  • 전신적으로 면역이 결핍된 백혈구 접착 결핍증 환아에서는 정기적인 구강 위생 관리는 매우 중요하다. 치면세균막의 제거를 통해 국소적인 자극원을 감소시킴으로써 구강 내 자극에 대한 동통이나 구취 감소가 가능하고 진행되는 치주조직의 파괴 양상을 어느 정도 지연시킬 수 있을 것으로 사료된다. 내원을 통한 관리도 중요하지만 잇솔질 교육을 통하여 환자 스스로 가정에서도 구강 위생 관리를 시행토록 하여야 하며 향후 감염원이 될 수 있는 원인을 감소시키는 노력이 필요하다.

지체장애인 맞춤형주택 디자인가이드라인의 사용자혜택 분석 (User Benefits Analysis of Customized House Design Guidelines for Physically Disabled People)

  • Most existing residential environment of disabled people has made them exposed to the risk of safety accidents, and therefore hinders their independent living. In this regard, the importance of desirable residential environment has emerged. Some preceding studies have addressed the characteristics of disabled people's residential environment plan; however, the information on what environmental benefits are offered through such characteristics is insufficient. This study aims to analyze the housing design guidelines for disabled people in respect of the benefit of disabled people who are actual users. In this study, essential design guidelines for disabled people verified through expert workshop panel was analyzed based on user benefit theory. According to the study results, the total guidelines have the most characteristics supporting behavioral facilitation, and mainly consist of physiological maintenance characteristics related to disabled people's health and safety. Also, it is founded that environmental affordance essentially required depending on each spatial function was different. The ratio of physiological maintenance item is high in access route, unlike living room, kitchen and bed room where behavioral facilitation is especially emphasized. These findings can be utilized as basic data for customized residential environment plan to realize residential welfare for disabled people.

In Vivo Non Invasive Molecular Imaging for Immune Cell Tracking in Small Animals

  • Clinical and preclinical in vivo immune cell imaging approaches have been used to study immune cell proliferation, apoptosis and interaction at the microscopic (intra-vital imaging) and macroscopic (whole-body imaging) level by use of ex vivo or in vivo labeling method. A series of imaging techniques ranging from non-radiation based techniques such as optical imaging, MRI, and ultrasound to radiation based CT/nuclear imaging can be used for in vivo immune cell tracking. These imaging modalities highlight the intrinsic behavior of different immune cell populations in physiological context. Fluorescent, radioactive or paramagnetic probes can be used in direct labeling protocols to monitor the specific cell population. Reporter genes can also be used for genetic, indirect labeling protocols to track the fate of a given cell subpopulation in vivo. In this review, we summarized several methods dealing with dendritic cell, macrophage, and T lymphocyte specifically labeled for different macroscopic whole-body imaging techniques both for the study of their physiological function and in the context of immunotherapy to exploit imaging-derived information and immune-based treatments.

  • Of all facial fractures in children, condylar fractures have the greatest propensity to produce a growth disturbance. This risk appears to be greatest when the injury is during the first 3 years of during adolescence. Yet, the ability of a child to undergo compensatory growth that decrease the effects of the injury is also the greatest. Fracture dislocation of the condyle in the preadolescent often results in excellent remodeling and function. Because of this factor and the higher risRs of avascular necrosis and ankylosis open reduction of a condylar fracture in a child is not widely recommended. This retrospective study analyzed mandibular condyle fractures in the children who admitted in dept. of Oral and Maxillofacial Surgery, Pusan paik hospital from 1984 to 1993 clinically and radiologically.

저선량 감마선 노출에 의한 비장세포의 염증 유발 작용에 대한 연구 (Effects of Low Dose Gamma Irradiation on the Inflammatory Response in Spleen Cells)

  • Gamma irradiation (${\gamma}IR$) is widely used for radiotherapy as a treatment of cancer cells although it has a risk to damage normal cells. Inflammation is regarded as one of side effects of ${\gamma}IR$ while the effect of low dose of ${\gamma}IR$ on inflammation has not been researched well. Here, we investigated the inflammatory responses of low dose of ${\gamma}IR$ on murine spleen cells. It was evaluated if ${\gamma}IR$ affected the mitogen-induced lymphocyte proliferation, the regulation of various inflammatory cytokines (IFN-${\gamma}$, IL-2, IL-17, IL-4, IL-10), and the involvement of Ikaros and MAPK/NF-${\kappa}B$ medicated mechanism. Exposure of $^{137}Cs-{\gamma}IR$ below 2 Gy decreased the lymphocytes proliferative response to mitogens (LPS, ConA) except at the lowest dose, 0.05 Gy. IL-17, IL-2 and IL-4 mRNA increased at 0.5 and 2 Gy, but not altered at 0.05 Gy. IL-10, anti-inflammatory cytokine, increased only at 0.05 Gy. In regard to intracellular signaling, p-JNK, p-p38 and p-$I{\kappa}B{\alpha}$ were not changed, whereas the activation of ERK and Ikaros increased at the lowest dose. These results suggest that exposure of ${\gamma}IR$ less than 0.5 Gy (or below 0.05 Gy) has beneficial effects as a radiation hormesis on immune function.

  • Until now, systems in financial companies have been constructed and operated based on great mainframe proved being stability. But it has had many disadvantages since they only implement maintenance adding and changing function. So they need construction of new systems(development of critical application, integration of various service channel, management of customer data). In spite of great construction costs and high risk, it is necessary that they construct e-financial system. Nowadays financial institutions must actively offer services to customers. In other words, the key of service is being moved from providers to customers. In oder to develop and sell new products in a timely manner, integrated management about appropriate and valid customer data is needed. And new system that covers expanded area of work is needed since the original parts of the area is being broken gradually. In this paper, we search construction processes of e-financial system of Nonghyup to respond to new financial environment flexibly and actively, concrete contents about innovation activities of e-financial system and the cases of service utilization. Also, we suggest the development direction of e- financial system for Nonghyup following day.

