Objectives: Obstructive sleep apnea (OSA) is common in people with epilepsy (PWE), and confers medical and seizure-related consequences when untreated. Positive airway pressure, the gold-standard for OSA management, is limited by tolerability. As serotonin is involved respiratory control and amelioration of seizure-induced respiratory events, this study aims to determine whether serotonin reuptake inhibitors (SRIs) may represent a potential therapeutic option. Methods: A retrospective study of 100 PWE and OSA ${\geq}18$ years of age was conducted. The primary outcome measure was OSA severity as function of SRI use, with rapid eye movement (REM)-related OSA as a secondary outcome. Results: Older age and depression were more common in those taking an SRI. There was no association between SRIs and OSA severity. However, the SRI group was less likely to have REM-related OSA. Conclusions: In PWE and OSA, SRI use is associated with reduced risk of REM-related OSA, and may represent a potential management strategy.
Purpose: The purpose of this study is to suggest a modified approach that compensates some shortcomings of RPN with relevant strength of ASIL for Safety System and suggests systematic and logical approach for FMEA. Methods: By comparing the objectives, determination procedures, and key conceptual differences of RPN and ASIL, a refined method of risk evaluation and a new risk metric are devised. Results: While the traditional FMEA provides only rough evaluation of relative risk for each failure, the proposed method compensates its shortcomings with relevant strength of ASIL and provides a more logical and practical procedure of risk evaluation. Conclusion: The new metric RPM provides not only a comparative priority rank but also the degree of physical seriousness. Besides, it may have even more benefits for various applications if the severity can be expressed as mone tary amount of losses.
본 연구에서는 승객, 공중 및 직원의 철도 사상사고를 대상으로 위험도 평가모델을 개발하였다. 이를 위해 철도 사상사고의 위험요인을 분석하여 관련 위험사건을 정의하였고, 위험사건의 발생을 초래하는 위험요인들의 논리적 연계성을 사건발생 시나리오로 구성하여 사건발생빈도 평가모델을 고장수목(Fault Tree)을 이용하여 개발하였다. 또한 사건수목(Event Tree)을 이용하여 인명피해를 초래하는 영향인자를 사건진전 시나리오로 구성하고, 위험사건별 사고 심각도를 등가사망지수로 환산하여 계산하는 위험도 평가모델을 개발하였다. 본 연구의 결과는 비용효과 분석, 안전대책의 민감도 분석 등에 다양하게 활용될 수 있다.
본 연구의 목적은 구조방정식모형을 활용하여 스마트폰중독 위험군 대학생의 대인관계문제, 스트레스대처방식, 정신건강의 관계를 분석하는 것이다. 연구 결과, 대인관계문제는 정신건강의 심각성을 악화시키는 직접적 영향력을 미치는 것으로 나타났다. 스트레스대처전략은 대인관계문제와 정신건강 사이의 영향관계에서 부분 매개효과를 갖는 것으로 나타났고, 소극적 대처는 위험요인, 적극적 대처는 보호요인으로 분석되었다. 이러한 결과를 바탕으로 스마트폰중독 위험군 대학생의 정신건강 증진을 위한 다양한 실천적 정책적 개입전략을 제시하였다.
The coastal composite fishery has the highest rate of around 30.0% in registered fishing vessels of Korea for the last three years. The risk analysis for the fisher's safety of coastal composite fishing vessels was conducted to serve as a basic data for improving the healthy and safe working environment of fisher using fisher's occupational accidents of the national federation of fisheries cooperatives (NFFC) from 2016 to 2018 (n = 1,105). As a result, the occupational accident occurrence rate of coastal composite fishery was 3.76% in all industries, 7.8 times the rate of that. In addition, death and missing rate was found to have a serious level management to 10.10‱. Among the accidents, 76.0% occurred at sea. The slipping, others and contact with machinery or fishing gear occurred more frequently in order in the frequency of accident occurrence pattern. In the risk assessment using frequency and severity, slipping, contact with machinery and others showed high risk during fishing. The results are expected to contribute to identification and assessment of safety hazard occurred in coastal composite fishing vessels.
