CONSERVATIVE ENDODONTIC TREATMENT OF PERMANENT TEETH WITH PERIAPICAL LESIONS : CASE REPORTS (치근단 병소를 가진 영구치의 보존적 치수 치료)

  • Recently, undifferentiated stem cells which exist in dental papillae of immature permanent teeth were newly discovered and these stem cells appear to be the origin of ameloblasts associated with the formation of root dentin. When treating immature permanent teeth, the preservation of these stem cells induce the continuous formation of the root. Therefore, it is reported that minimal invasion to periapical region in immature permanent teeth with periapical inflammation resulted in good-healing pattern in clinical and radiographic examination. In this case, a 10 year-old boy(mandibular right premolar) and a 8 year-old girl(maxillary left premolar) who visited the department of pediatric dentistry at Chosun University Dental Hospital were diagnosed with pulp necrosis and periapical abscess in clinical and radiographic examination. Endodontic instrumentation to the periapical region was limited and MTA(Mineral Trioxide Aggregate) was applied into the pulp canal. The periodic checks showed healing of periapical abscess and the development and growth pattern of roots. In permanent teeth with pulp necrosis and periapical abscess, preservation of pulp and dental papillae in the periapical region showed good prognosis during the periodic examinations. Therefore, a lot of clinical examination and long-term evaluation of conservative pulp treatment in immature permanent teeth are expected to be necessary.

NECROSIS OF ALVEOLAR BONE BY FORMOCRESOL : CASE REPORT (Formocresol에 의한 치조골 괴사의 치험례)

  • Various chemotherapeutic agents have been recommended for pulpotomy of primary teeth, and there are formocresol, ferric sulfate, and calcium hydroxide. Of those, formocresol has fixation effect of pulp tissue and high clinical success rate, so it is most commonly used agent. But formocresol has strong cytotoxic effects, thus many articles reported displacement and loss of permanent successor, amelogenesis imperfecta, mutation by general absorption, possibility of cancer induction. Recently, it has been reported that leakage by imperfect temporary sealing when FC-soaked cotton was inserted into the root canal caused necrosis of surrounding tissues. and that necrosis of alveolar bone related to the use of excessive formocresol. In this case, 2nd primary molar of upper left jaw was treated using formocresol in local clinic, but extracted because of lasting pain. Furthermore, symptoms didn't disappear so patient was refered to us. The patient was 8-year-old male, had foul odor from oral cavity and circular alveolar bone necrosis around the permanent successor' crown. Thus sequestrectomy was operated and observed through 19 months after operation, we found normal root development of permanent successor but no complete regeneration of alveolar bone defect and attached gingiva. Lesion of periodontal tissues by formocresol is irreversible, so we have to confirm the indication in using formocresol and pay attention to complete temporary sealing.

COMPARISON OF APICAL SEALING EFFICACIES USING DIFFERENT PLUGGING DEPTH IN CONTINUOUS WAVE OF OBTURATION TECHNIQUE (Continuous wave of obturation technique에서 플러거의 다양한 적용 깊이에 따른 근단부 폐쇄효율 비교)

  • The purpose of this study was to compare apical sealing ability of continuous wave canal filling technique according to various heat source plugging depths. Eighty one extracted human premolars with straight root were cleaned and shaped to size 35 using .06 taper rotary NiTi file. After cleansing and shaping, the teeth were divided into 5 groups following the heat source probing depths from the apex; 3, 4, 5, 6 and 7 mm. All specimens were filled using E&Q plus with #35/.06 tapered gutta-percha cone. The positive control teeth were not filled. All teeth were coated with nail varnish except the apical 1 mm around the apical foramen. Negative control teeth were completely sealed include the apical foramen. All specimens were immersed in 1% methylene blue solution for 72 hours. Then the specimens were sectioned horizontally at 1, 2 and 3 mm from the root apex. Each sectioned surface was photographed using a digital camera attached to the stereomicroscope at $12.5{\times}2.5$ fold magnification. All points at 1, 2 and 3 mm were summed as final score of one specimen. Statistical analysis of the collected data was performed. Under the condition of this study. there was no significant difference between the heat source plugging depths of 3, 4, 5, 6 and 7 mm in apical sealing ability. All of apical heat source plugging depth from 3 to 7 mm including Buchanan's protocol -from 5 to 7 mm- seems to be acceptable in clinical application.

