Recently the development of rotary instrument makes it possible that in root canal treatment operator saves much more time, maintans original curved canal shape and easily prepares continuous tapered root canal. The purpose of this experiment was to examine the smoothness of the internal surface of prepared root canal and the effectiveness of debridement in prepared root canal by SEM for the comparison of hand and Ni-Ti rotary instrument. 25 extracted human teeth were access opened and # 10 K-type file was introduced into canal until it was appeared at the apical foramen. The working length was established by subtracting 0.5mm from this measurement. Group 1. The root canal preparation was done to # 30 with working length and then step-back until # 45 with K-Flexofile (Maillefer, Swiss). Group 2. Root canal preparation was done by Naviflex Ni-Ti file (Brasseler, USA) as the same technique with group 1. Group 3. Canal was prepared by Profile .04 (Maillefer, Swiss) taper until #30. Group 4. With use of Quantec (Tycom, USA) root canal was prepared from file number 1 to 8. In group 1 and 2, the root canal irrigant was NaOCl and the other groups, NaOCl and RC-prep (Premine Dental Products, USA) was used. The prepared teeth were notched with high-speed bur as bucco-lingual direction and fractured with chisel and mallet, then examined with SEM. Group 1 showed smooth internal surface. There were scratches mainly to the axial direction. Group 2 showed similar characteristics to those in group 1. Group 3 showed more smoother and linear cutting surface with bised scratches. Group 4 has the almost same characteristics group 3 and there was no difference in the file design. Ni-Ti rotary root canal instrument prepare the dentinal wall more smoother than hand instrument. The effectiveness of debridement was not fully affected by file design. The isthmus area and accessory canals of the root canal system were not prepared in any group. According to the result, hand and rotary type instrumentation techniques were effective in removal of major amount of tissue from root canal but it was not complete. In the direction of cutting movement there was difference between them.

The main objective of nonsurgical retreatment is to disinfect the root canal space for periradicular healing. Thus, efficient removal of the filling material from the root canal system is essential to ensure a favorable outcome. Traditionally, the removal of root canal filling material was performed by stainless steel hand instrument and this procedure is time and effort-consuming. Recently, rotary NiTi retreatment instruments are developed to effective removal of root canal filling material. The aim of this study was to evaluate the effectiveness of stainless-steel hand instrument and rotary NiTi retreatment instrument when removing gutta-percha in curved root canals. For the remaining materials, there were no significant differences between groups. But the rotary NiTi system proved to be faster than hand instruments in removing root filling materials.
This study compared the shaping ability of nickel-titanium rotary files with different rake angle and radial land. The nickel-titanium files used in this study were Profile(Dentsply, Maillefer, Ballaigues, Switzerland), Hero 642(Micromega, Besancon, France) and K3 (SybronEndo, Glendora, Ca, USA) file. Resin blocks substituted for root canals. 36 resin blocks were divided into 3 groups with 12 canals each The time for canal preparation was recorded. The images of pre- and postoperative resin canal were scanned and those were superimposed Amounts of canal deviation, total canal widths, inner canal widths, and outer canal widths were measured at apical 1, 2 ,3, 4, 5, 6, and 7mm levels. The amount of canal deviation was the smallest in Profile group , and the time far canal preparation was the shortest in Hero 642 group. K3 group resulted in competent characteristics in both measurements. Positive rake angle seemed to result in fast shaping of root canal and radial land guide the instrument in center of the canals and around curvatures. Radial land also tended to reduce the sense of screwing into the root canal. The proper selection of the nickel-titanium fie based on the knowledge about file design is needed for the safer, simpler and faster root canal therapy.
Background: Various instrument kinematics used in single-visit endodontics influence the occurrence of pain after endodontic therapy. This study aimed to evaluate the occurrence of pain after mechanical instrumentation with Hyflex EDM (HEDM) and WaveOne Gold (WOG) during single-visit endodontic therapy. Methods: Sixty patients diagnosed with asymptomatic irreversible pulpitis and normal apical tissues in mandibular premolar teeth were included in the study for single-visit root canal therapy. The patients were divided into two groups (n = 30) according to the rotary instrument used during root canal preparation (group A [HEDM] and group B [WOG]). Pain was evaluated after endodontic therapy at 8, 24, and 48 h intervals using the visual analog scale (VAS). Data obtained were analyzed using the chi-square test, independent t-test, MannWhitney U test, and Wilcoxon matched-pairs test. Results: Statistically significant differences were observed between the two groups (P < 0.001) at 8, 24, and 48 h, with WOG exhibiting less pain than HEDM files. Conclusion: Postoperative pain was lower in the WOG file system than in the HEDM file system after single-visit root canal therapy at 8, 24, and 48 h.
The aim of this study was to evaluate the shaping abilities of four different rotary nickel-titanium instruments with anticurvature motion to prepare root canal at danger zone by measuring the change of dentin thickness in order to have techniques of safe preparation of canals with nickel-titanium files. Mesiobuccal and mesiolingual canals of forty mesial roots of extracted human lower molars were instrumented using the crown-down technique with ProFile, $GT^{TM}$ Rotary file, Quantec file and $ProTaper^{TM}$. In each root, one canal was prepared with a straight up-and-down motion and the other canal was with an anticurvature motion. Canals were instrumented until apical foramens were up to size of 30 by one operator. The muffle system was used to evaluate the root canal preparation. After superimposing the pre- and post-instrumentation canal. change in root dentin thickness was measured at the inner and outer sides of the canal at 1. 3, and 5 mm levels from the furcation. Data were analyzed using two-way ANOVA. Root dentin thickness at danger zone was significantly thinner than that at safe zone at all levels (p < 0.05). There was no significant difference in the change of root dentin thickness between the straight up-and-down and the anticurvature motions at both danger and safe zones in all groups (p > 0.05). ProTaper removed significantly more dentin than other files especially at furcal 3 mm level of danger and safe zones (p < 0.05) Therefore, it was concluded that anticurvature motion with nickel-titanium rotary instruments does not seem to be effective in danger zone of lower molars.

