상의 잔여부가 부착된 회전근 개 건실질 부위에서 발생한 완전 파열을 가진 드문 2예의 회전근 개 파열을 경험하였다. 두 예에서 모두 잔여건을 제거하지 않고 관절경적 건 대 건 봉합술을 시행하였으나 술 후 6개월에 시행한 자기 공명 영상에서 이전 파열 부위에서 회전근 개의 재파열을 보였다. 회전근 개 건실질부 파열에서의 건 대 건 봉합술의 유용성을 증명하기 위해서는 더 많은 증례 검토와 생역학적 연구가 필요할 것으로 사료된다.

관절경적 감압술에 의한 견봉하 충돌증후군의 치료 (Arthroscopic Decompression of Subacromial Impingement Syndrome)

  • The authors performed the arthroscopic decompression and cuff debridement on 47 cases in 45 consecutive patients with either stage II or stage III impingement syndrome from July 1990 to January 1994. The summarized results are as follows. 1. 47 cases in 45 consecutive patients had arthroscopic decompression for the subacromial impingement syndrome. 2. There were 31 males and 14 females and the mean age was 40 years for men and 46years for women. 3. The follow up duration was from 3 years 9 months to 1 year (average 2 years 1month). 4. Among 47 cases, 19 cases were found to have no cuff tear (stageII) 13 cases partial thickness cuff tear (stage IIIa) 10 cases complete tear on cuff less than 3 cm long (stage IIIb) and 5 cases complete tear on cuff more than 3 em in length (stage IIIc). 5. The arthroscopic subacromial decompression and rotator cuff debridement was a good treatment method in stage II and stage IIIa and stage IIIb. in the case of stage IIIc rotator cuff tear, it was useful for pain relief and improvement of shoulder function.

Degeneration Exists along the Entire Length of the Supraspinatus Tendon in Patients with a Rotator Cuff Tear

  • Background: The purposes of the study were to examine rotator cuff tendon degeneration with respect to harvesting location, to determine a rationale for debridement of the torn end, and thus, to determine adequate debridement extent. Methods: Twenty-four patients with a full-thickness rotator cuff tear were included in the study. Tendon specimens were harvested during arthroscopic rotator cuff repair from three locations; from torn ends after minimal regularization of fraying (native end group, NE group), from torn ends after complete freshening of the frayed end (freshened end group, FE group), and from the macroscopically intact portion just distal to the musculotendinous junction (musculotendinous junction group, MTJ group). Control samples were harvested from patients admitted for surgery for proximal humerus fracture. Harvested samples were evaluated using a semi-quantitative grading scale. Results: Mean total degeneration scores in the NE group ($13.3{\pm}3.21$), the FE group ($12.5{\pm}2.30$), and in the MTJ group ($10.8{\pm}3.10$) were significantly higher than those in the normal control group ($5.0{\pm}2.87$; all p<0.001). Mean total degeneration score in the NE group was significantly higher than that in the MTJ group (p=0.012), but was not from that of the FE group. Mean total degeneration score in the FE group was not significantly different from that of the MTJ group. Conclusions: Tendon degeneration exists throughout the entire tendon to the macroscopically intact portion of full-thickness rotator cuff tear. Therefore, aggressive debridement to grossly normal appearing, bleeding tendon is unnecessary for enhancing healing after repair.

개방적 완전 봉합술로 치료한 광범위 회전근 개 파열 환자에서 치료 전후의 방사선학적 및 임상적 소견의 변화 (The Radiologic and Clinical Changes after Open Complete Repair of Massive Rotator Cuff Tears)

  • 목적: 광범위 회전근 개 파열에 대해 개방적 완전 봉합이 가능하였던 환자들을 대상으로 수술 전, 후의 방사선학적 소견의 변화와 진행 정도를 회전근 개 재파열 정도 및 임상적 결과와 함께 비교 분석하고자 하였다. 대상 및 방법: 광범위 회전근 개 파열에 대해 개방적 완전 봉합술을 시행 받고 24개월 이상 추시가 가능하였던 33예를 대상으로 하였다. 임상적 평가는 American Shoulder and Elbow Surgeons(ASES)의 견관절 기능 평가법을 기준으로 하였으며, 광범위 회전근 개 파열과 관련한 관절염의 정도는 Hamada의 분류법을 기준으로 하였다. 결과: ASES 점수는 수술 전 평균 37.6점에서 85.6점으로 호전되었다. 수술 전 acromiohumeral interval (AHI)은 평균 6.5 mm였으며, 수술 직후 평균 9.3 mm로 증가하였으나, 최종 추시에서 평균 6.5 mm로 다시 감소되는 소견을 보였다. Hamada 분류법에 따른 관절염의 방사선학적 단계는 수술 전과 수술 후 모두 그 단계가 낮을수록 보다 좋은 임상적 결과를 보였다. 결론: 광범위 회전근 개 파열에 대한 수술적 치료로서 개방적 완전 봉합술은 비록 수술 후 재파열이 발생하더라도 임상적으로는 통증 경감과 견관절 기능 향상 면에서 상당히 만족할만한 결과를 보였다.

회전근 개 완전파열 환자에서의 나이 및 성별에 따른 견관절 동반 병소의 차이 (Differences in Associated Shoulder lesions Regarding Ages and Gender Among Patients with Complete Rotator Cuff Tear)

  • 목적: 회전근 개 완전 파열로 관절경적 시술을 받은 환자를 대상으로 나이와 성별에 따른 견관절 동반 병소의 차이를 알아보고자 하였다. 대상 및 방법: 2003년 6월부터 2006년 3월까지 본원 정형외과에서 회전근 개 완전파열로 수술하였던 109명, 109례의 환자들을 대상으로 견관절 동반 병소의 차이를 분석하였다. 성별 분포는 남자 59명, 여자 50명이었고 나이는 최소 24세에서 최고 75세로 평균 53.3세였다. 연령대에 따른 분류는 59세 이하의 환자 군과 60세 이상의 환자 군으로 임의로 분류하였고 분석 대상은 견봉 쇄골 관절염, 상부 관절와 순 병변, 상완 이두근 염의 동반 유무를 Khai-square를 이용하여 통계학적으로 분석하였다. 결과: 전반적으로 견봉 쇄골 관절염은 60세 이상의 환자 군에서 59세 이하 군에 비해 높은 동반율을 보였고 상부 관절와 순 병변은 59세 이하의 환자 군, 특히 남자 환자에서 60세 이상의 환자 군에 비해 높은 동반 비율을 보였다. 결론: 회전근 개 파열의 진단 및 치료를 함에 있어 연령대에 따른 견관절 동반 병소를 고려하여야 하며, 특히 젊은 남자환자에서는 상부 관절와 순 병변, 고령의 환자에서 견봉 쇄골 관절염의 동반 유무를 반드시 확인하여야겠다.