A CLINICAL AND RADIOGRAPHIC STUDY OF DENTIGEROUS CYSTS (함치성 낭의 임상 방사선학적 연구)

  • The purpose of this study was to obtain information on the clinical and radiographic features of the dentigerous cysts in the jaws. For this study, the authors examined and analysed the clinical records and radiographs of 233 patients who had lesions of dentigerous cyst diagnosed by clinical and radiographic or histopathological examinations. And the obtained results were as follows: 1. Dentigerous cysts occurred the most frequently in the 2nd decade(38.2%) and occurred more frequently in males(67.4%) than in females(32.6%). 2. The most common clinical symptom was swelling of the jaw(33.9%), and the lesions were treated by the method of surgical removal. 3. The type of lesions was mainly observed as central type(72.5%), and size of the lesion was most frequently observed 2 - 2.9cm in the widest length. 4. The lesions were most frequently observed well-defined outline with hyperostotic border(49.8%), and smooth margin(73.4%), and homogeneous lesional radiolucency(79.4%). 5. Cortical thinning and expansion of the lesions(82.0%) were observed, and their direction were most frequently observed toward buccal side(64.0%). 6. The effect on the causative tooth were observed as tooth displacement(41.2%) and delayed root development(l9.3%), and the distance between cemento-enamel junction and lesional wall attachment of the causative tooth was mainly observed as below 2mm(79.6%). 7. The effect on the adjacent tooth were observed as loss of lamina dura(66.8%), root resorption(33.9%), and tooth displacement(31.5%). 8. The effects on the adjacent anatomic structures were observed as displacement of the mandibular canal(46.5%) and maxillary sinus or nasal cavity(72.2%).

Effects of Root of Cibotii Rhizoma on Neuronal Damage of Spinal Cord Contusion Injury in Rats (구척(狗脊)이 흰쥐의 척수압박에 의한 신경세포 손상에 미치는 영향)

  • Objectives : This study was performed to evaluate the effects of root of Cibotii rhizoma(CR) ethanol extract on the tissue and neuronal damage of the spinal cord injury(SCI). Methods : SCI was induced by mechanical contusion following laminectomy of 10th thoracic vertebra in Sprague-Dawley rats. CR was orally given once a day for 7 days after SCI. Tissue damage and nerve fiber degeneration were examined with cresyl violet and luxol fast blue(LFS) histochemistry. HSP72(as neuronal damage marker), MAP2(as nerve fiber degeneration marker), c-Fos(immediate early gene), and Bax(pro-apoptotic molecule) expressions were examined using immuno-histochemistry. Individual immuno-positive cells expressing HSP72, MAP2, c-Fos and Bax were observed on the damaged level and the upper thoracic and lower lumbar spinal segments. Results : 1. CR reduced degeneration of nerve fibers and motor neuron shrinkage in the ventral horn of the lower lumbar spinal segment, but generally it did not seem to ameliorate the tissue injury following SCI. 2. CR reduced demyelination in the ventral and lateral funiculus of the lower lumbar spinal segment. 3. CR reduced HSP72 expression on the neurons in the peri-central canal gray matter adjacent to the damaged region. 4. CR strengthened MAP2 expression on the motor neurons in the ventral horn and on nerve fibers in the lateral funiculus of the lower lumbar spinal segment. 5. CR reduced c-Fos positive cells in the peri-lesion and the dorsal horn of the damaged level and in the ventral horn of the lower lumbar spinal segment. 6. CR reduced Bax positive cells in the peri-lesion and the dorsal horn of the damaged level and in the ventral horn of the lower lumbar spinal segment. Conclusions : These results suggest that CR plays an inhibitory role against secondary neuronal damage and nerve fiber degeneration. following SCI.