Objectives The aim of this study is to compare the quality of root canal preparation completed using MFile-$System^{(R)}$ instrument ( Komet, Gebr.Brsaseler, Germany) and conventional stainless steel file in the canals of Maxillary molar teeth that had a canal curvature between $25^{\circ}$ or more Materials & Methods Buccal canals of 24 first and second maxillary molar teeth, extracted for periodontal and prosthetic reasons were used. Tissue fragments and calcified debris were removed from teeth by scaling and the teeth were stored in 10% formalin solution for 24 hour. Then, teeth were stored in saline until used. To be included the roots had to have completed formed apices and angle of curvature ranging between $25^{\circ}$ or more according to the criteria described by Schneider(1971). Palatal and Second mesiobuccal canals were not included. Teeth were embedded into transparent acrylic. The teeth were randomly divided into two experimental groups. All teeth were scanned by Dental CT (PSR9000N, Asahi, Japan) to determine the root canal shape before instrumentation. Image slices were prepared from the apical end point to the pulp chamber. The first two sections were 2 mm from the apical end of root and 2 mm below the orifice. Further section was recorded, dividing the distance between the sections of apical and coronal levels into two equal lengths. 12 teeth were instrumented using stainless steel fileand another 12 teeth were instrumented using MFile-$System^{(R)}$. Following the completion of the instrumentation, the teeth were again scanned and compared with the cross sectional images taken prior to canal preparation. Amount of transportation and centering ability was assessed. Student's t-test was used for statistical analysis. Result Less transportation occurred with MFile-$System^{(R)}$ rotary instrumentation than stainless steel instrument. MFile-$System^{(R)}$ had better centering ability than stainless steel instrument. Conclusion MFile-$System^{(R)}$ rotary instrumentation transported canals less and had good centering ability.

Objectives: This study evaluated the amount of remaining root canal filling materials after retreatment procedures performed by undergraduate students using manual, rotary, and reciprocating techniques through micro-computed tomographic analysis. The incidence of instrument fracture and the instrumentation time were also evaluated. Materials and Methods: Thirty maxillary single rooted teeth were prepared with Reciproc R25 files and filled with gutta-percha and AH Plus sealer by the continuous wave of condensation technique. Then, the specimens were assigned to 3 groups (n = 10), according to the retreatment technique used: manual, rotary, and reciprocating groups, which used K-file, Mtwo retreatment file, and Reciproc file, respectively. Retreatments were performed by undergraduate students. The sample was scanned after root canal filling and retreatment procedures, and the images of the canals were examined to quantify the amount of remaining filling material. The incidence of instrument fracture and the instrumentation time were recorded. Results: Remaining filling material was observed in all specimens regardless of the technique used. The mean volume of remaining material was significantly lower in the Reciproc group than in the manual K-file and Mtwo retreatment groups (p < 0.05). The time required to achieve a satisfactory removal of canal filling material and refinement was significantly lower in the Mtwo retreatment and Reciproc groups (p < 0.05) when compared to the manual K-file group. No instrument fracture was observed in any of the groups. Conclusions: Reciproc was the most effective instrument in the removal of canal fillings after retreatments performed by undergraduate students.

