퇴행성 관절염과 류마티스 관절염 유병자의 악력 특성 -제 8기 국민건강영양조사 자료 분석- (Characteristics of Grip Strength in People with Osteoarthritis and Rheumatoid Arthritis: Data From the 8th Korean National Health and Nutrition Examination Survey )

  • Purpose: The purpose of this study was to investigate the characteristics and grip strength of people with osteoarthritis and rheumatoid arthritis using data from the Korea National Health and Nutrition Examination Survey. Methods: This was a retrospective study that analyzed raw data from the first year of the 8th Korea National Health and Nutrition Examination Survey (2019). The study population was 780 people in total, ranging in age from their teens to their 80s. These were people who had been diagnosed with osteoarthritis and rheumatoid arthritis, and the presence of arthritis in and grip strength of these subjects were determined using the average value of three measurements. Results: According to the National Health and Nutrition Examination Survey, the prevalence of arthritis in Korea was 2.5% in men, 10.7% in women, and 13.1% overall. More women than men had osteoarthritis and rheumatoid arthritis, and the number of people with osteoarthritis increased with age. In this study, of those with osteoarthritis, 13.5% were men and 88% were women of those with rheumatoid arthritis, 19.3% were men and 56.3% were women. The number of patients with osteoarthritis increased with age, and rheumatoid arthritis was more common in older people. Subjects with osteoarthritis had lower grip strength than those without the disease, and the older the age at which rheumatoid arthritis was first diagnosed, the lower the grip strength. Conclusion: Grip strength is lower in patients with osteoarthritis than in those without osteoarthritis, and it is possible to estimate the degree to which muscle strength decreases.

여성 류마티스 관절염 환자의 피로 예측 모형 (Prediction Model of Fatigue in Women with Rheumatoid Arthritis)

  • Rheumatoid arthritis is a chronic systemic autoimmune disease. Although the joints are the major loci of the disease activity, fatigue is a common extraarticular symptom that exists in all gradations of rheumatoid arthritis. Fatigue is defined as a subjective sense of generalized tiredness or exhaustion and has multiple dimensions. Therefore fatigue is a common and frequent problem for those with rheumatoid arthritis. In fact, 88-100% of individuals with rheumatoid arthritis experience fatigue. Especially the degree of fatigue is higher in women than men with rheumatoid arthritis. Despite the importance of fatigue among the patients with rheumatoid arthritis, the mechanism that leads to fatigue in rheumatoid arthritis is not completely understood. This study was intended to test and validate a model to predict fatigue in women with rheumatoid arthritis. Especially it was intended to identify the direct and indirect effects of the variables of pain, disability, depression, sleep disturbance, morning stiffness, and symptom duration to fatigue. Data were collected by questionnaires including Multidimensional Assesment of Fatigue(Tack, 1991), numeric scale of pain, graphic scale of joints, Ritchie Articular Index, Korean Health Assessment Questionnaire(Bae, et al., 1998), Inventory of Function Status(Tulman, et al., 1991), Center for Epidemiologic Studies-Depression, and Korean Sleep Scale(Oh, et al 1998). The sample consisted of 345 women with a mean duration of rheumatoid arthritis for 10.06 years and a mean age of 49.64 years. SPSS win and Win LISREL were used for the data analysis. Structural equation modeling revealed the overall fit of the model. Pain predicted fatigue directly and indirectly through disability, depression, and sleep disturbance. Disability, sleep disturbance predicted fatigue only directly, while depression only indirectly through disability and sleep disturbance. Also morning stiffness and symptom duration predicted fatigue through disability and depression. All predictors accounted for 65% of the variance of fatigue. Depression, pain, and disability predicted sleep disturbance. Depression had reciprocal relationship with disability and they both were predicted by pain directly and indirectly. In summary, pain, depression, disability, sleep disturbance, morning stiffness, and symptom duration contributed to the fatigue of patients with rheumatoid arthritis. The best predictor of fatigue was pain. This finding indicates that the modification of pain, depression, disability, sleep disturbance, morning stiffness could be nursing intervention for relief or prevention of fatigue.