CLINICAL APPLICATION OF MTA(MINERAL TRIOXIDE AGGREGATE) FOR APEXIFICATION (치근단 형성술(Apexification)에 있어서 MTA(Mineral Trioxide Aggregate)의 적용)

  • Traumatic injuries in young patients can result in the interruption of the development of the incompletely formed roots. In teeth with incomplete root-end formation and necrotic pulps, the root canals must be completely debrided. Because of a lack of an apical stop and the presence of thin and fragile walls in these teeth, it is imperative to perform apexification to obtain an adequate apical seal. Calcium hydroxide has become the material of choice for apexification. Despite its popularity for the apexification procedure, calcium hydroxide therapy has some inherent disadvantages that include variablility of treatment time, unpredictability of apical closure, difficulty in patient follow-up, and delayed treatment. An alternative treatment to long-term apexification procedure is the use of an artificial apical barrier that allows immediate obturation of the canal. MTA(Mineral Trioxide Aggregate) is a powder consisting of fine hydrophilic particles of tricalcium silicate, tricalcium aluminate, tricalcium oxide and silicate oxide. MTA has a pH of 12.5 after setting, similar to calcium hydroxide. This may impart some antimicrobial properties. MTA has low solubility and a radiopacity slightly eater than that of dentin. Also, MTA leaked significantly less than other materials and induced hard-tissue formation more than other materials.

THE EFFECT OF INTRACANAL BASE TO PROTECT THE CERVICAL LEAKAGE OF BLEACHING AGENTS (표백제의 치경부 누출을 방지하기 위한 근관 내 이장재의 효과)

  • Intracoronal bleaching is currently disregarded by many clinicians because of the potential consequence of cervical resorption. To prevent this complication it is recommended that intra coronal barrier materials be placed over the root canal obturation and sodium perborate be used with water rather than with hydrogen peroxide. The purpose of this study was to evaluate the amount of the hydrogen peroxide penetration according to the difference in intracanal base materials and sodium perborate preparation. Fifty extracted intact premolars were instrumented, and filled with gutta-percha. And then the outer surface of the teeth was sealed with wax exposing the CEJ. The prepared teeth were placed in plastic tubes containing 1.5ml distilled water with their entire root submerged into the solution, The teeth were divided into the following five groups. In the first two groups gutta-percha was removed without placement of barrier, and then water or superoxole(30% $H_2O_2$) with sodium perborate were used respectively for bleaching. In the other three groups, after removal of gutta-percha, an intracanal isolating barrier(ZPC, IRM, Fuji II LC) was placed and then bleached with sodium perborate and superoxole. The bleaching procedure was performed 4 times with 1 week interval. The results were as follows : 1. All the groups showed a tendency of increasing penetration amount with increasing treatment times(P<0.05). 2. After the 1st and 2nd treatments, there was no significant difference in microleakage among the groups. 3. After the 3rd bleaching with superoxole and sodium perborate, there was no significant difference in microleakage between gutta-percha alone group and gutta-percha with ZPC, Fuji II LC barrier group. But significant difference was found between IRM barrier group and other groups(P<0.01). 4. After the 4th bleaching with superoxole and sodium perborate, there was no significant difference between gutta-percha alone group and gutta-percha with barrier groups. 5. After the 4th treatment, the group bleached with sodium perborate and water without barrier showed lower hydrogen peroxide penetration than that of other groups(P<0.01).