Objectives: This study aimed to evaluate vital pulp tissue removal from different endodontic instrumentation systems from root canal apical third in vivo. Materials and Methods: Thirty mandibular molars were selected and randomly divided into 2 test groups and one control group. Inclusion criteria were a positive response to cold sensibility test, curvature angle between 10 and 20 degrees, and curvature radius lower than 10 mm. Root canals prepared with Hero 642 system (size 45/0.02) (n = 10) and Reciproc R40 (size 40/0.06) (n = 10) and control (n = 10) without instrumentation. Canals were irrigated only with saline solution during root canal preparation. The apical third was evaluated considering the touched/untouched perimeter and area to evaluate the efficacy of root canal wall debridement. Statistical analysis used t-test for comparisons. Results: Untouched root canal at cross-section perimeter, the Hero 642 system showed 41.44% ± 5.62% and Reciproc R40 58.67% ± 12.39% without contact with instruments. Regarding the untouched area, Hero 642 system showed 22.78% ± 6.42% and Reciproc R40 34.35% ± 8.52%. Neither instrument achieved complete cross-sectional root canal debridement. Hero 642 system rotary taper 0.02 instruments achieved significant greater wall contact perimeter and area compared to reciprocate the Reciproc R40 taper 0.06 instrument. Conclusions: Hero 642 achieved higher wall contact perimeter and area but, regardless of instrument size and taper, vital pulp during in vivo instrumentation is not entirely removed.
본 연구는 근관의 치관부는 GT Rotary file을 이용하여 성형하고, 근단부는 K-Flexofile, ProFile .04, Lightspeed, HERO642 file로 성형하여 근관용 file의 근관성형력을 비교하는 것이다. 실험 표본으로는 resin simulated root canal blocks(Dentsply, Swiss)를 이용하였고 4개의 군으로 분류하여 실험을 실시하였다. 모든 resin block을 GT Rotary file(Dentsply, Swiss)을 이용하여 치관부를 형성한 후 근단부는 K-Flexofile(Dentsply, Swiss), ProFile .04 taper file(Denstply. Swiss), Lightspeed file(Lightspeed Tech., USA), HERO642 file(Micromega, France)로 ISO size #35까지 형성하였다. 성형 후 preparation time과 instrument failure를 측정하였고 최종 사용한 file을 resin block 내에 재위치시켜 working length의 변화량을 0.5mm 단위로 측정하였다. Resin debris에 의한 canal blockage를 조사하였으며, 근관 내의 인상을 채득하여 canal form을 평가하였다. 성형 전과 성형 후의 사진을 촬영하고 computer software를 이용하여 중첩시킨 후 canal aberration, canal transportation을 평가하여 다음과 같은 결과를 얻었다. 1. Preparation time, working length의 변화량, canal blockage, canal aberration, canal transportation에 있어서 Ni-Ti file이 stainless steel file에 비하여 우수하였다(p<0.05). 반면에 근관성형력에 있어서 canal form과 canal transportation을 제외하고는 세 Ni-Ti file 사이에는 유의차가 없었다(p>0.05). 2. Canal form 중 taper에 있어서 Lightspeed는 적절하지 못한 taper를 보여주었다(p<0.05). 3. Canal transportation에 있어서 Lightspeed 군과 HER0642 군이 K-Flexofile 군과 ProFile .04 군에 비해 적게 나타났고(p<0.05), 특히 만곡의 끝 부분에서는 Lightspeed 군이 가장 적게 나타났다(p<0.05). 이상과 같은 연구결과로 보아 부적절한 taper를 지니고 있던 Lightspeed fie과 HERO642 file 중 HERO642 file은 GT Rotary file과 함께 사용함으로써 적절한 taper로 형성할 수 있었으나 Lightspeed file은 효과적이지 못하였으므로 적절한 taper를 형성하기 위한 file이 치근의 중간부를 형성하기 위해 필요함이 확인되었다.

The goals of root canal instrumentation are complete debridement of pulp tissue, removal of microbes and affected dentin, and proper cleaning and shaping of the root canal space before obturation. Instrumentation with stainless steel files has been shown to produce undesirable results in canals, regardless of the improved technique or modified file type used. Nickel-Titanium(Ni-Ti) alloy has been shown to be exceptionally elastic, having a lower bending moment and lower permanent set after torsion, compared with similar gauge stainless steel. The purpose of this study was to evaluate the change of root canal prepared by Ni-Ti rotary and stainless steel instruments. Thirty-four single rooted teeth of similar shape and canal size were divided into three groups. The teeth were scanned by computed tomography before instrumentation. In group 1, canals were instrumented using a step-back technique with K-file. In group 2, canals were prepared with K-flex file using the same technique as group 1. Group 3 was prepared with nickel-titanium(Ni-Ti) rotary instrument using a manufacture's instruction. Instrumented teeth were again scanned using computed tomography, and reformated images of the uninstrumented canals were compared with images of the instrumented canals. K-flex file and Ni-Ti file caused significantly less canal transportation than K-file in the 8mm root canal section from the apex(p<0.05). K-flex file and Ni-Ti file produced more centered canal preparation than K-file in the 2mm section(p<0.05). Ni-Ti file maintained more precisely the center of the canal than K-flex file in the 10mm section (p<0.05). There was no difference in the removed volume of canals among each groups.
