Mutational Analysis of the MTHFR Gene in Breast Cancer Patients of Pakistani Population

  • Objectives: Since methylenetetrahydrofolate reductase (MTHFR) maintains the balance of circulating folate and methionine and blocks the formation of homocysteine, its regulation in relation to different cancers has extensively been studied in different populations. However, information on Pakistani breast cancer patients is lacking. The MTHFR gene has two most common mutations that are single nucleotide additions which result in change of amino acids C677T to Ala222val and A1298C to Glu429Ala. Methodology: 110 sporadic breast patients with no prior family history of cancer or any other type of genetic disorders along with 110 normal individuals were screened for mutations in exons 1 to exon 9 using single strand conformational polymorphism, RFLP and sequencing analyzer. Results: The p values for the 677CC, 677CT, and 677TT genotypes were 0.223, 0.006, and 0.077, respectively. Those for the 1298AA, 1298AC, and 1298CC genotypes were 0.555, 0.009, and 0.003, respectively. Conclusions: We found an overall a significant, weak inverse association between breast cancer risk and the 677TT genotype and an inverse association with the 1298C variant. These results for MTHFR polymorphism might be population specific in sporadic breast cancer affected patients but many other factors need to be excluded before making final conclusions including folate intake, population and disease heterogeneity.

Primary angle-closure glaucoma, a rare but severe complication after blepharoplasty: Case report and review of the literature

  • Blepharoplasty is one of the most commonly performed aesthetic procedures. Surgical complications are rare, but can have severe consequences, such as permanent vision loss. In this report, we describe a patient who developed primary angle-closure glaucoma (ACG) with associated vision loss after a oculoplastic procedure using local anesthesia. So far, six similar cases have been described in the literature. It is believed that acute ACG is triggered by the surgical procedure in patients with predisposing risk factors such as a cataract. Surgical triggering factors include the use of buffered lidocaine/xylocaine with adrenaline/epinephrine, stress, and coverage of the eyes postoperatively. Due to postoperative analgesic use, the clinical presentation can be mild and atypical, leading to a significant diagnostic delay. Acute ACG should therefore be excluded in each patient with postoperative complaints by assessing pupillary reactions. If a fixed mid-wide pupil is observed in an ophthalmologic examination, an immediate ophthalmology referral is warranted. Surgeons should be aware of this rare complication in order to offer treatment at an early stage and to minimize the chance of irreversible vision loss.

Endometrial Cancer in Hospital Universiti Sains Malaysia

  • Background: Endometrial cancer is the most common gynecological malignancy among females worldwide, approximately 320,000 women being diagnosed with the disease each year and 76,000 dying. To date, there is limited knowledge of endometrial cancer in Malaysia. Objectives: To identify the epidemiological profile and prognostic factors of survival. Materials and Methods: A list of endometrial cancer patients in 2000-2011 was obtained from the hospital Record Department. Only cases confirmed by histopathology examination were included. We excluded those with incomplete medical records or referral cases. Simple and multiple Cox regression approaches were used for data analysis. Results: Only 108 cases were included with a mean (SD) age of 62.7 (12.3) years, with 87.0% Malay ethnicity. Grade of cancer was: 29.1% grade 1, 43.7% grade 2 and 27.2% grade 3. The majority of patients had non-endometrioid type (60.2%), with myometrial invasion (82.2%) and lymphovascular invasion (57.3%). The significant prognostic factors were age (HR 1.05; 95% CI: 1.02, 1.08, p=0.002) and having lymphovascular invasion (HR 2.15; 95% CI: 1.08, 4.29; p=0.030). Conclusions: Endometrial cancer patients should be diagnosed earlier to reduce the risk of mortality. The public should be given education on the signs and symptoms of the disease.

The Effects of the Combined Exercise Program on PhysicalFitness and Related Hormone in Elderly Women (복합운동프로그램이 여성노인의 체력과 항노화 호르몬에 미치는 영향)

  • PURPOSE: This study examined the effect of the combined exercise program on physical fitness and related hormone in elderly women. METHODS: The subjects included 40 elderly women who reside at B city. Upon thire agreements, the subjects were divided into either an experimental group and the control group. there 20 subjects in each. The combined exercise program was conducted during the 8 weeks, and the experimental group was underwent its associated program 5 times a week. there wear 2 subjects from each group that were excluded. The physical fitness wear measured and the related hormone(growth hormone, DHEA) were taken with blood serum density. RESULTS: After 8 weeks for intervention, there were statistically significant differences between in physical fitness and growth hormone, DHEA in experimental group(p<.05). however, this difference was not significantly different in the control group. Futher, there wear statistically significant difference between two group of all item(p<.05). CONCLUSION: The findings of this study show that combined exercise program helps the physical fitness and increase of growth hormone, DHEA in elderly women. In conclusion, the regular combined exercise program for 8 weeks is effective for increase physical fitness and related hormone in elderly women, and positive influence upon, thereby being thought to be able to lower risk in aging and weakness.

  • Obesity is the condition that excess fat stored in the body. Nowadays, the prevalence of obesity has greatly increased due to westernized dietary habits and changes in lifestyle. It has been proved that obesity is a risk factor of many diseases. Many methods were developed to find curative means for obesity and exercise and controlling diet were considered very effective ones. In order to find the theraputic effect of the exercise and food diary, we examined patients treated by electric accupuncture and ear accupuncture because of obesity. We observed 16 patients with obesity who visited KyungHee Univ., Kangnam Hospital of Oriental Medicine from November, 2002 to March, 2003. To rule out an bias, we excluded the patients who were treated for other disease or took medicine. Exercise in hospital was not effective for treatment of obesity, but keeping diary of diet was effcetive for the decrease of body weight(p<0.05).