  • Purpose: The purpose of this study was to evaluate the prevalence, awareness, treatment and control of hypertension, diabetes, and dyslipidemia among women with rheumatoid arthritis. Methods: This cross-sectional study of 288 women with rheumatoid arthritis was conducted in a university-affiliated hospital. Data were collected by reviewing the women's medical charts and self-report questionnaires. Results: The prevalence of hypertension, diabetes and dyslipidemia was 27.8%, 5.9% and 48.3%, respectively. Despite the high prevalence of dyslipidemia, the subjects'awareness of it and its treatment rates were the lowest, which resulted in a low control rate. Of the 288 subjects, 158 (55%) had at least one of the three conditions. The prevalence of the combination of cardiovascular risk factors increased with age, and 32.7% of those aged 60 or older had two or three of the cardiovascular risk factors. Combinations of cardiovascular risk factors were significantly related with severe pain and fatigue. Conclusion: Cardiovascular risk factors were highly prevalent among the women with rheumatoid arthritis, but the rates of awareness, treatment, and control were suboptimal. There is a pressing need to facilitate the prevention, early detection and appropriate management of hypertension, diabetes and dyslipidemia among patients with rheumatoid arthritis in order to prevent cardiovascular disease.

여성 류마티스 관절염 환자의 피로 요인에 관한 연구 (Factors Influencing Fatigue in Women with Rheumatoid Arthritis)

  • The purposes of this study were to identify the prevalence of fatigue. and factors influencing fatigue in women with rheumatoid arthritis. The subjects were 124 patients with rheumatoid arthritis. Most of all patients felt fatigue and mean score measured by Multidimensional Assessment of Fatigue was relatirely high. Fatigue was positively correlated with pain intensity, the number of pain sites, functional disability, depression. and sleep quality. A hierachical regression model was used to determine the variance which accounts for fatigue. Pain intensity, depression. sleep quality accounted for fatigue significantly. This finding indicates that pain influences fatigue through depression and sleep quality ; depression through sleep quality. The causative factors could be identified by further study of structural eqation model.

당뇨병전기 성인의 신체운동과 류마티스 발생률 (Physical Activity and Prevalence of Rheumatoid Arthritis in Korean Adults with Prediabetes in the 2009-2013 National Health Cohort Study)

  • Background: Rheumatoid arthritis is highly prevalent in overweight patients with type 2 diabetes mellitus and can be reduced by physical activity via altered proinflammatory parameters. However, the association between the frequency of physical activity and the prevalence of rheumatoid arthritis in prediabetic patients remains unclear and was evaluated in this study. Methods: Utilizing the Korean National Health Insurance Sharing Service database, 58,391 adults, who met the research criteria and underwent a general medical check-up between 2009 and 2013, were selected for this study. To analyze the data, a logistic regression with a proc survey logistic procedure was used. Results: The study revealed that the cumulative rheumatoid arthritis prevalence was lower in prediabetic patients compared to that in the control group (OR, 0.64; 95% CI, 0.483-0.840; p=0.001). More frequent physical activity (≥3 days/week) was significantly associated with a reduced rheumatoid arthritis prevalence in both groups (OR, 0.28; 95% CI, 0.039-0.521; p=0.044 vs. OR, 0.15; 95% CI, 0.063-0.237; p=0.007). Additionally, a 3.8-fold higher risk of rheumatoid arthritis development was observed in prediabetic adults with less frequent baseline physical activity (≤2 days/week). Overall, in prediabetes, the prevalence of rheumatoid arthritis was associated with the frequency of physical activity and not with the fasting plasma glucose levels. Conclusion: More frequent physical activity is associated with a low risk of developing rheumatoid arthritis in prediabetic patients. Thus, further studies are needed to confirm the clinical outcomes of frequent physical activity in rheumatoid arthritis prevention and control.

류마티스 관절염 환자의 무기력과 건강행위에 관한 연구 (Helplessness and Health Behavior of Patients with Rheumatoid Arthritis)

  • Purpose: This study was to find out the relationships between helplessness and health behavior of patients with rheumatoid arthritis. Method: The subjects were 293 rheumatoid arthritis patients who visit 2 rheumatology outpatient clinics in G city. Data were collected by questionnaires including Arthritis helplessness index(Nicassio, Wallston, Callahan, Herbert, & Pincus, 1985)and health behavior assessment scale(Jo, Oh & Choe, 2000). The data were analyzed with Pearson correlation coefficient, t-test, ANOVA and Duncan's multiple range test. Results: The majority of the subjects were women and the mean age was 50.4years. The mean duration of illness was 8.9years. There were negative correlations between the degree of helplessness and the degree of health behavior. The degree of helplessness was significantly high in those who were older, female, less educated, out of workforce, and those who had no chances to attend any arthritis-related classes. The degree of health behavior was significantly low in those who were older, high school graduate, in higher economic status, married, and those who had chances to attend arthritis-related classes. Conclusion: To reduce the helplessness of rheumatoid arthritis patients, the nursing intervention which is to increase the health behavior should be developed and provided to the rheumatoid arthritis patients.

