Study on the Different Effect of Same Taste of Herb;mainly based on "Chuhaisanghannon(注解傷寒論)" by Seongmugi(成無己) (본초(本草)의 동미이용(同味異用)에 대(對)한 연구(硏究);성무기(成無己)의 "주해상한론(注解傷寒論)"을 위주로)

  • We got some conclusion about the function of five tastes from individual peculiarity, from "Sanghanjapbyeongnon(傷寒雜病論)" by Sungmoogi, like below. 1. Five tastes[五味] have their general function, but herbs of one tastes, each has special function, we can define it as individual peculiarity 2. For example, sour taste generally make it convergent, astringent. The sourness of Paeoniae Radix Alba[芍藥] can convergent resin, and be help nutrition, but same of Phaseoli Semen[赤小豆] can make him vomit. 3. Bitterness generally make it down, dry, and solid. Scutellariae Radix[黃芩], Coptidis Rhizoma[黃連] can bring down fever of heart and spleen, but Rhei Radix Et Rhizoma[大黃] eliminates solid illness. 4. Sweetness make it strong, harmonic, relax. Sweetness of Glycyrrhizae Radix[甘草] flows into spleen, make it relax the part of beneath heart. Puerariae Radix[葛根] can make scatter the cold on skin. 5. General nature of hot taste is diffusion, sheen, rampancy. Cinnam omi Ramulus[桂技] eliminates the Pung(風) in Wigi(衛氣). same Zingiberis Rhi zoma[乾薑] make inner cold scattered, and warm stomach. 6. Salty make Gi(氣) down, and slacken solidity, salty Natrii Sulfas[芒硝]removes heat of body. But Alismatis Rhizoma[澤瀉] removes needless water. 7. To know peculiarity of each herb, not only one taste but other combined tastes, and areas medical, agricultural, biologic, etc.

Review of Recent Clinical Studies of Herbal Medicine Treatment for Pediatric Tonsillitis - Focused on Chinese Randomized Controlled Trials - (소아 편도염의 한약 치료에 대한 최신 임상 연구 동향 - 중국 RCT 연구를 중심으로 -)

  • Objectives The purpose of this study is to investigate recent clinical studies on the effect of herbal medicine for pediatric tonsillitis in China, and to seek better methods to treat and study for pediatric tonsillitis in Republic of Korea. Methods We searched clinical studies from the China National Knowledge Infrastructure (CNKI) by search formula (SU='扁桃体炎'+'扁桃炎'+'扁桃腺炎'+'乳蛾'+'喉蛾'+'蚕蛾'+'石蛾'+'珠蛾'+'肉蛾'+'乳蛾風'+爛乳風'+'tonsillitis') and (SU='中樂'+'中医樂'+'本草'+'湯'+'丸'+'散'+'方'+'顆粒'+'膠囊'+'自擬'+'herb'+'herbal'+'decoction'+'remedy'+'Chinese medicine'+'Korean medicine'+'kampo'+'formula'+'herbal drug'+'Chinese drug'+'plant'+'Chinese prescription'+'traditional medicine'+'Medicine, East Asian Traditional'+'Herbal Medicine') in professional search from January 2016 to August 2020. We analyzed the literature focusing on the treatment methods and results. Results Among 1464 searched studies, 35 randomized controlled trials were selected and analyzed. In most studies, the effectiveness of oral administration of herbal medicine on pediatric tonsillitis was significant. Most commonly used herbs were Fructus Forsythiae (連翹), Radix Glycyrrhizae (甘草), Radix Scutellariae (黃芩), Herba Menthae (薄荷), Fructus Arctii (牛蒡子), Radix Scrophulariae (玄蔘), Radix Platycodi (桔梗), Flos Lonicerae (金銀花), Radix Isatidis (板藍根), Radix Bupleuri (柴胡), Fructus Gardeniae (梔子), Rhizoma Belamcandae (射干), Radix et Rhizoma Rhei (大黃), Gypsum Fibrosum (石膏). Conclusions By analyzing the improvement of indicators such as total effective rate, cured rate, symptom disappearance time and symptom score, we found that herbal medicine treatment can help improve pediatric tonsillitis. However, additional studies are needed to solidify these findings.