Coronavirus disease 2019 (COVID-19) has become a major health burden worldwide, with over 600 million confirmed cases and 6 million deaths by 15 December 2022. Although the acute phase of COVID-19 management has been established, the long-term clinical course and complications due to the relatively short outbreak is yet to be assessed. The current COVID-19 pandemic is causing significant morbidity and mortality around the world. Interestingly, epidemiological studies have shown that fatality rates vary considerably across different countries, and men and elderly patients are at higher risk of developing severe diseases. There is increasing evidence that COVID-19 infection causes neurological deficits in a substantial proportion to patients suffering from acute respiratory distress syndrome. Furthermore, lack of physical activity and smoking are associated with severe acute respiratory syndrome coronavirus-2 (SARS-CoV-2) susceptibility. We should therefore explore why lack of physical activity, smoking, etc causing a population more susceptible to SARS-CoV-2 infection, and mechanism involved. Thus, in this review article, we summarize epidemiological evidence related to risk factors and lifestyle that affect COVID-19 severity and the mechanism involved. These risk factors or lifestyle interventions include smoking, cardiovascular health, obesity, exercise, environmental pollution, psychosocial social stress, and diet.
목적: 중등도 이상의 손상 환자를 대상으로 손상의 중증도에 따른 정형외과적 손상의 손상기전, 부위, 유형, 치료적 접근 및 그 치료결과를 분석하고자 한다. 대상 및 방법: 2014년부터 2019년까지 응급실/외상센터가 운영되지 않은 기간을 제외한 57개월 동안 국립중앙의료원 외상센터로 입원한 손상 중증도 점수(Injury Severity Score, ISS) 9점 이상의 환자들 중 정형외과적 손상이 동반된 778명을 대상으로 후향적 연구를 시행하였다. ISS와 생리학적 지표를 기준으로 한 손상의 중증도에 따라 중등도 손상군(1군, 679명)과 중증 손상군(2군, 99명)으로 분류하고 손상기전, 비정형외과적 손상을 평가하였다. 정형외과적 손상은 침범된 해부학적 영역과 골격구조의 수와 유형을 평가하였고, 치료적 접근 방법을 세분화하여 비교하였다. 치료결과는 재원기간을 포함하여 재원기간 내 발현된 전신 합병증과 사망률을 평가하였고 사망률에 미치는 요인을 분석하였다. 결과: 2군이 보다 젊고, 남성의 빈도가 높았고 고에너지 손상기전 및 동반 손상의 빈도가 유의하게 증가되었다. 정형외과적 손상 중 침범된 골격계의 수는 2군에서 유의하게 많아지면서 하지를 주로 침범한 1군에 비해 2군에서는 골반, 척추, 상지 영역의 침범이 유의하게 증가하였다. 정형외과적 손상은 환자의 임상적 상태에 따라 확정적 또는 단계적 치료가 시행되었으며 다발성 중증 손상을 보이는 2군에서는 일시적 외고정술을 이용한 단계적 수술의 빈도가 유의하게 높았고 1군에서는 생리학적 안정화를 이룬 뒤 확정적 수술을 시행하는 빈도가 유의하게 높았다. 중증도가 높아질수록 재원기간 역시 길어졌으며 재원기간 내 총 전신 합병증은 약 4.9%, 총 사망률는 약 4.5%였다. 중증도가 높아질수록 사망률은 유의하게 높아졌다(1군 2.9%, 2군 15.2%; p<0.0001). 사망률에 미치는 요인 중 고령과 높은 ISS가 중요한 요인으로 확인되었다. 결론: 손상의 중등도가 높을수록 고에너지 손상기전에 의한 손상의 발생 빈도가 높아지며 보다 젊고, 남성의 빈도와 동반 손상의 빈도가 높아졌다. 정형외과적 손상 역시 다발성 손상의 빈도와 중증도가 유의하게 증가되면서 정형외과적 외고정장치를 이용한 내고정술 등이 단계적 치료의 일환으로 시행된 경우가 유의하게 높아지는 경향을 보였다. 또한 재원기간, 전신 합병증 및 사망률 역시 유의하게 증가되는 경향을 보이며, 사망률에는 높은 연령과 ISS가 중요한 영향을 미치는 것으로 확인되었다.