극복력(Mastery)이 여성 류마티스 관절염 환자가 자각하는 불확실성 인지에 미치는 효과 (A Study on the Impact of Mastery on Appraisal of Uncertainty in Women Patients with Rheumatoid Arthritis)

  • Objectives: This study was conducted to investigate the effects of mastery on appraisal of uncertainty in women patients with rheumatoid arthritis. Methods : The study subjects consisted of 168 patients who were recruited from the outpatient clinic of a rheumatic center in Seoul. Self report questionnaires were used to measure the study variables that included uncertainty, mastery, danger appraisal of uncertainty, and opportunity appraisal of uncertainty. Cronbach's alpha reliabilities of these instruments ranged from .72 to .93. For data analysis. the SPSSWIN 10.0 program was utilized to exam descriptive statistics. Pearson's correlation. and regression analysis. Results: The results were as follows.: 1) The uncertainty scores of the subjects ranged from 33 to 87 with the mean score of 63.27. 2) The mastery scores of the subjects ranged from 10 to 27 with a mean score of 18.70. 3) The danger appraisal of uncertainty scores of the subjects ranged from 8 to 32 with a mean score of 20.22. 4) The opportunity appraisal of uncertainty scores of the subjects ranged from 7 to 28 with a mean score of 17.80. 5) Significant factors that explained the danger appraisal of uncertainty were mastery (=-.444. p<.001), and education level (=-.184. p<.05). 6) Significant factor that explained the opportunity appraisal of uncertainty was level of uncertainty (=-.328. p<.001). Conclusion: Among the independent variables. the most significant factor that explained the danger appraisal of uncertainty in the women patients with rheumatoid arthritis was mastery. Therefore, a nursing intervention with strategies to improve sense of mastery should be developed for women patients with rheumatoid arthritis.

자조관리 프로그램이 여성 류마티스 관절염 환자의 자기효능감과 삶의 질에 미치는 효과 (The Effects of a Self-management Program on Self-efficacy and Quality of Life for Korean Women with Rheumatoid Arthritis)

  • Purpose: The purpose of this study was to examine the effects of a self-management program on self-efficacy and quality of life for Korean women with rheumatoid arthritis. Methods: A quasi-experimental study using a nonequivalent control group pretest posttest design was used. The data were collected from July 1 to Sep. 15, 2007. Thirty one arthritis patients (15 in the experimental group and 16 in the control group) were participated in this study. A self-management program developed by Korean Society of Muscle and Joint Health in 1994 was used. The program was consisted of 6 sessions, 2 hours each and held every Friday for 6 weeks. General Self-Efficacy Scale and Quality of Life were used to test the effect of the program. Results: Hypothesis 1; "Self-efficacy in the experimental group after program would be higher than the control group" was supported. Hypothesis 2; "Quality of Life in the experimental group after program would be higher than the control group " was not supported. Conclusion: The self-management program was effective for Korean women with rheumatoid arthritis in terms of increasing self-efficacy. Therefore, to increase self-management skills and self-efficacy for female rheumatoid arthritis patients, the program can be useful.

여성 류마티스 관절염 환자의 피로 관련 요인 (Factors Related to Fatigue in Women with Rheumatoid Arthritis)

  • Purpose: The purpose of the study is to investigate the degree of fatigue and its related factors in women with rheumatoid arthritis. Method: The subjects were 143 patients with rheumatoid arthritis. Data were collected by questionnaires including Multi-Dimensional Assessment of Fatigue(Tack, 1991), Korean Health Assessment Questionnaire(Bae et al., 1991), numeric scale of pain(Lee & Song, 1987), Center for Epidemiologic StudiesDepression, and Korean Sleep Scale(Oh, et al., 1998). The data were analyzed by SPSS WIN 12.0 program using descriptive statistics, Pearson Correlation, and Stepwise Multiple Regression. Result: The result were as follows. 1. The scores of fatigue of subjects averaged $4.95{\pm}1.83$, degree of fatigue was $5.85{\pm}1.98$, and influence of fatigue was $4.04{\pm}2.09$. 2. The mean score of the degree of physical dysfunction, pain, sleep disorder, and depression were $1.42{\pm}0.38,\;8.15{\pm}3.58,\;1.86{\pm}0.67,\;and\;1.85{\pm}0.46$ points respectively. 3. The subject's total fatigue score, physical dysfunction, pain, sleep disorder, and depression was correlated positively(r=.44, r=.28, r=.29, r=.27, p< .01). 4. The main influencing factors on the fatigue were physical dysfunction and sleep disorder. These two main variables made it possible to explain 23.0% of the variance in fatigue. Conclusion: Therefore, nursing interventions for fatigue experienced women with rheumatoid arthritis would be focused to decrease physical dysfunction and sleep disorder.
