Coronavirus has been spreading rapidly around the world since it broke out in China in 2019. Respiratory diseases caused by coronavirus infection cause various diseases ranging from asymptomatic subclinical infections to severe pneumonia and cardiovascular complications, leading to death. In this regard, natural products are being studied to prevent various diseases caused by COVID-19. In current review, we would like to present mechanisms related to the inhibition of heart disease in ginseng and ginsenoside against SARS-CoV-2. In many previous studies, ginseng and ginsenoside are known to have antioxidant, blood flow improvement, improvement of vascular and heart function, blood pressure control, suppression of myocardial infarction and heart failure, and antiarrhythmia. Therefore, ginseng and ginsenoside have a possibility to suppress cardiovascular complications caused by COVID-19. Many of research provide evidence for ginseng and ginsenoside as treatments for the risk of cardiovascular complications. However, in this review, more specific contents on the proposition of the efficacy of ginseng and ginsenoside for COVID-19 should be presented. Therefore, we hope that researches to reduce cardiovascular complications of ginseng and ginsenoside for COVID-19 should be presented to reduce mortality for COVID-19.
Although chronic obstructive pulmonary disease (COPD) and interstitial lung disease (ILD) have distinct clinical features, both diseases may coexist in a patient because they share similar risk factors such as smoking, male sex, and old age. Patients with both emphysema in upper lung fields and diffuse ILD are diagnosed with combined pulmonary fibrosis and emphysema (CPFE), which causes substantial clinical deterioration. Patients with CPFE have higher mortality compared with patients who have COPD alone, but results have been inconclusive compared with patients who have idiopathic pulmonary fibrosis (IPF). Poor prognostic factors for CPFE include exacerbation, lung cancer, and pulmonary hypertension. The presence of interstitial lung abnormalities, which may be an early or mild form of ILD, is notable among patients with COPD, and is associated with poor prognosis. Various theories have been proposed regarding the pathophysiology of CPFE. Biomarker analyses have implied that this pathophysiology may be more closely associated with IPF development, rather than COPD or emphysema. Patients with CPFE should be advised to quit smoking and undergo routine lung function tests, and pulmonary rehabilitation may be helpful. Various pharmacologic agents and surgical approaches may be beneficial in patients with CPFE, but further studies are needed.
Total joint arthroplasty is a successful joint replacement treatment that improves joint function and overall quality of life and provides pain relief. However, the prevalence of periprosthetic joint infection (PJI) has become prevalent with the rise in the incidence of arthroplasty surgery. PJI occurs rarely following arthroplasty however presents with serious complications, including high morbidity. The identification of causative microorganisms is essential for the treatment of PJI. Managing PJI requires complex treatment strategies, including long-term antibacterial treatment, and significant medical costs can be incurred. The American Academy of Orthopedic Surgeons, the Centers for Disease Control and Prevention, and Surgical Care Improvement Project guidelines recommend that prophylactic antibiotics such as first-generation cephalosporins be infused completely 1 hour before surgical incision. However, these preventative antibiotics are very limited, therefore risk factors must be identified to diagnosis and treat patients effectively. Moreover, determining antimicrobial susceptibility during artificial joint surgery and choosing the most appropriate treatment strategy following an accurate diagnosis of microbial infections are essential. In the present review, we describe the management, including the etiology, diagnosis, and classification of PJI, and approaches to its diagnosis using the available novel molecular diagnostic methods.
Postoperative critical care management for lung transplant recipients in the intensive care unit (ICU) has expanded in recent years due to its complexity and impact on clinical outcomes. The practical aspects of post-transplant critical care management, especially regarding ventilation and hemodynamic management during the early postoperative period in the ICU, are discussed in this brief review. Monitoring in the ICU provides information on the patient's clinical status, diagnostic assessment of complications, and future management plans since lung transplantation involves unique pathophysiological conditions and risk factors for complications. After lung transplantation, the grafts should be appropriately ventilated with lung protective strategies to prevent ventilator-induced lung injury, as well as to promote graft function and maintain adequate gas exchange. Hypotension and varying degrees of pulmonary edema are common in the immediate postoperative lung transplantation setting. Ventricular dysfunction in lung transplant recipients should also be considered. Therefore, adequate volume and hemodynamic management with vasoactive agents based on their physiological effects and patient response are critical in the early postoperative lung transplantation period. Integrated management provided by a professional multidisciplinary team is essential for the critical care management of lung transplant recipients in the ICU.

Fragility curves are being more significant as a useful tool for evaluating the relationship between the earthquake intensity measure and the effects of the engineering demand parameter on the buildings. In this paper, the effect of different site conditions on the vulnerability of the structures was examined through the fragility curves taking into account different strength capacities of the precast columns. Thus, typical existing single-story precast RC industrial buildings which were built in Turkey after the year 2000 were examined. The fragility curves for the three typical existing industrial structures were derived from an analytical approach by performing non-linear dynamic analyses considering three different soil conditions. The Park and Ang damage index was used in order to determine the damage level of the members. The spectral acceleration (Sa) was used as the ground motion parameter in the fragility curves. The results indicate that the fragility curves were derived for the structures vary depending on the site conditions. The damage probability of exceedance values increased from stiff site to soft site for any Sa value. This difference increases in long period in examined buildings. In addition, earthquake demand values were calculated by considering the buildings and site conditions, and the effect of the site class on the building damage was evaluated by considering the Mean Damage Ratio parameter (MDR). Achieving fragility curves and MDR curves as a function of spectral acceleration enables a quick and practical risk assessment in existing buildings.

