An Integer Programming Formulation for Outpatient Scheduling with Patient Preference

  • Patients' satisfaction while receiving medical service is affected by whether or not their preferences can be met, including time and physician preference. Due to scarcity of medical resource in China, efficient use of available resources is urgently required. To guarantee the utilization ratio, the scheduling decisions are made after all booking information is received. Two integer models with different objectives are formulated separately, maximizing the degree of satisfaction and revenue. The optimal value of the two models can be considered as the bound of corresponding objectives. However, it is improper to implement any of the extreme policies. Because revenue is a key element to keep the hospital running and satisfaction degree is related to the hospital's reputation, neither the revenue nor the satisfaction can be missed. Therefore, hospitals should make a balance. An integrated model is developed to find out the tradeoff between the two objectives. The whole degree of mismatching that is related to patient satisfaction and other separate mismatching degree are considered. Through a computational study, it is concluded that based on the proposed model hospitals can make their decisions according to service requirement.

Patterns of Cancer-Related Risk Behaviors Among Construction Workers in Hong Kong: A Latent Class Analysis Approach

  • Background: Hong Kong's construction industry currently faces a manpower crisis. Blue-collar workers are a disadvantaged group and suffer higher levels of chronic diseases, for example, cancer, than the wider population. Cancer risk factors are likely to cluster together. We documented prevalence of cancer-associated lifestyle risk behaviors and their correlates among Hong Kong construction workers. Methods: Data were collected from workers at 37 railway-related construction worksites throughout Hong Kong during May 2014. Tobacco use, alcohol consumption, unbalanced nutrition intake, and physical inactivity were included in the analysis. Latent class analysis and multivariable logistic regression were performed to identify the patterns of risk behaviors related to cancer, as well as their impact factors among construction workers in Hong Kong. Results: Overall, 1,443 workers participated. Latent class analysis identified four different behavioral classes in the sample. Fully adjusted multiple logistic regression identified age, gender, years of Hong Kong residency, ethnicity, educational level, and living status differentiated behavioral classes. Conclusion: High levels of lifestyle-related cancer-risk behaviors were found in most of the Hong Kong construction workers studied. The present study contributes to understanding how cancer-related lifestyle risk behaviors cluster among construction workers and relative impact factors of risk behaviors. It is essential to tailor health behavior interventions focused on multiple risk behaviors among different groups for further enlarging the effects on cancer prevention.

Characterization of the in vitro Activities of the P1 and Helper Component Proteases of Soybean mosaic virus Strain G2 and Tobacco vein mottling virus

  • Potyviruses express their RNA genomes through the production of polyproteins that are processed in host cells by three virus-encoded proteases. Soybean plants produce large amounts of protease inhibitors during seed development and in response to wounding that could affect the activities of these proteases. The in vitro activities of two of the proteases of Soybean mosaic virus (SMV) and Tobacco vein mottling virus (TVMV) were compared in the rabbit reticulocyte lysate in vitro translation system using synthetic RNA transcripts. Transcripts produced from SMV and TVMV cDNAs that included the P1 and helper component-protease (HC-Pro) coding regions directed synthesis of protein products that were only partially processed. Unprocessed poly-proteins were not detected from transcripts that included all of the P1, HC-Pro, P3 and portions of the cylindrical inclusion protein coding regions of either virus. Addition of soybean trypsin inhibitor to in vitro translation reactions increased the accumulation of the unprocessed polyprotein from TVMV transcripts, but did not alter the patterns of proteins produced from SMV. These experiments suggest that SMV-and TVMV-encoded proteases are differentially sensitive to protease inhibitors.

