A Study on Risk Factors of Recurrent Otitis Media (소아 재발성 중이염의 위험인자에 대한 연구)

  • Purpose : Recurrent otitis media (ROM) is usually defined as ${\geq}$3 distinct and well-documented episodes within 6 months or ${\geq}$4 episodes within 12 months. ROM is sufficiently important to warrant consideration of chemoprophylaxis, tympanostomy tube placement. There also is evidence that children with ROM are at risk for both hearing loss and speech delay. However, studies of ROM have been notably insufficient. In addition, even though environmental, racial, and sociocultural differences can affect risk factors, few studies have been conducted with regard to recurrent otitis media in Korea. Methods : This study was conducted from July 2009 to January 2010 with infants and children who were younger than 60 months old, who visited the out-patient clinics at Han-Il General Hospital and Kyunghee University Hospital. Data were collected by interview using a pre-formed sheet. Among a total of 892 infants and children, 457 were excluded, and the remaining 435 were allocated to 104 with ROM and 331 as a control group. Results : Attendance at daycare centers (P<0.001, OR=2.85), allergic rhinitis (P=0.026, OR=2.32), past history of bronchiolitis (P=0.003, OR=2.33), and low socioeconomic status (P=0.005, OR=2.00) were found to have a close significant correlation with ROM. Risk factors such as sex, having a sibling, breast-feeding, use of pacifiers, atopy, pneumococcal vaccination, influenza vaccination, smoking of parents, and indoor smoking are not relevant. Conclusion : Attendance at daycare centers, allergic rhinitis, past history of bronchiolitis, and low socioeconomic status have been identified as risk factors for ROM.

Prospect for the Outbreak of War between U.S and China by Comparing of the U.S-Japan Relationship in the World WarII Era and the Modern U.S-China Relationship (태평양 전쟁 전 미일관계와 현재의 미중관계 비교를 통한 미중간 전쟁 발발 가능성에 관한 연구)

  • This paper aims to use crossover analysis to uncover similarities and differences between the U.S-Japan relationship in the World War II era and the modern U.S-China relationship, and to forecast the possibility of the outbreak of war between U.S and China by applying the steps to war theory. The steps to war theory argues that the probability of the outbreak of war between two states within five years would approach 90 percent, if they have ongoing territorial dispute, alliance, rivalry, and arms race. The comparison exposes some similarities with the territorial dispute, alliance, rivalry, but reveals dissimilarities with arms race. U.S-Japan relationship in the World War II era had the arms race, which does not exist the modern U.S-China Relationship. The result of comparison is that the probability for the Outbreak of War between U.S and China correspond to third stage(Risk Level). it means that the probability for the Outbreak of War between U.S and China is 55%. But, There are four elements(① Perception of Leader ② Mutual dependence of economy ③ Possession of nuclear weapon ④ Ravages of war) that reduce the probability for the Outbreak of War. Considering the four elements, the probability for the Outbreak of War between U.S and China is a slim chance. But the probability for the Outbreak of War between U.S and China is excluded because of territorial dispute, alliance, rivalry. So, This paper suggests three points.(① Developing military options ② Reducing the misconception of intend, ③ Promoting navy exchanges) to prevent of Outbreak of War.

Association between Ectopic Eruption of the Maxillary First Permanent Molar and Skeletal Malocclusion (상악 제1대구치의 이소맹출과 골격성 부정교합의 연관성)

  • This study assessed the association between ectopic eruption of the maxillary first permanent molar and skeletal malocclusion in 5- to 10-year-old children. As subjects, 786 children who attended the Wonkwang University Dental Hospital for orthodontic diagnosis were included. Children with unerupted first permanent molars or fully erupted second permanent molars were excluded. The study group demonstrated ectopic eruption of the maxillary first permanent molar, while the control group did not. Cephalometric radiographs taken between January 2003 and August 2015 were analyzed. Skeletal class III malocclusion was detected in 57.0% of the study group, which differed significantly from that in the control group (p<0.05). The SNA, ANB angles, and A to N-perpendicular distance were significantly smaller, whereas the A-B plane angle and APDI were significantly greater in the study group than in the control group (p<0.05). The SNB and mandibular plane angles were not significantly different between the groups. Thus, maxillary undergrowth is a risk factor for ectopic eruption of the maxillary first permanent molar.