기후변화의 영향으로 극심한 가뭄에 의한 피해가 증가하고 있으며, 이러한 피해를 줄이기 위하여 극한 가뭄에 대한 정량적인 분석이 필요하다. 따라서 본 연구에서는 극한 가뭄의 위험도에 대한 정량적 분석을 위해 임계수준방법을 측우기 강우자료, 관측 강우자료, 미래 기후변화 시나리오 강우 자료에 적용하여 가뭄사상을 정의하고 가뭄의 지속기간과 심도를 도출하였다. 또한, 코플라 함수를 활용하여 가뭄 지속기간 및 심도를 동시에 고려하는 이변량 가뭄빈도해석을 실시하였다. 이변량 가뭄빈도곡선을 바탕으로 과거 현재 미래에 대한 위험도를 산정했으며, 과거 및 현재를 기준으로 미래의 극한 가뭄에 대한 위험도를 분석하였다. 그 결과 과거 및 현재에 비해 미래의 평균 가뭄 지속기간은 짧게 나타났으나 평균 가뭄 심도는 매우 크게 나타났다. 따라서 미래에는 짧은 기간의 심한 가뭄들이 발생할 것으로 예측된다. 또한, 최대가뭄의 위험도를 분석한 결과 미래의 최대 가뭄 위험도는 과거 및 현재에 비해 각각 1.39~1.94배, 1.33~1.81배 큰 것으로 확인되었다. 최종적으로 미래에서 과거 및 현재의 기왕최대 가뭄 이상의 극한 가뭄위험도는 0.989와 1.0 사이의 범위를 가지는 것으로 나타나, 미래에는 극한 가뭄의 발생확률이 높은 것으로 판단된다.

Background: The incidence of colorectal cancer increases with vitamin D deficiency as shown in recently published studies. In addition, prospective investigations have indicated that low vitamin D levels may be associated with increased mortality of colorectal cancer, especially in stage III and IV cases. However, the exact incidence of vitamin D deficiency and the relation between vitamin D deficiency and osteopenia/osteporosis is still not known. The aim of this study is to identify severity of vitamin D deficiency and absolute risk factors of osteopenia/osteoporosis in colorectal cancer survivors. Materials and Methods: A total of 113 colorectal cancer survivors treated with surgery and/or chemotherapy ${\pm}$ radiotherapy were recruited from medical oncology outpatient clinics during routine follow-up visits in 2012-2013. Bone mineral densitometry (BMD) was performed, and serum 25-OH vitamin D levels were also checked on the same day of the questionnaire. The patients was divided into 2 groups, group A with normal BMD and group B with osteopenia/osteoporosis. Results: The median age of the study population was 58 (40-76). Thirty (30.0%) were female, whereas 79 (70.0%) were male. The median follow-up was 48 months (14-120 months). Vitamin D deficiency was found in 109 (96.5%); mild deficiency (20-30 ng/ml) in 19 (16.8%), moderate deficiency (10-20 ng/ml) in 54 (47.8%) and severe deficiency (<10 ng/ml) in 36 (31.9%). Osteopenia was evident in 58 (51.4%) patients whereas osteoporosis was noted in 17 (15.0%). Normal BMD was observed in 38 (33.6%). No apparent effects of type of surgery, presence of stoma, chemotherapy, radiotherapy and TNM stage were found regarding the risk of osteopenia and osteoporosis. Also, the severity of the vitamin D deficiency had no effect in the risk of osteopenia and osteporosis (p=0.93). In female patients, osteopenia/osteoporosis were observed in 79.5% patients as compared to 60.7% of male patients (p=0.04). Conclusions: In our study, vitamin D deficiency and osteopenia/osteoporosis was observed in 96.5% and 66.4% of colorectal cancer survivors, respectively. There is no defined absolute risk factor of osteopenia and osteoporosis in colorectal cancer survivors. To our knowledge, in the literature, our study is the first to evaluateall the risk factors of osteopenia and osteoporosis in colorectal cancer survivors.