This paper presents motion planning algorithm that yields to intervening side lane vehicles in a congested traffic flow based on vehicle to vehicle (V2V) communication. Autonomous driving in dense traffic situation requires advanced driving performance in terms of vehicle interaction and risk mitigation. One of the most important functions necessary for congested traffic autonomous driving is to predict the lane change intention of the side lane target vehicle. However, implementing this function by using only environmental sensors has limitations. In this study, V2V communication is used to overcome the limitations and determine the intention of cut-in vehicles. Lane change intention of the intervening side lane vehicle is inferred by its longitudinal speed, steering angle, and turn signal light information received by the on-board-unit (OBU). Once the yield decision is made, the subject vehicle decelerates to generate sufficient clearance for the target vehicle to enter. Validation of the algorithm was conducted with actual autonomous test vehicles.
Purpose: Recent studies have reported a significant association between skeletal muscle, muscle strength and non-alcoholic fatty liver disease (NAFLD). The effect of nutrient intake on the prediction of skeletal muscle mass and strength or its suggested correlation with metabolic diseases has been primarily reported in healthy individuals. The current study explores the association between energy intake and handgrip strength (HGS) in individuals with NAFLD. Methods: Data were obtained from the Korea National Health and Nutrition Examination Surveys 2016-2018. Data from 12,469 participants were extracted and 1,293 men and 1,401 women aged 20 years and older were included in the analyses of patients with NAFLD. The presence of NAFLD was determined using the hepatic steatosis index. To estimate relative skeletal muscle strength, HGS was measured using a digital dynamometer and calculated by adjusting the body mass index of the dominant arm. Study subjects in the NAFLD and non-NAFLD groups were separately categorized according to quartiles of the calculated HGS. Results: We found that individuals with low (EQ1) energy intake had lower odds of HGS compared to subjects with high (EQ4) energy intake, irrespective of their NAFLD status (p < 0.0001). However, the HGS did not differ based on the level of protein or fat intake ratio. Additionally, the effect of energy intake on HGS was more pronounced in men than in women. Conclusion: Energy intake was associated with the risk of weak HGS in men with NAFLD. The results indicate that energy intake may be a key factor in nutrition care for NAFLD patients with low muscle function.
Road transport allows all forms of land conditions to be met at less cost. Because of this function, despite numerous disadvantages, it becomes the most frequently used method of transport, especially in underdeveloped or developing countries. One of the most significant factors used in evaluating the atmosphere's air quality is the amount of CO2, increasing people's density in indoor spaces. The amount of CO2 indoors is, therefore, vital to determine. In this study, CO2 and temperature measurements made on nine different bus journey was made in Turkey. The minimum and maximum values were recorded as 555 ppm and 3000 ppm CO2, respectively, in the measurements. On all journeys, the average concentration is 1088.72 ppm. The minimum and maximum values were measured as 17.4℃ and 32.7℃ in the temperature measurements, and the average of all trips was calculated to be 25.76℃. In this study conducted before the Covid-19 pandemic, it was determined that the amount of CO2 increased with the density and insufficient ventilation in the buses. The risk of infection increases in places with high human density and low clean air. For situations such as pandemics, CO2 measurement is a rapid indicator of determining human density.

Decision making strategies should consider both adaptiveness and robustness in order to deal with two main characteristics of climate change: non-stationarity and deep uncertainty. Especially, robust strategies are different from traditional optimal strategies in the sense that they are satisfactory over a wider range of uncertainty and may act as a key when confronting climate change. In this study, a new framework named Robust Stochastic Dynamic Programming (R-SDP) is proposed, which couples previously developed robust optimization (RO) into the objective function and constraint of SDP. Two main approaches of RO, feasibility robustness and solution robustness, are considered in the optimization algorithm and consequently, three models to be tested are developed: conventional-SDP (CSDP), R-SDP-Feasibility (RSDP-F), and R-SDP-Solution (RSDP-S). The developed models were used to derive optimal monthly release rules in a single reservoir, and multiple simulations of the derived monthly policy under inflow scenarios with varying mean and standard deviations are undergone. Simulation results were then evaluated with a wide range of evaluation metrics from reliability, resiliency, vulnerability to additional robustness measures. Evaluation results were finally visualized with advanced visualization tools that are used in multi-objective robust decision making (MORDM) framework. As a result, RSDP-F and RSDP-S models yielded more risk averse, or conservative, results than the CSDP model, and a trade-off relationship between traditional and robustness metrics was discovered.

Recently an accurate quantification of streamflow under various climatological and anthropogenic factors and separation of their relative contribution remains challenging, because variation in streamflow may result in hydrological disasters. In this study, we evaluated the factors responsible for variation in streamflow in Korean watersheds, quantified separately their contribution using different Budyko-based functions, and identified hydrological breakpoint points. After detecting that the hydrological break point in 1995 and time series were divided into natural period (1966-1995), and disturbed period (1996-2014). During the natural period variation in climate tended to increase change in streamflow. However, in the disturbed period both climate variation and anthropogenic activities tended to increase streamflow variation in the watershed. Subsequently, the findings acquired from different Budyko-based functions were observed sensitive to selection of function. The variation in streamflow was observed in the response of change in climatic parameters ranging 46 to 75% (average 60%). The effects of anthropogenic activities were observed less compared to climate variation accounts 25 to 54% (average 40%). Furthermore, the relative contribution was observed to be sensitive corresponding to Budyko-based functions utilized. Moreover, relative impacts of both factors have capability to enhance uncertainty in the management of water resources. Thus, this knowledge would be essential for the implementation of water management spatial and temporal scale to reduce the risk of hydrological disasters in the watershed.