  • The result were as follows: 1. Yu Wan So(劉完素) regarded the cause of surgical disease as the heat(熱). He used the three method to drain(疏通), promoting pus drainage(托裏) and the balance between ying-energy(營氣) and wei-energy(衛氣). 2. Jang Jong Jeung(張從正) only refered to medicines without entire theory. 3. Lee Dong Won(李東垣) regarded the cause of surgical disease as the greasy diet(膏梁厚味), damp air(濕氣), wetness heat(濕熱). For each treatment he used the method of Yu Wan So's treatment and emphasized the balance between ying-energy(營氣) and wei-energy(衛氣), stomach energy(胃氣). 4. Ju Jin Heung(朱震亨) distinguished the cause of surgical disease from enternal(內), external(外) and channel(經). For each treatment he used the method to drain(疏通), promoting pus drainage(托裏) and the balance between ying-energy(營氣) and wei-energy(衛氣). The first stage used the clearing away heat and toxic materials(淸熱解毒) and the last stage used the invigoration and dispersion(補托). 5. Yu Wan So(劉完素) used to be very busy of Moschus(麝香), Alumen(白礬), Olibanum(乳香), Coptidis Rhizoma(黃連), Minium(黃丹) and Scutel1ariae Radix(黃芩) In classification of the medical action, great part of the medicine are activating blood circulation to dissipate blood stasis(活血祛瘀), clearing away heat and toxic materials( 淸熱解毒) and external application(外用). 6. Jang Jong Jeung(張從正) used to be very busy of Olibanum(乳香), Angelicae Gigantis Radix(當歸), Minium(黃丹), Myrrha(沒藥), Calomelas(輕粉), Rhei Radix Et Rhizoma(大黃) and Phellodendri Cortex(黃柏). In classification of the medical action, great part of the medicine arc activating blood circulation to dissipate blood stasis(活血祛瘀), clearing away heat and toxic materials(淸熱解毒) and diverged wind-cold evil(發散風寒). 7. Lee Dong Won(李東垣) used to be very busy of Forsythiae Fructus(連翹), Scuteliariae Radix(黃芩), Angelicae Gigantis Radix(當歸). Astragali Radix(황기), Glycyrrhizae Radix(炙甘草), Bupleuri Radix(紫胡), Phellodendri Cortcx(黃柏), and Coptidis Rhizoma(黃連). Two Cases of Acute Renal Failure Caused by Acute Paraquat Poisoning (급성 Paraquat 중독 후 발생한 급성 신부전 환자 2예)

  • Paraquat is a very potent herbicide which causes fatal toxicity when ingested, and there is no specific antidote against it. Human ingestion induces acute renal failure, hepatic dysfunction and progressive respiratory failure with high mortality rate. Clinical investigation and medical treatment were done on two cases of acute renal failure caused by paraquat poisoning admitted to the Department of Internal Medicine, Wonkwang University Oriental Chonju Medical Hospital. We report two cases of patients who survived after acute paraquat intoxication, by means of oriental medicine such as Gamdutang, a typical antidote of toxins, chinese ink as an absorbent and burned powder of Rhei Radix et Rhizoma for laxative and so on, western medicine such as gastric lavage, diuretics and fluid therapy. We suggest more experiments and studies related to such treatment for paraquat poisoning be conducted.

An Adult Case of Painful Hand Foot and Mouth Disease Treated with Galgeunhaegi-tang (심한 통증을 동반한 성인 수족구병을 갈근해기탕으로 치료한 증례)

  • Objectives The purpose of this case study is to report the improvement of hand foot and mouth disease after using Galgeunhaegi-tang Methods A Taeumin patient suffering from painful hand foot and mouth disease was treated with herb medicine Galgeunhaegi-tang, acupuncture and cupping therapy. Rhei radix et rhizoma was added or removed from herb medicine according to patient's stool condition. Severity of pain and general symptom was measured using Numerical Rating Scale(NRS), and the picture of patient's hand and foot was taken. Results & Conclusions After taking Galgeunhaegi-tang, symptoms including pain, rash, blisters, and general conditions improved as patient's stool condition improved from constipation to normal. In conclusion, this case report shows that Galgeunhaegi-tang can be effective to Taeumin hand foot and mouth disease patients.

Analysis for Compatibility of Daehwangmokdan-tang and Its Pharmacological Activities (대황목란탕(大黃牧丹湯)의 배오(配伍)분석과 그 약리활성)