Efficacy and Tolerability of Adjuvant Oral Capecitabine plus Intravenous Oxaliplatin (XELOX) in Asian Patients with Colorectal Cancer: 4-Year Analysis

  • Background: Although FOLFOX (infusional fluorouracil/leucovorin plus oxaliplatin) is established as a standard chemotherapeutic regimen, the long term efficacy of adjuvant XELOX (oral capecitabine plus intravenous oxaliplatin) in Asian colorectal cancer (CRC) patients remains anecdotal. Moreover, uncertainties persist as to whether pharmacogenetic differences in Asian populations preclude equally tolerable and effective administration of these drugs. Method: One hundred consecutive patients with resected colorectal cancer received adjuvant XELOX (oxaliplatin 130 $mg/m^2$ on day 1 plus capecitabine 900 $mg/m^2$ twice daily on day 1 to 14 every 3 weeks for 8 cycles) at Queen Mary Hospital, Hong Kong. Endpoints monitored during follow-up were disease-free survival (DFS) and disease recurrence, overall survival (OS) and adverse events (AEs). Results: The median patient age was 56 years, 56% were diagnosed with rectal cancer and 44% with colonic cancer. After a median follow-up of 4.3 years (95% confidence interval, 3.2-4.7), 24 recurrences were confirmed including 13 patients who died due to progressive disease. Four-year DFS was 81% in colon cancer patients and 67% in rectal cancer patients (p=0.06 by log-rank test). For the cohort as a whole, OS was 90% at 3 years and 84% at 5 years. Treatment-related AEs led to early withdrawal in four patients. The commonest non-hematological AEs were neuropathy (91%), hand-foot syndrome (49%) and diarrhea (46%), while the commonest grade 3/4 AEs were neutropenia (11%) and diarrhea (10%). Conclusion: These results confirm the favourable long term survival benefit with good tolerability in using adjuvant XELOX in treating East Asian colorectal cancer patients.

Ultimate strength behavior of steel-concrete-steel sandwich beams with ultra-lightweight cement composite, Part 2: Finite element analysis

  • Ultra-lightweight cement composite (ULCC) with a compressive strength of 60 MPa and density of $1,450kg/m^3$ has been developed and used in the steel-concrete-steel (SCS) sandwich structures. This paper investigates the structural performances of SCS sandwich composite beams with ULCC as filled material. Overlapped headed shear studs were used to provide shear and tensile bond between the face plate and the lightweight core. Three-dimensional nonlinear finite element (FE) model was developed for the ultimate strength analysis of such SCS sandwich composite beams. The accuracy of the FE analysis was established by comparing the predicted results with the quasi-static tests on the SCS sandwich beams. The FE model was also applied to the nonlinear analysis on curved SCS sandwich beam and shells and the SCS sandwich beams with J-hook connectors and different concrete core including ULCC, lightweight concrete (LWC) and normal weight concrete (NWC). Validations were also carried out to check the accuracy of the FE analysis on the SCS sandwich beams with J-hook connectors and curved SCS sandwich structure. Finally, recommended FE analysis procedures were given.

안드로이드 기반 스마트폰의 사용자 응답성 향상을 위한 프레임워크 지원 우선순위 부스트 기법 (Framework-assisted Priority boosting for Improving Interactivity of Android Smartphones)

  • 최근 안드로이드 플랫폼을 탑재한 스마트폰이 널리 보급되면서 안드로이드 플랫폼에 대한 관심은 더욱 커지고 있다. 하지만 안드로이든 스마트폰은 종종 양질의 사용자 응답성을 제공하지 못하는 것으로 알려져 있다. 이는 안드로이드 상에서 대화형 태스크가 다른 태스크와 구별되지 않고 동일한 우선순위로 스케줄링 되기 때문에 사용자 입력을 처리하는 동안 여러 번의 선점을 당해 긴 응답시간을 초래할 수 있기 때문이다. 이 논문은 안드로이드 스마트폰의 사용자 응답성 향상을 위해 프레임워크 지원 우선순위 부스트 기법을 제시한다. 제안된 기법은 프레임워크 레벨에서 대화형 태스크를 식별하고 이를 커널에게 전달하며, 커널 레벨에서는 식별된 태스크의 우선순위를 선별적으로 부스트 시킴으로써 선점 없이 사용자 입력을 처리할 만큼 충분한 시간을 보장해 준다. 실험 결과는 제안된 기법이 기존 시스템보다 최대 22% 단축된 응답 시간을 보여 제안된 기법의 효용성을 입증하였다.