Claimed Adverse Events of Korean Medicine in South Korea: Analysis of Cases in the Korea Medical Dispute Mediation and Arbitration Agency Databases (한의 의료와 연관된 조정 요청 이상반응: 한국의료분쟁조정중재원 접수사례 분석)

  • Objectives : The purpose of this study was to describe the type of claimed adverse events related to Korean Medicine practices in South Korea. Methods : Claims with regard to the Korean Medicine practice submitted to the Korea Medical Dispute Mediation and Arbitration Agency from April 2012 to December 2016 were collected. We analyzed claims that explicitly reported the type of Korean medicine intervention and were deemed as being adverse events as defined by the Korea Good Clinical Practice. Claims that did not mention the Korean medicine practice explicitly or those related to the patient's dissatisfaction to the service rather than adverse health outcomes were excluded. Types, related interventions and the suspected severity of claimed adverse events were summarized. Results : Of 197 claims obtained, 140 claim cases were eligible and 144 claimed events were deemed as possible adverse events of the Korean medicine practice. Pain(16%), local infection/inflammation(12%) and neurological symptoms(11%) were the most frequently reported types of claimed adverse events. Thirty-nine claimed serious adverse events(SAE) were identified, including pneumothorax(28.2%) and death(17.9%). Conclusions : A wide range of claimed adverse events were identified. Routine monitoring of claims data may provide undetected safety information with regard to the Korean medicine practice. High risk of misclassification of the intervention and claimed adverse events due to insufficient information is the main caveat of this study.

A Review on Treatment of Somatization Disorder in Traditional Chinese Medicine (신체화 장애에 대한 중의학 연구동향)

  • Objectives: This study was conducted to review studies on somatization disorder in traditional Chinese medicine. Methods: We reviewed studies in the China National Knowledge Infrastructure (CNKI) to 2017. Keywords were 軀體化障碍, Somatization disorder, somatic symptom disorder. We included Randomized Controlled Trial (RCT), and excluded non-Randomized Controlled Trial (nRCT), non-related somatization disorder or traditional Chinese medicine, non-clinical trials, dissertations for degrees. Jadad scale and Cochrane Library's Risk of Bias (RoB) were used for assessment of the quality of studies. Results: Twelve studies were selected. The Chinese Classification of Mental Disorders-3 (CCMD-3) was most frequently used as diagnostic criteria for somatization disorder. As for outcome measurement, Hamilton Rating Scale for Depression (HAMD) was used most commonly. Meta-analysis of 10 studies revealed effective rate of Chinese Herbal Medicine groups (CHM) was significantly higher than Western Medicine groups (WM) (RR: 1.14, 95% CI: 1.02 to 1.27, p=0.02, $I^2=40%$). There was no significant difference in effective rate of CHM+WM and WM (RR: 1.12, 95% CI: 0.84 to 1.49, p=0.46, $I^2=83%$). And also, effective rate of Acupuncture group (Acu) revealed no significant difference compared to that of WM (RR: 1.17, 95% CI: 0.95 to 1.44, p=0.13, $I^2=84%$). For HAMD, there was significant difference in CHM vs, WM group and Acu vs. WM group. Quality of selected 12 RCTs was low. Conclusions: Therapies practiced in traditional Chinese medicine may be effective options for somatization disorder. treatment. For further clinical studies in Korean medicine, this study could be groundwork for development of diagnosis and treatment on somatization disorder.

A Study on the Removal of Unusual Feature Vectors in Speech Recognition (음성인식에서 특이 특징벡터의 제거에 대한 연구)

  • Some of the feature vectors for speech recognition are rare and unusual. These patterns lead to overfitting for the parameters of the speech recognition system and, as a result, cause structural risks in the system that hinder the good performance in recognition. In this paper, as a method of removing these unusual patterns, we try to exclude vectors whose norms are larger than a specified cutoff value and then train the speech recognition system. The objective of this study is to exclude as many unusual feature vectors under the condition of no significant degradation in the speech recognition error rate. For this purpose, we introduce a cutoff parameter and investigate the resultant effect on the speaker-independent speech recognition of isolated words by using FVQ(Fuzzy Vector Quantization)/HMM(Hidden Markov Model). Experimental results showed that roughly 3%~6% of the feature vectors might be considered as unusual, and therefore be excluded without deteriorating the speech recognition accuracy.

Clinical Analysis about Complications of Stroke Treated Intensively in the Intensive Care Unit (뇌졸중으로 중환자실에서 집중치료한 환자들의 합병증에 관한 임상적 고찰)

  • Purpose : Cerebrovascular disease is the most frequent cause of death in Korea and it remains severe disabilities disturbing normal life, According to the previous studies, mortality of the stroke in the first one week is up to the 20% and 95% of stroke patients in the acute stage are accompanied by more than one complications. These complications affect not only the acute stage mortality but also the late stage rehabilitations. In Korea the oriental medicine is preformed in the treatment of stroke. Therefore it is important to recognize thoroughly the complications in the acute stage of stroke and to prevent them. But studies about complications of acute stage stroke is rarely presented in the fields of oriental medicine. So this study is prepared for investigating the characteristics and frequency of complications in the acute stage of stroke. And we are to assess the importance of theses acute complications by systemic reviewing the previous studies. Methods : Fifty one patients are included who had been admitted to ICU(intensive care unit) of Chenju Hospital of Wonkwang Oriental Medicine. Twenty nine patients are diagnosed as ischemic stroke and twenty one patients are diagnosed hemorrhagic stroke. Medical and neurological complications were investigated retrospectively based on medical notes excluding primary symptoms of stroke Le. motor weakness, sensory disturbance and speech disorder. And risk factors of stroke such as D.M. or hypertension are excluded. Results : Medical complications are more frequent than neurological ones. Most frequent medical complication is dysuria(61%) and constipation(45%), fever(30%) and aspiration pneumonia(22%) are followed in order of frequency. In Neurological complication dysphagia(56%), the exacerbation of infarction due to increased intracranial pressure(24%), irritability or insomnia(21%) is most frequently complicated in order of frequency. Conclusions : These complications are mostly caused by bed rest state in acute stage stroke. It is supposed that more aggressive management can prevent theses ones. And it is possible to improve the medical and neurological conditions by sticking theses study results.