  • Objectives : I analysed daehwangmokdan-tang's compatibility principle by the system of chief, deputy, assistant, and envoy and investigated pharmacological activities by categorizing with chemical components, molecular level, cellular level, animal level and human level based on Korean and Chinese studies for this formula. Methods : Daehwangmokdan-tang's compatibiltity principle was examined by the system of chief, deputy, assistant, and envoy. I looked into studies that presented in Korea from 1956 to 2016 about daehwangmokdan-tang through KOREA INSTITUTE OF ORIENTAL MEDICINE, Korean medicine information system (OASIS) and in Chinese for 20 years about daehwangmokdan-tang through China National Knowledge Infrastructure, CNKI. Then classify into chemical components, molecular level, cellular level, animal level and human level to analyse. Results : According to the system of chief, deputy, assistant, and envoy, chief herb is Rhei Radix et Rhizoma and Moutan Cortex, deputy herb is Natrii Sulfas and Persicae Semen, assistant and envoy herbs are Trichosanthis Semen. The amount of extraction of paeonol, total anthraquinone, and conjugated anthraquinone from daehwangmokdan-tang with the formulation of the system of chief, deputy, assistant, and envoy was the highest, and in the formulation of chief herb and deputy herb, the extraction amount of paeonol and conjugated anthraquinone was the lowest. With other formulations, the amount of extraction of total anthraquinone and conjugated anthraquinone was improved, although the degree was different. In particular, when it is blended with Persicae Semen as a deputy herb, the extraction amount of total anthraquinone and conjugated anthraquinone of Rhei Radix et Rhizoma as a chief herb is greatly increased, and the extraction amount of paeonol is rather different, but it is lowered. It was found that the amount of Mg Ca K Na in daehwangmokdan-tang was the highest. Pharmacological activities can be detected in inflammatory mediators and enzymes for molecular level. For cellular level, it can be determined in lipopolysaccharide (LPS)-stimulated RAW 264.7 cell line. In mouse and rats for animal level and human level, in inflammatory diseases (acute appendicitis, acute pancreatitis, acute cholecystitis, acute abdominal disease, ect.), pharmacological activities was caught. Conclusions : From the above results, daehwangmokdan-tang is composed in line with the system of chief, deputy, assistant, and envoy, suggesting that there is certain rationality and scientific. Pharmacological activities of daehwangmokdan-tang are effective to anti-inflammation, improvement of sepsis, analgesic, muscle relaxation, and improvement of intestinal flora and its metabolites. Daehwangmokdan-tang is consistent with the action of diuresis and anti-inflammation in terms of the content of elements, suggesting that there is action of purging fire and removing blood stasis, defecation detoxification, cooling blood and clearing heat and activating blood and dispersing stasis.

A Study on the medical and pharmacological theory of Interior-Overheated-Disease of Taeumin (태음인(太陰人) 이열병(裡熱病)의 병증(病證) 약리(藥理)에 대한 연구(硏究))

  • 1. Background and Purpose: I intended to understand the medical and pharmacological theory of Taeumin, through a study of the process through which Lee Je-ma discovered the Interior-Overheated-Disease of Taeumin and created the prescriptions for it. 2. Methods: I studied and analized the change in the medical and pharmacological theory, through a historical study on the quotations and prescriptions of "DongYi Soose Bowon". 3. Results: Through a literature study I could find that in the existing Oriental Medicine before Lee Je-ma, the difference of the Liver Febrile Disease of Taeumin and the Stomach Febrile Disease of Soyangin were recognized, and the prescriptions of the two diseases were a little distinguished, but the medical theories of those were not distinguished at all. And I found that the Liver Febrile Disease of Taeumin shows the pain in the eyes and the throat, and drying of the nose due to interior dry and heat, so it is different from the Stomach Febrile Disease of Soyangin that shows irritation of fever, headache, and the trouble in passing feces and urine. Also I could know that Radix Puerariae, Rhizoma Cimicifugae, Rhizoma Ligustici, Radix Angelicae Dahuricae, and Radix et Rhizoma Rhei are Taeumin's medicines because these medicines had been used for Taeumin's syndromes, and that Radix Scutellariae, Radix Platycodi, Semen Raphani, and Fructus Gleditsiae are Tae-Eum-In's medicines because these medicines are known as acting for lung.

Study on one Ryang of Guizhi-tang (계지탕 1량의 현대용량추정)

  • In order to convert the one ryang of Guizhi-tang into the modern weights and measures, herbs whose dose was expressed by number in Treatise on Feverile Disease of Zhongjing were measured actually. The results obtained are as follows. 1. Size of Jujubae fructus in Guizhi-tang is an average and if remove the seed, 12 Jujubae fructuses corresponds to 20g. These weights are the same as Belamcandae Rhizoma, Persicae Semen, Gleditsiae Fructus, Gardeniae Fructus and Armeniacae amarum Semen whose dose was expressed by number in Treatise on Feverile Disease of Zhongjing. If one ryang corresponds to 6.5g, 20g corresponds to three ryang. If one ryang corresponds to 15.6g, 20g corresponds to 1.3 ryang. When herbs were added in Guizhi-tang, the used weights were two more ryang. Almost unit of weights in Formulae of Zhongjing is the constantly multiplied. 2. Half sheng(升) of Pinelliae Rhizomae is 15 Pinelliae Rhizomas and corresponds to two and a half ryang. 14 Pinelliae Rhizomas are contained in 50 mL egg. 3.1 geun(斤), maximal amount of Gypsum Fibrosum and amount of one egg are the same weight supposing one ryang corresponds to 6.5g. 4. 2 ryang of Rhei Radix et Rhizoma is the 6.8 balls(碁子) of baduk and similar as 5-6 balls of baduk of Jisilchijasi-Tang. 5. General dose of Glycyrrhizae Radix is two ryang and 20mL, the size of thumb is 13g. 6. Half geun approaches the 3 flattened spoons. Consequently the presumption that one ryang in Guizhi-tang is 15.6g is wrong. One ryang is assumed as 6.5g.