  • 최근 들어 초고층 오피스 건축물의 건설이 크게 증가하고 있는 추세이며, 현재에도 많은 계획안이 추진중이며, 건설될 전망이다. 초고층 건축물은 건축물의 개념을 초월하여 도시의 개념을 지니고 있는데, 그 크기만큼 상당한 양의 에너지를 한 건축물에서 소비하고 있는 것이다. 따라서 초고층 건축물에서 소비되는 에너지를 절약하면 그 만큼 큰 효과를 볼 수 있는데, 공간에 적합하고 효율적인 조명설계와 시스템의 선택은 조명 에너지를 절약하기 위한 기초단계라고 할 수 있다. 본 연구에서는 국내 외 초고층 오피스 건축물의 조명설계 현황을 비교 분석하여 앞으로 건축될 초고층 오피스 건축물의 가이드라인으로 활용할 계획이다.

Modeling the Influence of Gas Pressure on Droplet Impact Using a Coupled Gas/liquid Boundary Element Method

  • An inviscid axisymmetric model capable of predicting droplet bouncing and the detailed pre-impact motion, influenced by the ambient pressure, has been developed using boundary element method (BEM). Because most droplet impact simulations of previous studies assumed that a droplet was already in contact with the impacting substrate at the simulation start, the previous simulations could not accurately describe the effect of the gas compressed between a failing droplet and the impacting substrate. To properly account for the surrounding gas effect, an effect is made to release a droplet from a certain height. High gas pressures are computationally observed in the region between the droplet and the impact surface at instances just prior to impact. The current simulation shows that the droplet retains its spherical shape when the surface tension energy is dominant over the dissipative energy. When increasing the Weber number, the droplet surface structure is highly deformed due to the appearance of the capillary waves and, consequently, a pyramidal surface structure is formed; this phenomenon was verified with our experiment. Parametric studies using our model include the pre-impact behavior which varies as a function of the Weber number and the surrounding gas pressure.

Ultimate strength behavior of steel-concrete-steel sandwich beams with ultra-lightweight cement composite, Part 1: Experimental and analytical study

  • Ultra-lightweight cement composite (ULCC) with a compressive strength of 60 MPa and density of $1450kg/m^3$ has been developed and used in the steel-concrete-steel (SCS) sandwich structures. ULCC was adopted as the core material in the SCS sandwich composite beams to reduce the overall structural weight. Headed shear studs working in pairs with overlapped lengths were used to achieve composite action between the core material and steel face plates. Nine quasi-static tests on this type of SCS sandwich composite beams were carried out to evaluate their ultimate strength performances. Different parameters influencing the ultimate strength of the SCS sandwich composite beams were studied and discussed. Design equations were developed to predict the ultimate resistance of the cross section due to pure bending, pure shear and combined action between shear and moment. Effective stiffness of the sandwich composite beam section is also derived to predict the elastic deflection under service load. Finally, the design equations were validated by the test results.

Coupled Finite Element Analysis for Semi-implicit Linear and Fully-implicit Nonlinear Scheme in Partially Saturated Porous Medium

  • The paper presents a comparison between a semi-implicit time integration linear finite element implementation and fully-implicit nonlinear Newton-Raphson finite element implementation of a triphasic small strain mixture formulation of an elastic partially saturated porous medium. The pore air phase pressure pa is assumed atmospheric, i.e., $p_a$ = 0, although the formulation and implementation are general to handle increase in pore air pressure as a result of loading, if needed. The solid skeleton phase is assumed linear isotropic elastic and partially saturated 'consolidation' in the presence of surface infiltration and traction is simulated. The verification of the implementation against an analytical solution for partially saturated pore water flow (no deformation) and comparison between the two implementations is presented and the important of the porosity-dependent nature of the partially saturated permeability is assessed on comparison with a commercial code for the partially saturated flow with deformation. As a result, the response of partially saturated permeability subjected to the porosity influences on the saturation of a soil, and the different behaviors of the partially saturated soil between staggered and monolithic coupled programs is worth of attention because the negative pore water pressure in the partially saturated soil depends on the difference.