Prognostic factors influencing clinical outcome of nonsurgical endodontic treatment (비외과적 근관치료의 임상적 성공에 영향을 미치는 예측 인자들의 평가)

  • Objectives: This study aimed to assess prospectively the clinical outcome of nonsurgical endodontic treatment and identify patient- and tooth-related factors, rather than treatment-related factors, that were the best predictors of this outcome. Materials and Methods: The inception cohort comprised 441 teeth (320 patients) and 175 teeth (123 patients) were followed up for 1-2 years. Age, gender, presence of medical disease, number of canals, previous endodontic treatment, presence of sensitivity and pain, pulp vitality, swelling or sinus tract of pulpal origin on the gingiva, periapical radiolucency and tendency of unilateral bite on the affected tooth were recorded at treatment start. Results: The outcome was classified on the basis of periapical radiolucency as healed or non healed. The overall healed rate in these cases, including nonsurgical retreatment, was 81.1%. Four tooth-related factors had a negative impact in the bivariate analysis: previous endodontic treatment, necrotic pulp, preoperative gingival swelling or sinus tract of pulpal origin, and preoperative periapical radiolucency. Stepwise logistic regression analysis including patient-, tooth-related factors and level of the root canal filling as a treatment-related factor showed that preoperative gingival lesion (odds ratio [OR]: 4.4; p = 0.005), preoperative periapical radiolucency (OR: 3.6; p = 0.011), and $\leq$ 1-2 mm under root filling length (OR: 9.6; p = 0.012) were significant predictors of failure. Conclusions: A preoperative gingival lesion of pulpal origin can influence the outcome of nonsurgical endodontic treatment in addition to preoperative periapical radiolucency.

A Case Study on Phytoremediation in Polluted Stream by Heilianthus annuus (Heilianthus annuus에 의한 오염된 하천에서의 Phytoremediation에 관한 연구)

  • This is the research to prepare a purification program for relatively less polluted stream by using phytoremediation. We calculated a treatment amount of nutrients followed by growth of Helianthus annuus (a kind of sunflower), setting up the plant reactor in the hothouse. Moreover, to investigate a field applicability, we could find increased contents of nitrogen, carbon and hydrogen in plants by setting up a H. annuus planted artificial floating island in an irrigation canal. When we changed the dissolved inorganic nitrogen(DIN) concentration of the influent from 28.5 to 199.2 mg/l and the dissolved inorganic phosphorus(DIP) concentration of the influent from 13.3 to 25.4 mg/l, growth disorder has not appeared though it is much higher than the criterion of water for irrigation. In this case, the removal rate of DIN was $81.7\sim98.6%$, and that of DIP was $81.9\sim98.4%$ in 3 days stay on average. It has appeared that the efficient hydraulic retention time(HRT) was 48 hours. The following contents of nitrogen, carbon and hydrogen of H. annuus appeared in the artificial floating island: nitrogen was $3.2\sim7.8%$ in the trunk and $3.0\sim6.3%$ in the root. Carbon was $40.1\sim57.7%$ in the trunk and $43.4\sim53.8%$ in the root.

Comparison of shaping ability between single length technique and crown-down technique using Mtwo rotary file (Mtwo 전동 파일을 사용한 single length technique과 crown-down technique의 근관성형 효율 비교)

  • The aims of this study were to compare the shaping effect and safety between single length technique recommended by manufacturer and crown-down technique using Mtwo rotary file and to present a modified method in use of Mtwo file. Sixty simulated root canal resin blocks were used. The canals were divided into three groups according to instrument and the manner of using methods. Each group had 20 specimens. Group MT was instrumented with single length technique of Mtwo, group MC was instrumented with crown-down technique of Mtwo and group PT was instrumented with crown-down technique of ProTaper. All of the rotary files used in this study were operated by an electric motor. The scanned canal images of before and after preparation were superimposed. These superimposed images were evaluated at apical 1 to 8 mm levels Angle changes were calculated. The preparation time, weight loss, instrument failure and binding, canal aberrations, and centering ratio were measured. Statistical analysis of the three experimental groups was performed with ANOVA and Duncan's multiple range tests for post-hoc comparison and Fisher's exact test was done for the frequency comparison. In total preparation time, group MT and group MC were less than group PT. In the aberrations, group MT had more elbows than those of group MC and group PT. The binding of group MC was least and group MT was less than group PT (P < 0.05). Under the condition of this study, crown-down technique using Mtwo rotary file is better and safer method than single length technique recommended by the manufacturer.