Feasibility of Early Definitive Internal Fixation of Pelvic Bone Fractures in Therapeutic Open Abdomen

  • Purpose: Damage control laparotomy has contributed to improved survival rates for severe abdominal injuries. A large part of severe abdominal injury occurs with a concomitant pelvic bone fracture. The safety and effectiveness of internal fixation of pelvic bone fracture(s) has not been established. The aim of the present study was to evaluate infection risk in the pelvic surgical site in patients who underwent emergent abdominal surgery. Methods: This single-center retrospective observational study was based on data collected from a prospectively maintained registry between January 2015 and June 2019. Patients who underwent laparotomy and pelvic internal fixation were included. Individuals <18 and ≥80 years of age, those with no microbiological investigations, and those who underwent one-stage abdominal surgery were excluded. Comprehensive statistical comparative analysis was not performed due to the small number of enrolled patients. Results: A total of six patients met the inclusion criteria, and the most common injury mechanism was anterior-posterior compression (67%). The average duration of open abdomen was 98 hours (range, 44-98), and the time interval between abdominal closure and pelvic surgery was 98 hours. One patient (16.7%) died due to multi-organ dysfunction syndrome. Micro-organisms were identified in the abdominal surgical site in five patients (83%), with no micro-organisms in pelvic surgical sites. There was no unplanned implant removal. Conclusions: Internal fixation of pelvic bone fracture(s) could be performed in the state of open abdomen, and the advantages of early fixation may countervail the risks for cross contamination.

Acupuncture for the Treatment of Burns: A Systematic Review of Randomized Controlled Trials and Case Controlled Trials (화상의 침 치료에 대한 체계적 문헌고찰)

  • Objectives The study was conducted to evaluate the acupuncture treatment for burns by reviewing international randomized controlled studies and case controlled studies. Methods In the review, ten academic literature archives; NDSL, OASIS, PubMed, Cochrane library, CNKI, WANFANG, CINAHL, J-STAGE, CiNii, and EMBASE were used as the main databases to search for the randomized controlled trials or the case controlled trials about acupuncture treatment of burns using the keywords "burns AND acupuncture", "burn AND acupuncture", and "scald AND acupuncture". Results Initially, a total of 852 studies were founded except duplicate studies. 801 studies were excluded after screening of title and abstract. After reviewing 51 papers, a total of four randomized controlled trials and two case controlled trials were selected. These studies were analyzed by year, subjects, treatment intervention, evaluation criteria, treatment effect, adverse events, and 'Risk of Bias' assessment for randomized studies and non-randomized studies. From the six papers out of 51 papers those were reviewed, patients with burns were divided into two groups. The experimental group received acupuncture treatment, while the control group did not. The results of the completed studies have shown that the experimental group receiving acupuncture treatment demonstrated significant improvement compared to the control group, and there was no serious adverse events. Conclusions According to some of the studies, acupuncture in burn treatment is worth to try. However, additional well-designed randomized controlled studies will be required to justify the effectiveness of acupuncture treatment of burns.

HIV/AIDS Knowledge, Attitudes, and Sexual Behavior among Adolescents in Seoul (서울시 중.고등학생의 에이즈 지식, 태도 및 성행동)

  • Objectives: The aim of this study was to describe HIV/AIDS knowledge, attitudes (discrimination towards HIV/AIDS and lives with HIV infected persons) and HIV/AIDS related sexual behaviors among middle school and high school students in Seoul, Korea. Methods: The population of this study was junior and senior high school students in Seoul, Korea. Eight junior high schools and eight senior high schools were randomly selected from Seoul City. Two thousand nine hundred and ninety-six responses from the sixteen schools were analyzed (fifty-nine were excluded). A self-administered questionnaire measuring socio-demographic variables, knowledge of HIV/AIDS, as well as discriminatory attitudes of HIV/AIDS was utilized. Results: The level of HIV/AIDS knowledge was low and the levels of discriminatory attitudes were high. The results indicated that students had considerable misconceptions about HIV transmission as well as stigmatizing attitudes towards HIV-infected people and their lives. The proportions of sexual intercourse were 1.3 % for middle school students and 7.7% for high school students. Only 30.7% of current sexually active subjects were to use condoms at the last step of sexual intercourse. The HIV/AIDS related risk-taking behavior were predicted by gender, school types, parents' marital status, reported academic performances, and attitudes towards to homosexuals. Conclusions: It is important to design HIV prevention strategies that improve in the knowledge of HIV transmission for adolescents in Korea.