Structural Equation Analysis of Social Support, Impaired Fasting Glucose, and Type 2 Diabetes

  • This study is to identify how the social support influences the impaired fasting glucose and type 2 diabetes as structural equation analysis reflects the health behavior and psychological path. It investigates participants who give consents to participate in the Korean Health Examine Cohort (KOEX), which collects surveys and biological samples at eight university hospitals nationwide. Of the 53,767 adults over 30 years of age enrolled in the 2nd to 7th KOEX study from 2004 to 2010 (1st to 7th), 18,618 participants with a history of chronic diseases related to type 2 diabetes, the main variable, were excluded. The number of participants were 34,316 with enough data on social support and diabetes, which are the main variables. The results show that social support affects health behaviors and lowers fasting blood sugar. Therefore, not only the management of risk factors of individuals, but also the practice of healthy behavior with the attention of social relations such as social support, etc., can prevent type 2 diabetes.

A systematic review of domestic oral health care programs for elementary school children (초등학생 대상의 국내 구강건강관리 프로그램에 대한 체계적 문헌고찰)

  • Objectives: This purpose of this study was to examine the trends in interventional research and analyze the contents and results of intervention through a systematic literature review of the domestic research literature, suggesting the effect of an oral care program on elementary school students. Methods: A total of 262 articles were retrieved initially, and the duplicate articles were then removed, excluded after screening titles, abstracts, and full-text articles. Finally, a total of 16 papers were used in the review. Results: The study included 9 articles with the nonequivalent control group pretest-posttest design and 7 with the one-group pretest-posttest design. All articles were of non-randomized studies. The most common index of the interventional effect was dental plaque. Further, oral health behaviors and knowledge, perception, self-efficacy, and DMFT index were common indices. In the quality assessment, in the "blinding of outcome assessment" among six items of ROBANS, 12 studies showed a high risk of bias. Conclusions: In order to develop an interventional oral health care program for elementary school students and evaluate the effectiveness, a strict research design and qualitative improvement of research reports are required, and continuous research should be conducted to develop a systematic protocol.

Plan for plastic surgeons to participate in trauma teams at regional trauma and emergency centers (권역외상 및 응급센터에서 성형외과의 참여방안)

  • A law revised in May 2012 provided support to regional and emergency centers for reducing the risk of preventable deaths. In particular, regional trauma centers have been established throughout the nation, with the goal of ensuring that any trauma patient can reach a trauma center within an hour. As a multidisciplinary approach is particularly important in treating severe trauma patients, activation teams are currently organized at each center to perform multiple simultaneous treatments. Under the present system, only 7 departments can participate in these trauma teams; emergency medicine, cardiothoracic surgery, general surgery, orthopedic surgery, neurosurgery, radiology, and anesthesiology. Plastic surgeons also play an essential role in treating trauma patients, and in fact currently treat many such cases. Especially in reconstruction procedures in patients with head and neck trauma and wide tissue defects, plastic surgeons possess unique expertise. However, since plastic surgeons are excluded from the trauma response teams due to institutional limitations, we describe the role and necessity of plastic surgery for trauma and emergency patients, and urge that the system be improved.

A systematic review of the accuracy and efficiency of dental movements with Invisalign®

  • We are currently living in an era where the use of computer-aided design/computer-aided manufacturing has allowed individualized orthodontic treatments, but has also incorporated enhanced digitalized technology that does not permit improvisation. The purpose of this systematic review was to analyze publications that assessed the accuracy and efficiency of the $Invisalign^{(R)}$ system. A systematic review was performed using a search strategy to identify articles that referenced $Invisalign^{(R)}$, which were published between August 2007 and August 2017, and listed in the following databases: MEDLINE, Embase, Cochrane Library, Web of Knowledge, Google Scholar, and LILACS. Additionally, a manual search of clinical trials was performed in scientific journals and other databases. To rate the methodological quality of the articles, a grading system described by the Swedish Council on Technology Assessment in Health Care was used, in combination with the Cochrane tool for risk of bias assessment. We selected 20 articles that met the inclusion criteria and excluded 5 due to excess biases. The level of evidence was high. Although it is possible to treat malocclusions with plastic systems, the results are not as accurate as those achieved by treatment with fixed appliances.

A Study on the Efficiency of the Foreign Exchange Markets: Evidence from Korea, Japan and China

  • Purpose - The purpose of this study was to examine the efficiency of the foreign exchange markets in Korea, Japan and China. Design/methodology/approach - This study collected 1327 observations each of the daily closing exchange rates of the three currencies against the US dollar for the sample period from January 1, 2015 to January 31, 2020, based on the tests for autocorrelation, unit root tests and GARCH-M(1,1) model estimation. Findings - We have found that the autocorrelation test indicates the lack of autocorrelation and unit root test confirms the existence of unit roots in all times series of the three currencies, respectively. The GARCH-M(1,1) test results, however, suggest that the exchange rates do not follow a random walk process. In conclusion, the recent spot foreign exchange markets in Korea, Japan and China are believed to be informationally inefficient. Research implications or Originality - These findings have practical implications for both individual and institutional investors to be able to obtain excess returns on their investments in the foreign exchange markets in three countries by using appropriate risk management, portfolio strategy, technical analysis, etc. This study provides the first empirical examination on the foreign exchange market efficiency in the three biggest economies in Asia including China, which has been excluded from research due to its exchange rate regime.
