Surgical treatment of rhinophyma using an ordinary bur

  • Rhinophyma is a painless benign tumor of the skin of the nose. It is the most severe form of rosacea. Rhinophyma is particularly rare among Asians. It can be treated with surgical resection using various methods, including scalpel excision, dermabrasion, cryosurgery, argon laser, carbon dioxide laser, and electrocautery. However, the gold standard treatment of rhinophyma remains unknown. In the present case, we debulked a giant rhinophyma with a scalpel and contoured it using a bur. Along with a relevant literature review, we present a case of rhinophyma in an Asian patient who was treated at no additional cost using a bur widely used in general plastic surgery.

  • Inflammatory skin diseases such as atopic dermatitis (AD) and rosacea were complicated by barrier abrogation and deficiency in innate immunity. The first defender of epidermal innate immune response is the antimicrobial peptides (AMPs) that exhibit a broad-spectrum antimicrobial activity against multiple pathogens, including Gram-positive and Gram-negative bacteria, viruses, and fungi. The deficiency of these AMPs in the skin of AD fails to protect our body against virulent pathogen infections. In contrast to AD where there is a suppression of AMPs, rosacea is characterized by overexpression of cathelicidin antimicrobial peptide (CAMP), the products of which result in chronic epidermal inflammation. In this regard, AMP generation that is controlled by a key ceramide metabolite S1P-dependent mechanism could be considered as alternate therapeutic approaches to treat these skin disorders, i.e., Increased S1P levels strongly stimulated the CAMP expression which elevated the antimicrobial activity against multiple pathogens resulting the improved AD patient skin.

주사(Rosacea)의 한약 치료에 대한 체계적 문헌고찰 및 메타분석 (Herbal Medicine for the Treatment of Rosacea: A Systematic Review and Meta-analysis of Randomized Controlled Trials)

  • Objectives : This review was conducted to validate the effectiveness and safety of herbal medicine combined with conventional therapy for rosacea. Methods : Randomized controlled trials(RCTs) reporting the effects of herbal medicine treatment on rosacea were searched through eight electronic databases from 2016 to March 17, 2020. This study collection and data extraction were performed by two independent reviews. The Cochrane risk-of-bias tool was used for the evaluation of the risk of bias in all included RCTs. Mean differences(MD) and Risk ratio(RR) of 95% Confidence intervals(Cls) were calculated and data synthesis was conducted using Review Manager(RevMan, ver.5.4) Results : Eighteen RCTs were included and all trials compared the combined therapy of herbal medicine with conventional western therapy to conventional therapy alone. The effective rate of the combination of herbal medicine with western medicine(RR 1.20, 95% CI : 1.13-1.28, p<0.00001, I2=0%), the effective rate of the combination of herbal medicine with laser-based therapy(RR 1.12, 95% CI : 1.04-1.21, p=0.004, I2=18%) and the effective rate of the combination treatment group using herbal medicine, western medicine and external drugs were all statistically higher that of the control group(RR 1.19, 95% CI : 1.11-1.28, p<0.00001, I2=0%). The score of non transient erythema(MD -0.36, 95% CI : -1.01 0.29, p=0.27, I2=93%), flushing(MD -0.69, 95% CI : -0.97, 0.41, p<0.00001, I2=32%), papules or pustules(MD 0.10, 95% CI : -0.15, 0.35 p=0.44, I2=0%) were also seen in the herbal medicine and western medicine combination group. The overall risk of bias of the included studies was some concerns. No serious adverse effects were observed. Conclusions : This review found the safety and effectiveness of the combined therapy of herbal medicine with conventional western therapy for rosacea.

피지세포에서 Akt/AMPK-SREBP-1 경로를 통한 CBD의 피지 합성 억제 효능 (Cannabidiol Inhibits Lipogenesis by Regulating Akt/AMPK-SREBP-1 Pathway in Sebocytes)