Effect of Samhwangsashimtang Extract on Lipopolysaccharide-Stimulated Inflammatory Response and Macrophage Activity (삼황사심탕(三黃瀉心湯) 추출물의 lipopolysaccharide 유도에 의한 염증 조절과 대식세포 활성에 대한 연구)

  • Objectives : Samhwangsashim-tang(SHSST), a mixture of Rhei radix et rhizoma, Scutellariae radix, and Coptidis rhizoma, has been regarded as being able to treat bleeding, gastric discomfort, dry mouth, insomnia and purpura due to Blood Heat. Currently, this herbal formula is applied to gastritis, gastric ulcer, hypertension, atherosclerosis or other types of vascular inflammatory disorders. Methods : We extracted this herbal mixture with 30% ethanol and examined for its effects on systemic inflammatory responses and in vitro macrophage activity. Mice were orally given to SHSST for 7 days and then lipopolysaccharide(LPS) was intraperitoneally injected. Tumor necrosis factor-${\alpha}$(TNF-${\alpha}$) levels in serum were measured 1 h after LPS challenge. Peritoneal macrophages were isolated from thioglycollate-injected mice and used for in vitro cellular activity. Cell death was measured using the MTT method and annexin V/propidium iodide staining. LPS-stimulated signaling molecules necessary for TNF-${\alpha}$ expression were determined by Western blotting. Results : Oral administration of SHSST for 7 days resulted in a significant reduction in LPS-stimulated TNF-${\alpha}$ release into serum. In vitro treatment of SHSST was cytotoxic in a concentration-dependent manner. However, SHSST caused a concentration-dependent reduction in necrosis and increase in apoptosis in mouse peritoneal macrophages. SHSST inhibited the activation of NF-${\kappa}B$, p38 and JNK signaling molecules in response to LPS. Conclusion : Taken together, our results demonstrated that SHSST was effective in lowering LPS-stimulated TNF-${\alpha}$ serum levels, possibly through its modulation of NF-${\kappa}B$, p38 and JNK in macrophages.

HDDM, a formula consisting of seven herbs, had anti-diabetic but no immunomodulatory activities in multiple low doses of streptozotocin-treated female of B6C3F1 mice

  • The objectives of this study were to determine the effect of herb formula HDDM, a modification of Huangdan decoction that has been shown to be effective in the treatment of glomerulonephritis and chronic renal failure, on the blood glucose levels in multiple low doses (MLD; 50 mg/kg for five consecutive days) of streptozotocin (STZ)-treated female B6C3F1 mice. Initial studies were performed to compare diabetes induction in five strains (e.g., B6C3F1, NOD, CD-1, C3H/HeN and C57BL/6) of mice by MLD-STZ, and immune changes following the treatment. The results suggested that the order of susceptibility to diabetes induction was NOD $\approx$ CD-1 > B6C3F1 $\approx$ C3H > C57BL/6. Furthermore, STZ modulation of T cell development, differentiation and activation might play a role in diabetes induction by MLD-STZ treatment. MLD-STZ-induced diabetes in female B6C3F1 mice was moderate, which allowed the evaluation of drug-induced protection or exacerbation of diabetes to be performed. As such, modulation of blood glucose by HDDM, which consisted of Da Huang (Radix Et Rhizoma Rhei), Huang Qi (Radix Astragali Seu Hedysari), Dan Shen (Radix Salviae Miltiorrhizae), Yin Yang Huo (Herba Epimedii), Yi Yi Ren (Semen Coicis or Coix lacryma-jobi), Mai Dong (Radix Ophiopogonis) and Shan Zhu Yu (Fructus Corni), was evaluated in MLD-STZ-treated female B6C3F1 mice. The results suggested that HDDM could lower the blood glucose levels, but it had no immunomodulatory activities. Additionally, HDDM-treated mice exhibited improved glucose tolerance. In conclusion, these studies have suggested that MLD-STZ-induced diabetes in female B6C3F1 mice is a useful model to evaluate drug modulation of diabetes, and that the herb formula HDDM possesses anti-diabetic effects.