임플란트 보철의 생역학과 교합 (Biomechanics and Occlusion for Implant-Supported Prosthesis)

  • There is an increasing appreciation of the vital role that biomechanics play in the performance of oral implant. The aim of this article is to provide some basic principles that will allow a clinician to formulate a biomechanically valid treatment plan. However, at this point in the history of oral implantology, the clinician should realize that we do not know enough to provide absolute biomechanical rules that will guarantee success of all implants in all situations. To examine the biomechanical questions, one must begin with an analysis of the distribution of biting forcess to implants. Related topics, such as stress transfer to surrounding tissues and interrelationships between bone biology and mechanical loading are major subjects, deserving a separate discussion. Once rigid fixation, angulation, crestal bone level, contour, and gingival health are achieved, stress beyond physiologic limits is the primary cause of initial bone loss around implants. The restoring dentist has specific responsibilities to reduce overload to the bone-implant interface. These include proper diagnosis, leading to a treatment plan designed with adequate retention and form, and progressive loading to improve the amount and density of bone and further reduce the risk of stress beyond physiologic limits. The major remaining factor is the development of occlusal concept in harmony with the rest of the stomagnetic system.

전두측두엽 치매 (Frontotemporal Dementia)

  • Frontotemporal dementia (FTD) is a degenerative disease characterized by the selective frontal and temporal lobe atrophy, and progressive deficits in behavior, executive function, or language. The prevalence and incidence of FTD are 15-22/100000 and 2.7-4.1/100000, respectively, in midlife. Hereditary is an important risk factor for FTD. Although there is some controversy regarding the further syndromatic subdivision of the different types of FTD, FTD is clinically classified into behavioral variant of frontotemporal dementia, semantic dementia and progressive nonfluent aphasia. FTD can be misdiagnosed as many psychiatric disorders because of similarity of the prominent behavioral features. Advances in clinical, imaging, and molecular characterization have increased the accuracy of FTD diagnosis, thus developing for the accurate differentiation of these syndromes from psychiatric disorders. We also discuss about therapeutic strategies for symptom management of FTD. Medications such as serotonin reuptake inhibitors, antipsychotics, and other novel treatments have been used in FTD with various rates of success. Further advanced research should be directed at understanding and developing new diagnostic and therapeutic modalities to improve the FTD patients' prognosis and quality of life.

육체운동에 의해 유발되는 화학물질에 대한 반응성의 변화 (Alteration in Response to Chemicals Induced by Physical Exercise)

  • Acute or repeated physical exercise affects a large number of physiological parameters including hemodynamics, respiration, pH, temperature, gastrointestinal function and biotransformation, which determine the pharmacokinetics of drugs and chemicals. The rate and the amount of a chemical reaching the active site are altered by physical exercise, which results in significant changes in pharmacolosical/toxicological activity of the chemical. This aspect of physical exercise has vast implication in therapeutics and in safety evaluation, particularly for chemicals that have a low margin of safety. However there appears to be a wide inter- and intraindividual variation in the effects of physical exercise depend-ing on the duration, intensity and type of exercise, and also on the properties of each chemical. It is suggested that more studies need to be done to determine which factor(s) plays a major role in the disposition of chemicals in human/animals performing physical exercise. Certain chemicals induce severe toxicity due to metabolic conversion to reactive intermediate metabolites. it is suggested that repeated exercise may enhance the free radical scavenging system by increasing the activity of antioxidant enzymes. This area of research remain to be explored to elucidate the interaction of exercise and chemical on the antioxidant system.

I-123 MIBG Cardiac SPECT의 임상적 적응증 (Clinical Application of I-123 MIBG Cardiac Imaging)

  • Cardiac neurotransmission imaging allows in vivo assessment of presynaptic reuptake, neurotransmitter storage and postsynaptic receptors. Among the various neurotransmitter, I-123 MIBG is most available and relatively well-established. Metaiodobenzylguanidine (MIBG) is an analogue of the false neurotransmitter guanethidine. It is taken up to adrenergic neurons by uptake-1 mechanism as same as norepinephrine. As tagged with I-123, it can be used to image sympathetic function in various organs including heart with planar or SPECT techniques. I-123 MIBG imaging has a unique advantage to evaluate myocardial neuronal activity in which the heart has no significant structural abnormality or even no functional derangement measured with other conventional examination. In patients with cardiomyopathy and heart failure, this imaging has most sensitive technique to predict prognosis and treatment response of betablocker or ACE inhibitor. In diabetic patients, it allow very early detection of autonomic neuropathy. In patients with dangerous arrhythmia such as ventricular tachycardia or fibrillation, MIBG imaging may be only an abnormal result among various exams. In patients with ischemic heart disease, sympathetic derangement may be used as the method of risk stratification. In heart transplanted patients, sympathetic reinnervation is well evaluated. Adriamycin-induced cardiotoxicity is detected earlier than ventricular dysfunction with sympathetic dysfunction. Neurodegenerative disorder such as Parkinson's disease or dementia with Lewy bodies has also cardiac sympathetic dysfunction. Noninvasive assessment of cardiac sympathetic nerve activity with I-123 MIBG imaging nay be improve understanding of the pathophysiology of cardiac disease and make a contribution to predict survival and therapy efficacy.