  • 여드름은 가장 흔한 피부 질환 중 하나로 청소년기에 주로 발생한다. 호르몬, 유전, 환경적 요인이 알려져 있으며, 이 외에도 피부 과각화 및 C. acnes의 과증식 등이 여드름 발병에 중요한 역할을 한다. CBD는 통증과 스트레스 완화 및 항염증 특성을 갖는 것으로 알려져 있다. 뿐만 아니라, CBD가 함유된 대마 추출물이 여드름 완화 및 치료에 효과적인 소재로 보고되었다. 그러나 이에 대한 연구는 부족한 실정으로, 본 연구를 통하여 피지세포에서 CBD의 항여드름 활성을 확인하고자 하였다. 본 연구진은 세포에 CBD를 처리하여 지질 합성과 증식에 대한 억제 효과를 확인할 수 있었다. 그런 다음 CBD가 SREBP-1를 통해 지방 생성에 대한 억제 효과를 가지는 것을 입증했다. 또한 SREBP-1의 상위 조절자인 Akt와 AMPK가 CBD에 의해 조절되는 것을 확인했다. 종합하면, 본 연구 결과를 통해 CBD가 Akt/AMPK-SREBP-1 경로 조절을 통해 지방 생성을 억제하여 여드름 완화 소재로 이용될 수 있음을 시사하였다. 과각화증으로 인한 염증에 대한 CBD의 효과를 확인하기 위한 추가 연구가 필요하며, 이는 여드름에 대한 CBD의 활용 가능성을 높일 수 있을 것으로 사료된다.

Treatment of Morbihan disease

  • Morbihan disease (MD) is a rare condition that involves rosaceous lymphedema or erythematous lymphedema of the middle and upper thirds of the face. It typically affects the periorbital region, forehead, glabella, nose, and cheeks. The etiology of MD remains unclear, and its diagnosis is challenging. MD often tends to be unresponsive to therapies commonly used to treat rosacea, including corticosteroids, isotretinoin, and antibiotics. Surgical treatments have therefore been attempted, but most cases showed unsatisfactory responses. These problems could have resulted from an incorrect recognition and interpretation of the pathophysiology of MD and inaccurate planning of the operation, resulting in recurrence or exacerbation of edema.

Effect of Application Method and Concentration of Plant Growth Retardants On Plant Quality of Potted Saxifraga rosacea Moench

  • 운간초의 분화 품질을 향상시키기 위하여 'Kumoma'와 'Kumoma-Gusa' 두 품종의 생장과 개화에 미치는 영향을 구명하고자 식물생장억제제 (PGRs) 4종을 처리하였다. 식물생장억제제 종류와 처리농도는 paclobutrazol (10, 20, 40, $80mg{\cdot}L^{-1}$), flurprimidol (5, 10, 20, $40mg{\cdot}L^{-1}$), daminozide (500, 1000, 2000, $4000mg{\cdot}L^{-1}$), chlormequat (50, 100, 200, $400mg{\cdot}L^{-1}$)이고 엽면살포와 토양관주 방법으로 처리하였다. Paclobutrazol $40mg{\cdot}L^{-1}$ 처리는 운간초의 두 품종에서 초장과 화경장을 줄이는데 효과적이었다. 'Kumoma' 품종에서, paclobutrazol $40mg{\cdot}L^{-1}$ 엽면살포와 토양관주 처리방법은 초장을 각각 12.6, 12.5 cm로, 화경장은 3.4, 3.3 cm로 줄일 수 있었다. 'Kumoma-Gusa' 품종에서는 paclobutrazol 토양관주는 처리 농도가 증가함에 따라 초장은 13.2-10.7 cm, 화경장은 3.9-2.0 cm까지 줄일 수 있었지만 꽃의 수는 대조구와 비교했을 때 현저하게 감소하여서 20개까지 감소하였다. Flurprimidol $20mg{\cdot}L^{-1}$ 스프레이와 토양관주 처리는 두 품종 모두 초장과 화경장을 줄일 수 있었다. 'Kumoma' 품종에서 flurprimidol $10mg{\cdot}L^{-1}$ 스프레이 또는 토양관주 처리는 개화 품질을 향상시킬 수 있었고 대조구와 비교해 꽃의 수는 44.7개 이상, 출하 일수는 5일 정도 빨리 출하 할 수 있었다. 'Kumoma-Gusa' 품종에서 PGRs처리에 의한 초장과 화경장 억제효과가 가장 큰 것은 paclobutrazol $80mg{\cdot}L^{-1}$과 flurprimidol $40mg{\cdot}L^{-1}$ 이었고 paclobutrazol과 flurprimidol 스프레이와 토양관주 처리에서 각각, 초장은 10.7, 9.9 cm, 화경장은 2.0, 1.5 cm 로 생장 억제효과가 가장 큰 것을 알 수 있었다. 그러나 flurprimidol $40mg{\cdot}L^{-1}$ 토양관주 처리에서 출하일수는 농도에 따라 3-13일 정도 대조구보다 늦어졌고 꽃의 수는 15.6개로 가장 적은 것을 관찰 할 수 있었다. 두 품종 모두에서 daminozide 처리는 초장과 화경장이 줄어들지 않았지만 'Kumoma' 품종에서는 대조구와 비교했을 때 꽃의 수가 증가하였다. Chlormequat 처리는 paclobutrazol 과 flurprimidol 처리와 비교했을 때 화경장 감소효과는 작았고 처리 식물의 잎은 다른 처리에 비해 연한 녹색을 보여주었다. 두 품종 모두에서 chlormequat 과 daminozide 처리는 생육과 개화 품질에 효과적으로 영향을 주지는 않았지만 꽃의 수는 모든 처리농도에서 41개 이상을 보여주었고, 'Kumoma-Gusa' 품종에 chlormequat 토양관주 처리는 최대 꽃의 수 63개를 볼 수 있었다. 이번 결과들은 paclobutrazol 과 flurprimidol 처리가 운간초 분화의 품질 향상을 위한 화경장의 생육을 조절하는 생장억제제로서 사용 될 수 있음을 보여 주었고, 'Kumoma' 품종에서는 paclobutrazol $40mg{\cdot}L^{-1}$ 스프레이는 생육특성을, flurprimidol $10mg{\cdot}L^{-1}$ 토양관주 방법은 개화관련 특성의 품질을 향상시키는 생장억제제의 농도로 추천 될 수 있고, 'Kumoma-Gusa' 품종에서는 flurprimidol $20mg{\cdot}L^{-1}$ 스프레이와 flurprimidol $5mg{\cdot}L^{-1}$ 토양관주 방법이 운간초 분화의 생육특성과 개화품질을 향상시키는 생장억제제의 농도로 추천 될 수 있다.

방풍통성산(防風通聖散)에 대한 문헌적(文獻的) 고찰(考察) (The Literatural study of Bangpungtongsungsan)

  • I have come to conclusion as follows about Bangpungtongsungsan after literatural study. 1. Bangpungtongsungsan is the medical treatment of heat, wind and dryness. 2. Bangpungtongsungsan is used in head-wind, dizziness, both eye disease, tinnitus, both ear deafness, nasal polyp, uriticaria, sajuabi, beard and hair falling, apoplexy, paralysis of hands and feet, dull mentality, tetanus, epidemic disease characterized by swelling and redness of face, carbuncle, daepungchang, pustule, syphilis, tinea capitis and so on. 3. Bangpungtongsungsan is used in the disease of cerebral hemorrhage, hypertension, arteriosclerosis, constipation, rosacea, hemorrhoids, cutaneous disease, empyema, eye disease, diabetes, asthma, obesity, beriberi, erysipelas, baldhead, fatty heart, chronic nephritis and so on. 4. Bangpungtongsungsan fits in Taeumin who has much wetness-heat and those who have much heat or have much heat but don't give off well. And it doesn't fit in the disease of intolerance to cold and fever in the form of weakness headache, asthenia of the spleen and stomach, cold by internal disorder and so on.

Morphological Description, DNA Barcoding, and Taxonomic Review of Five Nudibranch Species (Gastropoda) from South Korea

  • The nudibranch is one of the most colorful gastropod species found in oceans worldwide. Unlike many other gastropod groups, the nudibranch loses an external shell in the adult stage, but instead develops various chemical defense systems. More than 2,500 nudibranch species have been reported worldwide, and 73 species are currently recorded in Korean waters. In this study, we present morphological descriptions, DNA barcode information of mtDNA cox1 sequence, and taxonomic review for five nudibranch species: Apata pricei (MacFarland, 1966), Doto rosacea Baba, 1949, Janolus toyamensis Baba and Abe, 1970, Polycera abei (Baba, 1960), and Trinchesia sibogae (Bergh, 1905). Of these, we also provide in-depth discussion of taxonomic issue of A. pricei that was previously subdivided into two subspecies, A. pricei pricei and A. pricei komandorica. Our morphological examination and molecular analyses of the mtDNA cox1 sequences indicate that these two subspecies are not taxonomically warranted. The phylogenetic information for the other nudibranch species from mtDNA cox1 sequence analysis is also included, providing a molecular basis for species identification and inferring their local phylogenies within each of the species groups discussed herein.
